Community based Survey Methodology for Maternal Healthcare Utilization: Gujarat, India

background: About 60% of institutional births occur in the private sector in Gujarat due to limited availability of obstetricians in the government. Chiranjeevi Yojana (CY), a voucher-like program initiated in 2007, accredits private obstetricians who are reimbursed by the state government to provide free delivery care to eligible women i.e. below poverty line and tribal. One million women have delivered under the CY program yet there are no large community based studies of the program. Methodology of a prospective community study is described here. Methods/Designs: A prospective cohort study was done in 142 villages across 3 districts in Gujarat between July, 2013 and November, 2014. A detailed survey was done by trained researchers to ascertain maternal healthcare information including antenatal, intra-partum and post-partum care, place of delivery, birth outcomes, out of pocket expenses etc. Results: 54,955 households were surveyed. 73% of all households had poverty documentation. 4274 mothers who delivered in the study period were included. Discussion: This paper is description of the methodology of a large community based survey and household and individual level characteristics. The survey was nested in a larger project to evaluate the CY program in the state of Gujarat.

background: The Tuberculosis (Tb) disease has immense impact on physical, psychological, economic and social well-being of an individual. It is desired that the patient with any kind of Tb disease should lead a respectable and happier life during their course of Tb treatment. Currently, the quality of life (QOL) is an important indicator to assess the well-being of a person and there is paucity of such information among Tb patients. Hence, we conducted to assess and compare the QOL of Revised National Tb Control Programme registered Drug sensitive Tb patients, Drug resistant Tb patients and general population of Gadag district in Karnataka, India. Methods: A cross-sectional study was conducted in Gadag district, Karnataka from March 2017 through March 2018 among drug sensitive, drug resistant Tb patients and general population. A Non-probability purposive sampling was adopted to select the samples that were matched for age, gender and ward. The select patients were administered WHO QOL-bREF questionnaire by trained personnel. Data were analyzed using SPSS version 12 analysis software. Results: The scores obtained for the four domains of QOL were (a) physical health: 69.62 + 18.29 (b) Psychological: 66.96 + 18.62 (c) Environment: 60.99 + 15.05 and (d) Social relationships: 53.5 + 19.93. Conclusions: The drug resistant Tb patients have poor QOL when compared to drug sensitive and general population.

Ectopic colonic varices development from liver cirrhosis and portal hypertension is uncommon. They are part of the spectrum of portal hypertensive colopathy. Colonic variceal bleeding remains a rare cause of lower gastrointestinal tract (GI) bleeding. Due to the paucity of cases, there are no well-established conventional treatments for bleeding colonic varices. Different treatments have been reported. Here, we report a case of a 55-year-old gentleman, with a history of alcoholic liver cirrhosis, presenting with severe lower GI bleeding and symptomatic anaemia. An esophagogastroduodenoscopy revealed large esophageal varices with high-risk bleeding stigmata requiring endoscopic variceal ligation. A cross-sectional computed tomography scan showed colonic portosystemic shunts. In light of this and that the severe lower GI bleeding seemed out of proportion to the esophageal varices seen on upper endoscopy, an urgent unprepped colonoscopy was performed which revealed possible bleeding diverticula disease which required endoscopic mechanical hemoclip therapy. However, despite this, patient had recurrence of lower GI bleeding prompting a second colonoscopy. This relook colonoscopy showed ectopic ascending colon varices with high-risk bleeding stigmata. High-dose intravenous vasoactive agent somatostatin (500 mcg/hour) and subsequently terlipressin (2 mg every 4 hours) were used. The patient subsequently underwent successful balloon-occluded retrograde transvenous obliteration (b-RTO) and sclerotherapy. The non-selective beta-blocker (NSbb) carvedilol was started and bridged together with the vasoactive agent until stabilisation of portal hypertension. This difficult case illustrates the dynamic nature of portal hypertensive bleeding. It also highlights the presence of confounding non-variceal pathology complicating diagnosis of portal hypertensive colonic variceal bleeding, and that ectopic ascending colonic variceal bleeding can be treated successfully with b-RTO and scle

Research on the effects of ultraviolet-b (UV-b) radiation on soybean seed quality is limited. The objective of this study was to quantify UV-b doses, 0, 5, 10 & 15 kJ•m–2•d–1, on soybean growth and seed quality. The experiment was conducted in the Soil-Plant-Atmosphere-Research (SPAR) facility. Chambers located at the R.R. Foil Plant Science Research Facility of Mississippi State University, Mississippi, USA, were used. Each SPAR chamber consists of a steel soil bin to accommodate the root system, a Plexiglas chamber to accommodate plant canopy and a heating, and cooling system connected to air ducts that pass conditioned air to cause leaf flutter through the plant canopy. The SPAR units, supported by an environmental monitoring and control systems, are networked to provide automatic acquisition and storage of the data, monitored every 10 seconds throughout the day and night. Soybean cultivar Pioneer 93Y92 (maturity group IV, Roundup Ready) was used in the study. The desired UV-b radiation was supplied by square-wave UV-b supplementation systems under near ambient PAR and delivered to plants for eight hours, each day, from 08:00 to 16:00 h by eight fluorescent UV-313 lamps. The results showed that increased UV-b did not influence many of the growth parameters because the treatments were imposed at mid-fruiting period. Seed quality parameters that are important for seed industry and human and animal nutrition were all affected by UV-b. Protein and palmitic and oleic acids declined linearly, while oil and linoleic and linolenic acid contents increased with increased UV-b. Sucrose, stachyose, and stearic acid contents showed quadratic trends, increased to about 4 - 5 kJ of UV-b and declined at higher doses. Thus, both current and projected UV-b radiation levels can modify soybean growth and seed quality. The crystal structure of spiro[2.2”]acenaphthene-1”-onespiro[3.3’]-5’-(2,3-dichlorophenylmethylidene)-1’-methylpipe-ridin-4’-one-4-(2,3-dichorophenyl) octahydroindolizine was elucidated by single crystal X ray diffraction. The title compound C37H30Cl4N2O2, crystallizes in the orthorhombic system, space group P212121 with a = 8.4610(4) , b = 16.0926(6) , c= 23.8997(11) and Z = 4. The central piperidine ring adopts twisted conformation, the piperidine of octahydroindolizine ring is in chair conformation and the pyrrole ring is in slightly twisted envelope conformation. Details of the synthesis, NMR, crystal structure determination and intra- and inter- molecular interactions of the compound are given.

Hot forging of Mg-4Al-2ba-2Ca(Aba X422) alloy and validation of processing map

A cup-shaped component of Mg-4 Al-2 ba-2 Ca（Aba X422） alloy was forged in the temperature range of 300-500 °C and at speeds in the range of 0.01-10 mm/s with a view to validate the processing map and study the microstructural development. The process was simulated through finite-element method to estimate the local and average strain rate ranges in the forging envelope. The processing map exhibited two domains in the following ranges：（1） 300-390 °C and 0.0003-0.001 s^-1, and（2） 400-500 °C and 0.0003-0.3 s^-1 and both represented dynamic recrystallization（DRX）. The map revealed a wide flow instability regime at higher strain rates and temperatures lower than 400 °C, in which flow localization occurred. Forgings produced under conditions of the above two domains were sound and symmetrical, and had finer grain sizes when being forged in the first domain. However, when being forged in the flow instability regimes, the alloy fractured before the final shape was reached. The experimental load-stroke curves for the conditions within the domains correlated well with the simulated ones, whereas the curves obtained in the instability regime were uneven.

The effect of Sn, Ca, Al, Si and Zn addition on the compressive strength of cast Mg-Sn-Ca （TX） alloys was studied in the temperature range of 25-250 °C and correlated with the microstructure. The Sn to Ca mass ratio up to 2.5 contributes to the formation of Mg2Ca phase at the grain boundaries and CaMgSn in the matrix, while a ratio of 3 gives only CaMgSn phase mostly in the matrix. While the compressive strength decreases with the increase in temperature, for Sn/Ca up to 2.5, a plateau occurs in 100-175 °C, which is attributed to the strengthening by Mg2Ca. However, for ratio of 3, the strength is lower and decreases more gradually. Mg-3Sn-2Ca （TX32） has the highest strength and the addition of 0.4%Al increases its strength but simultaneous addition of Si lowers the strength. Likewise, the addition of Zn improves its strength but simultaneous addition of Al slightly decreases the strength. The results are correlated with the types of intermetallic phases that form in various alloys.
