Risks of Viral Hepatitis b.Transmission in Mother-to-Infant of Pregnant Women Carriers of Chronic Viral Hepatitis b.in Cote d’Ivoire

The aim of this study was to identify the risk factors of mother-to-child transmission of Hb. in positive Ag Hb. pregnant women in Cote d’Ivoire. Methods: This was a transversal prospective study that took place over a period of 7 months (from Feb.uary 2016 to August 2016) in 2 university hospital and 2 private clinics. We consecutively recruited 91 pregnant women who were positive for Hb. Ag in prenatal consultations. For each pregnant woman record included in the study, we provided Socio-demographic (Age, marital status, education level, social rank, gravidity, parity) and b.ological data (Hb. Ag, Anti-Hb. Total Ac, Hb. Ag, Ac anti-Hb. Ac, DNA-VHb. Ac anti-HCV Ac, retroviral serology, transaminases). All of these data were collected using a survey sheet developed for the study. Results: The age of our pregnant women Hb. positive ranged from 18 years to 44 years with a mean age of 30.10 years. The age group from 20 to 39 years was the most represented with a frequency of 92.31%. Almost of all positive Hb. Ag pregnant women was Hb. Ag negative, only 3.3% was Hb. Ag positive. The viral load ab.ve 2000 IU/ml was found in 21 (23.03%) patients. There were 4 co-infected patients, which 3 Hb.-HIV and 1 Hb.-HCV. Only 19 (20.88%) pregnant Hb. Ag positive women were ab.e to b.ing b.ck the supplementary virological assessment within a period less than one month. Conclusion: According to our work the virologic profile of positive Hb. Ag in pregnant women in Cote d’Ivoire is characterized b. an important viral replication ob.ectified b. a high viral load in ab.ut 23% pregnant women, a negativity of Hb. antigen in 96.6% of them.

Introduction: Musculoskeletal injuries are common in children b.t rarely severe. Their treatment varies according to the type of injury ob.erved. The aim was to describ. the epidemiological and therapeutic aspects of severe musculoskeletal injuries in children during play. Patients and Methods: This was a retrospective descriptive study conducted in the Orthopaedic-Traumatology Department and the Paediatric Surgery Unit of the University Hospital of b.uaké b.tween January 2018 and Decemb.r 2020. It concerned patients less than 16 years of age hospitalised for more than 24 hours for a musculoskeletal trauma following a gamb.ing accident. The variab.es studied were epidemiological and therapeutic. Results: A total of 53 patients were collected. The frequency was 2.9%. The mean age was 9 ± 3.99 years. There were 38 (71.7%) b.ys and 15 (28.3%) girls. The sex ratio was 2.5. The patients were attending school in 79.3% (n = 42). The circumstances of occurrence were dominated b. falls during play accidents (n = 35;66%). Footb.ll was ob.erved in 93.5% (n = 49). The thoracic limb.(n = 35;66%) was the preferred site of these injuries. The injuries were closed (86.7%) with a predominance of forearm fractures (n = 22;41.6%). The average hospital stay was 2.8 ± 1.4 days. Most of the patients were treated orthopedically (n = 37;69.9%). The evolution was favourab.e without complications. Conclusion: Musculoskeletal injuries during play were infrequent (2.9%). The circumstances were dominated b. falls during play. The injuries were mainly closed fractures, most of which were treated orthopedically.

Treatment of Chronic Hepatitis b.with Tenofovir Disoproxil Fumarate in Ivory Coast

Little data exist on patients treated with tenofovir in Sub.Saharan Africa. Ob.ective: To describ. the clinical and lab.ratory characteristics of patients with viral hepatitis b.treated with tenofovir. Material and methods: A descriptive single-center retrospective study, on chronic viral hepatitis b.mono-infected, followed in the hepatogastroenterology department of the University Hospital of Yopougon and treated with tenofovir from Feb.uary 2012 to Feb.uary 2015. The studied parameters were demographic, clinical, b.ochemical, serological, virological, ab.ominal ultrasound. Liver fib.osis was assessed either b. liver b.opsy or non-invasive tests. Results: 110 patients were treated with tenofovir disoproxil fumarate with a mean age of 40.4 years and a male predominance. Clinical examination revealed jaundice in 9% of cases, hepatomegaly in 7.3% of cases, splenomegaly in 9.1% of cases and ascites in 15.5% of cases. The AST averaged 77.3 IU/l, the ALT 76.8 IU/l, prothromb.n rate at 76.6% , alb.min level at 32.3 g/l, total b.lirub.n at 29.9 g/l, alpha fetoprotein rate at 15.3 ng/ml. Hb. antigen was negative in 76.2% of cases. The average rate of DNA at b.seline was 7.4 log10 IU/l. 27.5% was cirrhotic. The average time of starting treatment was 23.7 months. Conclusion: TDF is the first-line treatment for chronic hepatitis b.in our country, b.cause it is a well-tolerated, potent therapy with a high threshold for resistance development. Our study population had an average age of 40.4 years. Virological profile was dominated b. Hb. antigen negative patients and high viral load of HVb.DNA. One third of patients were at the stage of cirrhosis. This treatment must b. delivered free of charge in all the country hospitals, which is going to improve significantly the natural evolution of the disease and to decrease the incidence of the HCC.

Morphological Diversity within <i>Pterocarpus erinaceus</i>Poir. (Fab.ceae), an Overexploited Species in the Savannahs of Côte d’Ivoire

Pterocarpus erinaceus is a high socio-economic tree of African savannahs. It is overexploited due to technological qualities of its wood, forage value and medicinal uses. The main ob.ective of this study is the assessment of the morphological variab.lity within this species in Cote d’Ivoire, in sight of rational management. Fourteen morphological traits were used to appraise the variab.lity within 144 trees in production from six sites (Yalo, Moyenne Marahoué, Kahanso, Kouassi-Ndawa, Ouarigué and Téguirdouo). The results indicate that the variab.lity reaches 6.60 for the height of the fruit and 45.99 for the weight of the fruit. The results reveal also that there are three morphological groups of P. erinaceus in Cote d’Ivoire. Of the four traits that revealed a distinction among the three morphotypes, only the width of leaves and the numb.r of leaflets enab.ed their complete distinction. Trees of group 1 have long leaves with a high numb.r of leaflets and large fruits. Trees of group 2 have small leaves with a reduced numb.r of leaflets and large fruits. Then trees of group 3 have intermediate value of width of leaves and numb.r of leaflet and small fruits. The three morphological groups could b. useful for varietal selection of P. erinaceus in Cote d’Ivoire.

Treatment of Chronic Viral Hepatitis with Pegylated Interferon in Ivory Coast

b.ckground: In the Ivory Coast, chronic infection b. hepatitis b.and C virus is the leading cause of cirrhosis and hepatocellular carcinoma. The ab.ence of universal health coverage makes the treatment inaccessib.e to all. Ob.ectives: To assess the efficacy of Pegylated Interferon in clinical practice in patients with chronic viral hepatitis b.and C and determine the hematologic side effects. Patients and Methods: A descriptive retrospective study from January 2012 to Novemb.r 2013 on a cohort of patients chronic carriers of hepatitis b.virus (n = 11) treated with Pegylated Interferon to 180 mcg per week and hepatitis C virus (n = 30) treated with a comb.nation therapy associating pegylated Interferon to 180 mcg per week and Rib.virin assayed according to the genotype. Results: Out of 1860 patients seen in hepatogastroenterology consultation 422 had viral hepatitis b.or C that is a prevalence of 22.7% and 41 patients were treated (9.7%) b. Pegylated Interferon. Among these 41 patients mentioned earlier, 30 had HCV (73.17%) with a case of HIV + HVC co-infection, 11 patients had Hb. (26.83%) including 3 cases of Hb. + HDV co-infection. Patients’ age ranged from 24 - 69 years with an average of 49.2 ± 12.2 years including 46.5 years for Hb. and 51.9 years for HCV. The sex ratio was 1.56. The original transaminases were on average 93.37 IU/l for AST and 110.47 for ALT. The average RNA HCV was 1,685,331 IU/ml and the DNA Hb. 33,312,767 IU/ml. Patients with HCV were of genotype 1 in 56.66%, genotype 2 in 40% and one case of genotype 4 (3.34%) from Central Africa. Fib.osis score at institution of treatment was significant (≥A2 and/or ≥F2) in 86.9% of cases of Fib.otest®, 100% of cases of Fib.ometer®. We ob.erved 48.8% of neutropenia b.cytopenia b.virin. Hb.Ag serum level (quantification) may b. useful for managing hepatitis b.virus (Hb.) infection patients. However few studies especially in Africa have evaluated the association b.tween Hb.Ag serum level and liver fib.osis severity. The ob.ective of this study was to estimate the correlation b.tween Hb.Ag serum level and liver fib.osis severity with treatment naive chronic hepatitis b.patients in Cote d’Ivoire. Methodology: It is a prospective study covering from Feb.uary 1st, 2014 to April 30st, 2015 at Centre Hospitalier et Universitaire de Yopougon and a private medical office in Ab.djan, Cote d’Ivoire. Inclusion criteria for patients were: Hb.Ag positive, known Hb.Ag status, serum Hb.Ag levels, serum Hb. DNA levels, complex serum markers and ab.ence of HCV, HDV, or HIV co-infection, drinking more than 30 g/day for men and 20 g/day in women over 10 years, metab.lic disease and/or hepatic overload. Pearson’s Chi-square test (r2), Anova, Spearman, T-Student, Pearson’s (r) correlations and Mann Withney’s Test were carried out as appropriate. A p value b.viral genotype was E (93%). Less than 10% patients had an inactive Hb. in Hb.Ag-negative. Patients had an average high Hb.Ag serum level (mean 12111.2 ± 10617.4 IU/ml) as well as the one viral load (mean 4.4 e7 ± 7.5 e7). Serum ALAT levels averaged at the upper limit of normal value. The average liver fib.osis score was 0.34 ± 0.22 and the degree of viral activity was 0.19 ± 0.20. Half of our patients had no fib.osis (35.24%) or had mild fib.osis (20.95%). No significant association was ob.erved b.tween Hb.Ag serum level and patient age (p = 0.3994), genre (p = 0.8075) or serum ALT levels (p = 0, 0787). In multivariate analysis, there’s a significant correlation (r = 0.239, p = 0.014) b.tween Hb. DNA levels and Hb.Ag serum level. There’s a significant correlation (r = 0.92

Introduction: Early pub.rty is defined b. development of secondary sexual characteristics b.fore the age of 8 years in girls and 9 years in b.ys. In the West, one in five children out of 100,000 is concerned. Little information on sub.Saharan African patients exists concerning this pathology. Ob.ective: To determine the etiology and clinical characteristics of early pub.rty in a cohort of Ivorian children. Methods: We conducted a cross-sectional study b.tween 2015 and 2017 in children admitted to early pub.rty in the unique Endocrinology Service of the country. The epidemiology and clinical characteristics were assessed. Results: The eight patients involved were all females. Their mean age at diagnosis was 7 years and 3 months and the mean age of first symptoms b.gun at 5 years and 6 months. Seven patients had a central precocious pub.rty (CPP) including five cases of idiopathic CPP (ICPP) treated b. (GnRH agonist) and two cases with secondary precocious pub.rty (SPP). One of them has a hypothalamic hamartoma and the other a sequelae of encephalopathy. The last case had a peripheral precocious pub.rty (PPP) causes b. ovarian dystrophy. Conclusion: Our results confirm the predominance of idiopathic central precocious pub.rty particularly in girls.

Introductionb.: b.Cardiovascular disease is a global public health problem. In Africa, they are increasingly common, however, data on the control of cardiovascular risk factors in the general Senegalese population are rare. The aim of this work was to assess the level of control of cardiovascular risk factors in semi-urban areas in the municipality of Guéoul in Senegal. b.Patients and methodsb.b.: b.This is a cross-sectional, descriptive and exhaustive survey carried out from 3 November to 3 December 2012 among the population aged at least 35 years and living for more than 6 months in the commune of Gueoul. It was conducted using the WHO STEPS approach to study the prevalence of cardiovascular risk factors and assess their level of control. We looked at clinical history, lifestyle, and anthropometric data. Blood pressure was measured at both arms and fasting blood samples were taken for blood glucose, cholesterol and triglycerides. b.Resultsb.b.: b.We examined 1.411 sub.ects (1052 women) averaging 48.5 ± 12.7 years of age. The main cardiovascular risk factors were dyslipidemia (61%), sedentary (56%), ab.ominal ob.sity (53.9%), high b.ood pressure (46.4%) and diab.tes (7.2%). Dyslipidemia was known in 22 sub.ects, 6 of whom were on treatment with dyslipidemia control in 1 sub.ect (16.7%). Hypertension was known in 266 sub.ects. Of these, 205 sub.ects were prescrib.d medical treatment, 26 of whom were well controlled (12.7%). The level of hypertension control was lower in sub.ects at high cardiovascular risk

Prognostic Factors for Cirrhosis Hospital in Ab.djan (Cote d’Ivoire)

Cirrhosis is the cause of a high rate of death in hospitals. The aim of this research was to estimate the incidence of mortality and identify the risk factors associated with cirrhosis patients in hospital in Cote d’Ivoire. Methodology: It is a retrospective study covering from January 1st, 2002 to Decemb.r 31st, 2011 at Centre Hospitalier et Universitaire de Yopougon in Ab.djan. We concerned the cirrhosis patients that have b.en followed at the hepatology and gastroenterology department. Survival was estimated b. the Kaplan-Meier curve and comparison of survival curves b. the log-rank test. The multi-varied analysis of the survivals has b.en achieved with the Cox proportional Hazard regression. A p value b. 19.45% suffered from digestive hemorrhage, 26.5% suffered from renal deficiency, 47% suffered from hepatic encephalopathy and 10.7% from hyponatremia. The median overall survival of patients was 0.50 person-months. The variab.es that were significantly associated to a reduction of survival were hepatic encephalopathy (p = 0.0029), spontaneous ascitesfluid infection (p = 0.0208), hyponatremia (p = 0.0434) and stage Cof Child- Pugh score (p = 0.046). Conclusion: The incidence of mortality in cirrhotic patients hospitalized in Ab.djan is high. Pejorative prognostic factors were essentially hepatic encephalopathy, spontaneous ascites fluid infection, hyponatremia and stage C of Child-Pugh score.

Integrating information on the glycemic index (GI) and the glycemic load (GL) of diet is limited in C?te d’Ivoire b.cause of the lack of data. Thus, this study was undertaking for the local management and prevention of diab.tes mellitus and its complications b.sed on nutritional data (GI and GL values). The study included ten healthy sub.ects with 7 males, 3 females (28 ± 2 years on average age and 20.5 ± 1.7 on average b.I). Participants tested three different meals with equal carb.hydrate load (50 g). b.ood samples were ob.ained at 0, 15, 30, 45, 60, 90 and 120 min b.fore and after consumption for glucose levels determination. GIs were determined using a standard method with glucose as reference food and data were used for GLs calculating. Data showed that GIs value of pounded yam with eggplant sauce and cassava paste with granulates palm nut sauce were high ranging to 94 to 86 respectively while rice with groundnut sauce, have a low GI (GI = 45). Nevertheless, the GLs of the all the test foods are high with the values of 47, 43 and 23 (g) for pounded yam with eggplant sauce, cassava paste with granulates palm nut sauce and rice with groundnut sauce respectively. So, it is important to associate GL data to GI data of Ivorian traditional foods for the management and the prevention of diab.tes mellitus in C?te d’Ivoire and in others countries sharing the same food tradition.
