New perspectives in occult hepatitis c virus infection

Occult hepatitis c virus (HcV) infection, defined as the presence of HcV RNA in liver and in peripheral blood mononuclear cells (PBMcs) in the absence of detectable viral RNA in serum by standard assays, can be found in anti-HcV positive patients with normal serum levels of liver enzymes and in anti-HcV negative patients with persistently elevated liver enzymes of unknown etiology. Occult HcV infection is distributed worldwide and all HcV genotypes seem to be involved in this infection. Occult hepatitis c has been found not only in anti-HcV positive subjects with normal values of liver enzymes or in chronic hepatitis of unknown origin but also in several groups at risk for HcV infection such as hemodialysis patients or family members of patients with occult HcV. This occult infection has been reported also in healthy populations without evidence of liver disease. Occult HcV infection seems to be less aggressive than chronic hepatitis c although patients affected by occult HcV may develop liver cirrhosis and even hepatocellular carcinoma. Thus, anti-HcV negative patients with occult HcV may benefit from antiviral therapy with pegylated-interferon plus ribavirin. The persistence of very low levels of HcV RNA in serum and in PBMcs, along with the maintenance of specific T-cell responses against HcV-antigens observed during a long-term follow-up of patients with occult hepatitis c, indicate that occult HcV is a persistent infection that is not spontaneously eradicated. This is an updated report on diagnosis, epidemiology and clinical implications of occult HcV with special emphasis on anti-HcV negative cases.

cu-Ni-Al alloys at different concentrations were obtained using a high frequency induction melting unit, keeping a balance in the nominal compositions. Light alloys are important to be used in industrial applications. Aluminum additions result in a positive hardness increment of the ternary alloys in comparison with the binary cu-Ni alloys. Generalized wear mechanisms of the alloys with low aluminum content are basically type abrasive, while samples with 5 and 10 at.% Al present an oxidative-adhesive wear mechanism. Wear results have indicated that aluminum addition affects positively the wear resistance, mainly in samples with high aluminum content product of the creation during the test of different oxides corresponding to the elements present in the alloys.

OBJEcTIVES Elderly patients show a higher incidence of ischemic and bleeding events after percutaneous transluminal coron-ary intervention(PcI).We sought to investigate outcomes in elderly patients treated with antithrombotic strategy guided by bleeding and ischemic risks after revascularization with last generation everolimus-eluting stent(EES).METHODS Prospective multicenter registry including patients over 75 years revascularized with EES and antithrombotic ther-apy guided by clinical presentation,PcI complexity and PREcISE DAPT ***-primary safety endpoints were:(1)composite of cardiac death,myocardial infarction and stent thrombosis and;(2)bleeding(BARc 2-5).Primary efficacy endpoint was target lesion revascularization.A matched group of patients revascularized with current drug-eluting stents and no such tailored antith-rombotic therapy was used as *** Finally,1064 patients were included in SIERRA-75 cohort,80.8±4.2 years,36.6%women,71%acute coronary syn-dromes(AcS)and 53.6%complex ***-primary safety endpoint of major adverse cardiovascular events was met in 6.2%,co-primary safety endpoint of bleeding in 7.8%and primary efficacy endpoint of TKLR in 1.5%.The multivariable adjusted model showed no significant association of the prescribed short/long dual antiplatelet therapy(DAPT)durations with any endpoint suggesting a well tailored *** stent thrombosis reported in the subgroup with 1-3 months DAPT *** compared to control group,bleeding BARc 2-5 was significantly lower in SIERRA-75 group(7.4%vs.10.2%,P=0.04)as well as the net safety-efficacy endpoint(14.3%vs.18.5%,P=0.02).cONcLUSIONS In elderly population,the application of this risks-adjusted antithrombotic protocol after revascularization with last generation EES seems to be associated with an improved prognosis in terms of ischemic and bleeding outcomes.

The liver is frequently affected by severe acute respiratory syndrome-coronavirus 2(SARS-coV-2)*** most common manifestations are mildly elevated alanine aminotransferase and aspartate aminotransferase,with a prevalence of 16-53%among *** with severe coronavirus disease 2019(cOVID-19)seem to have higher rates of acute liver dysfunction,and the presence of abnormal liver tests at admission signifies a higher risk of severe disease during *** with chronic liver diseases also have a higher risk of severe disease and mortality(mainly seen in patients with metabolic-associated fatty liver disease).Several pathways of damage have been proposed in the liver involvement of cOVID-19 patients;although,the end-cause is most likely *** liver tests have been attributed to the expression of angiotensin-converting enzyme 2 receptors in SARS-coV-2 *** enzyme is expressed widely in cholangiocytes and less in *** factors attributed to liver damage include drug-induced liver injury,uncontrolled release of proinflammatory molecules(“cytokine storm”),pneumonia-associated hypoxia,and direct damage by the *** steatosis,vascular thrombosis,fibrosis,and inflammatory features(including Kupffer cell hyperplasia)are the most common liver histopathological findings in deceased cOVID-19 patients,suggesting important indirect mechanisms of liver *** this translational medicinebased narrative review,we summarize the current data on the possible indirect mechanisms involved in liver damage due to cOVID-19,the histopathological findings,and the impact of these mechanisms in patients with chronic liver disease.

In this work, we make a number of proposals to explain how a world of (4 + 4)-dimensions can be useful for a better understanding of both dark matter and quantum gravity. The key idea is to look for some advantage of considering self-dual invariants in (4 + 4)-dimensions rather than in a separate context of (1 + 3)-dimensions or (3 + 1)-dimensions. In fact, we show that by considering the duality concept in (4 + 4)-dimensions we may provide an alternative meaning of a framework for loop quantum gravity. Moreover, considering the Dirac equation in (4 + 4)-dimensions for a particle without electric charge and mass, we show that when it is projected into the (1 + 3) and (3 + 1)-worlds may describe a system with electric charge and mass. We also discuss the relation between the three physical scenarios;(4 + 4)-world, black-holes and dark matter.

Using agent development tools to construct an agent-based system is a well applied approach. However, the development tools usually do not have the function to check the feasibility about the workflow of the agent system during it implementation stage. Therefore, to develop an evaluation approach to analyze the feasibility of a developing agent system such that the improper workflow of an agent system can be found in the early design stage is a necessary task to reduce the risk of implementation. In this research, a Petri Net （PN） based three-stage evaluation approach was developed. In the conceptual stage, the pitfall of the current agent system developing process was examined and an improvement analysis process was specified. Then, in the system design stage, an evaluation approach which extracted the process log file from a developing agent system into a PN model in terms of a process mining approach-or algorithm was proposed. This model was simulated in a PN simulation package. The agent system performance was evaluated in terms of analyzing the deadlock phenomena of the PN model. Finally, in the implementation stage, the proposed concept was implemented by using an agent developing tool JADE and a PN simulation tool cPN. An agent-based robotic assembly system was used to examine the possible deadlock of the agent system.

The objective of the study was to investigate the effect of the organo-modified nanosepiolite(ONSep)on improving the fire safety of polypropylene(PP).The composites based on PP,flame retardant master batch(MB-FR,25 wt%PP+50 wt%decabromodiphenyl ether(DBDPE)+25%antimony trioxide(ATO))and ONSep were prepared via melt *** results of the limiting oxygen index(LOI)and vertical burning rating(UL-94)test indicated that PP/40 wt%MB composites had no rating with seriously dripping phenomenon,while the LOI value was only ***,as 4 wt%ONSep was added in PP/40 wt%MB composites,the composites achieved UL94 V-0 rating and the LOI value was *** comparison,PP/50 wt%MB composites could not reach the V-0 rating *** TGA results revealed that the addition of ONSep enhanced the thermal stability of the PP/MB-FR *** cone calorimeter results indicated that the heat release rate,average mass loss rate,smoke production rate and smoke temperature of the PP/40 wt%MB-FR/4 wt%ONSep composites decreased in comparison with those of PP/40 wt%MB-FR ***,the Young modulus and impact strength were also much better improved with the increase of ONSep ***,the synergistic flame retardancy of ONSep in PP/MB-FR matrix significantly containing a halogen based flame retardant(DBDPE)significantly improved the fire safety and mechanical properties of the composites,and allowed to decrease the total amount of brominated fire retardants.
