Prevalence and Factors Associated with Positivity of Antinuclear Antibodies (ANA) Patterns, Native Anti-DNA and Extractable Nuclear Antigens (ENA) Antibodies: Experience from a Laboratory in Dakar

Background: Diagnosis of autoimmune diseases (AID) is challenging, due to overlapping features with other non-immune disorders. Anti-nuclear antibodies (ANA) are sensitive screening tests but anti-deoxyribonucleic acid-antibody (anti-DNA), and anti-extractable nuclear antigens (anti-ENA) are specific for AIDs. We aimed to look at ANA patterns in our patients and correlated them with anti-ENA for proper interpretation and better patient management cost-effectively. Methods: A retrospective study was conducted over 1 year from January to December 2022 who were tested for ANA at biology medical laboratory of Pasteur Institute of Dakar. Anti-ENA and anti-DNA results were also analyzed for ANA-positive patients. Statistical analysis was performed using STATA 14.0, p Results: 216 patients were analyzed. Women predominated at 79.2% and mean age was 48 years [CI 95%, 46 - 50], with extremes of 10 and 89. Most represented age group was [41 - 60] with 38%. ANA was positive in 27 (12.5%) of patients, 59.2% of whom were strongly positive (titer of 1/1000, 1/3200 or 1/6400). The most common pattern was nuclear speckled, which was found in 77.8% of samples. Anti-ENA and anti-DNA positivity in ANA-positive patients was found respectively in 63% (17/27) and 1.4% (3/27) of the samples analyzed. Most commonly identified anti-ENA was anti-Sm 29.6%, anti-SSA 29.6%, anti-Ro-52 25.9%, anti-RNP 18.5% and anti-SSB 14.8% which was associated with speckled pattern. Association results indicated a significant relationship between both tests and between ANA titer in the anti-ENA- and ANA-positive patients (p 0.001). Conclusions: ANA, Anti-ENA and anti-DNA antibodies are essential for AIDS diagnosis. However, the testing repertoire should follow an algorithm comprising of clinical features, followed by ANA results with nuclear, mitotic, and cytoplasmic patterns, anti-ENA, and anti-DNA for a more meaningful, and cost-effective diagnostic approach.

Ambulatory Uretrocystoscopy in the Urology Andrology Department of Grand Mbour Hospital

Background: Urethrocystoscopy is a method of endoscopic exploration that allows direct visualization of the urethra and bladder for diagnosis or treatment of diseases of the lower urinary tract. Purpose: To determine the epidemiological profile of patients who underwent urethrocystoscopy and to present the indications and the results of this endoscopic examination. Patients and Methods: This is a retrospective, descriptive study of all patients who underwent outpatient urethrocystoscopy in the Urology and Andrology department of Grand Mbour hospital. The epidemiological data of the patients, the indications, the results of the endoscopic exploration and the additional procedures performed were entered and analyzed with Excel 2016. We performed descriptive statistics. Results: Of the 216 patients who underwent outpatient urethrocystoscopy, we counted 179 men (82.87%) and 37 women (17.13%). The sex ratio was 4.84. The mean age of the patients was 53.71 ± 18.76 years (age range 17 to 91 years). The main indications were lower urinary tract disorders (60.18%) and hematuria (28.70%). Endoscopic exploration revealed prostatic tumor in 69 patients (31.9%), 29 cases of bladder tumors (13.4%) and 19 cases of bilharzia cystitis (8.8%). The urethrocystoscopy was normal in 32 patients (14.8%). Conclusion: Urethrostoscopy is an endoscopic exploration examination that can be performed on an outpatient basis. Lower tract urinary disorders and hematuria were the main indications.

Phenotypic Detection of Enterobacterales Strains Susceptible of Producing OXA-48 Carbapenemase

Background: Nowadays, emergence of Carbapenemase-Producing Enterobacterales (CPE) throughout the world has become a public health problem, especially in countries with limited resources. In recent years, CPE of type OXA-48 (Ambler class D) have been identified in Dakar. The aim of this study was to evaluate the phenotypic detection of OXA-48 CPE using a temocillin disc (30 μg). Methodology: A retrospective study was carried out at Medical Biology Laboratory of Pasteur Institute in Dakar on Ertapenem-Resistant Enterobacterales (ERE) strains isolated from 2015 to 2017. These strains were then tested with a 30 μg temocillin disc. Any strain resistant to temocillin (inhibition diameter Results: Forty-one ERE isolated during the study period were tested, of which 34 (82.9%) were OXA-48 based on phenotypic detection using temocillin disc and confirmed by PCR (100%). OXA-48 CPE strains detected were composed of Klebsiella pneumoniae (n = 14;41.2%), Enterobacter cloacae (n = 8;23.5%), Escherichia coli (n = 7, 20.5%), Citrobacter freundii (n = 3;8.8%), Cronobacter sakazakii (n = 1;3%) and Morganella morganii (n = 1;3%). Conclusion: Temocillin resistance has a good positive predictive value for detecting OXA-48 CPE by phenotypic method, confirmed by PCR. Temocillin is therefore a good marker for detection of OXA-48 CPE except Hafnia alvei.

Influence of the Nature of the Incoming Sludge on the Performance of a Vertical Flow Reed Beds in Dakar-Senegal

This work investigates the influence of the type sludge on drainage, plant development, purification performances and biosolids quality. Drainage properties were measured through the frequency of clogging, the percentage of leachate recovered and the dryness of accumulated sludge. Plant development was measured through the density, the height and the stem diameter. Purification performance was evaluated from the reduction rate. Biosolids quality was measured after 3 months of maturation. The results show that the clogging frequencies were 9.5%;0% and 3.7%;the volume of leachate recovered was 42.2%;20.4% and 24.7% and, the dryness was 33.4%;61.1% and 52.4% for FS-ST, FS-STT and SS respectively. Plants densities were about, with densities 197.1, 171.3 and 178.3 plants/m2 in beds fed respectively with FS-ST, FS-STT and SS. Despite the high removal rates, the concentrations of pollutants in the leachates are above the Senegalese standard NS 05-061 for discharge into the environment. The biosolids are all mature with C/N and NH4+/NO3?ratios lower than 12 and 1 respectively. The biosolids are also rich in organic and mineral elements. The concentrations of Ascaris eggs are higher than the WHO recommendations. These biosolids should be stored for additional time or composted.

Long-Term Impact of Acute Retinoic Acid Supplementation at the Young Age on Testicular Architecture of Wistar Albino Rats

Introduction: Inappropriate and excess vitamin supplementation, particularly for vitamin A, is increasingly recognized as a public health problem in developed countries. On the other hand, blind supplementation of vitamin A, for children in developing countries is a subject of controversy in the literature. The crucial role of vitamin A in the process of spermatogenesis in adult rodents is well established, but only a few publications are consecrated to the long-term effect of vitamin A intake at a young age on testicular development and differentiation. Objectives: Our study aimed to evaluate the long-term effects of acute supplementation at an early age, in the post-natal period, on spermatogenesis and testicular trophicity at adult age. Material and Methods: Young Wistar Albinos rats of 22 days received an acute high dose of supplementation of vitamin A (retinyl palmitate). The control group, group 1, received only extra virgin olive oil, Group 2 a dose of 7000 IU/kg of retinyl palmitate, group 3, 14,000 IU/kg, and Group 4 a dose of 28,000 IU/kg. At 10 weeks of age, the testes’ testosterone levels were measured by ELISA. For histological assessment, sections were stained with Hematoxylin eosin, and the Johnsen score was used to evaluate spermatogenesis in the seminiferous tubules. Results: The average testicular weights of rats were significantly lower in group 4 (p < 0.05), and so was the testosterone level in the testis compared to the control group (p .01). Most of the seminiferous tubules were concerned by an arrest of spermatogenesis and the Johnsen score was decreased with a mean score of 5.96 ± 1.60 (p .001) in that Group. In Group 3, Johnsen’s score was significantly better than the one obtained with the control. Conclusion: We observed a negative effect in the long term with a high acute dose of supplementation of retinyl palmitate at a young age, on testicular development and differentiation. Giant cerebriform nevus cell nevus of the scalp is an extremely rare form of congenital melanocytic nevus. Giant cerebriform nevus of the scalp has a major psychosocial impact because of its unsightly appearance with fetid maceration. We report the case of a 35-year-old woman who had a painless, malodorous swelling of the cerebriform scalp measuring 20 × 17 cm in diameter with a wide base of insertion at the occipital level adhering to the deep planes. The excision associated with a skin plasty was carried out. Histology concluded that there was a giant cerebriform naevo-cellular nevus of the scalp.

Hydrogeochemical Characterization of Aquifer Systems and Surface Water/Groundwater Relations in the Lower Senegal River Valley

This study assesses the chemical quality of water resources in the Lower Senegal River valley, based on 35 samples collected in November 2022. Major ion concentrations in surface water and groundwater were analyzed using classical geochemical interpretation diagrams (Piper, GIBBS, etc.) and multivariate geostatistical analyses, including hierarchical cluster analysis (HCA) and principal component analysis (PCA). The results revealed three types of facies: Ca-Mg-HCO3-type facies, characteristic of poorly mineralized waters such as surface waters and groundwater from dune formations and the alluvial plain close to the hydraulic axis;Na-Cl type facies associated with well waters located in the alluvial plain that tap Inchirian or Nouakchottian shallow reservoirs and Maastrichtian deep borehole waters;and mixed Ca-Cl and Na-HCO3 type facies observed in certain floodplain and dune reservoirs. The results showed a strong correlation between sodium, chlorides, bromides, and electrical conductivity, indicating a significant contribution of these ions to groundwater mineralization. The various sources of water mineralization include mixing processes between surface water or rainwater, or calcite or dolomite dissolution processes (for weakly mineralized waters), basic exchanges or inverse basic exchanges between the aquifer and the water table (for moderately mineralized waters), and evaporation processes, halite dissolution, and paleosalinity during periods of marine transgression and regression (for highly mineralized waters). The study also highlighted the high vulnerability of the alluvial aquifer to pollution from intensive irrigated agriculture, as significant quantities of sulfates and nitrates were measured in some samples. These results also highlight the importance of water quality management in the Lower Senegal Valley, particularly as concerns the protection of the alluvial aquifer against pollution from irrigated agriculture.

Identification of Antioxidant Additives in Food Products Sold in Dakar Markets

The use of food additives in industrial production has the advantage of improving sensory properties, technological quality and extending the shelf life of foods. Among the most widely used additives are antioxidants, which prevent oxidation, browning and rancidity reactions in foods. However, the strong presence of these additives on the market is not without risks for human health, and should be controlled to guarantee food safety. Analysis of the risks associated with consumption of foods containing these additives requires, among other things, information on the frequency of use of these additives in various consumer products. The aim of this study is therefore to identify the antioxidants present in industrial food products distributed in Dakar. The methodology adopted consists of a qualitative analysis based on the identification of additives from food labels. Investigations were carried out in 9 stores, 4 superettes and 2 supermarkets located in different districts of Dakar. The results revealed the presence of 12 antioxidant additives, dominated by citric acid (53%) and ascorbic acid (29%). These studies have also highlighted the simultaneous use of several antioxidants in the same food product. Moreover, for some artificial antioxidants identified antioxydant such as BHA and BHT, health risks are associated with their consumption. The results of this study open up prospects for the development of information databases on food additives.

Regional Climate Models in the Simulation of the Drought of the 1970’s and 1980’s Years in Senegal (In West Africa)

West Africa was hit by an unprecedented drought in the 1970’s and 1980’s years, with dramatic consequences for surface and groundwater resources. In the context of climate change, there are many studies for the prediction of the increase in the occurrence of these droughts. To predict this situation in the Senegalese region, it is necessary to use regional climate models, which carrying out the study. This work deals with the interest to examine the capacity of the RCMs (regional climate models) in order to reproduce the deficit on the 1970’s year rainfall in Senegal. In this work, we used daily precipitation data from five (5) regional climate models to characterize the droughts in Senegal by using the SPI (Standardized Precipitation Index) on different time scales (3, 6, 12 and 24 months). For this purpose, the index was calculated over two distinct periods: 1951-1969 and 1970-1990. The results show that the period 1970-1990 was drier than the period 1951-1969. For the zonal average, the results show that the North of Senegal was more affected by this deficit rainfall than the South part. The analysis of the interannual variability of rainfall for some stations in Senegal shows that the drought did not start at the same time throughout the zone.

Electrochemical Analysis of Zirconiumin Aqueous and Organic Media

For the challenging nature of the zirconium environment analysis, this study consists to analyze the electrochemical behavior of Zirconium in both aqueous and organic media. To that end first the electrolytic media was selected on the basis of the Pourbaix potential-pH diagram, which provides informations on the predominance of Zr(IV) ion and Zr in aqueous media. In aqueous media, analyzes were first carried out in acidic media then in basic media. Studies have thus revealed that the acidic environment is not favourable for the electrochemical analysis of zirconium. Voltammograms obtained in an acidic environment show no zirconium detection signal;this is due to the strong presence of H+ ions in the solution. We have also observed in acidic media the phenomenon of passivation of the electrode surface. In aqueous alkaline media (pH = 13), we have drawn in reduction several Intensity-Potential curves by fixingsome technical parameterslike scanning speed, rotation speed of the electrode. The obtained voltammograms show cathodic waves, starting from -1.5 V/DHW and attributed to the reduction of Zr (IV) to Zr (0). The last phase of this study focused on the electrochemical analysis of zirconium in an organic media. In this media, several intensity-potential curves were plotted in reduction and in cyclic voltammetry with various parameters. Through several reduction analysis, the Zr (IV) was reduced to Zr (0) to the potential of -1.5 V/DHW. The electrochemical analysis of zirconium in organic media seems globally easier to achieve thanks to its large solvent window (i.e. dimethylformamide (DMF) solvent window > 6 V).
