Numerical modeling of bed deposition in rapid flow-like landslides:a case study of the Gaolou landslide in Shaanxi Province,China

Some flow-like landslides tend to lose materials while moving on a relatively dry sliding *** phenomenon is called bed *** contrast to the bed entrainment phenomenon,bed deposition is relatively poorly ***,an improved depth-averaged model is proposed to quantify this *** deposition depth is calculated according to the momentum conservation of the deposited mass,and the rheological property of the sliding mass on the bottom is modified considering an abrupt increase in the depth of the sliding surface after *** the proposed model,the Gaolou landslide,a typical flowlike landside occurring on October 6 th,2006 with an obvious bed deposition phenomenon in Shaanxi Province of China,is simulated to investigate the influence of bed deposition on its propagation *** results indicate that the proposed model can effectively depict the bed deposition phenomenon in the Gaolou *** deposition dissipates part of the kinetic energy of this landslide;thus,the simulated debris inundation area would likely be overestimated when this effect is *** the other hand,the thin liquefied layer formed by the bed deposition process reduces the friction energy dissipation,contributing to the high mobility of this landslide.

Two-stage surgical treatment for septic non-union of the forearm

AIM To investigate the effectiveness of a two-stage surgical procedure for the treatment of septic forearm *** Septic non-unions are rare complications of forearm fractures. When they occur, they modify the relationship between forearm bones leading to a severe functional impairment. Treatment is challenging and surgery and antibiotic therapy are required to achieve infection resolution. It is even harder to obtain non-union healing with good functional results. The aim of this study is to present a two stages surgical treatment for septic forearm non-union with revision and temporary stabilization of the non-union until infection has cleared and subsequently perform a new synthesis with plate, opposite bone graft strut and intercalary graft. We retrospectively reviewed 18 patients with a mean age at the time of primary injury of 34.5 years(19-57 years) and a mean follow-up of 6 years(2-10 years). All patients presented an atrophic nonunion with a mean length of the bone defect of 1.8 cm(1.2-4 cm). Complications and clinical results after surgical treatment were recorded. RESULTS Mean time to resolution of the infectious process was 8.2 wk(range 4-20 wk) after the first surgery and specific antibiotic therapy. All the non-union healed with an average time of 5 mo(range 2-10 mo) after the second step surgery. Cultures on intraoperative samples werepositive in all cases. No major intraoperative complications occurred. Two patients developed minor complications and one needed a second surgical debridement for infection resolution. At the last follow-up functional results were excellent in 5(27.8%) patients, satisfactory in 10(55.5%) and unsatisfactory in 3(16.7%) patients. No activities of daily living(ADLs) limitations were reported by 12(66.6%) patients, slight by 3(16.6%) and severe limitation by 3(16.6%) patients. A flexible,fully organic detector for proton beams is presented *** detector operates in the indirect mode and is composed of a polysiloxane-based scintillating layer coupled to an organic phototransistor,that is assessed for flexibility and low-voltage operation(V=−1 V),with a limit of detection of 0.026 Gy min^(−1).We present a kinetic model able to precisely reproduce the dynamic response of the device under irradiation and to provide further insight into the physical processes controlling *** detector is designed to target real-time and in-situ dose monitoring during proton therapy and demonstrates mechanical flexibility and low power operation,assessing its potential employment as a personal dosimeter with high comfort and low risk for the *** results show how such a proton detector represents a promising tool for real-time particle detection over a large area and irregular surfaces,suitable for many applications,from experimental scientific research to innovative theranostics.

背景在本前瞻、随机、双盲研究中，我们检验了50mg 1％无防腐剂的2-氯普鲁卡因可以提供比相同剂量1％单纯利多卡因恢复更快的脊髓阻滞这一假设。方法30例ASAⅠ-Ⅱ级准备接受膝关节镜检查的门诊患者随机分组接受50mg 1％单纯利多卡因（n=15）或1％无防腐剂单纯氯普鲁卡因（n=15），手术前静脉注射咪哒唑仑（0．03mg／kg）。由一位不知道分组情况的观察者记录感觉（针刺感觉消失）和运动（改良Bromage评分）阻滞的进程直至阻滞完全消退，同时记录可以自主行走和排尿的时间。手术后24小时和第7天进行电话回访。结果氯普鲁卡因组2例患者（13％）和利多卡组1例患者（7％）手术中需要辅助芬太尼（100μg Ⅳ）（P=0．99），但没有患者需要改行全麻完成手术。2-氯普鲁卡因的感觉恢复、运动恢复和自主行走时间的中位数（范围）[95（68-170）分钟、60（45-120）分钟和103（70-191）分钟]均比利多卡因者[120（80-175）分钟、100（60-140）分钟和152（100～185）分钟]要快（分别为P=0．019，P=0．0005和P=0．003）。氯普鲁卡因组的初次排尿时间[180（100-354）分钟]和利多卡因组[190（148-340）分钟]并无不同（P=0．191）。短暂神经症状在利多卡因组患者有5例（33％）而氯普鲁卡因组没有（0％）（P=0．042）。结论50mg 1％无防腐剂2-氯普鲁卡因鞘内注射后感觉和运动功能恢复和自主行走时间比相同剂量1％利多卡因更快，同时较少发生短暂神经症状。
