Sarcoidosis is a chronic multisystemic granulomatous disease of unknown origin, which can involve nearly all organs. In the case of an infrequent gastrointestinal tract involvement in systemic sarcoidosis, granulomas of the liver are most commonly describ.d while isolated pancreatic sarcoid lesions are rarely seen. We report a case of systemic sarcoidosis with exclusive extrapulmonal involvement of the liver and the pancreas in a 71-year- old white man. The diagnosis of liver involvement was confirmed b. b.opsy. Pancreatic surgery was needed b.cause preoperative evaluation could not exclude pancreatic cancer and for b.liary decompression. An extensive literature review of systemic sarcoidosis, focusing on reported cases with unusual presentation of sarcoidosis in the liver and the pancreas, its diagnosis, treatment, and prognosis was made.

PURPOSE: To evaluate the rate of infectious and noninfectious endophthalmitis after intravitreal injection of a high-dosage of triamcinolone acetonide. DESIGN: Clinical interventional case-series study. METHODS: The study included 1135 intravitreal injections of approximately 20 mg triamcinolone performed for 915 eyes with diab.tic macular edema (n=257),exudative age-related macular degeneration (n=561),retinal vein occlusions (n=82),and other reasons. Among the injections were 220 reinjections. Triamcinolone was filtered to remove the solvent agent. Mean follow-up was 8.1± 7.4 months. RESULTS: In none of the eyes,signs of an infectious or noninfectious endophthalmitis were ob.erved such as noncrystalline hypopyon,cellular infiltration,or amorphous opacification of the vitreous,retinal infiltration,or pain. One patient developed infectious endophthalmitis 2 days after a traumatic rupture of a previous corneoscleral cataract incision. CONCLUSIONS: The rate of infectious or noninfectious endophthalmitis after an intravitreal high-dosage triamcinolone injection may b. approximately 1:1000,if the solvent agent was removed.

Using a melt-cooling process with the host glass,Dy_(2)O_(3)(1 wt%,2 wt%,3 wt%,and 4 wt%) doped tellurite glasses were produced.A(65-x)TeO_(2)-20b.(2)O_(3)-10Al_(2)O_(3)-SGeO_(2)-xDy_(2)O_(3) glass system was analyzed b. Xray *** patterns have a hump at 2θ=27°-29° and no sharp peaks,indicating that all the glass samples are *** Vickers microhardness determines how resistant a glass is to persistent deformation produced b. a harder *** scanning calorimetry(DSC) at 300-550℃ was used to evaluate the thermal properties of the Dy_x glass *** glass transition temperature T_g(small endothermic peak),start crystallization temperature T_x and crystallization temperature T_c can all b. seen on the DSC thermogram(exothermic peaks).The UV-Vis transmittance and ab.orption spectra were *** optical data show that the values of E_d^(ir),E_g^(Ind) and E_U decrease as the amount of Dy_(2)O_(3)increases,indicating a narrowing of the tails due to localized states in the forb.dden gap.A concentration quenching event occurs when Dy^(3+) concentrations are ab.ve a certain threshold(1%).The addition of Dy_(2)O_(3) increases glass density,while the addition of other Dy(Ⅲ) oxides increases sample *** radiation shielding values change as a consequence of density *** sample with the highest amount of Dy_(2)O_(3)(Dy_(4)) has stronger radiation shielding characteristics;hence,it is superior in terms of shielding outcomes at all energy *** Dy_x samples can b. used in various forms of ionizing radiation shielding.

目的　建立一种简便、快速、敏感和特异的检测巨细胞病毒 (HCMV)糖蛋白gb.gH基因型变异的方法。方法　以HCMV糖蛋白基因gH、gb.、gb.lv为靶序列 ,设计 17条具有型特异性的寡核苷酸探针 ,聚合酶链反应 (PCR)扩增目的片段 ,反向杂交检测 2 3例中国和 6例德国移植患者HCMVgH、gb.、gb.lv基因型变异 ,其结果与直接测序进行比较。结果　 2 3例中国患者HCMV糖蛋白为gH1和 2型 ,gb.、2和 3型 ,未见gb.型 ;而 6例德国患者的HCMV糖蛋白涵盖gH1和 2型 ,gb.、2、3及 4型。中、德两国患者均可同时感染 2种不同基因型的病毒株。反向杂交技术对检测患者同时感染不同基因型病毒株优于直接测序。结论　PCR 反向杂交技术具有简便、快速、敏感和特异的特点 ,适用于临床实验室检测HCMV糖蛋白基因型变异。

Pure and Molyb.enum (Mo) doped Potassium Titanyl Phosphate (KTP) inorganic crystals were grown b. high temperature solution growth (HTSG) from poly phosphate (K6P4O13) flux using different KTP/Flux ratios. The pure and doped KTP crystals of size 20x13x5 mm3 and 7x5x2.5 mm3 respectively were grown successfully b. spontaneous nucleation. The grown crystals were characterized b. XRD, UV, FTIR and Hardness studies. Micro hardness studies show that the pure crystals are harder than the Mo doped crystals.

To report on visual outcome of patients receiving intravitreal triamcinolone a cetonide for treatment of diffuse diab.tic macular edema. Prospective, comparati ve clinical interventional study. setting: Institutional. patient population: Th e study included 25 consecutive patients (50 eyes) with b.lateral diab.tic macul ar edema. intervention procedure: Unilateral intravitreal injection of ab.ut 20 mg triamcinolone acetonide into the eye (study group) more severely affected b. diab.tic maculopathy. The contralateral eyes served as control group. Mean follo w-up was 7.1 ±.4.1 months. main outcome measure: Visual acuity, intraocular pr essure. In the study group, visual acuity increased significantly (P ≤. 001) b. 3.0 ±.2.6 Snellen lines to a peak at two to six months after the injection, an d decreased significantly (P=. 001) towards the end of follow up. At the end of follow-up, visual acuity was higher, not significantly (P=. 18) higher, than at b.seline. An increase in visual acuity was found in 23 eyes (92%)-. In the co ntrol group, differences b.tween visual acuity at b.seline and at any of the re -examinations during follow-up were not significant (P > . 10). In an intra-i ndividual intereye comparison, gain in visual acuity was significantly (P b.ained up to four months a fter injection. Intravitreal triamcinolone acetonide may temporarily increase vi sual acuity in eyes with diab.tic macular edema.
