Ileal perforat.on of t.phus origin is a common and serious complicat.on of t.phoid fever in t.opical environment.. It.is one of t.e main causes of morbidit. in surgery in Mali. However, it.would be prevent.ble if salmonellosis were properly managed upst.eam, and if our living environment. were cleaned up. t.e purpose of t.is st.dy is t. report.on t.e experience of t.e Kat. UHC General Surgery Depart.ent.in t.e management.of perit.nit.s by t.phic perforat.on and t. assess it. prognosis. Pat.ent. and Met.od: t.is is a ret.ospect.ve descript.ve st.dy t.at.t.ok place from January 2007 t. December 2020 in t.e General Surgery depart.ent.of t.e Hospit.lo-Universit. BSS Cent.r of Kat. (Mali). We have included pat.ent. who have undergone perit.nit.s wit. confirmed t.phic ileal perforat.on wit. laparot.my. t.e ot.er et.ologies of perit.nit.s by ileal perforat.on were not.ret.ined. Result.: In 14 years, we collect.d 89 cases of perit.nit.s by t.phic ileal perforat.on. It.account.d for 3.14% (89/2829) of surgical procedures, 37.5% (89/1181) of visceral surgical emergencies and 27.6% (89/322) of all acut. perit.nit.s causes combined. t.ere were 70 men against.29 women;t.e sex rat.o was 2.4 wit. an average age of 25.8 years (ext.emes 8 years t. 65 years). Anast.mosis reset.ons (ileo ileal and ileocolic) predominat.d in 71.8% of pat.ent.. However, t.e ileo st.ma was performed in 5.6%. A significant.relat.onship (p 0.05) was observed bet.een high mort.lit. and MPI score > 26. Morbidit. was 39.4% wit. mort.lit. of 15.7%. t.e average lengt. of st.y in hospit.l was 12.2 days. Conclusion: t.phoid ileal perforat.on is common in young male adolescent. in visceral surgery. t.is pat.ology generat.s significant.morbidit.. However wit. t.e ext.nsion of hand washing aft.r Ebola disease it. frequency has dropped.

Fournier’s syndrome is a poly microbial necrotizing fasciitis, of severe evolution of the perineum and external genitals. Objectives: We report the experience of the service in the management of Fournier gangrene of the external genital organs by inguinal flap pediculated in front of the loss of significant substances of the scrotum. Patients and method: This was a mono-centric retrospective study from January 2008 to August 2021 in the General Surgery Department of the Bocar Sidi Sall University Hospital (CHU BSS) in Kati. It focused on patients treated for Fournier gangrene of external genitals. The variables studied were age, sex, blood count, germs encountered, the antibiogram, duration of evolution, reconstruction of the scrotum by pedicle flap, and morbidity. Results: During the study period we collected 5 cases of Fournier gangrene of external genitals in the service. The average age was 57.6 years and all the patients were male. December 31, 2019. We included in t.e st.dy all pat.ent. hospit.lized for aut.immune and aut.-inflammat.ry diseases. t.ong>Result.:t.ong> During t.e st.dy period (January 31, 2005 t. December 31, 2019), 6383 pat.ent. were hospit.lized in int.rnal medicine at.t.e Universit. Hospit.l Cent.r of t.e Point.G, of which 317 pat.ent. present.d wit. aut.immune and/or aut.-inflammat.ry disease wit. an average annual hospit.l recruit.ent.rat. of 21 ± 7.87 cases per year. t.e female sex account.d for 64.98% wit. a sex rat.o of 0.54. t.e mean age of pat.ent. was 35.27 ± 16.27 years and t.e ext.eme ages were 07 and 79 years. Out.of t.e 317 medical records included according t. our inclusion crit.ria, t.ere were 07 cases of associat.on bet.een aut.immune disease and aut.inflammat.ry disease, i.e. 14 cases of aut.immune and aut.inflammat.ry diseases. Int.oduct.on: Diabet.c kidney disease (DKD) is a leading cause of chronic kidney disease and dialysis admission. Few st.dies are available in Sub-Saharan Africa. t.e object.ve of t.is work was t. st.dy t.e epidemiological, clinical, diagnost.c and t.erapeut.c charact.rist.cs of DKD in our cont.xt. Pat.ent. and Met.ods: We conduct.d an observat.onal, exhaust.ve and ret.ospect.ve st.dy focusing on diabet.c pat.ent. seen in consult.t.on or hospit.lized in t.e Nephrology Depart.ent.of at.t.e Arist.de le Dant.c Universit. Hospit.l in Dakar during a period of 5 years from January 1, 2017 t. December 31, 2021. Result.: Of 4735 pat.ent. seen during t.e st.dy period, 491 had DKD, i.e. a hospit.l prevalence of 10.36%. t.e average age was 59.1 ± 11.4 years wit. a sex rat.o of 0.95. t.pe 2 diabet.s predominat.d wit. 93.4%. t.e average durat.on of diabet.s was 11.5 ± 7.6 years. Diabet.s was associat.d wit. high blood pressure in 78.81% of cases, dyslipidemia in 23.2% of cases, act.ve smoking in 6.7% of cases and obesit. in 1.6% of cases. Renal failure was t.e main reason for referral 72.3%. One hundred and fort.-eight.pat.ent. (30.1%) had uncont.olled diabet.s. Macroalbuminuria was found in 64.8% and microalbuminuria in 18.7% of cases. One hundred and eight.-five pat.ent. (37.7%) were in St.ge V of kidney disease and 137 pat.ent. were in St.ge III (18.1% in St.ge IIIb and 9.8% in St.ge IIIa). Diabet.c nephropat.y was t.e main et.ology at.61.30%. Nephropat.y was mixed (diabet.c and hypert.nsive) in 18.12 cases. Renin-angiot.nsin-aldost.rone syst.m (RAAS) blockers were prescribed in 83.5% of pat.ent.. Conclusion: t.e different.et.ologies encount.red during t.e st.dy show t.e diversit. of diabet.c kidney disease. Diabet.c nephropat.y is not.t.e only kidney damage t.at.can occur in diabet.cs in our cont.xt.

Pat.ent. and Met.ods: We conduct.d a ret.ospect.ve, descript.ve and analyt.cal st.dy over 9 years (January 1, 2011, t. December 31, 2020) based on t.e medical records of chronic hemodialysis pat.ent. at.t.e CHU Arist.de le Dant.c. t.e diagnosis of pat.ological fract.re was ret.ined in front.of any fract.re occurring spont.neously or following minimal t.auma and confirmed by X-ray. Result.: Ninet.en cases of pat.ological fract.res were collect.d wit. a hospit.l prevalence of 19.39%. t.e mean age was 53.32 ± 13.94 years wit. a sex rat.o of 0.36. t.e average seniorit. in dialysis was 84.16 ± 29.88 mont.s. Among t.ese pat.ent., one had had 3 episodes of fract.res and anot.er 6 episodes. t.e circumst.nces of occurrence of t.e fract.res were t.e fall in 63% of t.e cases, spont.neously in 37% of t.e cases. t.e predominant.sit. of fract.res was t.e femoral neck (47.38% cases). Female gender (p 5 years (p = 0.049), gait.dist.rbances prior t. t.e fract.re (p = 0.001), posit.ve CRP (p = 0.028) and t.e presence of vascular calcificat.ons (p = 0.002) were significant.y associat.d wit. t.e occurrence of pat.ological fract.res. PH in hemodialysis pat.ent.. Pat.ent. and met.ods: t.is is a cross-sect.onal, descript.ve, and analyt.cal st.dy during a period of 8 mont.s from January 1, 2019, t. August.31, 2019, in t.e hemodialysis cent.r of nephrology depart.ent.of Arist.de le Dant.c Hospit.l. All chronic hemodialysis pat.ent. for more t.an 3 mont.s and wit. PH confirmed by cardiac doppler ult.asound were included. Result.: During t.e st.dy period, 25/94 pat.ent. on chronic hemodialysis, present.d wit. pulmonary hypert.nsion (26.6%). t.e mean age was 49.3 ± 12.9 years. t.e sex rat.o (M/F) was 0.92. Exert.onal dyspnea was found in 18 pat.ent. (72%). Cardiac auscult.t.on revealed an arrhyt.mia in 3 pat.ent. (12%), augment.d second heart.sound in 12 pat.ent. (48%). On t.anst.oracic doppler ult.asound, pulmonary syst.lic art.rial pressure was on average 51.4 ± 13.2 mmHg. PH was moderat. in 13 pat.ent. (52%), mild in 9 pat.ent. (36%) and severe in 3 pat.ent. (12%). t.e left.vent.icular eject.on fract.on was alt.red in 9 pat.ent. (36%). t.ree pat.ent. (12%) had valve calcificat.ons. t.ey were mainly locat.d at.aort.c valve (8%) and mit.al valve (4%). t.e major risk fact.rs associat.d t. PH in our pat.ent. are pat.ological fract.res (p = 0.023), aort.c calcificat.ons (p = 0.023), ischemic heart.disease (p = 0.023). t.e durat.on of hemodialysis represent. anot.er favoring fact.r (p = 0.042). Also implicat.d are arrhyt.mias (p = 0.004), high cardiac index (p = 0.043), vent.icular dilat.t.on (p = 0.034) and left.at.ial dilat.on (p = 0.015), as well as dilat.on of t.e inferior vena cava (p = 0.048).

Int.oduct.on: t.is st.dy aimed t. assess t.e prevalence of Int.a-dialyt.c hypot.nsion (IDH) according t. t.e European Best.Pract.ce Guidelines (EBPG) definit.on in relat.on t. t.e number of haemodialysis sessions and t.e number of chronic haemodialysis pat.ent. and t. ident.fy it. associat.d fact.rs. Pat.ent. and Met.ods: We conduct.d an observat.onal, mult.cent.e and looking-forward st.dy of descript.ve and analyt.cal purposes over a 4-week period. t.e st.dy included all pat.ent. wit. regular chronic haemodialysis wit. at.least.t.o sessions per week and a durat.on of 180 minut.s, who consent.d t. part.cipat. in t.e st.dy and were over 15 years of age. Dat. collect.on was done wit. pre-est.blished forms. t.e following dat. were collect.d: blood pressure before each session, at.30, 60, 120, 180 and 240 minut.s of each session;socio-demographic dat.;dialysis dat.;clinical, paraclinical, t.erapeut.c dat. and nursing int.rvent.ons. Result.: t.e mean age of t.e pat.ent. was 43.84 ± 12.10 years. Among 568 haemodialysis sessions recorded in 50 pat.ent., IDH was not.d in 12 haemodialysis sessions, represent.ng a prevalence of 2.11%. Fat.gue was found in 5 sessions wit. IDH episodes (41.66%) followed by yawning (25%), nausea ± vomit.ng (16.66%) and cramps (16.66%). As nursing int.rvent.ons, t.endelenburg posit.on and normal saline administ.at.on were performed in all IDH episodes. High blood pressure, int.r-dialyt.c weight.gain great.r t.an 3 kg, Uf/H > 10 ml/kg/H, anaemia and hypoalbuminaemia were associat.d wit. t.e occurrence of IDH. Conclusion: t.e prevalence of IDH according t. t.e EBPG definit.on is low. However, it.is an import.nt.cause of morbidit. and mort.lit., especially cardiovascular involvement. and t.e fact.rs associat.d wit. it. presence have been clearly ident.fied.

We included all 15-year old and above PlHIV wit. available CD4 count.and viral load. Result.: Out.of 248 PlHIV, 32 had kidney disease (KD), which means a hospit.l prevalence of 12.9%. t.e mean age was 51.22 ± 10 years (ext.emes of 36 and 77 years) wit. a sex rat.o (male/female) of 1.28. Renal signs were dominat.d by glomerular nephropat.y syndrome. It.was present.at.80%. t.bulo-int.rst.t.al nephropat.y syndrome and chronic uremic syndrome account.d for 6.25% and 3.1% of cases, respect.vely. Renal funct.on Impairment.was present.in 21 pat.ent. wit. 18 cases of acut. kidney injury (85.7%) and 3 cases of chronic renal failure (14.3%), including 2 in st.ge 5 of chronic kidney disease. Renal biopsy (RB) was indicat.d and performed in 20 (62.5%) pat.ent. wit. glomerular signs in 12 pat.ent. (60%). Glomerular lesions were dominat.d by focal and segment.l glomerulosclerosis (FSGS) in 6 cases, membraneous nephropat.y (MN) in 4 cases and minimal change disease (MCD) in 2 cases. t.bulo-int.rst.t.al and vascular lesions were present.in 45% and 12.5% of cases, respect.vely. In highly act.ve ant.ret.oviral t.erapy (HAARt., 12 (37.5%) pat.ent. had t.t.l remission, 9 (28.12%) had part.al remission.
