The value of toxicological analysis in acute poisoning patients with uncertain exposure histories:a retrospective and descriptive study from an institute of poisoning

baCKGROUnD:In clinical practice,some patients might not be able or unwilling to provide a thorough history of medication and poison *** aim of this study was to use toxicological analysis to examine the clinical characteristics of patients with acute poisoning whose exposure history was uncertain from a toxicological analysis ***:This was a retrospective and descriptive study from an institute of *** registration information and test reports spanning the period from April 1,2020 to March 31,2022,were *** with uncertain exposure histories and who underwent toxicological analysis were *** manifestations and categories of toxics were ***:Among the 195 patients with positive toxicological analysis results,the main causes of uncertain exposure history was disturbance of consciousness(62.6%),unawareness(23.6%)and unwillingness or lack of cooperation(13.8%).The predominant clinical manifestations were disturbed consciousness(62.6%),followed by vomiting and nausea(14.4%)and liver function abnormalities(8.7%).A comparison of clinical manifestations between patients with positive and negative(n=99)toxicological analyses results revealed significantly different proportions of disturbances in consciousness(63%vs.21%),dizziness(1.5%vs.5.1%),multi-organ failure(1.5%vs.7.1%),and local pain(0 vs 4%).The main categories of substances involved were psychiatric medications(23.1%),sedatives(20.5%),insecticides(13.8%),and herbicides(12.8%).COnCLUSIOn:The clinical manifestations of acute poisoning in patients with an uncertain exposure history are diverse and nonspecific,and toxicological analysis plays a pivotal role in the diagnosis and differential diagnosis of such patients.

The Role of Renewable Protocatechol Acid in Epoxy Coating Modification: Significantly Improved Antibacterial and Adhesive Properties

It is of great significance to design epoxy coatings with superior antibacterial properties and high adhesive properties, as well as excellent processing, superior durability, and high transparency. However, it is still a challenge because of the common complex design and synthesis. Herein, the bio-based monomer protocatechuic acid(PCA) was used as raw material, the catechol structure with high bonding and antibacterial properties was introduced into the flexible alkane segment of ethylene glycol diglycidyl ether(EGDE) through an efficient, and green method, and it was cured with isophorone diamine(IPDA) to prepare corresponding thermosets. The cured resins exhibited excellent allaround qualities, particularly in bonding and antibacterial. When 30% PCA was added to pure epoxy resin, the adhesion between substrate and coating increased from 4.40 MPa to 13.60 MPa and the antibacterial rate of coating against E. coli and S. aureus could approach 100%. All of this is due to the fact that the catechol structure present in PCA has the ability to interact with various substrates and alter the permeability of bacterial cell membranes. The architecture of this method offers a fresh approach to dealing with the issues of challenging raw material selection and complex synthesis techniques.

Investigation of Fracture Strength of Current Post Materials (Peek, Fiber, Cast Metal) in Different Ferrule Conditions

Aim: The fracture resistance of the post materials used nowadays significantly impacts the fracture type of the post material. The current study aimed to examine the impact of the fracture resistance of PEEK posts, whose use as a post material has not yet become widespread, on the fracture pattern that will occur. Materials and Methods: Sixty-six extracted upper central incisor human teeth were used in the study. Six groups of posts were produced from metal, fiber, and PEEK materials (n = 11). Afterward, the fracture resistance test was applied to the samples. Samples were classified according to fracture type as repairable or irreparable. The AnOVA and test were used in statistical analysis. Post-hoc tests were carried out using Tukey’s and Tamhane’s T2 tests. On the other hand, Pearson’s chi-square test and the Fisher-Freeman-Halton test were conducted for comparisons between categorical variables. The statistical significance level was accepted as p Results: According to the fracture resistance test results, the groups where metal personal posts were used displayed higher fracture resistance in comparison with all other groups. According to the results of the optical microscope examination, the incidence of repairable fractures was significantly higher in the PEEK post group with ferrule preparation (p 0.05). Conclusion: In the absence of the ferrule effect, fractures are catastrophic, regardless of the material used. The use of PEEK as a post material contributes to keeping fractures at a repairable level.

Spatial Morphology Evolution Characteristics Analysis of the Resident Population Distribution in Henan, China

The population spatial distribution pattern and its evolving pattern play an important role in regional allocation of social resources and production factors, formulation of regional development plans, construction of a better life society, and promotion of regional economic development. based on the resident population statistics data of Henan province from 2006 to 2021, with county as the basic study unit, the paper studies the spatial morphology characteristics and its evolution patterns of resident population distribution, by using spatial analysis methods such as population distribution center, standard deviation ellipse, and spatial auto correlation analysis. The results show that: the resident population spatial distribution shows unbalanced state, the population agglomeration areas mainly distribute in the northeast part and north part, where the resident population growth rate is significantly higher than other regions, over time, this trend is gradually becoming significant. The resident population distribution has a trend of centripetal concentration, with the degree and trend of centripetal gradually strengthening. The resident population distribution has obvious directional characteristics, but the significance is not high, the weighted resident population average center is approximately located at (4.13740˚n, 113.8935˚E), and the azimuth of the distribution axis is approximately 11.19˚. The population distribution has obvious agglomeration characteristics, with the built-up areas of Zhengzhou and Luoyang as their centers, where have a significant siphon effect on the surrounding population. The southern and southwestern regions in the province form a relatively stable belt area of Low-Low agglomeration areas.

Multi-aspect analysis of ureteral access sheath usage in retrograde intrarenal surgery: A RIRSearch group study

Objective:To evaluate the effect of ureteral access sheath(UAS)use and calibration change on stone-free rate and complications of retrograde intrarenal surgery(RIRS).Methods:Data from 568 patients undergoing RIRS for kidney or upper ureteral stones were retrospectively ***,patients were compared after 1:1 propensity score matching,according to UAS usage during RIRS(UAS used[+]87 and UAS non-used[−]87 patients).Then all UAS+patients(n=481)were subdivided according to UAS calibration:9.5-11.5 Fr,10-12 Fr,11-13 Fr,and 13-15 *** outcomes of the study were the success and complications of ***:Stone-free rate of UAS+patients(86.2%)was significantly higher than UAS−patients(70.1%)after propensity score matching(p=0.01).Stone-free rate increased with higher caliber UAS(9.5-11.5 Fr:66.7%;10-12 Fr:87.0%;11-13 Fr:90.6%;13-15 Fr:100%;pns of UAS+patients(11.5%)were significantly lower than UAS−patients(27.6%)(p=0.01).Complications(8.7%)with 9.5-11.5 Fr UAS was lower than thicker UAS(17.2%)but was not statistically significant(p=0.09).UAS usage was an independent factor predicting stone-free status or peri-and post-operative complications(odds ratio[OR]3.654,95%confidence interval[CI]1.314-10.162;OR 4.443,95%CI 1.350-14.552;OR 4.107,95%CI 1.366-12.344,respectively).Conclusion:Use of UAS in RIRS may increase stone-free rates,which also increase with higher caliber *** usage may reduce complications;however,complications seemingly increase with higher UAS calibration.

baCKGROUnD Idiopathic omental infarction(IOI)is challenging to diagnose due to its low incidence and vague *** differential diagnosis also poses difficulties because it can mimic many intra-abdominal organ *** hypercoagulability and thrombosis are among the causes of omental infarction,venous thromboembolism scanning is rarely performed as an etiological *** SUMMARY The medical records of the 5 cases,who had the diagnosis of IOI by computed tomography,were *** majority of the patients were male(n=4,80%)and the mean age was 31 years(range:21-38).The patients had no previous abdominal surgery or a history of any chronic *** main complaint of all patients was persistent abdominal *** infarction was detected in all patients with contrast-enhanced computed *** treatment was initially preferred in all patients,but it failed in 1 patient(20%).After discharge,all patients were referred to the hematology department for thrombophilia *** 1 patient applied for thrombophilia screening and was homozygous for methylenetetrahydrofolate reductase(A1298C mutation)and heterozygous for a factor V Leiden *** IOI should be considered in the differential diagnosis in patients presenting with progressive and/or persistent right side abdominal *** risk factors such as hypercoagulability in patients with IOI is also important in preventing future conditions related to venous thromboembolism.

Giant cerebriform nevus cell nevus of the scalp is an extremely rare form of congenital melanocytic nevus. Giant cerebriform nevus of the scalp has a major psychosocial impact because of its unsightly appearance with fetid maceration. We report the case of a 35-year-old woman who had a painless, malodorous swelling of the cerebriform scalp measuring 20 × 17 cm in diameter with a wide base of insertion at the occipital level adhering to the deep planes. The excision associated with a skin plasty was carried out. Histology concluded that there was a giant cerebriform naevo-cellular nevus of the scalp.

Management of Penetrating Cranioencephalic Trauma Caused by Sharp Metal Objects—Therapeutic and Evolutionary Aspects: 12 Cases at the Renaissance University Hospital in n’Djamena

Introduction: Cranioencephalic trauma caused by bladed weapons is rare, and that caused by sharp objects is exceptional. The aim of our study was to describe the clinical, therapeutic and evolutionary aspects. Materials and method: This was a descriptive and analytical study over a 48-month period at CHU la Renaissance from January 1, 2018 to December 31, 2021, concerning patients admitted for penetrating cranioencephalic trauma by pointed object. Results: Twelve cases, all male, of penetrating cranioencephalic sharp-force trauma were identified. The mean age was 34 ± 7 years, with extremes of 11 and 60 years. Farmers and herders accounted for 31% and 25% of cases respectively. The average admission time was 47 hours. Brawls were the circumstances of occurrence in 81.2% of cases. Knives (33%), arrows (25%) and iron bars (16.6%) were the objects used. Altered consciousness was present in 43.8% of cases, and focal deficit in 50%. Scannographic lesions were fracture and/or embarrhment (12 cases), intra-parenchymal haematomas (6 cases) and presence of object in place (4 cases). Surgery was performed in 11 patients. Postoperative outcome was favorable in 9 patients. After 12 months, 2 patients were declared unfit. Conclusion: Penetrating head injuries caused by sharp objects are common in Chad. Urgent surgery can prevent disabling after-effects.
