background In Viet Nam, tuberculosis (TB) represents a devastating life-event with an exorbitant price tag, partly due to lost income from daily directly observed therapy in public sector care. Thus, persons with TB may seek care in the private sector for its flexibility, convenience, and privacy. Our study aimed to measure income changes, costs and catastrophic cost incurrence among TB-affected households in the public and private *** Between October 2020 and March 2022, we conducted 110 longitudinal patient cost interviews, among 50 patients privately treated for TB and 60 TB patients treated by the National TB Program (NTP) in ha Noi, hai Phong and Ho Chi Minh City, Viet Nam. Using a local adaptation of the WHO TB patient cost survey tool, participants were interviewed during the intensive phase, continuation phase and post-treatment. We compared income levels, direct and indirect treatment costs, catastrophic costs using Wilcoxon rank-sum and chi-squared tests and associated risk factors between the two cohorts using multivariate *** The pre-treatment median monthly household income was significantly higher in the private sector versus NTP cohort (USD 868 vs USD 578;P=0.010). However, private sector treatment was also significantly costlier (USD 2075 vs USD 1313;P=0.005), driven by direct medical costs which were 4.6 times higher than costs reported by NTP participants (USD 754 vs USD 164;P<0.001). This resulted in no significant difference in catastrophic costs between the two cohorts (Private: 55% vs NTP: 52%;P=0.675). Factors associated with catastrophic cost included being a single-person household [adjusted odds ratio [(aOR=13.71; 95% confidence interval (CI): 1.36–138.14;P=0.026], unemployment during treatment (aOR=10.86; 95%CI: 2.64–44.60;P<0.001) and experiencing TB-related stigma (aOR=37.90; 95%CI: 1.72–831.73;P=0.021).Conclusions Persons with TB in Viet Nam face similarly high risk of catastrophic costs whether treated in the

Randomized controlled trial of minimally invasive surgery using acellular dermal matrix for complex anorectal fistula

AIM: To compare the efficacy and safety of acellular dermal matrix (ADM) bioprosthetic material and endorectal advancement flap (ERAF) in treatment of complex anorectal fistula. METHODS: Ninety consecutive patients with complex anorectal fistulae admitted to Anorectal Surgical Department of First Affi liated Hospital, Xinjiang Medical University from March 2008 to July 2009, were enrolled in this study. Complex anorectal fistula was diagnosed following its clinical, radiographic, or endoscopic diagnostic criteria. Under spinal anesthesia, patients underwent identification and irrigation of the fistula tracts using hydrogen peroxide. ADM was securely sutured at the secondary opening to the primary opening using absorbable suture. Outcomes of ADM and ERAF closure werecompared in terms of success rate, fecal incontinence rate, anorectal deformity rate, postoperative pain time, closure time and life quality score. Success was defined as closure of all external openings, absence of drainage without further intervention, and absence of abscess formation. Follow-up examination was performed 2 d, 2, 4, 6, 12 wk, and 5 mo after surgery, respectively. RESULTS: No patient was lost to follow-up. The overall success rate was 82.22% (37/45) 5.7 mo after surgery. ADM dislodgement occured in 5 patients (11.11%), abscess formation was found in 1 patient, and fistula recurred in 2 patients. Of the 13 patients with recurrent fistula using ERAF, 5 (11.11%) received surgical drainage because of abscess formation. The success rate, postoperative pain time and closure time of ADM were significantly higher than those of ERAF (P < 0.05). However, no difference was observed in fecal incontinence rate and anorectal deformity rate after treatment with ADM and ERAF. CONCLUSION: Closure of fistula tract opening with ADM is an effective procedure for complex anorectal fistula. ADM should be considered a first line treatment for patients with complex anorectal fistula.

A case of congenital obstruction of magendie's foramen:embryologic analysis and treatment

background: Fourth ventricle isolated Magendie's foramen primitive obstruction is rare. The etiology and the pathophysiology of the constitution of this obstruction in congenital cases remain elusive. We report a case of congenital obstruction of Magendie's foramen in an adult patient and discuss the embryogenic mechanism and our management of this rare *** presentation: A 20 years old female patient without any medical history was referred for headaches and vomiting. Emergency CT scan revealed major hydrocephalus subsequently she underwent a ventriculoperitoneal shunt as initial *** diagnosis of fourth ventricle obstruction was made 5 months later when the patient came back complaining of headaches, cerebellar signs and cystic dilatation of the fourth ventricle on CT scan and MRI. Fourth ventricle Magendie's foraminoplasty via classic posterior fossa surgery brings complete ***: Magendie's foramen obstruction is rare. The embryological development of the posterior fossa and its content could explain the primitive obstruction which can be managed by classic surgery in case of unavaibility of endoscopy.

目的:对比无托槽隐形矫治与固定矫治错[牙合]畸形患者的治疗效果及对龈沟液炎症因子水平、牙齿咬合功能的影响。方法:回顾性收集我院2021年5月至2023年5月收治的92例错[牙合]畸形患者的病例资料,按手术方案不同分为两组(n=46),观察组接受无托槽隐形矫治,对照组接受固定矫治。对比两组患者矫治6m后矫治效果;矫治前、矫治3 m、6 m后牙周健康指标,包括牙龈指数(Gingival index,GI)、龈沟出血指数(Sulcus bleeding index,SBI)、菌斑指数(Plaqueindex,PLI)、牙周探诊深度(Periodontaldepth,PD)以及并发症发生率,并采用Tee Tester咬合力检测仪检测矫治前、矫治3 m、6 m后牙齿咬合功能([牙合]力总值、[牙合]力中心点最大位移、[牙合]力不对称指数),采用酶联免疫吸附试验检测矫治前、矫治3 m、6 m后龈沟液炎症因子,如白细胞介素-6(Interleukin-6,IL-6)、可溶性细胞间粘附分子-1(Soluble intercellular adhesion molecule-1,sICAM-1)、白细胞介素-1β(Interleukin-1β,IL-1β)水平。结果:观察组总有效率97.83%(45/46)相较于对照组78.26%(36/46)更高(P<0.05);与矫治前相比,各治疗组的GI、SBI、PLI、PD均明显降低(P<0.05),其中观察组更为显著(P<0.05);与矫治前相比,各治疗组的[牙合]力总值均明显升高,[牙合]力中心点最大位移、[牙合]力不对称指数均明显降低,其中观察组更为显著(P<0.05);与矫治前相比,各治疗组的龈沟液IL-6、sICAM-1、IL-1β水平均明显升高,其中对照组更为显著(P<0.05);观察组并发症发生率4.35%(2/46)相较于对照组21.74%(10/46)更低(P<0.05)。结论:经无托槽隐形矫治错[牙合]畸形患者整体矫治效果优于固定矫治。

新型冠状病毒Beta变异株S1重组蛋白的制备及免疫效果评价

根据CHO细胞密码子偏好性进行SARS-CoV-2 Beta变异株S1基因的优化与合成,并将其克隆至pSN载体,构建重组表达质粒pSN-Beta-S1,转染CHO细胞,获得重组蛋白Beta-S1,通过SDS-PAGE和Western blot方法鉴定重组蛋白Beta-S1的表达,对培养液上清... 详细信息

根据CHO细胞密码子偏好性进行SARS-CoV-2 Beta变异株S1基因的优化与合成,并将其克隆至pSN载体,构建重组表达质粒pSN-Beta-S1,转染CHO细胞,获得重组蛋白Beta-S1,通过SDS-PAGE和Western blot方法鉴定重组蛋白Beta-S1的表达,对培养液上清进行亲和层析纯化。将纯化后的重组蛋白Beta-S1联合氢氧化铝佐剂免疫baLB/c小鼠,检测血清中特异性IgG抗体及其亚型,评价针对SARS-CoV-2不同变异株的交叉中和抗体活性。结果显示,成功构建pSN-Beta-S1重组质粒,经CHO细胞表达系统表达了重组蛋白Beta-S1,重组蛋白条带单一,大小约为120 kDa,纯度可达85%以上,可与SARS-CoV-2 RBD蛋白多克隆抗体和Strep标签单克隆抗体特异性结合。S1重组蛋白对baLB/c小鼠具有良好的免疫原性,第3次免疫后14 d,针对RBD和S1蛋白的特异性IgG抗体效价平均可达1∶66260和1∶133120,抗体亚型偏向于IgG1,针对SARS-CoV-2野生株,Beta、Delta、Omicron ba1和Omicron ba2变异株的中和抗体分别为1∶629、1∶1720、1∶374、1∶77和1∶101。本研究利用CHO细胞表达系统成功制备了SARS-CoV-2 Beta变异株S1重组蛋白,免疫baLB/c小鼠后获得了良好的免疫原性和针对不同变异株的交叉中和抗体活性,为广谱SARS-CoV-2疫苗的研究提供了一定的借鉴意义。

We study the large-time dynamics of Cucker-Smale(C-S)flocking particles interacting with nonNewtonian incompressible *** of particles and fluids were modeled using the kinetic Cucker-Smale equation for particles and non-Newtonian Navier-Stokes system for fluids,respectively and these two systems are coupled via the drag force,which is the main flocking(alignment)mechanism between particles and *** present a global existence theory for weak solutions to the coupled Cucker-Smale-Navier-Stokes system with shear *** also use a Lyapunov functional approach to show that sufficiently regular solutions approach flocking states exponentially fast in time.
