Cutaneous Leishmaniasis as Imitator of Skin Diseases and a Diagnostic Challenge

background: Cutaneous leishmaniasis (CL) usually has a well clinical presentation that rarely confused with other dermatological diseases, but when it presents with atypical picture, it may be a diagnostic challenge to dermatologist. Objective: To record the cases of CL those have very unusual presentations or sites in the Iraqi patients that might be confused with other dermatoses. Patient and methods: This is a clinical descriptive study that has been performed in the outpatient of Department of Dermatology baghdad Teaching Hospital, baghdad, Iraq, during the period from January 2015-February 2017, where fifty six patients were enrolled in the study. The age of the patients ranged from 1 - 67 years old with median is 30 years, 32 males and 24 females with M:F ratio is 1.3:1, the diagnosis was confirmed by skin smear and/or biopsy. Results: Fifty six cases of CL with atypical presentations and unusual sites that categorized as follow: diffuse nodular 4 cases, paronychia like 1 case, sausage-like CL 4 cases, lupus vulgaris like 2 cases, rosacea like 1case, squamous cell carcinoma-like 3 cases, keratoacanthoma like 1 case, pyoderma gangrenosum-like 2 cases, erysipeloid like 1 case, b-cell lymphoma like 1 case, grouping pattern 1 case, panniculitis-like 2 cases, dermatitis like 2 cases, ecthyma like 1 cases, verruca like 1 case, keloid like 1 case, CL on the scalp 1 case, palmer CL 1 case, CL on the eyelid 1 case, CL on the ear 1 case, CL on the lips 3 cases, one genital CL case and twenty cases of sporotrichosis like spread. Conclusion: Unusual presentations of the CL with atypical lesions should be well recognized and investigated to confirm the diagnosis so that not to be confused with other skin diseases especially in countries where cutaneous leishmaniasis is not endemic

Radiomics analysis with three-dimensional and two-dimensional segmentation to predict survival outcomes in pancreatic cancer

bACKGROUND Radiomics can assess prognostic factors in several types of tumors,but considering its prognostic ability in pancreatic cancer has been *** To evaluate the performance of two different radiomics software in assessing survival outcomes in pancreatic cancer *** We retrospectively reviewed pretreatment contrast-enhanced dual-energy computed tomography images from 48 patients with biopsy-confirmed pancreatic ductal adenocarcinoma who later underwent neoadjuvant chemoradiation and *** were segmented using TexRad software for 2-dimensional(2D)analysis and MIM software for 3D analysis,followed by radiomic feature *** proportional hazard modeling correlated texture features with overall survival(OS)and progression-free survival(PFS).Cox regression was used to detect differences in OS related to pretreatment tumor size and residual tumor following *** Wilcoxon test was used to show the relationship between tumor volume and the percent of residual ***-Meier analysis was used to compare survival in patients with different tumor densities in Hounsfield units for both 2D and 3D *** 3D analysis showed that higher mean tumor density[hazard ratio(HR)=0.971,P=0.041)]and higher median tumor density(HR=0.970,P=0.037)correlated with better OS.2D analysis showed that higher mean tumor density(HR=0.963,P=0.014)and higher mean positive pixels(HR=0.962,P=0.014)correlated with better OS;higher skewness(HR=3.067,P=0.008)and higher kurtosis(HR=1.176,P=0.029)correlated with worse *** entropy correlated with better PFS(HR=0.056,P=0.036).Models determined that patients with increased tumor size greater than 1.35 cm were likely to have a higher percentage of residual tumors of over 10%.CONCLUSION Several radiomics features can be used as prognostic tools for pancreatic ***,results vary between 2D and 3D *** tumor density was the only variable that could reliably predict OS,irrespective

The Electroreduction of Gadolinium and Dysprosium Ions in Equimolar NaCl-KCl Melt

The mechanism of rare earth metals (Gd and Dy) chloride complexes electroreduction on the tungsten electrode in equimolar NaCl-KCl melt at 973 K has been studied by linear and cyclic voltammetry. Some kinetic parameters of processes were calculated. It was shown that the tungsten electrode was indifferent to gadolinium and dysprosium which were reduced on the surface. We found that the discharge mechanism of gadolinium and dysprosium chloride complexes was described by three-electron step when the steady-state conditions of polarization were limited by the mass transfer stage. The conditions of nonstationary polarization made the slowness of the charge transfer stage. The diffusion coefficient of gadolinium and dysprosium ions was calculated, the diffusion coefficient of GdCl3-6 ions was (0.9 ± 0.2) × 10-5 cm2.s-1, and for DYCI3-6 ions, it was (1.60 ± 0.2) × 10-5 сm2.s-1.

Estimation of the Economic Value of Irrigation Water in Jordan

Insight into the different values of water is essential to support rational decision making about policies, management and investments in the water sector. The main objective of this paper is to estimate an economic value of irrigation water in Jordan by choosing appropriate methodology fit with available data. The Residual Imputation Method （RIM） is used to determine the average economic value of irrigation water used in agriculture across crops. The results showed that the weighted average of water value used in field crops is JD 0.44 m＂3 and JD 1.23 m3 for vegetable crops and JD 0.23 m3 for fruit trees. The overall weighted average water value in irrigation is estimated with JD 0.51 m＂3. With regard to individual crops, cucumbers had the highest water values with about JD 6.05 m3, followed by string beans with JD 2.64 m3, and sweet pepper with JD 2.54 m3. The lowest returns per m3 were provided by squash, radish and hot pepper. For fruit tress banana has the highest water value JD 0.79 m3 and olive tress has the lowest with only JD 0.069 m3. The current practice of some banana producers is economically rational by installing Reverse Osmosis unit to irrigate banana, since water value is twice the desalination costs of one cubic meter. The estimated values represent the maximum price that farmers might be willing to pay for water under the current market conditions. Water subsidy distorts farmers＇ perception of water as a scarce and thus valuable resource. Low water prices are thus likely to engender excessive use. It is necessary to allow water prices to recover the real cost of water supply and to ensure financial sustainability of water utilities.

Flow Variation of the Major Tributaries of Tigris River Due to Climate Change

Iraq relies greatly on the flow of the Euphrates and Tigris Rivers and their tributaries. Five tributaries namely Khabour, Greater Zab, Lesser Zab, AlAdhiam and Daylia, which are the major tributaries of Tigris River, sustain Northern Iraq Region, a semi-arid, mainly a pastureland. These tributaries contribute about 24 km3 of water annually. The discharge in the tributaries, in recent times, has been suffering increasing variability contributing to more severe droughts and floods apparently due to climate change. This is because there were no dams constructed outside Iraq previously. For an appropriate appreciation, Soil Water Assessment Tool (SWAT) model was used to evaluate the impact of climate change on their discharge for a half-centennial lead time to 2046-2064 and a centennial lead time to 2080-2100. The suitability of the model was first evaluated, and then, outputs from six GCMs were incorporated to evaluate the impacts of climate change on water resources under three emission scenarios: A1b, A2 and b1. The results showed that water resources are expected to decrease with time.

Some new fluorinated 3-N-acyl/3-N-alkylaminophenyl-1,2,4-triazino[3,4-b][1,3,4]thiadiazolones (2-12) have been obtained from treatment of 2-(4'-fluorophenyl)-6-(2'-amino-5'-fluorophenyl)-1,2,4-triazino[3,4-b][1,3,4]thiadiazol-4-one(1) with active functional oxygen, sulfur and halogen compounds in different conditions. Former structures of the products have been characterized from elemental and spectral data (UV, IR, NMR and Mass). The new products were evaluated as potential anthelmintic drugs.

The Need for Cardiac Pacing after Isolated Coronary Artery bypass Graft Surgery

background: The aim of this study is to assess the necessity of temporary cardiac pacing and identify the predictors of pacing after coronary artery bypass graft (CAbG). Methods: This was a retrospective observational study of 150 patients who had isolated CAbG between November 2013 and December 2021. Patients were classified into two groups: group A, (n = 135) who did not need to be paced and group b, (n = 15) who needed temporary epicardial pacing. Only 10% (15/150) of patients in the study needed pacing. The primary outcome variable was assessment of the need for postoperative temporary cardiac pacing (TCP) (patients were paced during chest closure or at any time during hospital admission). The incidence of pacing during the postoperative period was observed. Univariate and multivariate predictors for postoperative pacing were determined and specicific predictors for cardiac pacing were found. Results: In both univariate and multivariate analysis, Old age > 65 years, diabetes mellitus, pacing to come off cardiopulmonary bypass (CPb), CPb time > 100 min, cardioversion to leave OR, antiarrhythmics to leave OR and new onset of atrial fibrillation (AF), were found to be significant predictors for the need to cardiac pacing. Conclusion: After coronary artery bypass surgery, a small percentage of patients require TCP. We emphasize unique predictors for postoperative pacing in this study.

Physiological Races of Fusarium oxysporum f. sp. Ciceris in Iraq

Twenty isolates of Fusarium oxysporum f. sp. ciceris were isolated from wilted chickpea plants obtained from different districts of north part of Iraq to assess variability in pathogenicity of the populations. Each isolate was tested on 12 differential chickpea varieties. Isolates showed highly significant variation in wilt severity on the differential varieties. based on the reaction types that induced on differential varieties, isolates were grouped into four groups, First group included isolates FocSl, FocQ7, FocQ 10, FocFI3, FocH 17 and FocHl8; the second group included isolates FocS2, FocS3, FocS4, FocQ5, FocQ8, FocQ9, FocF11, FocF12, FocFl4 and FocH19; the third group included isolates FocF15, FocHl6, FocH20; where the isolate FocQ6 was placed in the fourth group. Results showed that the percentage of genetic similarity was ranged 42% to 100% and was 42% between the first group and other groups and 72% between the three groups the rest and thus this indicate the presence of four races of the fungus which are O, 4, 5 and 1b/C, this represent the first record of these races in lraq.
