Efficacy Study of Metronomic Chemotherapy in Metastatic Triple Negative breast Cancer and Correlation with VEGF, TSP Levels

background: Treatment refractory metastatic breast cancer patients are at best treated palliatively. We evaluated the effects of metronomic chemotherapy on survival outcomes in this population. Methods: Twenty eight subjects with treatment refractory (n = 21) and treatment naive (n = 7) MbC were included in an open label single arm efficacy study of metronomic chemotherapy. Patients were given a chemotherapy regimen of Tab. Cyclophosphamide 50 mg once daily and Tab. Methotrexate 2.5 mg twice in a week over a minimum period of 3 months or until the progression of their disease whichever was earlier. Monitoring of serum VEGF and Thrombospondin levels were done to correlate the response rates. Data were analysed using chi square test for proportions and Kaplan Meir Survival analysis. Results: The mean age of the study population was 51.5 ± 14.2 years. The mean duration of metronomic chemotherapy was 123.89 ± 97.6 days. Overall 71.4% had progressive disease and 28.6% had stable disease. 55.6% with treatment naive metastatic breast cancer had stable disease compared to 15.8% of treatment refractory metastatic breast cancer. There was also a significant improvement in progression free survival in those with tumor load less than 5 cms compared to >5 cms and in grade 2 compared to grade 3 disease. There was no correlation of serum VEGF levels before and after chemotherapy. There is no significant decrease in TSP levels. Conclusion: The results suggest stable response in one third of study patients. Performance status and tumor load are important predictors in this category of population. There is no significant correlation of serum VEGF and TSP levels before and after chemotherapy. Also, there was no significant correlation of biomarker levels in responding and non-responding patients.

Clinical Profile and Results in Cancers Treated with Nivolumab: A Single Centre Study

Introduction: Immunotherapy is also called as biologic therapy or bio-therapy. Immunotherapy is quickly becoming an important component in the multipronged approaches being developed to treat certain forms of cancer. The goal of immunotherapy is to strengthen the body’s own natu-ral ability to combat cancer by enhancing the effectiveness of the immune system. This study tries to compile response rates and side effect profile of Nivolumab. Methods: Patients with performance status of 3 or less than 3, who failed on 2 or more lines of therapy except melanoma and have received minimum 3 cycles of Nivolumab were taken in to this study. Study subjects followed for minimum of 3 cycles for assessment. Each patient received 240 mg of Nivolumab once in 15 days for 3 - 6 cycles. Results: The median age of 20 patients was 53 years (26 - 79 yrs). 18 patients were male and 2 patients were female. Progressive disease (PD) was observed in 12 patients with immunotherapy treatment and partial response (PR) was observed in 3 patients, and stable disease (SD) in 5 patients. Conclusion: This is one of the Indian study mainly reporting experiences with Nivolumab. Large sample size is required to conclude about the efficacy of the study. Nivolumab is more tolerated in Indian patients compared to other reported studies.

Diffusion-weighted magnetic resonance imaging to predict response of hepatocellular carcinoma to chemoembolization

AIM: To investigate whether intra-procedural diffusion- weighted magnetic resonance imaging can predict response of hepatocellular carcinoma (HCC) during trans- catheter arterial chemoembolization (TACE). METHODS: Sixteen patients (15 male), aged 59 ±11 years (range: 42-81 years) underwent a total of 21 separate treatments for unresectable HCC in a hybrid magnetic resonance/interventional radiology suite. Ana- tomical imaging and diffusion-weighted imaging (b = 0, 500 s/mm2) were performed on a 1.5-T unit. Tumor enhancement and apparent diffusion coefficient (ADC, mm2/s) values were assessed immediately before and at 1 and 3 mo after TACE. We calculated the percent change (PC) in ADC values at all time points. We compared follow-up ADC values to baseline values using a paired t test (α = 0.05). RESULTS: The intra-procedural sensitivity, specificity, and positive and negative predictive values (%) for detecting a complete or partial 1-mo tumor response using ADC PC thresholds of ±5%, ±10%, and ±15% were 77, 67, 91, and 40; 54, 67, 88, and 25; and 46, 100, 100, and 30, respectively. There was no clear predictive value for the 3-mo follow-up. Compared to baseline, the immediate post-procedure and 1-mo mean ADC values both increased; the latter obtaining statistical significance (1.48 ± 0.29 mm2/s vs 1.65 ± 0.35 × 10-3 mm2/s, P 15% predicted 1-mo anatomical HCC response with the greatest accuracy, and can provide valuable feedback at the time of TACE.

In the present scenario, there has been a rapid attention in research and development in the natural fiber composite field due to its better formability, abundant, renewable, cost-effective and eco-friendly features. This paper exhibits an outline on natural fibers and its composites utilized as a part of different commercial and engineering applications. In this review, many articles were related to applications of natural fiber reinforced polymer composites. It helps to provide details about the potential use of natural fibers and its composite materials, mechanical and physical properties and some of their applications in engineering sectors.

Esophageal achalasia in children is rare but ultimately requires endoscopic or surgical treatment. Historically, Heller esophagomyotomy has been recommended as the treatment of choice. The refinement of minimally invasive techniques has shifted the trend of treatment toward laparoscopic Heller myotomy(LHM) in adults and children with achalasia. A review of the available literature on LHM performed in patients ble data suggest that LHM is safe and effective. There is a paucity of evidence on the need for and superiority of concurrent antireflux procedures. In addition, a more complete portrayal of complications and long-term(> 5 years) outcomes is needed. Due to the infrequency of achalasia in children, these characteristics are unlikely to be defined without collaboration between multiple pediatric surgery centers. The introduction of peroral endoscopic myotomy and single-incision techniques, continue the trend of innovative approaches that may eventually become the standard of care.

b>Introduction:b> Apart from smoking as the known risk factor for lung cancer, recent developments implicate occupational exposure to carcinogens, indoor air pollution and dietary factors as other causative agents. In our study, we have analyzed the clinical and pathological profile of lung cancer patients treated at our center over a period of 8 years. b>Aim:b> To find the demographic and clinicopathological profile of lung cancer patients admitted to the oncology unit. b>Methods:b> This retrospective record based analysis includes a cohort of 1248 patients diagnosed with lung cancer, at a tertiary cancer care center in bangalore, South India. This study includes data of patients admitted during the period 2010 to 2018, retrieved from the Hospital’s emergency medical records (EMR). Their demographic profile, clinical correlates, radiological profile and diagnostic details were studied. benign tumours, malignant pleural disease and sarcomatoid tumors were excluded from this study. b>Results:b> Adenocarcinoma (AC) was detected among 70.4% of patients, Squamous cell carcinoma (SCC) among 15.3% and Small cell lung cancer (SCLC) among 14.3% of lung cancer admissions. Male to female ratio was 2.95:1. It was found that the median age of lung cancer patients was 61 years. >60% of lung cancer patients were from the 51 - 70 year age-group. Across the three types of cancers among the patients in Stage 3b to 4 strata, >35% received palliative chemotherapy and >20% received palliative chemotherapy + radiation. b>Conclusion:b> In our study, AC is the most common histological subtype (>70%) of lung cancer. The outcome of lung cancer patients can be considerably impacted by addressing risk factors through preventive measures implemented in the community.

A pot culture experiment was carried out for the Screening of 11 popular genotypes against to Meloidogyne incognita and Fusarium chlamydosporum wilt complex on Coleus forskohlii. Genotype viz., yellow tubers and Rabakavi local showed resistant reaction, Orange tuber and Nimbanur local showed moderately resistant reaction. Rest of the seven genotvDes showed suscentible reactions.
