Exploring the Retailer＇s Relational Satisfaction in a Non-western Context： Evidence From the Tunisian Post 2011 ＇s RevolutionSetting

The current research aims to find out whether the model of the retailer＇s relational satisfaction, developed in a western setting is relevant in a different economic and cultural field of investigation： the Tunisian post 2011 revolution＇s emerging context, and in a different industry： the retail industry. It explores the distinctive economic and cultural features that affect the retailer＇s relational satisfaction with its consumer goods manufacturers. The study employs a survey based on a face-to-face administered questionnaire. Two hundred and thirty four responses from a survey with Tunisian retailers as well as international retailers operating in Tunisia were obtained. All constructs are measured using five-point Likert type scales （1 = strongly disagree, 5 = strongly agree）. Analyses are conducted using exploratory factor analysis and reliability analysis. The results support the original model with however some differences derived from the Tunisian economic setting as a whole, the characteristics of the Tunisian consumer goods industry as well as the clan characteristic feature of the Tunisian collectivistic culture. Thus, the amount of discounts obtained, does no longer contribute to the retailer＇s economic satisfaction. Moreover, when it comes to social satisfaction, the respondents get uncomfortable, even to deal with a potential negative interaction with their suppliers. For retail companies involved in internationalization in the Maghreb Region, and especially in Tunisia, it seems that other competitive advantages than prices are to be developed, such as the development of private label or other services that create positive perceived value for the Tunisian customer. From a cultural standpoint, western managers have rather to go away from a formalized channel relationship management, while taking into account the importance of interpersonal influence. Total mesopancreas excision is the better staging tool of the mesopancreas in pancreatic head carcinoma

Preoperative imaging staging based on tumor,node,metastasis classification cannot be effective to avoid R1 resection because only further improvements in imaging technologies will allow the precise assessment of perineural and lymphatic invasion and the occurrence of microscopic tumour deposits in the ***,waiting for further improvements in imaging technologies,total mesopancreas excision remains the only tool able to precisely assess mesopancreatic resection margin status,maximize the guarantee of radicality in cases of negative(R0)mesopancreatic resection margins,and stage the mesopancreas.

Relationship between antral distension and postprandial symptoms in functional dyspepsia

AIM: To investigate in patients with functional dyspepsia (FD) after an every-day meal whether (1) gastrointestinal (GI) and extra-GI symptoms had any relation with the degree of antral volume, (2) the onset of postprandial symptoms was associated with, and may predict, delayed gastric emptying. METHODS: In 94 symptomatic FD patients, antral volume variations and gastric emptying were assessed with ultrasonography after a 1050 kcal meal. Symptoms were evaluated with a standardized questionnaire. The association of GI and extra-GI symptoms with antral volumes and gastric emptying were estimated with logistic regression analysis. RESULTS: Forty percent of patients did not report any symptoms after a meal. Compared to the healthy controls, the antrum was more distended in patients throughout the entire observation period and 37 (39.4%) patients had delayed gastric emptying. Only postprandial drowsiness was associated with antral volume variations (AOR = 1.42; P < 0.001) and with delayed gastric emptying (AOR = 3.59; P < 0.03). CONCLUSION: In FD patients, GI symptoms are neither associated with antral distension nor with gastric emptying. Drowsiness is associated with antral distension and delayed gastric emptying. The onset of drowsiness is preceded by an increment of antral distension and the duration of the symptom appears to be related to the persistence of antral distension.

Impact of tumour rupture risk on the oncological rationale for the surgical treatment choice of gastrointestinal stromal tumours

Tumour rupture of gastrointestinal stromal tumours(GISTs)has been considered to be a remarkable risk factor because of its unfavourable impact on the oncological *** tumour rupture has not yet been included in the current tumor-node-metastasis classification of GISTs as a prognostic factor,it may change the natural history of a low-risk GIST to a high-risk ***,tumour rupture was defined as the spillage or fracture of a tumour into a body cavity,but recently,new definitions have been *** definitions distinguished from the prognostic point of view between the major defects of tumour integrity,which are considered tumour rupture,and the minor defects of tumour integrity,which are not considered tumour ***,it has been demonstrated that the risk of disease recurrence in R1 patients is largely modulated by the presence of tumour ***,after excluding tumour rupture,R1 may not be an unfavourable prognostic factor for ***,after the standard adjuvant treatment of imatinib for GIST with rupture,a high recurrence rate *** review highlights the prognostic value of tumour rupture in GISTs and emphasizes the need to carefully take into account and minimize the risk of tumour rupture when choosing surgical strategies for GISTs.

The "meso" of the rectum and the "meso" of the pancreas: similar terms but distinct concepts in surgical oncology

A correspondence between the ＂meso＂ of the rectum and of the pancreas has recently been reported. Here we highlight the differences between mesorectum and mesopancreas. Based on anatomical findings from a series of 89 consecutive pancreaticoduodenectomies and 71 consecutive total mesorectal excisions, we observed that in contrast to the mesorectum, the mesopancreas did not have well-defined anatomic boundaries and was continuous and connected through its components with the para-aortic area. In rectal cancer,tumor deposits and nodal involvement could be confined to the mesorectum（i.e., within the mesorectal fascia）, whereas in pancreatic carcinoma, tumor deposits and nodal metastases occurred in the boundless mesopancreatic area. Total mesorectal excision was made en bloc with the rectum by dissecting along the mesorectal fascia; this was not the case for mesopancreatic excision since anatomical demarcation of the mesopancreas did not exist. Moreover, the growth pattern of pancreatic cancer showed greater dispersion, which was more prominent at the invasive front of the tumor and could potentially affect the status of the resection margin. These findings indicate that the mesorectum and mesopancreas are completely distinct from the pathological, surgical, and oncological standpoints.

Enucleation for gastrointestinal stromal tumors at the esophagogastric junction:Is this an adequate solution?

The authors discussed the proposal by Coccolini and colleagues to treat gastrointestinal stromal tumors (GISTs) at the esophagogastric junction with enucleation and,if indicated,adjuvant therapy,reducing the risks related to esophageal and gastroesophageal *** concluded that,because the prognostic impact of a T1 high-mitotic rate on esophageal GIST is worse than that of a T1 high-mitotic rate on gastric GIST,enucleation may not be an adequate surgery for esophagogastric GISTs with a high mitotic rate in which the guarantee of negative resection margins and adjuvant therapies can be the only chance of survival.

On the installation of an in situ large-scale vertical SEALing (VSEAL) experiment on bentonite pellet-powder mixture

Recently,the Institute for Radiological protection and Nuclear Safety(IRSN)has launched VSEAL(Vertical SEALing)project to investigate the impact of gas migration on the long-term performance of bentonite based vertical sealing systems(VSS).The first VSEAL in situ test was emplaced in IRSN’s Underground Research Laboratory(URL)in Tournemire(France)in 2019 and was equipped with 76 wired and wireless *** test is still in progress,but the collected set of data provides already valuable information of the hydro-mechanical behavior of VSS during *** swelling core consists of a mixture of highdensity pellets and powder of MX80 bentonite in a ratio of 80/20(in dry mass).An innovative method was adopted to drill a 1-m diameter and w10-m deep shaft in order to minimize the rock perturbation at the *** a specific protocol was adopted to install the bentonite mixture together with a careful characterization of the core during construction,VSEAL 1 constitutes the unique in situ sealing test with a well-known initial structural distribution of the pellets and the *** heterogeneities occurred within the experiment during the installation process:a damaged zone developed around the shaft walls due to the interruption of the installation operations caused by COVID19 lockdown in France;a technological gap with a variable thickness between the last pellets layer and the top confining lid and a heterogeneous distribution of the bentonite powder at some layers inducing large inter pellets voids close to the bentonite-rock *** injected water volume,relative humidity,water content and swelling pressure in both radial and axial directions were *** of the results showed that the presence of installation-induced heterogeneities led to the generation of preferential flow paths that influenced the swelling pressure evolution at radial and axial directions.
