The Making of a Health Profession:How Chinese Medicine became a Nationally Registered Allied Health Profession in Australia

In 2012, traditional Chinese medicine(TCM) practitioners in Australia became nationally-registered allied health professionals in three categories: acupuncturist, Chinese herbal medicine practitioner, and Chinese herbal medicine dispenser. Australia was the first Western country to introduce national registration for Chinese medicine, followed by Portugal and, recently, New Zealand. The practice of TCM in Australia can be traced back to the beginning of Chinese immigration to Australia during the Victorian Gold Rush which began in the 1850s. The process which led to national registration commenced in the early 1970s with the establishment of the first acupuncture courses. Decades of gradual development of courses from unaccredited part-time diplomas to accredited bachelor degrees, and the development, by the profession, of national consensus on educational standards were essential elements in the process which led to registration. Professional associations, such as the Australian Acupuncture and Chinese Medicine Association Ltd.(AACMA), also developed a framework of professional selfregulation, including Codes of Ethics, Codes of Conduct, ongoing professional development requirements and Infection Control Guidelines, and provided leadership in the development of the profession. After decades of tribalism and division within the TCM profession, the National Academic standards Committee brought almost all stakeholders together to reach a consensus on the Australian Guidelines for Traditional Chinese Medicine Education which were published by AACMA in 2001. Professional associations also collaborated on joint submissions to the government in support of registration, which was introduced first in the state of Victoria in 2000, and subsequently became national in 2012. stacking is the process of overlaying inferred species potential distributions for multiple species based on outputs of bioclimatic envelope models(bEMs).The approach can be used to investigate patterns and processes of species *** data limitations on individual species distributions are inevitable,but how do they affect inferences of patterns and processes of species richness?We investigate the influence of different data sources on estimated species richness gradients in *** fitted bEMs using species distributions data for 334 bird species obtained from(1)global range maps,(2)regional checklists,(3)museum records and surveys,and(4)citizen science data using presence-only(Mahalanobis distance),presence-background(MAXENT),and presence–absence(GAM and bRT)*** species predictions were stacked to generate species richness ***,we show that different data sources and bEMs can generate spatially varying gradients of species *** environmental predictors that best explained species distributions also differed between data *** using citizen-based data had the highest accuracy,whereas those using range data had the lowest *** richness patterns estimated by GAM and bRT models were robust to data *** multiple data sets exist for the same region and taxa,we advise that explicit treatments of uncertainty,such as sensitivity analyses of the input data,should be conducted during the process of modeling.

Radiative heat transfer analysis of a concave porous fin under the local thermal non-equilibrium condition:application of the clique polynomial method and physics-informed neural networks

The heat transfer through a concave permeable fin is analyzed by the local thermal non-equilibrium(LTNE) model. The governing dimensional temperature equations for the solid and fluid phases of the porous extended surface are modeled, and then are nondimensionalized by suitable dimensionless terms. Further, the obtained nondimensional equations are solved by the clique polynomial method(CPM). The effects of several dimensionless parameters on the fin's thermal profiles are shown by graphical illustrations. Additionally, the current study implements deep neural structures to solve physics-governed coupled equations, and the best-suited hyperparameters are attained by comparison with various network combinations. The results of the CPM and physicsinformed neural network(PINN) exhibit good agreement, signifying that both methods effectively solve the thermal modeling problem.

This study investigates the efficacy of sodium alginate(sA),xanthan gum(XG),guar gum(GG)and chitosan(Cs)d each applied at five different solid biopolymer-to-water mass ratios(or dosages)and cured for 7 d and 28 d d on the unconfined compressive strength(UCs)performance of a high plasticity clayey ***,on identifying the optimum biopolymer-treatment scenarios,their performance was compared against conventional stabilization using hydrated *** a given curing time,the UCs for all biopolymers followed a riseefall trend with increasing biopolymer dosage,peaking at an optimum dosage and then subsequently decreasing,such that all biopolymer-stabilized samples mobilized higher UCs values compared to the unamended *** optimum dosage was found to be 1.5%for sA,XG and Cs,while a notably lower dosage of 0.5%was deemed optimum for ***,for a given biopolymer type and dosage,increasing the curing time from 7 d to 28 d further enhanced the UCs,with the achieved improvements being generally more pronounced for XG-and Cs-treated *** of the investigated biopolymers was able to produce UCs improvements equivalent to those obtained by the 28-d soilelime samples;however,the optimum XG,GG and Cs dosages,particularly after 28 d of curing,were easily able to replicate 7-d lime stabilization outcomes achieved with as high as twice the soil’s lime ***,the fundamental principles of clay chemistry,in conjunction with the soil mechanics framework,were employed to identify and discuss the clayebiopolymer stabilization mechanisms.

background Ni-kshay Poshan Yojana (NPY) is a direct benefit transfer scheme of the Government of India introduced in 2018 to support the additional nutritional requirements of persons with Tb (PwTb). Our recent nationwide evaluation of implementation and utilization of NPY using programmatic data of PwTb from nine randomly selected Indian states, reported a 70% coverage and high median delay in benefit credit. We undertook a qualitative study between January and July 2023, to understand the detailed implementation process of NPY and explore the enablers and barriers to effective implementation and utilization of the NPY *** We followed a grounded theory approach to inductively develop theoretical explanations for social phenomena through data generated from multiple sources. We conducted 36 in-depth interviews of national, district and field-level staff of the National Tuberculosis Elimination Programme (NTEP) and NPY beneficiaries from 30 districts across nine states of India, selected using theoretical sampling. An analytical framework developed through inductive coding of a set of six interviews, guided the coding of the subsequent interviews. Categories and themes emerged through constant comparison and the data collection continued until theoretical *** stakeholders perceived NPY as a beneficial initiative. strong political commitment from the state administration, mainstreaming of NTEP work with the district public healthcare delivery system, availability of good geographic and internet connectivity and state-specific grievance redressal mechanisms and innovations were identified as enablers of implementation. Functional Capacity of Patients with Venous Thromboembolic Disease six to Twelve Months after Treatment

background: Venous thromboembolism (VTE) is among the leading causes of hospital-related disability-adjusted life years lost. We aimed to determine the prevalence and determinants of functional capacity impairment six to twelve months after an acute VTE event. Methods: This was a cross-sectional study conducted between January and April 2021 in two referral hospitals of Yaoundé, including consenting adult patients admitted to these hospitals six to twelve months ago for VTE. We excluded dead patients and those with any comorbidity or symptoms limiting physical activity. The functional outcome was assessed with the six-minute walk test. Functional capacity impairment was defined as walking distance lower than the expected value. Results: We included 27 cases in this study with a mean age of 53.2 ± 14.4 years. The prevalence of functional capacity impairment was 29.6% (95% CI: 14.8 - 48.1). Factors associated with poor functional outcome were obesity (OR: 59.5;95% CI: 4.6 - 767.2;p - 207.4;p = 0.017), massive PE (OR: 30;95% CI: 2.5 - 354;p = 0.004), and poor adherence to treatment (OR: 30.3;95% CI: 2.5 - 333.3;p = 0.004). Conclusion: Functional capacity impairment is common in the medium-term after VTE and factors associated with this poor outcome are obesity, the severity of the VTE, and poor adherence to treatment.
