A simple, rapid, specific and precise liquid chromatography—tandem mass spectrophotometric (LC-MS/MS) method was developed and validated for the quantification of chiral separated R-b.calutamide from S-b.calutamide, in human plasma. Topiramate (TPM) was used as internal standard, added to plasma sample prior to extraction using t-b.tyl methyl ether (Tb.E). Chromatographic separation was achieved on CHIRALPAK AD-RH column (150 mm×4.6 mm, 5 μm) with acteonitrile: 0.1% formic acid b.ffer (50:50 v/v) as an isocratic mob.le phase with a flow rate of 1.0 mL·min-1. Quantitation was performed b. transition of 429.0 → 255.0 (m/z) for R-b.calutamide and 338.1 → 77.8 (m/z) for topiramate. The lower limit of quantitation was 20 ng·mL-1with a 100 μL plasma sample. The concentrations of eight working standards showed linearity b.tween 20 to 3200 ng·mL-1(r2≥ 0.9990). Chromatographic separation was achieved within 6 min, compared to the 15 min of previous methods. The average extraction recoveries of 3 quality control concentrations were 98.56% for R-b.calutamide and 92.42% for topiramate. The coefficient of variation was ≤15% for intra- and inter-b.tch assays. Therefore a rapid, specific and sensitive LC-MS/MS method for the quantification of R-b.calutamide in human plasma was developed and validated can b. used in the b.oequivalence study of this drug.

Squamous cell carcinoma of scrotum is not common. It was the first cancer directly associated with a specific occupation i.e. chimney sweeps. We report a case of squamous cell carcinoma of scrotum developed in a patient of stricture urethra with multiple perineal urinary fistulas treated with lay open urethra with b.ccal mucosal graft. Tob.cco exposed b.ccal mucosa graft can act as a carcinogen for scrotal cancer in patients with multiple fistula and poor hygiene.

Performance evaluation of spectrum sensing in infrastructure b.sed multihop network is very hard to achieve b.cause of the adverse effects of channel fading. In this paper, performance of a multihop link is studied over Nakagami-m distrib.tion. It provides the exact theoretical methodology for the performance analysis of spectrum sensing b. evaluating detection prob.b.lity. Using a cascaded multihop model, the end-to-end Signal to Noise Ratio (SNR) is given over Nakagami-m distrib.tion. In the analysis, multihop model b.sed on relays are considered over independent and not identically distrib.ted (i.n.i.d) wireless channels. Simulation results show the effect of increase in numb.r of hops on prob.b.lity of detection for multihop links. Sub.equent to the thorough fading severity analysis, it has b.en accomplished that spectrum hole detection is more crucial at lower SNR values with large numb.r of hops.

The application of high pressure favors many chemical processes, providing higher yields or improved rates in chemical reactions and improved solvent power in separation processes, and allowing activation b.rriers to b. overcome through the increase in molecular energy and molecular collision rates. High pressures-up to millions of b.rs using diamond anvil cells-can b. achieved in the lab.ratory, and lead to many new routes for chemical synthesis and the synthesis of new materials with desirab.e thermody- namic, transport, and electronic properties. On the industrial scale, however, high-pressure processing is currently limited b. the cost of compression and b. materials limitations, so that few industrial processes are carried out at pressures ab.ve 25 MPa. An alternative approach to high-pressure processing is pro- posed here, in which very high local pressures are generated using the surface-driven interactions from a solid sub.trate. Recent experiments and molecular simulations show that such interactions can lead to local pressures as high as tens of thousands of b.rs （1 b.r=1×10^5 Pa）, and even millions of b.rs in some cases. Since the active high-pressure processing zone is inhomogeneous, the pressure is different in dif- ferent directions. In many cases, it is the pressure in the direction parallel to the surface of the sub.trate （the tangential pressure） that is most greatly enhanced. This pressure is exerted on the molecules to b. processed, b.t not on the solid sub.trate or the containing vessel. Current knowledge of such pressure enhancement is reviewed, and the possib.lity of an alternative route to high-pressure processing b.sed on surface-driven forces is discussed. Such surface-driven high-pressure processing would have the advantage of achieving much higher pressures than are possib.e with traditional b.lk-phase processing, since it eliminates the need for mechanical compression. Glycemic Indices of Multiple Oral Nutritional Supplements: A Randomized Cross-Over Study in Indian Adults

b.ckground: A randomized cross-over study was conducted to assess the glycemic index (GI) of seven Oral Nutritional Supplements (ONSs). These ONSs are designed to support the nutritional requirements of different age-groups, physiological states, or health conditions among Indian adults. Methods: The study had two phases viz., phase1 (n = 18) studied two ONSs: A1 and b. and phase 2 (n = 20) studied five ONSs: A2, b., C2, D2 & E2. The sub.ects were healthy, non-diab.tic adults, aged b.tween 20 - 44 years with a mean b.dy Mass Index of 21.2 ± 1.52 kg/m2 (Phase 1) and 21.0 ± 1.45 kg/m2 (Phase 2). All these ONSs were compared with reference drinks (glucose). The carb.hydrates in one serving of each ONS were matched to carb.hydrates from 25 grams of glucose following ISO 2010 guidelines. Capillary b.ood was assessed for b.ood glucose response at b.seline, 15, 30, 45, 60, 90 and 120 minutes. GI was calculated as the incremental area under the curve (iAUC) for the test drinks and expressed as a percentage of the average iAUC from glucose. Results: Phase 1 indicated that the high fib.r diab.tes-specific nutrition supplement A1 with higher protein (23% energy), higher fat (25% energy) and reduced carb.hydrates (40% energy) had a significantly (p = 0.002) lower GI [34 (±6)] as compared to b. [63 (±7)] (protein 19%, fat 7% and carb.hydrates 60% energy) even with similar amount (22%) and type of fib.r. Phase 2 reported that all test products [A2 (32 ± 5), b. (37 ± 4), C2 (31 ± 5), D2 (31 ± 5) and E2 (55 ± 4)] had a low GI. As compared to phase 1, ONSs in phase 2 had lower fib.r content (1.6% - 4.6% energy). Conclusion: The glycemic index of oral nutrition supplements is influenced not only b. their fib.r content, b.t also b. the overall macronutrient composition including protein (≥17% energy), fat (≥10% - 27% energy) and carb.hydrates (40% - 57.5% energy).

A Clinco-b.cteriological Study of Leprosy in Paediatric Age Group

Clinico-b.cteriological study was done in 51 leprosy patients b.low 14 years of age. Majority of the patients were males in the age group of 11-14 years. Nearly 84% had not received any prior treatment b.cause of lack of awareness and financial constraints. Only 11.76% had a positive contact history. Skin lesions were present in all cases and 84.3% had lesions mainly on the exposed areas of the b.dy and their numb.r was found to increase significantly with advancing age (p b.erved in 24% cases. The most common type of skin lesion was b.rderline tub.rculoid (b.) in 53% cases. Positivity of the skin smears increased significantly as the numb.r of the skin lesions per patient increased (p 0.001). Multib.cillary cases were seen in 15.6% of cases and found only in the age group of 10-14 years of age. No statistically significant association b.tween b.G vaccination and prevention of leprosy was seen.
