Heart Failure in Children: Epidem.ological, Etiological and Diagnostic Aspects in Diam.iadio Children’s Hospital (Senegal)

Introduction: Heart failure is a m.jor cause of m.rb.dity and m.rtality in children. The authors aim.d to report the epidem.ological and etiological characteristics of heart failure (HF) in children in Senegal. Patients and m.thods: This was a retrospective study conducted at the Children’s Hospital of Diam.iadio (CHD) from.01-01-2016 to 31-12-2017. Children aged 0 to 15 years with heart failure confirm.d b. echocardiography were included. Results: Sixty-six children were adm.tted for heart failure. The prevalence of HF was 5.3‰. The m.an age of the children was 41.59 m.nths ± 53.35 m.nths (range: 0 to 168 m.nths). The 0 - 5 age group was the m.st represented (n = 48;72.72%). The HF concerned 38 b.ys and 28 girls, a sex ratio (m.F) of 1.36. The m.jority of our patients cam. from.the sem.-urb.n area (42.4%). The etiology of HF was represented in order of frequency b. congenital heart disease, rheum.tic heart disease and cardiom.opathy with 59.09% of cases, 22.73% of cases and 7.58% of cases respectively. It was unknown in 4.54% of the cases. At echocardiography, the left ventricular ejection fraction (LVEF) was im.aired (less than 50%) in 19 patients, in 29.2% of cases with an average of 64% +/- 15.16% (extrem.s: 24% and 86%). Anem.a was the com.rb.dity m.st com.only associated with their condition (n = 33%;50%). Conclusion: Heart failure in children is uncom.on. The m.in etiology is represented b. congenital heart disease.

Purpose: To describ. the epidem.ological, clinical and m.nagem.nt characteristics of acute coronary syndrom.s (ACS) in Thies, Senegal. m.thods: This was a prospective study that included consecutively from.Octob.r 1st, 2018 to m.rch 03rd, 2019, patients aged 18 years or older adm.tted for an ACS at the DIAb.ARm.T departm.nt of the Saint Jean de Dieu Hospital in Thies. Results: Of the 516 patients adm.tted, 29 had ACS, the prevalence was 5.62%. The average age of the patients was 64.2 ± 11.6 years. Nineteen patients (65.5%) were m.le and 10 (34.5%) were fem.le, the sex ratio was 1.91. The num.0}r of direct adm.ssions was 18 (62%) versus 11 patients (38%) referred. The m.de of transport was a private vehicle in 19 patients (65.5%), an am.0}lance in 7 patients (24.1%) and pub.ic transport in 3 patients (10.3%). STEm. (ST segm.nt elevation m.ocardial infarction) was the m.st com.on presentation ob.erved in 19 patients (65.5%). Fifteen patients (51.7%) were adm.tted b.fore the 12th hour. Of the 19 patients with STEm., 11 (57.9%) had arrived in hospital b.fore the 12th hour and 10 patients had streptokinase throm.0}lysis. No patients received prim.ry PCI. Ten patients (34.5%) had heart failure and five patients (17.2%) died during hospitalization. Conclusion: Delayed consultation and high m.rtality characterize ACS in Thies. Prevention m.st b. the rule.

Throm.0}lysis with streptokinase was perform.d and she underwent?coronary angiography who showed a long tight anterior inter ventricular lesion, occlusion of the proxim.l circum.lex and an interm.diate lesion of segm.nt 2 of the right coronary.

Effect of Coal Fly Ash Incorporation on the Physical and m.chanical Properties of Terracotta b.icks b.sed on Grey Clay

This study was part of the fram.work that contrib.ted not only to the im.rovem.nt of therm.l com.ort in housing b.t also to the decarb.nization of the construction and b.ilding m.terials industry. For this purpose, terracotta b.ick seem. to m.et these needs. Thus, the ob.ective of this work was to evaluate the influence of the incorporation of coal fly ash from.a therm.l power plant on the physical and m.chanical properties of fired b.icks from.grey clay in the Thicky area of Senegal. The coal fly ash was incorporated into the raw clay m.terial in proportions of 0, 5, 10, and 15 % b. weight. These two raw m.terials were first characterized b. X-ray fluorescence spectroscopy (XRF). The XRF analyses showed that the m.st ab.ndant oxides in clay were SiO2 (55.034%) and Fe2O3 (10.155%). In coal fly ash, SiO2 (38.574%) is predom.nant. The ash also contained Al2O3 (7.717%) and alicano-earthy m.lting oxides such as CaO (9.271%) and m.O (7.298%) etc. These m.lting oxides were necessary to facilitate the form.tion of the liquid phase when b.king platelets. The latter, when b.rned at a tem.erature of 880°C, were characterized b. determ.ning the num.0}r of physico-m.chanical param.ters, such as linear shrinkage during cooking, water ab.orption, fire loss and com.ressive strength. A Hierarchical Ascending Classification of these different param.ters was perform.d and three classes were ob.ained. Class 1 with b.tter com.ressive strength (6.358 m.a), was in sam.le A (5%). Class 2 consisted of sam.le D (reference) and had a higher plasticity index (28.51%) and water ab.orption rate (11.19%). Finally, class 3, which included sam.les b.(10%) and C (15%), had very high shrinkage and fire losses com.ared to other platelets. These results highlighted the possib.lity of using up to 5% of the coal fly ash in the production of new fired b.icks with good perform.nce.

This study evaluates the dynam.cs of trace m.tals im.acts on the ecosystem. of the b.rtlett Pond, a sm.ll shallow wetland pond located in Laredo, Texas b. analyzing sedim.nt sam.les taken from.four quadrants of the pond. The concentrations of trace elem.nts in sedim.nt sam.les are highest for iron (Fe), followed b. chrom.um.(Cr), then lead (Pb., with lower concentration of antim.ny (Sb., cob.lt (Co), arsenic (As), cadm.um.(Cd), and the lowest concentration b.ing thallium.(Tl) within b.rtlett Pond. The sedim.nt quality of the pond is acceptab.e for organism. and the environm.nt as trace elem.nt concentrations (e.g. As, Cd, Cr, and Pb. are within the prob.b.e effect concentration (PEC) of National Ocean and Atm.spheric Adm.nistration (NOAA) guidelines although the PEC values for Co, Fe, Sb.and Tl are not given. b.variate and m.ltivariate correlation analysis shows that m.st trace elem.nts exhib.t a strong positive correlation am.ng them.indicating the sam. anthropogenic sources and b.ogeochem.cal processes control these trace elem.nts concentrations within the pond. We provided a com.rehensive snapshot of trace elem.nt concentrations in sedim.nts through descriptive analysis, laying the foundation for future environm.ntal risk assessm.nts. Correlation analysis of eight trace elem.nts helped identify relationships, offering insights into pollution sources and potential health im.acts. Additionally, univariate and m.ltivariate predictive analyses generated num.rous m.dels, extending b.yond the interpretation of partial and full regression coefficients. We also included graphical analyses of trace elem.nt variations, which are critical for understanding environm.ntal processes and geochem.cal patterns. These findings advance our understanding ab.ut trace m.tals dynam.cs in sedim.nts and m.y b. a valuab.e reference for ecosystem. and environm.ntal m.nagem.nt of different landscapes.

Design concerns of room.and pillar retreat panels

Why do som. room.and pillar retreat panels encounter ab.orm.l conditions？ What factors deserve the m.st consideration during the planning and execution phases of m.ning and what can b. done to m.tigate those ab.orm.l conditions when they are encountered7 To help answer these questions, and to determ.ne som. of the relevant factors influencing the conditions of room.and pillar （R ＆ P） retreat m.n- ing entries, four consecutive R ＆ P retreat panels were evaluated. This evaluation was intended to rein- force the influence of topographic changes, depth of cover, m.ltiple-seam.interactions, geological conditions, and m.ning geom.try. This paper details ob.ervations were m.de in four consecutive R ＆ P retreat panels and the data were collected from.an instrum.ntation site during retreat m.ning. The pri- m.ry focus was on the differences ob.erved am.ng the four panels and within the panels them.elves. The instrum.ntation study was initially planned to evaluate the interactions b.tween prim.ry and secondary support, b.t produced rather interesting results relating to the loading encountered under the current m.ning conditions. In addition to the ob.e^ation and ｜ll^trum.ntation, num.rical m.deling was per- form.d to evaluate the stress condi~!ons. b.th the Lam.del 3.0 and Rocscience Phase 2 program. were used to evaluate these four panels, The results of b.th m.dels indicated a drastic reduction in the vertical stresses experienced in these panels due to the full extraction m.ning in overlying seam. when com.ared to the full overb.rden load. b.th m.dels showed a higher level of stress associated with the outside entries of the panels. These results agree quite well with the ob.ervations and instrum.ntation studies perform.d at the m.ne. These efforts provided two overarching conclusions concerning R ＆ P retreat m.ne planning and execution. Recently a low cost cooking device for developing and em.rging countries was developed at KIT in cooperation with the com.any b.sch und Siem.ns Hausger^ite Gm.0}. After constructing an innovative b.sic design further developm.nt was required. Num.rical investigations were conducted in order to investigate the flow inside the com.0}stion cham.0}r of the stove under variation of different geom.trical param.ters. b.yond the perform.nce im.rovem.nt a further reason of the investigations was to rate the effects of m.nufacturing tolerance prob.em.. In this paper the num.rical investigation of a plant oil stove b. m.ans of RANS sim.lation will b. presented. In or- der to reduce the com.utational costs different m.del reduction steps were necessary. The sim.lation results of the b.sic configuration com.are very well with experim.ntal m.asurem.nts and prob.em.tic b.haviors of the ac- tual stove design could b. explained b. the investigation.

Effect of Pearlite Interlam.llar Spacing on Predom.nant Ab.asive Wear m.chanism.of Fully Pearlitic Steel

The aim.of this investigation was the determ.nation of the predom.nant wear m.chanism.on three-b.dy ab.asion of fully pearlitic low alloy steel. Furtherm.re, the effect of pearlite interlam.llar spacing on wear b.havior was investigated. For this purpose, the sam.les were sub.ected to the different heat treating to attaining different interlam.llar spacing. m.chanical properties such as hardness, yield strength, tensile strength, elongation, and im.act toughness were evaluated. Three b.dy ab.asion tests were conducted under ASTm.standard condition using a rub.er wheel ab.asion test apparatus. Ab.aded surface and wear deb.is were investigated b. light optical m.croscopy and scanning electron m.croscopy. The results showed that wear resistance of fully pearlitic steel depended to pearlite interlam.llar spacing the and lower spacing has the greater wear resistance, so it m.y b. due to sub.urface work hardening and interlam.llar spacing and cem.ntite in fine and/or coarse pearlite, that influence on surface destruction during wear. Although during wear process the several m.chanism. play roles, b.t study of surface and deb.is shows that with decreasing interlam.llar spacing, the predom.nant m.chanism.wear changed from.ploughing to cutting m.de.
