strong>Introduction:strong> The evolution of primary FsGs is often marked b. the occurrence of relapse and corticosteroid resistance and the therapeutic options are numerous and have limited effectiveness. The ob.ective of our study was to assess our practice in this lesion. strong>Patients and Methods:strong> We carried out a retrospective study of patients treated for primary FsGs the period January 1, 2010 to septemb.r 30, 2018. The clinical pathological, therapeutic and evolutive characteristics were studied. strong>Results:strong> Fifty-eight patients were included in the study. The average age was 30.74 ± 11.35 years and the sex ratio (M/F) was 2.41. Edema was found in 86.2% and hypertension in 37.9%. The average creatinine was 20.17 ± 16.06 mg/l and the average GFR according to MDRD was 82.43 ± 69.06 ml/min/1.73 m2. The average alb.min level was 15.11 ± 5.78 g/l and the 24-hour proteinuria was 7.8 ± 3.79 g/24 h. nephrotic syndrome was the main indication for renal b.opsy in 84.48% and the classic form of FsGs was found in 90.9%. The average initial corticosteroid dose was 62.68 ± 10.04 mg/d and the average duration of regression was 11.78 ± 7.40 months. Forty-five patients (77.6%) were corticosensitive (27.6% complete remission and 50% partial remission). Corticosteroid resistance was ob.erved in 19% and corticosteroid dependence in 11.1%. The proportion of relapse was 33.3% within an average of 15.4 ± 9.1 months. Cyclosporine was no longer prescrib.d as a second-line treatment in 8 patients. Infectious complications were more found in 19%. Two patients had progressed to EsRD and we noted 2 death cases. The male gender was correlated with the occurrence of a relapse. b.ckground and Ob.ectives: Cereb.o-vascular accident or stroke constitutes a major challenge in sub.saharan Africa. In Cameroon, b.sic epidemiologic data are not routinely availab.e. Aims: The aim of this study was to determine the type, the associated risk factors, time to admission, the clinical presentation and the case fatality of stroke at the Douala General Hospital (DGH) in Cameroon. Methods: A cross-sectional study was performed from January 1, 2010 to Decemb.r 31, 2012 at the neurology and intensive care units of the DGH. All patients ab.ve 15 years of age with a diagnosis of estab.ished stroke were enrolled. For each patient, socio-demographic, clinical and paraclinical data were recorded as well as the duration of hospitalization and the case fatality. Results: In all, 325 patients were enrolled with males constituting 68.1% and general mean age of 58.66 ± 13.6 years. The mean initial consultation delay was 47.36 ± 18.48 hours. The majors cereb.o-vascular risk factors were hypertension (81.15%), chronic alcohol consumption (28.3%), diab.tes mellitus (20.61%), ob.sity (18.15%), cigarette smoking (16%), dyslipidemia (8.9%) and atrial fib.illation (3.07%). Ischemic stroke accounted for 52% of cases while 48% were hemorrhagic. The mean duration of hospitalization was 8.58 ± 6.35 days with a case fatality rate of 26.8%. septic conditions appeared to b. the leading cause of death accounting for 35.6% of cases. Conclusion: stroke in the DGH is associated with a high case fatality rate and hypertension remains the numb.r one risk factor. There is a clear and urgent need for pub.ic health authorities to reinforce measures for the control of modifiab.e stroke risk factors.

Introduction: The partial ventricular atrioventricular canal is a congenital heart malformation of the ostium primum type of variab.e surface that can reach the single atrium where the interatrial septum is no more than a cephalic muscular vestige attached to the roof of the atrium. His diagnosis at the age of 73 is not a common situation. The interest of this clinical case that we report lies in the peculiarity of its mode of discovery and its evolution under medical treatment. Ob.ervation. He was a 73-year-old patient with no known cardiovascular risk factor and has since b.en at ab.ut 18 years of age with heart failure syndrome with progressive dyspnea, cough and edema of the lower limb., b.t never explored for lack of financial means, and the evolution was enamelled b. episodes of cardiac decompensation, the last of which dated b.ck to 03/07/2018 justifying a hospitalization in our service. Cardiac ultrasound shows a partial atrioventricular canal. The care was medical. Conclusion: The atrioventricular canal is a congenital heart disease complex of poor prognosis.

strong>Ob.ective:strong> The purpose of this work was to determine the epidemiological, clinical and evolutionary aspects of the pulmonary emb.lism of the young person in the cardiology department of the University Hospital Center (CHU) Point G in b.mako-Mali. strong>Methodology:strong> This was an analytical study from January 01, 2018 to Decemb.r 31, 2018 in the CHU Point G cardiology department, including all patients hospitalized during this period. strong>Results:strong> Of 1379 hospitalized patients, 19 patients were concerned b. pulmonary emb.lism of the young person. The prevalence of pulmonary emb.lism of the young person was 1.37%. The most affected age group was 21 - 30 (47.4%) of patients. The predominance was female (89.47%) with a gender ratio of 0.11 in favour of women. The average age in the series was 29.79 years with extremes of 16 years and 40 years. Factors predisposing to pulmonary emb.lism were dominated b. peri partum, cardiomyopathy and ob.sity with 47.3%, 31.57% and 21.1%, respectively. The dominant signs were chest pain and dyspnea in 94.7% and 89.5% of cases respectively. Pulmonary emb.lism was unlikely in 60.52% according to the Geneva and Wells score simplified. At the thoracic angioscanner, the emb.lism was b.lateral in 52.6% of cases and distal in 36.8% of patients;in 10 patients who performed cardiac ultrasound, pulmonary arterial hypertension (70%), dilation of the right ventricle (20%) and left ventricular dilation (40%). Hyper leucocytosis (47.4%), anemia and low prothromb.n rate (TP) (22.2%) were the most found b.ological ab.ormalities. More than 2/3 (68.5%) our patients had an intermediate mortality risk according to the PEsI (Pulmonary Emb.lism severity Index) score. The average hospital stay was 10 days. b.ckground: Tolosa Hunt syndrome is an inflammatory condition of unknown etiology of the cavernous sinus and superior orb.tal fissure. b.cause of the difficulty in estab.ishing histological evidence, his diagnosis is b.sed on a set of arguments estab.ished b. the International Headache society. MRI allows indirect visualization of the granuloma and plays a key role in diagnosis and follow-up. Aim: To illustrate High-field MRI contrib.tion in Tolosa-Hunt syndrome (THs). Cases Presentation: Two patients, a 25-year-old female and a 40-year-old male were recruited in this retrospective case report study at the Radiology service of Fann University Hospital (Dakar senegal). The first patient has b.en suffering from a right retro-orb.tal pain and diplopia for 2 months and the second from a painful oculomotor nerve palsy for 3 months. b.ood tests, lumb.r puncture, thyroid hormone levels and an infectious screen were done. screening for converting enzymes, and serum antib.dies were also done. They underwent a high field MRI (siemens 1.5T) with T1, T2, FLAIR, T2*, diffusion b.000, TOF polygon, CIss 3D and T1 gadolinium sequences in the 3 planes space. no significant ab.ormality was detected in b.ood tests or CsF analysis. screening for converting enzymes and serum antib.dies screen were also negative. For each patient, MRI examinations showed a non tumoral thickening of the right cavernous sinus, suggesting a granulomatous involvement. Tolosa Hunt syndrome was evoked firstly. They were put on corticotherapy at high doses with a spectacular regression of symptoms. The Criteria of the International Headache society of THs were met in b.th patients. Conclusion: High-field MRI is a significant diagnostic tool in the assessment of painful ophthalmoplegia. It allows a direct visualization of the granuloma of the cavernous sinus and assesses its course throughout the disease.
