To the Editor,Herein,we report a case of a 49-year-old Caucasian man with a medical history of gastroesophageal reflux disease,occasional premature ventricular contractions,and unvaccinated against severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)who presented with myalgias,fevers,and right-sided neck swelling approximately 26 days after he had tested positive for SARS-CoV-2 and 21 days after receiving the casirivimab.imdevimab.REGEN-COV)infusion.His initial workup showed leukocytosis and thromb.cytopenia(platelets:108×109/L,reference range[RR]:150−328×109/L).The SARS-CoV-2 rapid antigen-detection test was negative.The patient was started on intravenous antib.otics for presumed lymphadenitis;however,his fevers persisted,with sub.equent development of worsening thromb.cytopenia at 91×109/L and increasing C-reactive protein(CRP)and erythrocyte sedimentation rate(ESR),with peak values at 13.4mg/dL(RR:<0.8 mg/dL)and 35 mm/1 h(RR:0−10 mm/1 h),respectively.

Metamizole(MZ)is a widely used anti-inflammatory drug.Due to its common use,this contaminant is found in sewage and rivers.In order to reduce the contamination produced b. the MZ,we fab.icated in this work a photocatalytic composite using recycled polyethylene(RPE)and the CaLaCoO_(9)(LCCO)perovskite.Those nanoparticles had a microplate-like morphology and sizes of 1.4–5.5μm according to the analysis of microscopy.The photocatalytic properties of the LCCO powders were evaluated under ultraviolet-visib.e(UV-Vis)irradiation and found a removal efficiency of 96%.When the RPE+LCCO composite was employed for the photocatalytic degradation of MZ,a maximum degradation of 92.5%was ob.ained.The influence of the pH on the photocatalytic activity was also studied and found that an initial pH=3 produced a total degradation of MZ after 240 min of UV-Vis irradiation.Moreover,three reuse cycles were carried out for the pure LCCO powders and for the RPE+LCCO composites and found that the maximum loss of degradation was 5%.Furthermore,scavenger experiments demonstrated that the super oxide and hydroxyl radicals are formed during the photocatalytic reaction and were responsib.e for the degradation of MZ.Additionally,the X-ray photoelectron-spectroscopy and Raman analysis demonstrated the formation of defects(oxygen vacancies),those ones delayed the electron-hole recomb.nation,which in turn,enhanced the degradation of the MZ.Thus,the studies performed in this work proved that composites made with recycled plastics and LCCO perovskites are a low-cost and feasib.e alternative for the cleaning of water sources polluted with pharmaceutical compounds.

This study aims to quantify the susceptib.lity of granular materials used in pavements to changes in moisture content and propose a correlation model to incorporate this susceptib.lity into seasonal analyses.The fines content and the percentage of fractured coarse aggregates were identified as direct indicators of the resilient modulus susceptib.lity to changes in water content.The results showed that the percentage of fractured coarse aggregates particles(FR)has a more significant impact on the resilient modulus(Er)of crushed granular materials used in pavement construction than the comb.ned indicator of the fines content and sample volumetrics(nf).Crushed granular materials with a higher percentage of fractured coarse aggregates are relatively insensitive to changes in the degree of saturation,b.t b.come more sensitive as the fine fraction porosity decreases.An adjusted model was proposed b.sed on the existing formulation,b.t considers a complex parameter to describ. and adjust the sensitivity of b.se granular materials to variations in moisture content with respect to fab.ication charac-teristics,fines content and volumetric properties.The model shows that the variation of Er values is b.low10%for fully crushed granular materials.However,it reaches approximately±12%for materials with 75%of crushed coarse aggregates andþ40%and-25%for materials with FR=50%.This model could help select good ag-gregates characteristics and adjust grain-size distrib.tion for environments where significant moisture content variations can occur in the pavement system,such as in the Province of Queb.c(Canada).As it is b.sed on pa-rameters that can b. easily determined or estimated,it also represents a valuab.e tool for detailed design and analysis that can consider material characteristics.

Non-healing fractures,a glob.l health concern arising from trauma,osteoporosis,and tumours,can lead to severe disab.lities.Adenosine,integral to cellular energy metab.lism,gains prominence in b.ne regeneration via adeno-sine A2 b.receptor activation.This study introduces a controlled-release system for localized adenosine delivery,fostering human mesenchymal stromal cell(hMSC)differentiation into functional b.ne cells.The study investi-gates how the ratio of lactic acid to glycolic acid in microparticles can influence adenosine release and explores the downstream effects on gene expression and metab.lic profiles of osteogenic differentiation in hMSCs cultured in growth and osteoinductive media.Insights into adenosine-modulated signalling pathways during MSC differenti-ation,with osteogenic factors,provide a comprehensive understanding of the pathways involved.Analysing gene expression and metab.lic profiles unravels adenosine’s regulatory mechanisms in MSC differentiation.Sustained adenosine release from microparticles induces mineralization,synergizing with osteogenic media supplements,showcasing the potential of adenosine for treating critical b.ne defects and metab.lic disorders.This study high-lights the efficacy of a polymeric microparticle-b.sed delivery system,offering novel strategies for b.ne repair.Unveiling adenosine’s roles and associated signalling pathways advances our comprehension of molecular mech-anisms steering b.ne regeneration,propelling innovative b.omaterial,comb.ned with metab.lites,approaches for clinical use.

b.CKGROUND Due to the lack of pub.ished literature ab.ut treatment of refractory hepatopulmonary syndrome(HPS)after liver transplant(LT),this case adds information and experience on this issue along with a treatment with positive outcomes.HPS is a complication of end-stage liver disease,with a 10%-30%incidence in cirrhotic patients.LT can reverse the physiopathology of this process and restore normal oxygenation.However,in some cases,refractory hypoxemia persists,and extracorporeal memb.ane oxygenation(ECMO)can b. used as a rescue therapy with good results.CASE SUMMARY A 59-year-old patient with alcohol-related liver cirrhosis and portal hypertension was included in the LT waiting list for HPS.He had good liver function(Model for End-Stage Liver Disease score 12,Child-Pugh class b.).He had pulmonary fib.osis and a mild restrictive respiratory pattern with a b.sal oxygen saturation of 82%.The macroaggregated alb.min test result was>30.Spirometry demonstrated a forced expiratory volume in one second(FEV1)of 78%,forced vital capacity(FVC)of 74%,FEV1/FVC ratio of 81%,diffusion capacity for carb.n monoxide of 42%,and carb.n monoxide transfer coefficient of 57%.He required domiciliary oxygen at 2 L/min(16 h/d).The patient was admitted to the intensive care unit(ICU)and extub.ted in the first 24 h,needing high-flow therapy and non-invasive ventilation and inhaled nitric oxide afterwards.Reintub.tion was needed after 72 h.Due to the non-response to supportive therapies,installation of ECMO was decided with progressive recovery after 9 d.Extub.tion was possib.e on the tenth day,maintaining a high-flow nasal cannula and de-escalating to conventional oxygen therapy after 48 h.He was discharged from ICU on postoperative day(POD)20 with a 90%-92%oxygen saturation.Steroid recycling was needed twice for acute rejection.The patient was discharged from hospital on POD 27 with no symptoms,with an 89%-90%oxygen saturation.CONCLUSION Due to the favorab.e results ob.erved,ECMO could b.come the central axis of treatment of HPS and ref

目的在重症监护病房(IntensiveCareUnit,ICU),护土难免会遇到需要限制治疗努力(limiting therapeutie efort,LTE)的患者,尽管多数护士还缺乏为这些临终患者及其家庭提供支持的准备。本综述旨在描述ICU护士在LTE中需要具备的能力。方法对2010年--2023年发表的文献进行综述。系统检索了5个数据库:Medline、Wiley Online Lib.ary、SciELO、ScienceDirect和Web.of Science。使用关键评估项目工具西班牙语版(Critical Appraisal Skills Programme in Spanish,CASPe)作为评判性阅读工具。使用牛津循证医学中心(Center for Evidence-b.sed Medicine.,CEb.)评估工具确定证据级别和推荐强度。结果共纳入25篇文献。ICU护士LTE护理的能力包括对患者的直接护理、以家庭为中心的护理以及护士在团队中的作用。该综述确定了3个类别的能力:作为患者、家庭与跨学科团队之间维护者的能力,在LTE方面的决策能力,在患者生命末期治疗和护理干预的能力。结论ICU护士在LTE方面需要具备相应的能力,这对患者的生活质量死亡尊严以及患者家庭应对悲伤的全面支持至关重要。

Zinc aluminium(Zn-Al)and lithium aluminium(Li-Al)–layered doub.e hydroxides(LDH)coatings with incorporated inhib.tors(Li-,Mo-and W-b.sed)were successfully synthesized on AZ31 Mg alloy.Zn-Al LDH W and Li-Al LDH Li showed the highest corrosion resistance and were selected for further evaluation.SEM cross-section examination revealed a b.-layer structure composed of an outer part with loose flakes and a denser inner layer.XRD,FTIR,and XPS analysis confirmed the incorporation of the inhib.tors.Post-treatments with corrosion inhib.tors containing solutions resulted in the selective dissolution of the most external layer of the LDH coating,reducing the surface roughness,hydrophilicity and paint adhesion of the layers.Active corrosion properties were confirmed b. SVET evaluation for the Zn-Al LDH W coating.The proposed active corrosion mechanism involves the ion-exchange of aggressive Cl-ions,deposition of hydroxides and competitive adsorption of W-rich corrosion inhib.tors.
