Analysis of the therapeutic evolution in the management of airway infantile hemangioma

AIM: To analyze the evolution in the management of airway infantile hemangioma(AIH) and to report the results from 3 pediatric tertiary care ***: A retrospective study of patients with diagnosis of AIH and treated in 3 pediatric tertiary care institutions from 1996 to 2014 was performed. RESULTS: Twenty-three patients with diagnosis of AIH were identified. Mean age at diagnosis was 6 mo(range, 1-27). Single therapy was indicated in 16 patients and 7 patients received combined therapy. Two therapeutic groups were identified: Group A included 14 patients who were treated with steroids, interferon, laser therapy and/or surgery; group B included 9 patients treated with oral propranolol. In group A, oral corticosteroids were used in 9 patients with a good response in 3 cases(no requiring other therapeutic option), the other patients required additional treatment options. cushing syndrome was observed in 3 patients. One patient died of a fulminant sepsis. Open surgical excision and endoscopic therapy were performed in 11 patients(in 5 of them as a single treatment) with a response rateof 54.5%. Stridor persisted in 2 cases, and one patient died during the clinical course of bronchial aspiration. In group B, oral propranolol was used in 9 patients(in 8 of them as a single treatment) with a response rate of 100%, with an mean treatment duration of 7 mo(range, 5-10); complications were not observed. cONcLUSION: Our experience and the medical literature support the use of propranolol as a first line of treatment in AIH.

A case about a HIV woman with nasal ulcers is described in this paper. In every inmunodepressed patient who has mu-cosal or cutaneous ulcers, infection by leishmanial parasites needs to be ruled out, especially when ulcers have not re-gression with usual treatments. The Leishmanial nasal disease usually shows swelling and mucosal ulcers, that may progress to necrosis. Delaying in appropriate therapy might cause irreversible damage.

Estimating the geographic range of a threatened shark in a data-poor region： cetorhinus maximus in the South Atlantic Ocean

The distribution of the planktivorous basking shark cetorhinus maximus is influenced by zooplankton abundance at small scales and temperature at medium scales in the North Atlantic. Here, we estimate the distribution of basking sharks on South Atlantic continental shelves, and the relative importance of chlorophyll concentration, as a proxy for zooplankton abun- dance, and temperature in determining habitat suitability for basking sharks at large scales. We used maximum entropy （MaxEnt） and maximum likelihood （MaxLike） species distribution modelling to test three hypotheses： the distribution of basking sharks is determined by （1） temperature, （2） chlorophyll concentration, or （3） both chlorophyll and temperature, while considering other factors, such as oxygen and salinity. Off South America, basking shark habitat included subtropical, temperate and cool-temperate waters between approximately 20°S and 55°S. Off Africa, basking shark habitat was limited to cool-temperate waters off Namibia and southern South Africa. MaxLike models bad a better fit than MaxEnt models. The best model included minimum chlorophyll concentration, dissolved oxygen concentration, and sea surface temperature range, supporting hypothesis 3. However, of all variables included in the best model, minimum chlorophyll concentration had the highest influence on basking shark distribution. Unlike the North Atlantic distribution, the South Atlantic distribution of basking sharks includes subtropical and cool-temperate waters. This difference is explained by high minimum chlorophyll concentration off southern Brazil as compared to North Atlantic subtropical areas. Observations in other regions of the world support this conclusion. The highest habitat suitability for basking sharks is located close to nearshore areas that experience high anthropogenic impact [current Zoology 61 （5）： 811-826, 2015].

Relevant Aspects of Expert Interest in the Perception of the Facial Aesthetic Damage

Forensic aesthetic facial damage—AFD expert’s valuation is a concerning matter in court as expressively vary in between experts. In fact, differences can be quite significant suggesting examiners age, sex and professional qualification may influence this valuation. Aiming to offer a systematic and more objective evaluation, some methods for assessing AFD had been proposed lately. Known as Aesthetic Impairment Impact Perception (AIPE), cobo-Plana’s revised methodology (2010) was idealized to minimize the examiner’s subjectivity by means of answering an ordered sequence of questions related to the specific damage perception. This research aims to access AFD perception differences (simulated cases) in between Lawyers, Heath professionals (Physicians and Dental surgeons) and general professionals (laypeople) under AIPE methodology in order to allow a better comprehension of AFD when dealing with forensic expertise cases. Within Groups perceptions varied (p cored higher AFD lesions, and rater’s sex (as an independent variable) did not express any significant difference in AFD scoring. The tendency of under-rating AFD lesions below very important severity grade was observed being a matter of major concern considered their prevalence and the effective consequences involved to the victim. These results recommend extra caution when performing AFD evaluation and consequently expertise reports, claiming a more judicious analysis based on consolidated standards and suggest, examiners to be more perceptive to physical damage evaluation in order to include social perceptions and reactions, mainly, victims feelings and constrains over their aesthetic loss.

A survey for Infectious Hypodermal and Haematopoietic Necrosis Virus (IHHNV) and White Spot Syndrome Virus (WSSV) was performed during two catching seasons of native shrimps in the Yucatan coast in the Gulf of Mexico. The mtDNA cOI barcode identified two endemic species;the southern pink shrimp Penaeus notialis (Pérez Fantante, 1967) and the northern pink shrimp Penaeus brasiliensis (Latreille, 1987) (previously described as Farfantepenaeus brasiliensis). The prevalence of IHHNV was of 18.18% in 2016, and of 8.57% in 2017. All organisms tested negative for WSSV. This is the first identification of wild shrimps in the state of Yucatan Mexico by mtDNA cOI barcode as well as the first identification of IHHNV in such species. The presence of IHHNV in wild shrimps populations has a potential of persisting in the coast of Yucatan with putative detrimental effect on local fisheries because once established in natural waters and hosts;such pathogens are almost impossible to eradicate.

AIMTo analyze the diagnostic yield (DY), therapeutic impact (TI) and safety of capsule endoscopy (cE).METHODSThis is a multi-centre, observational, analytical, retrospective study. A total of 163 patients with suspicion of celiac disease (cD) (mean age = 46.4 ± 17.3 years, 68.1% women) who underwent cE from 2003 to 2015 were included. Patients were divided into four groups: seronegative cD with atrophy (Group-I, n = 19), seropositive cD without atrophy (Group-II, n = 39), contraindication to gastroscopy (Group-III, n = 6), seronegative cD without atrophy, but with a compatible context (Group-IV, n = 99). DY, TI and the safety of cE were *** overall DY was 54% and the final diagnosis was villous atrophy (n = 65, 39.9%), complicated cD (n = 12, 7.4%) and other enteropathies (n = 11, 6.8%; 8 crohn’s). DY for groups I to IV was 73.7%, 69.2%, 50% and 44.4%, respectively. Atrophy was located in duodenum in 24 cases (36.9%), diffuse in 19 (29.2%), jejunal in 11 (16.9%), and patchy in 10 cases (15.4%). Factors associated with a greater DY were positive serology (68.3% vs 49.2%, P = 0.034) and older age (P = 0.008). On the other hand, neither sex nor clinical presentation, family background, positive histology or HLA status were associated with DY. cE results changed the therapeutic approach in 71.8% of the cases. Atrophy was associated with a greater TI (92.3% vs 45.3%, P < 0.001) and 81.9% of the patients responded to diet. There was one case of capsule retention (0.6%). Agreement between cE findings and subsequent histology was 100% for diagnosing normal/other conditions, 70% for suspected cD and 50% for complicated *** has a high DY in cases of suspicion of cD and it leads to changes in the clinical course of the disease. cE is safe procedure with a high degree of concordance with histology and it helps in the differential diagnosis of cD.

Preventative effects of a probiotic, Lactobacillus salivarius ssp. salivarius, in the TNBS model of rat colitis

AIM： To investigate the intestinal anti-inflammatory effect and mechanism of a probiotic Lactobacillus salivarius ssp. salivariuscEcT5713 in the TNBS model of rat colitis. METHODS： Female Wistar rats （180-200 g） were used in this study. A group of rats were administered orally the probiotic L. salivarius ssp. salivarius （5 × 10^8 cFU suspended in 0.5 mL of skimmed milk） daily for 3 wk. Two additional groups were used for reference, a non-colitic and a control colitic without probiotic treatment, which received orally the vehicle used to administer the probiotic. Two weeks after starting the experiment, the rats were rendered colitic by intracolonic administration of 10 mg of TNBS dissolved in 0.25 mL of 500 mL/L ethanol. One week after colitis induction, all animals were killed and colonic damage was evaluated both histologically and biochemically. The biochemical studies performed in colonic homogenates include determination of myeloperoxidase （MPO） activity, glutathione （GSH） content, leukotriene B4 （LTB4） and tumor necrosis factor α （TNF-α） levels, as well as inducible nitric oxide synthase （iNOS） expression. In addition, the luminal contents obtained from colonic samples were used for microbiological studies, in order to determine Lactobacilli and Bifidobacteria counts. RESULTS： Treatment of colitic rats with L. salivarius ssp. salivarius resulted in amelioration of the inflammatory response in colitic rats, when compared with the corresponding control group without probiotic treatment. This antiinflammatory effect was evidenced macroscopically by a significant reduction in the extent of colonic necrosis and/or inflammation induced by the administration of TNBS/ethanol （2.3±0.4 cm vs3.4±0.3 cm in control group, P〈0.01） and histologically by improvement of the colonic architecture associated with a reduction in the neutrophil infiltrate in comparison with non-treated colitic rats. To evaluate the effect of different levels of citric acid (cA) combined with phytase and calcium levels in the diets of laying hens on the calcium and phosphorus (P) contents of eggs: yolk-albumen and shell, yolk color and egg quality. A study was conducted with 24-week-old laying hens fed with a diet based on sorghum and soybean meal with 2.7 Mcal of EM/kg, 15% crude protein, 3.25% calcium and 0.25% available P. The levels of cA, phytase and calcium were 0%, 0.6% and 1.2%;0 and 300 units of phytase (PHYU)/kg;and 3.0% and 3.25%, respectively. Phytase was added as an ingredient into to the diets, contributing 0.1% P and 0.3% calcium. A completely randomized factorial experiment was performed with a 3 × 2 × 2 design and 4 replications. The cA increased (P combination of 1.2% cA, 300 PHYU phytase and 3.25% calcium increased (P calcium in the eggshell. There was a cA × phytase × calcium interaction (P cting egg yolk pigmentation;there were no differences in egg quality. It was concluded that adding cA to the diets of laying hens increases eggshell calcium content, and improves the phytase response. 1.2% cA × Phytase × 3.25% calcium significantly increases shell calcium by 0.56 g. Under the conditions of the present investigation, it would imply greater resistance of the eggs to be broken in the handling from the farm to the sales centers and consumer. Phytase interacts negatively with 0.6% cA at low calcium levels to degrade the yellow pigmentation of the yolk.
