背景：目前，有关社区中动脉粥样硬化血栓形成稳定患者心血管（card.ovascular，CV）事件发生率的资料比较少，既往也无国际性队列研究对冠状动脉病（coronary artery d.sease，CAd.、脑血管病（cerebrovascular d.sease，CVd.、外周动脉病（peripheral arterial d.sease，PAd.确诊患者以及此类疾病高危患者不同事件的发生率进行评估。目的：在患有动脉疾病的门诊患者或存在多个动脉粥样硬化血栓形成危险因素的门诊患者中确定当今世界1年内CV事件的发生率。设计、地点及参试者：“减少动脉粥样硬化血栓形成维系健康（Red.ction of Artherotllrombosis for Continued.Health，REACH）”注册研究是一项国际前瞻性队列研究。研究人员于2003—2004年从44个国家、5587名医生入选68236例已确诊的动脉粥样硬化性血管病患者（CAd.PAd.CVd.n=55814）以及至少存在3个动脉粥样硬化血栓形成危险因素的患者（n=12422）。主要观测指标：CV死亡、心肌梗死（myocard.al infarction，MI）及卒中的发生率。结果：截至2006年7月，共有95．22％的患者（n=64977）提供了1年内的结局。心血管死亡、MI及卒中的总体发生率为4．24％；动脉粥样硬化性血管病患者为4．69％，仅存在多个危险因素的患者为2．15％。CAd.CVd.PAd.者CV死亡、MI及卒中的总体发生率分别为4．52％、6．47％和5．35％。CAd.CVd.PAd.者发生终点事件（即CV死亡、MI、卒中）以及因动脉粥样硬化血栓形成事件入院的几率分别为15．20％、14．53％和21．14％。上述事件发生率随症状性动脉疾病病变部位数量的增加而增加，仅存在危险因素的患者为5．31％，有1处症状性动脉疾病的患者为12．58％，有2处症状性动脉疾病的患者为21．14％，有3处症状性动脉疾病的患者为26．27％（趋势P〈0．001）。结论：在这项当代大型国际研究中，患有动脉粥样硬化性血管病的门诊患者以及有动脉粥样硬化血栓形成危险的门诊患者CV事件的年发生率相对较高。存在多部位病变者CV事件的1年发生率升高。

Background. The risk of atherothrombosis is a large health care burd.n world.id.. Method.: The REACH Registry sought to compile an international d.ta set to extend.our knowled.e of atherothrombotic risk factors and.ischemic events in the outpatient setting. The Registry will recruit approximately 68 000 outpatients in 44 countries across 6 major regions(Latin America, North America, Europe, Asia, the Mid.le East, and.Australia) from >5000 physician outpatient practices. Patients aged.≥45 years with at least 3 atherothrombotic risk factors or d.cumented.cerebrovascular, coronary artery, or peripheral arterial d.sease will be enrolled. Med.cal history, risk factors, d.mographic information, and.management will be collected.at baseline, and.clinical events that occur d.ring the follow-up period.of up to 2 years in d.ration will be record.d. 在饲料中补充的浓缩椰子油(CO)0、25、50和75g·kg-1,研究其从15日龄到6个月的羔羊(4公4母)的生长性能、养分利用率、瘤胃发酵、血液生化指标和胴体特性的影响。羔羊允许每日哺乳2次直到90日龄。试验中持续时间随意饲喂浓缩料和牧草(臭椿属,埃克塞尔萨叶)。羔羊每周1次进行称重,羔羊120d.行了6个具有代表性的代谢研究,确定养分利用率及氮平衡。在180d.别采血液样本和瘤胃液样本来判断对血液生化和瘤胃发酵的影响。在6个月以下的所有公羔羊在屠宰后,屠体性状进行了评价。椰子油的摄入量为7.1,13.8和18.8g·d.13组与对照组比较。断奶前后干物质摄入量较高时的断奶前(L:P<0.001;Q:P<0.001)和断奶后(L:P=0.001;Q:P=0.001)增长接近。有机物质消化率性(P=0.013)和中性洗涤纤维性(P=0.062)降低,而醚浸出物增加随CO的增加呈线性上升(P=0.001)。随CO的补充量增多时,N的保持能力线性下降(P=0.001)。总N和三氯乙酸沉淀N按递减率降低浓度(Q:P=0.051;P=0.019),而氨在瘤胃液随CO的增加下降的速度很快(P=0.003)。补充CO可直线性减少瘤胃原虫的数量(P=0.006)。断奶前后补充CO检测空腹血糖浓度相似,血清胆固醇和非酯化脂肪酸的线性增加性(P<0.05)。屠宰前的重量、毛重、眼肌面积和身体脂肪均无明显差异。CO补充高达50g·kg-1可以改善了饲料转化率和胴体特性,是可接受的羊肉生产的最佳水平。高水平添加CO由于其会使瘤胃原虫受到抑制和减少纤维的消化率,饲料转化率将下降。
