Multi-aspect analysis of ureteral access sheath usage in retrograde intrarenal surgery: A RIRSearch group study

Objective:To evaluate the effect of ureteral access sheath(uAS)use and calibration change on stone-free rate and complications of retrograde intrarenal surgery(RIRS).Methods:Data from 568 patients undergoing RIRS for kidney or upper ureteral stones were retrospectively ***,patients were compared after 1:1 propensity score matching,according to uAS usage during RIRS(uAS used[+]87 and uAS non-used[−]87 patients).Then all uAS+patients(n=481)were subdivided according to uAS calibration:9.5-11.5 Fr,10-12 Fr,11-13 Fr,and 13-15 *** outcomes of the study were the success and complications of ***:Stone-free rate of uAS+patients(86.2%)was significantly higher than uAS−patients(70.1%)after propensity score matching(p=0.01).Stone-free rate increased with higher caliber uAS(9.5-11.5 Fr:66.7%;10-12 Fr:87.0%;11-13 Fr:90.6%;13-15 Fr:100%;ppostoperative complications of uAS+patients(11.5%)were significantly lower than uAS−patients(27.6%)(p=0.01).Complications(8.7%)with 9.5-11.5 Fr uAS was lower than thicker uAS(17.2%)but was not statistically significant(p=0.09).uAS usage was an independent factor predicting stone-free status or peri-and post-operative complications(odds ratio[OR]3.654,95%confidence interval[CI]1.314-10.162;OR 4.443,95%CI 1.350-14.552;OR 4.107,95%CI 1.366-12.344,respectively).Conclusion:use of uAS in RIRS may increase stone-free rates,which also increase with higher caliber *** usage may reduce complications;however,complications seemingly increase with higher uAS calibration.

circRNA3669 promotes goat endometrial epithelial cells proliferation via miR-26a/RCN2 to activate pI3K/AKT-mTOR and MApK pathways

The development of receptive endometrium(RE) from pre-receptive endometrium(pE) for successful embryo implantation is a complex dynamic process in which the morphology and physiological states of the endometrial epithelium undergo a series of significant changes, including cell proliferation and apoptosis. However, the molecular mechanisms are not yet fully understood. In this study, a higher circRNA3669 level was observed in pE than in RE of goats. Functional assays revealed that this overexpression promoted the proliferation of goat endometrial epithelial cells(GEECs) by activating the expression of genes related to the pI3K/AKT-mTOR and MApK pathways,thereby inhibiting apoptosis in vitro. Furthermore, circRNA3669 functioned as a competing endogenous RNA(ceRNA) to upregulate Reticulocalbin-2(RCN2) expression at the post-transcriptional level by interacting with and downregulating miR-26a in GEECs. In addition, RCN2, which is highly expressed in the pE of goats, was found to be regulated by β-estradiol(E2) and progesterone(p4). Our results demonstrated that RCN2 also affected the key proteins pI3K, AKT, mTOR, JNK, and p38 in the pI3K/AKT-mTOR and MApK pathways, thereby facilitating GEECs proliferation and suppressing their apoptosis in vitro. Collectively, we constructed a new circRNA3669-miR-26aRCN2 regulatory network in GEECs, which further provides strong evidence that circRNA could potentially play a crucial regulatory role in the development of RE in goats.

Investigation of Fracture Strength of Current post Materials (peek, Fiber, Cast Metal) in Different Ferrule Conditions

Aim: The fracture resistance of the post materials used nowadays significantly impacts the fracture type of the post material. The current study aimed to examine the impact of the fracture resistance of pEEK posts, whose use as a post material has not yet become widespread, on the fracture pattern that will occur. Materials and Methods: Sixty-six extracted upper central incisor human teeth were used in the study. Six groups of posts were produced from metal, fiber, and pEEK materials (n = 11). Afterward, the fracture resistance test was applied to the samples. Samples were classified according to fracture type as repairable or irreparable. The ANOVA and test were used in statistical analysis. post-hoc tests were carried out using Tukey’s and Tamhane’s T2 tests. On the other hand, pearson’s chi-square test and the Fisher-Freeman-Halton test were conducted for comparisons between categorical variables. The statistical significance level was accepted as p Results: According to the fracture resistance test results, the groups where metal personal posts were used displayed higher fracture resistance in comparison with all other groups. According to the results of the optical microscope examination, the incidence of repairable fractures was significantly higher in the pEEK post group with ferrule preparation (p 0.05). Conclusion: In the absence of the ferrule effect, fractures are catastrophic, regardless of the material used. The use of pEEK as a post material contributes to keeping fractures at a repairable level.

Study on the Impact of post-Mastectomy Radiotherapy on Immediate Breast Reconstruction and Complications: A Meta-Analysis

background and objective:Immediate breast reconstruction not only reduces the number of surgeries for patients after mastectomy but also decreases psychological and physical trauma,making it increasingly ***,there is currently no consensus on the integration of post-mastectomy radiotherapy(pMRT)with reconstruction *** review evaluates the impact of pMRT on complications following immediate breast reconstruction,providing guidance for clinical treatment ***:pubMed,Web of Science,Embase,and other databases were searched for studies published in the past 15 years on outcomes of implant-based breast reconstruction in the context of radiotherapy to identify articles for *** 5.4 software was used to analyze the risks of seroma,infection,wound dehiscence,flap necrosis,implant exposure,capsule contracture,and reconstruction ***:A total of 1l relevant studies were included,comprising 6323 cases of immediate breast *** was found that breasts receiving postoperative irradiation had a significantly increased risk of complications,with statistically significant differences in seroma(p=0.004),infection(pund dehiscence(p=0.04),implant exposure(ppsule contracture(puction failure(pp necrosis(p=0.88).Conclusion:The results indicate that postoperative radiotherapy significantly increases the risk of complications for patients undergoing immediate implant-based *** measures may be taken in advance with the assistance of healthcare providers if necessary.

Effect of Mushroom Extract Hericium erinaceus on Spatial Memory and Morphology of Neurons in the CA1 and CA3 Regions of the Hippocampus in Ovariectomized Rats

Various studies indicate that low levels of estradiol negatively impact cognitive abilities. Extracts from the fungus Hericium erinaceus (HE) contain bioactive components that promote the proper functioning of the nervous system and potential effects on protection against neurodegenerative diseases, including dementia and motor dysfunctions. The objective was to evaluate the effects of the administration of the HE mushroom extract on visuospatial memory and morphology of neurons in the CA1 and CA3 regions of the hippocampus in ovariectomized rats. 40 young Wistar rats weighing 90 ± 10 g BW were used, which were distributed into four groups of 10 animals;Control Group, non-ovariectomized and untreated rats;Group E2, ovariectomized rats treated with estradiol (2 μg/kg/body weight);Group HE, ovariectomized rats treated with the extract of the fungus Hericium erinaceus (0.5 mg/kg body weight) and Group Ovx/ST, ovariectomized rats, without treatment. The animals were tested in the barnes and Open Field maze, then they were sacrificed, and their brains were obtained to perform a histological analysis of neuronal morphology in the CA1 and CA3 areas of the hippocampus. The most outstanding results showed that the Ovx/ST group recorded the longest time to arrive at the escape box and stay in the barnes maze. A correlation was observed between neuronal damage and function;in the groups that did not present satisfactory performance in the maze tests, morphological alterations were identified such as the presence of some neuronal somata with degeneration characteristics such as pyknosis, nuclear basophilia and shrinkage of the cells. Its soma, as well as a decrease in the nuclear area of CA1 and CA3 neurons. It is concluded that the fungus Hericium erinaceus exerted a neuroprotective effect on the neuronal bodies of the hippocampus, associated with better performance in the visuospatial recognition memory test.

目的探讨中老年人血液常见氨基酸水平与早期轻度脑白质疏松症(leukoaraiosis,LA)的关系及其对轻度LA的筛查诊断价值。方法本研究纳入2020年8月至2021年11月因头晕或头痛等主诉于锦州医科大学附属第一医院神经内科住院的患者,根据头颅磁共振检查结果入组21例非LA患者和31例轻度LA患者,使用串联质谱法采集患者23种血液氨基酸代谢物水平,比较两组患者血液氨基酸水平差异,使用Logistic回归分析模型和受试者工作特征(receiver operating characteristic,ROC)曲线研究轻度LA的独立危险因素以及血液氨基酸水平对轻度LA的筛查诊断价值。结果年龄、冠状动脉粥样硬化性心脏病史、酪氨酸和缬氨酸水平在两组间差异具有统计学意义(ppp<0.05)。结论血液中常见氨基酸水平变化与早期轻度LA具有相关性,年龄增长和缬氨酸水平升高是轻度LA的独立危险因素,两者在筛查轻度LA方面具有一定的临床价值。
