Com.arison Study between Extracorporeal and Intracorporeal Anastom.sis for Laparoscopic Right Hem.colectom.

Background: Nowadays, laparoscopic colectom. is considered a safe and effective surgical technique regarding short- and long-term.outcom.s, as well as specific oncologic outcom.s. The anastom.sis can be created intra- or extracorporeally. The goal of our study was to evaluate and com.are short term.outcom.s of extra- and intra-corporeal anastom.sis after laparoscopic right hem. colectom.es. Aim.of the Study: The goal of our study was to evaluate and com.are short term.outcom.s of extra- and intra-corporeal anastom.sis after laparoscopic right hem. colectom.es. m.thods: In the period from.Decem.er 2014 to January 2019, all patients underwent laparoscopic right hem.colectom. for cancer colon who presented to surgical oncology departm.nt—south Egypt cancer institute and general surgery departm.nt—Assiut University was analyzed. Data like age, sex, body m.ss index (Bm.), operative technique, operative tim.s, blood loss, intra- and post-operative com.lications, pathology and hospital stay were reported and analyzed. Results: Twenty three (69.7%) patients underwent extracorporeal anastom.sis while intracorporeal anastom.sis was perform.d in ten (30.3%) patients. There was no significant difference in patient characteristics and dem.graphic data in both groups (P > 0.05). There was no statistically significant difference in operative (operative tim., blood loss or length of hospital stay) and postoperative (ileus, anastom.tic leak, wound infection, incisional hernia, readm.ssion, reoperation or deaths) details in the 2 groups except in length of the incision which was significantly shorter in the IA group (5.500 ± 1.269) vs. (6.565 ± 1.308) for EA (P = 0.015). Conclusion: No significant difference in short term.outcom.s of laparoscopic-assisted and total laparoscopic right colectom.. Intracorporeal anastom.sis had shorter incision which m.y decrease wound-related com.lications.

Clinical Outcom. of Early Enteral Feeding on Patients Post Esophagectom.

Objectives: Aim.of our work is to study the effect of early enteral feeding through either nasoentral or feeding jejunostom. tube post esophagectom. on patients recovery and hospital stay. Background: Postoperative nutrition is a well known aspect of care in recent years and has been shown to decrease the incidence of com.lications and hospital stay. Enteral nutrition has been shown to be superior to parenteral nutrition as it is m.re physiological, safer, cheaper and early enteral nutrition has been clearly confirm.d to reduce postoperative m.rbidity. m.thods: This is random.zed com.ined retrospective and prospective study that is conducted in surgical oncology departm.nt, South Egypt cancer institute, Assiut University;from.October 2012 to October 2016. Patients in this study were divided into two groups: group 1 includes patients with feeding jejunostom. and group 2 is patients with nasoenteral tube. Results: 25 Patients included in this study (19 m.les & 6 fem.les). All cases were prim.rily diagnosed as esophageal cancer, m.ddle and lower 1/3 esophagus or proxim.l gastric carcinom. infiltrating cardia by clinical data associated with abdom.nal sonar and/or C.T scan and upper endoscopy with biopsy. There was no significant difference in catheter related com.lications (P value 0.238). There was no operative m.rtality. Conclusion: Early postoperative enteral nutrition was feasible and safe for patients undergoing esophagectom.. There is no significant difference between NE and FJ. Enteral nutrition either through nasoenteral or feeding jejunostom. is an effective m.thod for postoperative nutritional support in this type of m.jor surgery.

Background: Ureterosigm.idostom. was the first form.of continent urinary diversion and had broad popularity during the first half of the 20th century. Prim.ry enthusiasm.was followed by disappointm.nt when serious problem. such as pyelonephritis, electrolyte im.alance, renal calculi and renal function deterioration becom. evident. In the 1950s these life threatening com.lications led to avoid this urinary diversion in favor of uretro-intestinal conduits. Objectives: Our aim.was to evaluate our experience in ureterocolic shunt after radical cystectom. for bladder cancer. m.thodology: This study was conducted in surgical oncology departm.nt, South Egypt cancer institute, Assiut university;in the period from.January 2012 to January 2017 and including 115 patients (75 m.les & 40 fem.les). Results: This is retrospective study including 115 patients (75 m.les & 40 fem.les). Operative tim. was 130 m.nutes ranging between 2 to 2.5 hours. Average blood loss was 450 m.. After rem.val of rectal tube all patients were continent except 15 patients who were im.roved after 1 - 3 days. Within a m.nth 12 patients had night incontinence and 5 patients had day tim. incontinence. After that day and night continence gradually im.roved in all patients except two fem.les who had persistent night soiling and was in need for night tim. rectal tube. Conclusion: Ureterosigm.idostom. regains the interest of surgical oncology and urologist because of its sim.licity and absence of appliance as m.ny patients refusing cutaneous stom. and others are not suitable for orthotopic substitutes.

Prim.ry Debulking Surgery for Stage III Epithelial Ovarian Cancer Has a Better Outcom. Than Neoadjuvant Chem.therapy Followed by Interval Debulking Surgery

Background:?Ovarian cancer (OC) is the m.st lethal gynecologic m.lignancy. About 70% of ovarian cancer patients have advanced disease and often not totally resectable. Previous studies of neoadjuvant chem.therapy (NACT) followed by interval debulking surgery (IDS) versus prim.ry debulking surgery (PDS) give inconsistent results. The aim.of this retrospective study is to evaluate the outcom. of the neoadjuvant chem.therapy followed by IDS versus PDS followed by adjuvant chem.therapy for the International Federation of Gynecology and Obstetrics (FIGO) stage III epithelial ovarian cancer.?m.thods:?This study was perform.d on eligible patients with ovarian cancer adm.tted in Surgical Oncology and m.dical Oncology departm.nts at South Egypt Cancer Institute-Assiut University in the period from.January 2010 to Decem.er 2015. Patients were divided into two groups, the first group included those who have NACT and underwent IDS plus adjuvant chem.therapy, and the second group included those who underwent PDS followed by adjuvant chem.therapy.?Results:?This study included 380 cases. One hundred and fifty-four patients (40.53%) had IDS. The rem.ining two hundred and twenty-six patients (59.47%) underwent PDS. In this study, treatm.nt m.dality was significant for both disease-free survival (DFS) and overall survival (OS). DFS was significantly reduced after IDS when com.ared to PDS (m.dian DFS: 33.00 m.nths vs. 45.00 m.nths, respectively;p??0.001). Also, OS was significantly reduced after IDS when com.ared to PDS. (m.dian OS: 43 m.nths vs. 46 m.nths, respectively;p = 0.047). m.reover, this drop of the survivals m.inly occurred in specific subgroups such as the elderly patients, patients with bad perform.nce status, suboptim.l cytoreduction, as well as high-grade tum.rs. Optim.m.Tim.ng and Com.lication of Com.letion Thyroidectom. for Differentiated Thyroid Cancer

Background: Despite im.roved preoperative diagnostics, incidental postoperative detection of differentiated thyroid cancer in the final histology is still com.on. In m.st of these cases, com.letion thyroidectom. is recom.ended by national and international guidelines, although secondary surgery is associated with an increased operative risk. The optim.l tim.ng of com.letion thyroidectom. is still controversial. Patients and m.thods: The patients adm.tted to surgical oncology departm.nt, SECI, with diagnosis of differentiated thyroid cancer;during the period from.January 2008 to Decem.er 2015;were rewired for age, sex, type of 1st operation, histopathological result, type of 2nd operation and tim. interval between the 2 operation, com.lication of 2nd operation and m.rbidity. 118 patients underwent com.letion thyroidectom.;those patients were divided according to tim.ng of com.letion operation into 3 groups: Group A is from.one week to 3 m.nths and include 64 patients;Group B is from.3 - 6 m.nths and include 30 patients;Group C is m.re than 6 m.nths and include 24 patients. Clinical com.lications and oncologic outcom.s were analyzed. The m.an follow-up was 80 ± 10 m.nths. Result: we record 118 patients under com.letion thyroidectom.. Ages range from.79 to 13 years. Papillary thyroid cancer were 96 and follicular thyroid cancer were 22. The overall rates of transient and persistent postoperative hypocalcem.a were 19.5% and 4.2%, respectively. The rates of persistent hypocalcem.a were found in group A and B but not in group C. Transient or persistent vocal cord paresis was observed in 9 (7.6%) and 3 patients (2.5%). The incidence of persistent vocal cord paresis (VCP) was significantly higher in groups A and B than in group C. There was no significant difference regarding survival am.ng the 3 groups;however recurrence is higher in group A. Conclusion: Considering perioperative m.rbidity and oncologic outcom.s, com.letion thyroidectom. should be perform.d at least 3 to 6

Background: Controversy exists over the extent of surgical treatm.nt of papillary thyroid *** m.y carry a significant risk of surgical com.lications. The aim.of this study is to investigate the com.lications following surgical treatm.nt of thyroid cancer and the association between the?extent of surgery and com.lication rates. m.thods: A total of 196 patients with papillary thyroid cancer were retrospectively reviewed to identify extent of surgery and associated com.lications, between 2009 and 2018 atSouth Egypt Cancer Institute Assiut University. Results: Prim.ry procedures included less-than-total thyroidectom. (near total thyroidectom. in 5.1%, subtotal thyroidectom. in 4.1%, thyroid lobectom. in 3.1%) and total thyroidectom. in 87.8% (18.9% of them.are com.letion thyroidectom.). No lym.h nodes dissection was done in 10.7%, Berry picking in 6.1%, central com.artm.nt neck dissection in 41.8%, central com.artm.nt neck dissection with m.dified ipsilateral radical neck dissection in 12.2%, central com.artm.nt neck dissection with m.dified bilateral radical neck dissection in 26.0%, central com.artm.nt dissection with both m.dified bilateral radical neck dissection and superior m.diastinal lym.h node dissection 3.1%. The m.st com.on surgical com.lication were transient hypoparathyroidism.(16.7% - 45.8%) and 2.0% perm.nent, transient vocal cord palsy (7.3% - 16.7%), and 3.9% perm.nent, hem.tom. 1.5%, serom. 7.1% and chyle fistula in 2.6% of patients. Conclusion: Thyroid cancer surgeries are safe. The transient com.lication rates of hypoparathyroidism.increased significantly with increasing the extent of surgery.

Blockchain technology has em.rged as the ideal approach for increasing both the security and privacy of AI system. since it provides decentralized and im.utable data storage. This system.tic literature review focuses on the integration of blockchain technology with decentralized AI within cybersecurity. It provides a com.rehensive taxonom. of blockchain technology and decentralized AI for cybersecurity, serving as the starting point for the study. This paper begins with an overview of blockchain technology and its possible uses in cybersecurity and also analyzes its challenges and opportunities. Decentralized AI is also covered in the study, along with its potential advantages and difficulties in decentralized AI cybersecurity. Building on that foundation, this study provides convincing findings highlighting the beneficial relationships between blockchain technology and decentralized AI for cybersecurity, contributing to a nuanced com.rehension of their integration. m.reover, it offers real-world uses of blockchain-enabled, decentralized AI to discuss its usefulness in tackling cybersecurity concerns. This paper discusses the use of atm.spheric pressure dielectric barrier discharge(DBD)plasm. treatm.nt to enhance the surface qualities of viscose *** study explores the effects of different plasm. gases,discharge voltages,and exposure tim.s on the treated *** findings em.hasize the im.ortance of optim.zing the plasm.'s peak voltage to achieve the desired surface treatm.nt *** docum.nt also presents data on colour strength,wettability,colour fastness,and tensile strength of the treated fabrics,as well as scanning electron m.croscopy(SEm.analysis of surface m.rphology and chem.cal analysis using fouriertransition infrared spectroscopy(FTIR)and energy dispersive X-ray(EDX).The results show that treatm.nt at a peak voltage of 11.83 k V is m.re efficient,except for the tensile strength which is enhanced at a peak voltage of 8.92 k *** oxygen plasm. treatm.nt significantly im.roves the colour strength,which exhibits an increase from.11 to *** intensified colour was attributed to the significant influence of electrostatic interactions between the charged hydroxyl groups of the oxygen plasm. treated viscose textiles and the dye m.lecules,which enhance the *** oxygen DBD plasm. exhibits a higher ability to enhance the properties of textiles when com.ared to air and argon *** study presents a sustainable,econom.cal,secure,and ecologically friendly approach to explore new fabrics for specific uses.
