The direct interfacing of photonic crystal fiber to a metallic nanoantenna has widespread application in nanoscale imaging, optical lithography, nanoscale lasers, quantum communication, in vivo sensing, and medical *** report on the fabrication of a needle-shaped plasmonic nanoantenna on the end facet of a photonic crystal fiber using electron-beam-induced evaporation of platinum. We demonstrate the coupling of light from the fiber waveguide mode to the subwavelength nanoantenna plasmonic mode focusing down to the apex of the plasmonic needle using a polarization-resolved far-field side-scatter imaging technique. Our work provides an important step toward widespread application of optical fibers in nearfield spectroscopic techniques such as tip-enhanced Raman and fluorescence microscopy, single-photon excitation and quantum sensors, nanoscale optical lithography, and lab-on-fiber devices.

Two-dimensional materials are generating great interest due to their unique electrical and optical *** particular, transition metal dichalcogenides such as molybdenum disulfide(MoS_2) are attractive materials due to the existence of a direct band gap in the monolayer limit that can be used to enhance nonlinear optical phenomena, such as Raman spectroscopy. Here, we have investigated four-wave mixing processes in bulk MoS_2 using a multiplex coherent anti-Stokes Raman spectroscopy setup. The observed four-wave mixing signal has a resonance at approximately 680 nm, corresponding to the energy of the A excitonic transition of MoS_2. This resonance can be attributed to the increased third-order nonlinear susceptibility at wavelengths near the excitonic transition. This phenomenon could open the path to using MoS_2 as a substrate for enhancing four-wave mixing processes such as coherent anti-Stokes Raman spectroscopy.

Bladder cancer(BC)is one of the most frequently diagnosed types of urinary *** advances in treatment methods,no specific biomarkers are currently in *** and untargeted profiling of metabolites and elements of human blood serum from 100 BC patients and the same number of normal controls(NCs),with external validation,was attempted using three analytical methods,i.e.,nuclear magnetic resonance,gold and silver-109 nanoparticle-based laser desorption/ionization mass spectrometry(LDI-MS),and inductively coupled plasma optical emission spectrometry(ICP-OES).All results were subjected to multivariate statistical *** potential serum biomarkers of BC,namely,isobutyrate,pyroglutamate,choline,and acetate,were quantified with proton nuclear magnetic resonance,which had excellent predictive ability as judged by the area under the curve(AUC)value of *** elements,Li and Fe,were also found to distinguish between cancer and control samples,as judged from ICP-OES data and AUC of 0.807(in validation set).Twenty-five putatively identified compounds,mostly related to glycans and lipids,differentiated BC from NCs,as detected using *** serum metabolites were found to discriminate between tumor grades and nine metabolites between tumor *** results from three different analytical platforms demonstrate that the identified distinct serum metabolites and metal elements have potential to be used for noninvasive detection,staging,and grading of BC.

AIM To examine the practice pattern in Kaiser Permanente Southern California(KPSC), i.e., gastroenterology(GI)/surgery referrals and endoscopic ultrasound(EUS), for pancreatic cystic neoplasms(PCNs) after the regionwide dissemination of the PCN management *** Retrospective review was performed; patients with PCN diagnosis given between April 2012 and April 2015(18 mo before and after the publication of the algorithm) in KPSC(integrated health system with 15 hospitals and 202 medical offices in Southern California) were ***2558(1157 pre-and 1401 post-algorithm) received a new diagnosis of PCN in the study period. There was no difference in the mean cyst size(pre-19.1 mm vs post-18.5 mm, P = 0.119). A smaller percentage of PCNs resulted in EUS after the implementation of the algorithm(pre-45.5% vs post-34.8%, P < 0.001). A smaller proportion of patients were referred for GI(pre-65.2% vs post-53.3%, P < 0.001) and surgery consultations(pre-24.8% vs post-16%, P < 0.001) for PCN after the implementation. There was no significant change in operations for PCNs. Cost of diagnostic care was reduced after the implementation by 24%, 18%, and 36% for EUS, GI, and surgery consultations, respectively, with total cost saving of 24%.CONCLUSION In the current healthcare climate, there is increased need to optimize resource utilization. Dissemination of an algorithm for PCN management in an integrated health system resulted in fewer EUS and GI/surgery referrals, likely by aiding the physicians ordering imaging studies in the decision making for the management of PCNs. This translated to cost saving of 24%, 18%, and 36% for EUS, GI, and surgical consultations, respectively, with total diagnostic cost saving of 24%.

BACKGROUND Hepatitis B virus(HBV)nucleos(t)ide analog(NA)therapy reduces liver disease but requires prolonged therapy to achieve hepatitis B surface antigen(HBsAg)*** is limited North American real-world data using non-invasive tools for fibrosis assessment and few have compared 1st generation NA or lamivudine(LAM)to tenofovir disoproxil fumarate(TDF).AIM To assess impact of NA on virological response and fibrosis regression using liver stiffness measurement(LSM)(i.e.,FibroScan®).METHODS Retrospective,observational cohort study from the Canadian HBV *** collected included demographics,NA,HBV DNA,alanine aminotransferase(ALT),and *** were HBV monoinfected patients,treatment naïve,and received 1 NA with minimum 1 year *** In 465(median 49 years,37%female,35%hepatitis B e antigen+at baseline,84%Asian,6%White,and 9%Black).Percentage of 64(n=299)received TDF and 166 were LAM-treated with similar median duration of 3.9 and 3.7 years,*** mean baseline LSM was 11.2 kPa(TDF)vs 8.3 kPa(LAM)(P=0.003).At 5-year follow-up,the mean LSM was 7.0 kPa in TDF vs 6.7 kPa in LAM(P=0.83).There was a significant difference in fibrosis regression between groups(i.e.,mean-4.2 kPa change in TDF and-1.6 kPa in LAM,P<0.05).The last available data on treatment showed that all had normal ALT,but more TDF patients were virologically suppressed(<10 IU/mL)(n=170/190,89%)vs LAM-treated(n=35/58,60%)(P<0.05).None cleared *** In this real-world North American study,approximately 5 years of NA achieves liver fibrosis regression rarely leads to HBsAg loss.

Screening gene function in vivo is a powerful approach to discover novel drug targets. We present high-throughput screening （HTS） data for 3 762 distinct global gene knockout （ko） mouse lines with viable adult homozygous mice generated using either gene-trap or homologous recombination technologies. Bone mass was determined from DEXA scans of male and female mice at 14 weeks of age and by microCT analyses of bones from male mice at 16 weeks of age. Wild-type （WT） cagemates/littermates were examined for each gene ko. Lethality was observed in an additional 850 ko lines. Since primary HTS are susceptible to false positive findings, additional cohorts of mice from ko lines with intriguing HTS bone data were examined. Aging, ovariectomy, histomorphometry and bone strength studies were performed and possible non-skeletal phenotypes were explored. Together, these screens identified multiple genes affecting bone mass： 23 previously reported genes （Calcr, Cebpb, Crtap, Dcstamp, Dkkl, Duoxa2, Enppl, Fgf23, Kissl/Kisslr, Kl （Klotho）, Lrp5, Mstn, Neol, Npr2, Ostml, Postn, Sfrp4, S1c30a5, Sic39a13, Sost, Sumf1, Src, Wnt10b）, five novel genes extensively characterized （Cldn18, Fam20c, Lrrkl, Sgpll, Wnt16）, five novel genes with preliminary characterization （Agpat2, RassfS, Slc10a7, Stc26a7, Slc30a10） and three novel undisclosed genes coding for potential osteoporosis drug targets.

In a previous transcriptomic study of human bone marrow stromal cells(BMSCs,also known as bone marrow-derived^mesenchymal stem cells#,)/SFRP2 was highly over-represented in a subset of multipotent BMSCs(skeletal stem cells,SSCs),which recreate a bone/marrow organ in an in vivo ectopic bone formation *** modulate WNT signaling,which is essential to maintain skeletal homeostasis,but the specific role of SFRP2 in BMSCs/SSCs is ***,we evaluated Sfrp2 deficiency on BMSC/SSC function in models of skeletal organogenesis and *** skeleton of Sfrp2-def\c\ent(ko)mice is overtly normal;but their BMSCs/SSCs exhibit reduced colony-forming efficiency,reflecting low SSC self-renewal/***2 ko BMSCs/SSCs formed less trabecular bone than those from WT littermates in the ectopic bone formation ***,regeneration of a cortical drilled hole defect was dramatically impaired in Sfrp2 ko ***2-deficient BMSCs/SSCs exhibited poor in vitro osteogenic differentiation as measured by Runx2 and Osterix expression and calcium ***,activation of the Wnt co-receptor;Lrp6,and expression of Wnt target genes,Axin2,C-myc and Cyclin Dl,were reduced in Sfrp2-deficient BMSCs/*** of recombinant Sfrp2 restored most of these activities,suggesting that Sfrp2 acts as a Wnt *** demonstrate that Sfrp2 plays a role in self-renewal of SSCs and in the recruitment and differentiation of adult SSCs during bone ***2 is also a useful marker of BMSC/SSC multipotency,and a factor to potentially improve the quality of ex vivo expanded BMSC/SSC products.

In the ChineseAItai,on the northern side of the Erqis fault,the-10-m-wide Qiemuerqieke gabbro is composed almost entirely of hornblende and *** relative crystallization sequence is olivine,hornblende,plagioclase,and it shows a narrow compositional range in SiO (48.7-50.2 wt%),MgO(6.33-8.54 wt%),FeO(5.27-6.46 wt%),NaO(3.06-3.71 wt%),and ko(0.26-0.37 wt%).These contents result in a high Mg# value(68-72)and a low ko/NaO ratio of～*** has(Sr/Sr) ratios of 0.70339-0.70350,ε(t)values of 4.8-6.0,and zirconε(t) of 11.4-15.8;these values demonstrate a mantle-derived source,and a whole-rockδO of～6.7‰suggests a mantle wedge *** presence of magmatic hornblende suggests a relatively high water fugacity,and the crystallization temperature(715-826℃)calculated using Ti-in-zircon thermometry is considerably lower than that of a normal mafic melt but consistent with an origin from a water-bearing magma. The gabbro has a secondary ion mass spectrometry zircon U-Pb age of 276.0±2.1 Ma,which is coeval with the 275 Ma mantle plume in the northernTarim craton,but the Qiemuerqieke petrological and geochemical data do not indicate an abnormally high mantle temperature or a deep mantle signature,which would commonly characterize a mantle plume *** results integrated with published data support a model of juvenile crustal growth by a subduction-related process.
