Objectives:Analyze the obstructive sleep apnea (OSA) treatment history in a group of participants who enrolled in a hypoglossal nerve stimulation trial Methods:Moderate-severe OSA patients with difficulty adhering to CPAP presented for enrollment in a multicenter ***-reported history on prior OSA medical therapy was collected at enrollment,including OSA diagnosis date,CPAP start and stop dates,oral appliance trial,and reasons for discontinuation or ***:The cohort consisted of 929 participants,83％ male,with a mean age (53.9 ± 10.5) *** percent (n =835) had complete CPAP information including 47％ (n =435) who discontinued therapy prior to enrollment and 43％ (n =400) who were still attempting CPAP but had inadequate *** rates were 60％ at 1-year,73％ at 3-years,and 86％ at *** appliance therapy was attempted by 171 patients for mean (1.8 ± 2.3) years,with 81％ abandonment at 1 year,89％ at 3-years,and 94％ at ***:In this CPAP-refractory cohort,high rates of CPAP abandonment were reported in the first several years with approximately half of the participants not receiving any treatment despite being diagnosed for ＞5 *** clinical follow-up and consideration of alternative treatment options is indicated in all OSA patients in order to ensure adequate longitudinal care.

background: Excessive daytime sleepiness (EDS) is common in adults. A need exists for an easier and faster objective clinical measures of EDS. The autonomic nervous system controls pupil size and prior pupillometry studies have demonstrated associations with sleepiness. We used a novel portable device to assess pupillometry and prospectively evaluated a sleep clinic cohort. Methods: Following IRb approval Pupillometry (The ForSiteTM, NeurOptics, Irvine, CA), was performed on 113 sleep clinic patients. Constriction and dilation velocity and latencies, minimum and maximum aperture were obtained along with Epworth Sleepiness Score (ESS), 10 point Visual Analog Scale (VAS), bMI, gender, age and AHI. Three sets of measures were obtained and analyzed with ANOVA, t-test, Linear Regression and Pearson correlation coefficients (SAS, Cary, NC). Results: both constriction velocity and latency correlated with VAS (n = 88, r = 0.28, p = 0.007 and r = 0.31, p = 0.004). Only constriction velocity correlated with AHI (n = 78, r = -0.27, p = 0.016). Multivariate linear regression which includes VAS and age predicted constriction velocity (r = 0.36, p = 0.002) and latency (r = 0.38, p = 0.001). Using Pearson correlation, AHI and VAS combined were associated with constriction velocity (-0.273 (0.016), and 0.284 (0.007), respectively). Using a maximum constriction velocity threshold value (age adjusted) of 2.8, VAS ≥ 6 was predicted with a sensitivity of 83% and specificity of 84%. Conclusions: Pupillary constriction velocity and latency predict self-reported VAS state of sleepiness. While both are affected by age, only constriction velocity is affected by apnea severity. These data suggest that a portable pupillometer may provide a method to identify individuals with abnormal sleepiness.

Molar-tooth(MT) structure is an enigmatic sedimentary structure consisting of variously-shaped cracks and voids filled with a characteristically uniform,equant calcite *** is globally distributed but temporally restricted to rocks from Neoarchean to Neoproterozoic *** origin of MT structures has been debated for more than a century and the topic continues to be highly *** features of MT structure occurring in micritic limestones of the Mesoproterozoic Gaoyuzhuang Formation(ca.1500 Ma to ca.1400 Ma),Jixian section,Tianjin City,North China show that:1) there is a definite interface or lining,rich in organic material and pyrite,between the MT crack-filling calcite microspar and the micritic host rock,which is also rich in organic matter;2) the micritic host rocks are notable for the absence of stromatolites and microbial laminites;3) distinctive conglomeratic lag deposits made up of intraclasts of MT microspar result from storm reworking of the MT structures;4) the MT structure is associated with possible algal megafossils such as Chuaria;5) the MT microspar is made up of the larger calcite crystal and the MT crack is marked by the diversity of configurations;6) both the TOC content and the carbon-isotopic value(δ^(13)C_(PDb)) among the host rock, the MT microspar and the possible algae fossil are obviously *** the forming mechanism of the Gaoyuzhuang MT structure,these features can still indicate that:A) the MT microspar was formed by rapid precipitation and lithification;b) the MT microspar precipitated directly within the cracks; C) the decomposition of organic matter within the host micrite might be the chief mechanism producing gas bubbles;D) microscale gas-sediment interaction led to the generation of the MT cracks and the precipitation of microspar therein;E) the MT cracks might represent the track of migration and expansion of gas bubbles,and that the recrystallization of host micrites cannot be eliminated during forming proc

Social issues are increasingly recognized as significant inhibitors to mineral development projects. Increasingly, social risk is being recognized as a key factor determining the success of a mineral investment. Groups opposed to a mine for social or political reasons otten use environmental impacts, real or perceived, to prevent mine development. These risk factors depend largely on cultural perceptions of mining activities and must be understood as such in order to be appropriately managed. A first step to addressing social issues is inclusive, transparent and meaningful engagement of stakeholders. This process allows stakeholders to understand what the other parties value in order to collectively establish a common currency for development and the creation of mutual value. Expanding the scope of benefits and values a mine can bring is of increasing importance to mining companies who typically consult outside specialists remote from the mine site and late in the development timeline for this purpose. Training technical staff, engineers and geologists, who make initial and ongoing contact with local interests, in a holistic approach to mine development is crucial to successful and economic mineral development projects. Further extending this conversation to the genera/ public, media governments and non-govemmental organizations is a necessary step in developing a meaningful discourse on the benefit of mining activities.
