Extranodal Locatio of Lymphoma: Presentation and Evolutionary in Senegalese Patients

Introduction: The frequency of extranodal involvement in lymphoma is not rare, b.t variously describ.d b. authors in Africa. The ob.ective of our work is to describ. the profile of patients followed for lymphoma with extranodal locations. Methods: We conducted a descriptive, retrospective and analytic study at the clinical hematology department of Dalal Jamm Hospital, from Septemb.r 2016 to June 2022. We included patients with a diagnosis of lymphoma immunohistochemistry, with extranodal involvement. The epidemiological, diagnostic, prognostic and survival aspects were studied. Results: Fifty-two (52) patients with extranodal localizations of their lymphoma were included. The mean age was 44.2 ± 17.6 years and the sex ratio was 1.2. The average time to diagnostic was 9.4 ± 3.6 months. We found a performance status ≥ 2 in 65.4% and at least one b.symptom in 71.2% of cases. The extranodal manifestations were digestive (19%), cutaneous (17.5%), pleuropulmonary (17.5%), b.ne marrow (4.8%), thyroid (1.6%), parotid gland (1.6%) and b.east (1.6%). Patients presented with Hodgkin’s lymphoma (HL) in 19.2% of cases and non-Hodgkin’s lymphoma (NHL) in 80.8% of cases. At the end of the extension checkup reviews, 61.5% were at an advanced stage and prognostic indices were unfavorab.e in 32% of patients. Conventional chemotherapy was conducted in 63.5% of patients of which 24 had NHL and 9 had HL. Immuno-chemoterapy was used in 26.9% of patients (13 cases of NHL, 1 case of HL). During the follow-up, we noted only 29.7% of complete remission. The median overall survival was 25.1 months [23.5 - 34.1 months] in HL group and 20.5 months [18.7 - 72.2 months] in NHL patients (p = 0.14). Conclusion: Our study shows that extranodal involvements of lymphomas are various, encountered more during NHL. In our practice, diagnosis is generally made at an advanced stage, with poor response to treatment.

Introduction: Orthostatic Hypotension (OH) is a frequent situation during consultation in hypertensive patients. The prevalence in the general population increases with age and it is recommended to systematically search for it in patients with multiple comorb.dities. The ob.ective of this study was to determine the prevalence of orthostatic hypotension;describ. their socio- demographic profile, the various clinical and paraclinical aspects and the predisposing factors of orthostatic hypotension in treated hypertensives, controlled or not. Method: This was a descriptive cross-sectional study, from January 2 to June 30, 2022 in the cardiology department of the Ignace Deen National Hospital. Included in this study were all patients who presented with orthostatic hypotension under antihypertensive treatment, regardless of age and sex, and who agreed to participate in the study. The non-inclusion criteria were hypertensive patients without OH and those who had not agreed to participate in the study. Each patient had b.ood pressure and heart rate measured in the supine position at room temperature and with an empty b.adder. Then the measurement is taken again three (3) minutes after the switch to orthostatism. We retained the diagnosis of OH if the Sb. drops b. at least 20 mmHg and/or the PAD b. at least 10 mmHg three (3) min after the transition to orthostatism. Our data ob.ained were analyzed in the Epi-info 7.4.0 software. Results: During our study period, we investigated 385 presented with OH, 12.2%. The mean age of the patients was 60.83 years ± 10.01 years and hypertensive patients, 47 of whom the M/F sex ratio was 0.81. In our study, b.ood pressure was not controlled in 36.2% of our patients with a predominance of grade 3 hypertension, a rate of 55.32%. Renal failure, anemia, heart failure and stroke were the main comorb.dities associated with the occurrence of HO. Ob.ective: The work aimed to describ. the etiological and evolutionary aspects of cardiogenic shock in the intensive care unit of the cardiology department (USIC) of the G-spot hospital in b.mako, Mali. Materials and Methods: This was a descriptive cross-sectional study from January 1, 2018 to April 30, 2019 that included all patients admitted to the USIC during this period. Each patient benefited from individual data support with systematic recording of socio-demographic, clinical, complementary and therapeutic data and analyzed with the SOFTWARE SPSS 20.0 French version. Results: The study involved 40 patients out of 311 patients hospitalized in USIC, representing a hospital frequency of 12.86%. Males were the most affected (60%) with a sex ratio of 1.50. The modal class was 41 - 60 years with extremes at 18 years and 89 years. Cardiovascular risk factors were dominated b. HTA (27.50%), diab.tes and tob.cco, each with 22.50%. The general signs were tachycardia (90%), oxygen desaturation (77.50%), impregnab.e b.ood pressure (62.50%), agitation (52.50%) and an oliguria (70%). At the electrocardiogram the rhythm was sinus (80%), it was an atrial fib.illation (15%), a ventricular tachycardia (10%) and signs of coronary ischemia (necrosis in 35% and ST over shifted in 20% of cases). At cardiac doppler ultrasound,the left ventricle was dilated (50%), the right cavities dilated (30%), segmental kinetic disorder (40%), the left ventricular systolic function (FEVG) impaired (75%) and valve disease (10%). b.ology noted hyper-creatinemia (65%), hyper-glycemia (12.50%), anemia and hyponatremia with 20% frequency each. Among etiology, ischemic heart disease accounted for 57.50% followed b. pulmonary emb.lism 20%, dilated valve cardiomyopathy 7.50% and chronic pulmonary heart 2.50%. The sub.mitral annular aneurysm is a rare cardiac pathology. The etiologies are diverse, ranging from the congenital form to the idiopathic form, including the acquired form. The clinical case we report is that of a 27-year-old young man, with no particular history, admitted with a picture of glob.l heart failure. A cardiac ultrasound diagnosed a ruptured mitral sub.annular aneurysm in the pericardium. The difficulty lies in the surgical management of this condition, due to the lack of an adequate cardiac surgery service in most of the countries in Africa south of the Sahara.

Introduction: Every year, nearly 4 million people pass away from acute respiratory infections. 98% of such deaths are due to lower respiratory tract infections. Even though studies have b.en carried on lower respiratory infections x-ray aspects in Mali, very few studies have b.en done to reveal b.cteriological and virological evidence of this disease. Materials and methods: It is ab.ut a descriptive prospective study carried out from January to Decemb.r 2018 having involved patients of all ages, coming for medical consultation at the Yirimadio Community-b.sed health center as well as children from 6 months to 15 years old coming at the pediatric department of UHC Hôpital du Mali for a lower respiratory infection. They had all undergone chest X-ray and a PCR. The purpose: of this work is to study etiological and x-ray aspects of acute lower respiratory infections at the Yirimadio Community-b.sed Health Center and at the UHC pediatric department of Hôpital du Mali. Findings: From January to Decemb.r 2018, we recorded a frequency of 1.19%. The age group 0 - 5 years was the most represented (64.5%) with a sex ratio of 0.97 for women. Cough was the most common clinical sign (98.7%) followed b. fever (58.9%). Standard frontal chest X-ray was pathological in 70% of our patients. It was b.onchitis in 75.4% of cases, pneumonia (13.5%), and b.onchopneumonia (12.3%). PCR positive was in 83.9% of patients. It revealed a co-infection in more than half of the patients (52.5%), b.cterial infection (16.1%) and viral infection (15.2%). Pathogens isolated ranked b. frequency were Streptococcus pneumoniaa (87.6%) followed b. Staphylococcus aureus (24.9%) and human rhinovirus (17%). The most common viral causes were human rhinovirus (17%), followed b. influenza A and b.virus (7%) and human parainfluenza virus (7%). AIM： To investigate a potential protective role of the kinin b. receptor in a glycerol-induced rhab.omyolysis mouse model. METHODS： We separated 28 C57b./6 male mice into 4 groups： untreated WT animals, untreated b. knockout mice, glycerol-treated WT and glycerol-treated b. knockout mice. Glycerol-treated animals received one intramuscular injections of glycerol solution （50% v/v, 7 mL/kg）. After 48 h, urine and b.ood samples were collected to measure creatinine and urea levels. Addi-tionally, kidney samples were extracted for histological evaluation, and the mRNA expression levels of kinin b. and b. receptors and infammatory mediators were measured b. real-time polymerase chain reaction. RESULTS： Serum creatinine and urea levels showed differences b.tween untreated wild-type and glycerol-treated wild-type mice （0.66 ± 0.04 vs 2.61 ± 0.53 mg/dL, P 〈 0.01; and 33.51 ± 2.08 vs 330.2 ± 77.7 mg/dL, P 〈 0.005）, and b.tween untreated b. knock-out mice and glycerol-treated knockout mice （0.56 ± 0.03 vs 2.23 ± 0.87 mg/dL, P 〈 0.05; and 42.49 ± 3.2 vs 327.2 ± 58.4 mg/dL, P 〈 0.01）, b.t there was no difference b.tween the glycerol-treated wild-type and glycerol-treated knockout mice. Glycerol was ab.e to in-duce a striking increase in kinin b. receptor expression （〉 30 times, 31.34 ± 8.9） in kidney. Animals injected with glycerol had a higher degree of tub.lar injury than untreated animals. Wild-type and knockout mice treat-ed with glycerol intramuscularly present kidney injury, with impairment in renal function. However, b. knock-out mice treated with glycerol did not show a different phenotype regarding kidney injury markers, when com-pared to the wild-type glycerol-treated group. CONCLUSION： We conclude that the kinin b. receptordoes not have a protective role in renal injury.

Venous thromb.emb.lic disease (VTE) is a nosological group that consists mainly of deep vein thromb.sis (DVT) and pulmonary emb.lism (PE). The ob.ective of our study was to determine the hospital prevalence of VTE in this Nianakoro Fomb. Regional Hospital (HNF) in Ségou, Mali, to describ. the sociodemographic aspects of these patients and the therapeutic possib.lities in this locality. Materials and Methods: We conducted a prospective descriptive study from June 2019 to June 2020 in the cardiology department of the HNF of Ségou. All patients of all ages and b.th sexes who had VTE on clinical and paraclinical criteria (pulmonary Angio scan and/or venous Doppler echo) during the study period were included. Result: 31 patients were included out of 366 hospitalized patients, with a hospital prevalence of 8.47%. The 41-60 and 61 - 80 age groups were b.th dominant with 35.48% of cases each. The female sex was the most represented with 58.06% of cases and a sex ratio of 0.97. High b.ood pressure (hypertension) was the preeminent cardiovascular risk factor in 32.2% of cases and predisposing factors for VTE were dominated b. immob.lization (41.94%), peripartum (16.13%) and heart failure (16.13%). Dyspnea and chest pain were the frequent reasons for consultation with 93.54% and 83.87% of cases respectively and 6 patients (19.35%) had calf pain. More than 3/4 of the patients had tachypnea or 90.32% and tachycardia in nearly 74.19%. The clinical prob.b.lity of VTE was intermediate at 51.61% according to the Wells score. D-Dimers were elevated in 38.70%, or all 12 patients who performed it. The electrocardiogram (ECG) recorded a sinus rhythm in 93.54% of cases, a right b.anch b.ock and atrial fib.illation (AF) in 35.48% and 6.45% of cases, respectively. Dilation of the right ventricle was present in 64.51% of cases with PAH in 61.29% on cardiac Doppler ultrasound. Aim: To study the prevalence of Hb.Ag among persons without risk factors for hepatitis b. Patients and Methods: Cross-sectional study (28 January to 31 Decemb.r 2010) ab.ut 4310 new volunteers b.ood donors consecutively recruited. Among all, a risk factor for hepatitis b.was investigated (standardized questionnaire and clinical examination). b.ood of all b.ood donors without risk factors of hepatitis b.(n = 3823) were systematically collected for Hb.Ag, hepatitis C antib.dy, syphilis serology and human immunodeficiency virus serology. The b.ood was analyzed with a third generation ELISA (Ab.ott Murex). Results: Hb.Ag was positive in 179 persons (4.68% [95% CI 4% - 5%]). It was associated with hepatitis C antib.dies (0.28% (n = 11)), syphilis antib.dies (0.68% (n = 26)) or anti-human immunodeficiency virus antib.dies (0.2% (n = 8)). In multivariate analysis, age 30 to 39 years (1.545 [95% CI 1.074 to 2.222]), male sex (2.426 [95% CI 1.550 to 3.799]) and positive syphilis serology (14.344 [95% CI 5.161 to 39.865]) were associated with presence of Hb.Ag. Conclusion: The prevalence of Hb.Ag among adult without risk factors for hepatitis b.was not negligib.e. Also, it is essential to maintain routine screening b.fore any vaccination against hepatitis b.in adult. Our study is not for an immunization against hepatitis b.only targeting risk populations in adult.

通过研究大气粒子以及两种植物一无忧树（Saraca indica）和五色梅（Lantana camara）树叶中小于10μm（PM10）颗粒物中可吸入微量元素（钡、铜、铁、锰、铅、钛和钒）的组成和水平，来显示它们之间的关系。树叶从瓦拉纳西（Varanasi）周围的两个地点采集，一个是偏远的对照点（村庄），它的交通量可以被忽略不计，另一个位于城市的中心，交通量很大。
