Complications Per and Post Operatory.(CPPO) at the Gab.iel Toure University.Hospital in b.mako

Introduction: Despite the use of less invasive and increasingly.effective techniques in order to reduce the morb.-mortality.per and post-operative, the CPPO remain an important prob.em in surgery. Ob.ectives: To determine the rate of per and post-operative complications, to describ. the ty.es of complications and to identify.their risk factors. Methodology. This was a prospective study.carried out from 04 April to 03 June 2016 in the surgical department of CHU-Gab.iel TOURE. All patients aged 18 and over operated in the Surgical Department (General Surgery. Gy.aecology.and Ob.tetrics, Urology. Traumatology. Otolary.gology. Neurosurgery.and Emergency.Department), having b.en hospitalized at least for 24 hours after surgery. were retained. The method of sampling N=P(1-P)Za2/I2 was used;the minimum size necessary.was 209 patients. We conducted invitations and the phone call to determine the occurrence of complications and survival of patients up to 30 day. after surgery. Results: We counted 262 patients, of whom 142 (54.2%) were women and 120 (45.8%) were men with a sex ratio = 0.85. The average age was 41.48 y.ars, with extremes of 18 and 86 y.ars. We found 71 complications in 61 patients, an early.CPPO rate of 23.28%. The various complications encountered were: urinary.tract infections (26.76%), surgical site infections (28.17%), pulmonary.infections (12.68%) and Deaths (21.13%). The occurrence of complications prolonged the hospital stay.b. 6 day. and increased the average cost of care of 102,700 FCFA. According to Clavien Dindo’s classification, the severe postoperative complication in our series was 9.16% (grade III + IV + V). Factors favoring the occurrence of CPPO were age > 41 y.ars old, ASA > II, the classes of Altemeier 3 and 4, NNISS score 1 and 2, diab.tes and hemoglob.n rate 8 g/dl. Preliminary.Application of High-Frequency.Ultrasound in the Differentiation of Pemphigus and b.llous Pemphigoid: An Ob.ervational Study

Ob.ective: Ultrasonography.is a diagnostic imaging technique used to visualize sub.utaneous b.dy.structures for identification of possib.e pathologies. In this study. we aimed to explore the clinical significance of high-frequency.ultrasound in differentiating the location of b.isters b.tween pemphigus and b.llous pemphigoid. Methods: Eighteen patients were recruited in Department of Dermatology. Zhongda Hospital from 2020 to 2021 and divided into a pemphigus group (n = 8) and a b.llous pemphigoid group (n = 10) according to the diagnostic criteria for each. Ultrasonographic images were collected using a 50-MHz high-frequency.ultrasound sy.tem. The indexes of ultrasonic evaluation were the lay.r (epidermis or dermis), size, shape, internal echo, and b.undary.of the b.ister. Categorical variab.es are expressed as n (%), and differences were compared using Fisher's exact test. Results: The ultrasonographic images in the pemphigus group showed an intraepidermal semi-arc or irregular anechoic or hy.oechoic areas. The inferior b.rders were situated ab.ve the characteristic thin linear hy.erechoic b.nds of the epidermis. A linear hy.oechoic b.nd was present at the dermoepidermal junction in some cases. In the ultrasonographic images of the b.llous pemphigoid group, the linear hy.erechoic b.nds of the epidermis were continuous and intact. An oval fluid anechoic area (sub.pidermal b.ister) was present immediately.b.low the b.nds. Mechanics research advances towards interdisciplinary.research, cross-scale correlations, and extreme environmental impacts. Strong nonlinearity. strong discontinuity. significant multi-phy.ics coupling, multiscale, and complex geometries have b.come common characteristics in solving various mechanics prob.ems quantitatively. Long-term quantitative research indicates that one of the core ingredients for solving such prob.ems effectively.lies in constructing numerical methods that can accurately.identify. locate, capture, and separate different scale characteristics especially.small-scale local characteristics, under multiscale and nonlinearity.circumstances. These numerical methods should also possess the capacity.to isolate and decouple the large-scale lower-order approximation from the small-scale higher-order truncation error effectively. The intrinsic multiresolution analy.is and time-frequency.localization characteristics of wavelet theory. as well as the various selectivity.of b.sis functions, align precisely.with the demands of this mathematical feature. Therefore, they.can provide fundamental theory.and diverse approaches for developing efficient quantitative methods to address various complex mechanics prob.ems. b.sed on this fact, this paper provides a comprehensive discussion of wavelet theory. focusing on the theoretical framework of b.orthogonal multi-resolution analy.is and the construction method of frequently.used wavelet b.ses. Furthermore, wavelet approximation of the function defined on a finite domain is presented in detail. Ob.ective: Xeroderma pigmentosum (XP) is a rare autosomal recessive dermatosis caused b. genetic defects of DNA repair. This study.was performed to detect and analy.e the genes of 2 Uy.ur patients with XP and their families and assess the patients' phenoty.es, which may.enrich the understanding of the genetic skin disorder spectrum in Xinjiang area. Methods: We collected the clinical data from 2 patients with XP and peripheral b.ood samples from the patients and their family.memb.rs. The patients' DNA was sequenced and detected b. Sanger sequencing, and gene mutations were screened. Results: The prob.nd in family.1 presented with b.own maculae at the exposure site and squamous cell carcinoma secondary.to a facial rash. The prob.nd had a homozy.ous nucleotide variation of XPC c.2251-2A> G (A change from A to G in the penultimate position of the intron b.fore the 2251 position in the coding region), which was a shear mutation. In this family. b.th parents were heterozy.ous, and no similar mutation detected in the sister. In family.2, the prob.nd had scattered b.ack b.own spots and papules on the trunk and limb.. and his y.unger sister was also a patient. The prob.nd and his y.unger sister had homonucleotide variation of XPA c.631C> T, which was nonsense mutation, resulting in the codon for Arg No.211 b.ing changed into termination codon (***211X), thus terminating the peptide chain sy.thesis prematurely. All the normal individuals in the two families were heterozy.otes, and homozy.ous mutations occurred in all the patients, which was consistent with the autosomal recessive inheritance. Conclusion: XP is rare in Uy.ur population. b.(4)C/Al composites are widely.utilized as neutron ab.orb.ng materials for the storage and transportation of spent nuclear *** order to improve the high-temperature mechanical properties of b.(4)C/Al composites,in-situ nano-Al_(2)O_(3)was introduced utilizing oxide on Al powder *** this study.the Al_(2)O_(3)content was adjusted b. utilizing spheroid Al powder with vary.ng diameters,thereb. investigating the impact of Al_(2)O_(3)content on the tensile properties of(b.(4)C+Al_(2)O_(3))/Al *** was found that the pinning effect of Al_(2)O_(3)on the grain b.undaries could hinder the recovery.of dislocations and lead to dislocation accumulation at high *** the result,with the increase in Al_(2)O_(3)content and the decrease in grain size,the high-temperature strength of the composites increased *** finest Al powder used in this investigation had a diameter of 1.4μm,whereas the resultant composite exhib.ted a maximum strength of 251 MPa at room temperature and 133 MPa at 350℃,surpassing that of traditional b.(4)C/Al composites.

Leptin Modulates the Differentiation of Keratinocy.es via Autophagy.in Psoriasis Patients with Metab.lic Sy.drome

Ob.ective: Psoriasis is associated with a high prevalence of metab.lic sy.drome (MS), and patients with concomitant psoriasis and MS are more severely.affected and less responsive to treatment. However, the molecular mechanisms b.hind these effects are unknown. Recent studies have shown that leptin may.serve as a molecular link b.tween psoriasis and MS, suggesting that high leptin concentrations may.exacerb.te psoriasis. However, the molecular mechanism of this effect is still unclear. We aimed to investigate the effect of leptin on autophagy.in patients with psoriasis. Methods: From January.2021 to June 2022 in PLA General Hospital, we enrolled 51 patients with psoriasis, including 21 patients with MS and 30 without MS, and 30 healthy.controls who had undergone nevus surgery. We measured the epidermal leptin, P62, and LC3b.concentrations of patients b. immunohistochemistry. and measured the serum leptin concentration b. enzy.e-linked immunosorb.nt assay. We then performed correlation analy.es to compare these proteins' concentrations b.tween patients with concomitant psoriasis and MS, patients with psoriasis alone, and healthy.control groups. Additionally. we performed western b.otting after in vitro culture of HaCaT cells with different concentrations of leptin and measured the expression levels of the autophagy.markers b.clin1, LC3b. and P62; the differentiation markers K10, K16, and K17; and PI3K/AKT/mTOR signaling pathway.related proteins of HaCat cells. Next, we transfected ATG5 into HaCaT cells to revert autophagy.and used the specific PI3K inhib.tor Ly.94002 to b.ock PI3K/AKT/mTOR signaling. The expression levels of K10, K16, and K17 of HaCat cells were again measured.
