Introduction: The elbow is a superficial joint, particularly exposed to direct impact, forced movement, and overstrain. Our work aimed to study magnetic resonance imaging (MRI) pathologies of the soft tissues of the elbow through illustrative cases. Methodology: This was a retrospective and prospective cross-sectional study covering a period of one year, from June 2020 to June 2021, at the Saint Camille Hospital in Ouagadougou and the Polyclinique Notre Dame de la Paix. Results: In general, this study found that the pathologies diagnosed on MRI were lateral epicondylitis, subcutaneous type V elbow lipoma and liposarcoma, anteromedial cortical fracture of the radial cup, cortical detachment fracture of the lateral epicondylitis, medial epicondylitis, villonodular articular synovitis, simple dermo-hypodermatitis, sequellar fibrosis of the ulnar nerve, Workman's syndrome (median and ulnar nerves) and osteoarthritis of the elbow. Lateral epicondylitis was the most frequent pathology, and most patients consulted for elbow pain predominantly associated with pressure on the epicondyle, with relative functional impotence and, occasionally, elbow swelling. Conclusion: MRI, as a complement to ultrasound and radiography, remains the most informative examination for exploring soft-tissue pathologies of the elbow.

Diagnosis of Acute Appendicitis in Sub-Saharan Africa

Objective: The aim of our study was to propose a solution for rationalizing requests for ultrasound examination and CT scan in suspected acute appendicitis, based on use of the Alvarado score. Patients and Methods: We included in our study all patients consulting for pain of the right iliac fossa, whatever their age and sex, who had been diagnosed with acute appendicitis and who had undergone appendectomy. We used the Alvarado scoring system, based on clinical items and laboratory measurements, to diagnose appendicitis. Results: One hundred eleven patients, of whom 77 were men (69.4%) and 34 were women (30.6%), with a sex ratio of 2:2.6, were included. Mean age was 31.29 ± 1.31 years, with a range of 1 to 81 years. The diagnosis of definite appendicitis was established in 54 patients (48.6%). Abdominal ultrasound examination had been performed in 80 patients (72.1%). It established a diagnosis of appendicitis in 69 patients (86.2%), while in 11 patients (13.8%) the appendix was described as normal or was not visualized. Conclusion: Our study demonstrated good agreement between peroperative diagnosis and the result of the Alvarado score applied a posteriori. Ultrasound was not always contributory to the diagnosis of acute appendicitis. It could be reserved for patients whose clinical and laboratory score indicated that appendicitis was unlikely.

Incomplete Situs Inversus Revealed by Acute Pain of the Right Hypochondrium

Situs inversus is an abnormal placement of the thoracic and/or abdominal organs that are inverted right/left from normal. It is a rare congenital malformation often discovered in childhood. In adults, it can lead to misdiagnosis. Clinical case: A 35-year-old female patient, seen in a hepatogastroenterology consultation for acute spontaneous pain in the right hypochondrium. She had no particular clinical history. A diagnosis of hepatopathy was suspected. Abdominal and pelvic CT scans showed the left liver, stomach and spleen in the right hypochondrium, but with the heart in place, suggesting incomplete situs inversus. The evolution in our patient was spontaneously resolved with analgesics and antispasmodics, which leads us to believe that the volvulus was probably partial and without other complications. Conclusion: In developing countries, antenatal diagnosis of situs inversus is rare and is usually made during a pathology that leads the patient to a medical consultation. CT is one of the key paraclinical examinations for its diagnosis as genetic tests are not widely available.

background: Psychomotor delays in children are mainly investigated by CT imaging, due to the low availability and accessibility of magnetic resonance imaging in our context. Our aim was to study the CT aspects of psychomotor retardation (PMR) in children under 5 years of age at the University Hospital of Ouagadougou. Materials and Methods: Descriptive study with retrospective collection carried out over a period of twelve months (from September 2020 to August 2021). Our study focused on the reports of cranioencephalic CT scans performed on children aged 0 to 5 years, in whom a clinical diagnosis of psychomotor retardation had been made. Variables studied included socio-demographic data, clinical data, technique of CT examination, description of lesions and normality or not of the conclusion. Results: Cranioencephalic CT in children aged 0 - 5 years represented 9.78% of all CT scans performed in this age group. The sex ratio was 1.39. The mean age of the subjects in these reports was 18.31 ± 13.97 months and the modal class was [13 - 24] months. The CT scan was considered pathological in 85.10% of cases (n = 57). The most frequently noted encephalic anomalies were cerebral atrophy isolated in 54.4% of the cases (n = 31) or associated with other lesions in 30.1% of the cases (n = 17). Hydrocephalus was noted in 19.5% of the cases (n = 11). Conclusion: The lesions were mostly sequelae of cerebral ischaemia or cerebromeningeal infections. There were few cerebral malformations. There was no tumour etiology noted.

background: There are no recommendations for the use of standardised CT reports in oncology in our country. The aim of this study was to evaluate CT reports of pancreatic tumors in the city of Ouagadougou. Materials and Methods: Descriptive, multicenter, cross-sectional study conducted from 1st January 2013 to 31 December 2021. It concerned CT scan reports from five public and private imaging centers in the city of Ouagadougou. During the study period, 41 reports of pancreatic tumors were collected. We evaluated the reports using the standardized model developed by the Society of Abdominal Radiology and the American Pancreatic Association as a reference. Results: CT scan reports were not standardised. The writing style was free. Concerning the lesion, the aspect of the tumor at the pancreatic time was the item with the least information (24.4%). The status of the superior mesenteric artery was mentioned in 17%, the celiac trunk and the portal trunk in 12.2% of cases and 9.7% for the superior mesenteric vein. No report noted the appearance of the common hepatic artery. The status of the lymph nodes, liver and peritoneal cavity was mentioned in all reports. The exact location of the lymph nodes was not specified. The analysis of the reports classified them into two groups: potentially resectable tumours in the absence of secondary locations in 31.7% of cases and locally advanced tumours or presence of metastases in 68.3% of cases. Conclusion: The CT reports were not standardized. The items allowing evaluating the loco-regional extension of the tumor were the least specified. This may suggest the high rate of potentially resectable tumours in our study.

Context: In spite of the development of cross-sectional medical imaging and endoscopy means, the prescription of specialized radiographic examinations is still relevant in the pediatric environment in our country. Objective: Show the contribution of specialized radiographic examinations performed in pediatric settings. Methodology: This was a retrospective descriptive study implemented over a five year-period, from June 10th, 2014 to June 10th, 2019 within the radiology department of Charles De Gaulle Pediatric Teaching Hospital in Ouagadougou. This study included the results of 192 specialized radiographic examinations collected within the concerned department. Results: The average age of the patients was 4.21 years with extremes of 8 days and 15 years. Infants (29 days to 24 months) were the predominant age group with a rate of 53.13% (n = 102 patients). boys represented 76.51% of the sample with a sex ratio estimated at 3.25. Specialized radiographic examinations represented 0.46% of the overall imaging examinations and 0.72% of radiographic examinations performed during the study period. 67.19% of pathological examinations were reported. Colonic enema and retrograde urethrocystography (RUC) were the most performed examinations respectively accounting for 33.34% and 27.6%. The radiographic findings were consistent with the diagnostic hypotheses in 41.6% of cases. Conclusion: Specialized radiographic examinations keep their place in the exploration of several pathologies in pediatric settings.

background: The assessment of liver fibrosis is an essential part of the fol-low-up of patients with chronic HbV. Ultrasound and fibroscan are two com-monly used non-invasive examinations and the purpose of this study is to as-sess the correlation between the results of these two examinations in the as-sessment of liver fibrosis. Methodology: This was a descriptive and analytical cross-sectional study with retrospective collection over a period of 30 months from January 01, 2018 to June 30, 2020 on the correlation between the ultra-sound aspects and the results of the fibroscan of patients with chronic carriers of Hepatitis b virus at the Teaching Hospital of bogodogo and at the faith-based health center CANDAF. Results: A total of 85 patients with fibrosis were collected. The 30 - 40 age group was the most represented (41.18%), with a male predominance of 52.94%. The patients with a married life were the most represented (77.64%), the social stratum the most represented were the middle managers (32.94%). University level was predominant among educa-tion levels (45.64%). The personal history was traditional circumcision (34.12%), excision (21.18%), manicure (16.47%). The circumstances of the findings were blood donation (34.12%), screening (21.18%), fortuitous findings (16.47%). The association between echostructure and fibrosis on the fibroscan scan was significant (homogeneous hyperechoic echostructure p = 0.0028 and granular echostructure p = 0.0001). Fibrosis on scans had a significant associa-tion with liver contour (Regular contours p = 0.0009 and p = 0.0002). Fibrosis on CT scan had a significant association for the diagnosis of fibrosis on ultra-sound (p = 0.0002 for hepatic steatosis and cirrhosis and p = 0.0013 for hepat-ic dysmorphia), on the other hand, no significant association for hepatomegaly (p = 0.8883). Imaging Findings of Children Stroke in burkina Faso and etiological Approach

Introduction: Stroke is defined as a sudden onset focal neurological deficit of presumed vascular origin. Although it is essentially adult and elderly condition, stroke can occur in children. Their diagnosis essentially radiological is based on two main imaging modalities namely Computed Tomography (CT) and/or Magnetic Resonance Imaging (MRI). Our study aimed to study radiological features of stroke in children with an etiologic approach in three health facilities of Ouagadougou. Material and Method: We conducted a retrospective study with descriptive focus during 11-year period from January 1st, 2006 to September 1st, 2017 among children aged between 02 months and 16 years who underwent brain CT scan and/or MRI;in whom diagnosis of stroke had been established in radiology department of Yalgado Ouedraogo Teaching Hospital (CHU-YO), Schiphra Methodist Medical Center (CM Schipphra) and Saint Camille Hospital of Ouagadougou (HOSCO). Results: 40 cases of pediatric stroke were collected including 23 at HOSCO, 15 at CHU-YO and 03 at CM Schipphra. Mean age was 6.45 years, with extremes of 2 months and 192 months. Unilateral contralateral motor weakness was the most common clinical presentation (32.5%) followed by seizures (17.5%). Ischemic stroke dominated in 82.5% of cases. Sino-venous thrombosis was rare and suspected in only one case. Sylvian artery was mostly affected in ischemic stroke for 56.25% of patients. The main etiologic factors found were hematologic (67.50%) then infectious factors (25%). Sickle cell disease was always incriminated in hematologic factor. Homozygous SS patients were the most numerous (82%). Conclusion: Stroke diagnosis in children is based on etiologic investigation, biology with a major role of medical imaging. Sickle cell disease accounts a lot in occurrence of stroke in children in our context.

background: Acute intestinal ischemia is an abdominal suffering occasioned by a sudden reduction of mesenteric blood circulation. It is a rare and potentially serious medical-surgical emergency, deadly in 50% to 100% of cases. Diagnosis is known to be difficult at the early stage during which a well-conducted treatment may help reduce mortality. Multi-detector scanners were assessed to be far more sensitive and appropriate for the diagnosis of mesenteric ischemia than angiography. Objective: Study the tomodensitometric aspects of acute intestinal ischemia, and the traps of late diagnosis, in order to optimize the care and improve the prognosis of this disease. Patients and Methods: This is a prospective, longitudinal and descriptive study covering a period of nine (9) months. We have included all cases of acute intestinal ischemia. Results: Twenty (20) cases of acute intestinal ischemia were registered i.e. a frequency of 2.2 cases per month. The average age of the patients was 67.8 years with 0.7 as sex ratio. Abdominal pain was noted in all cases. History of cardiovascular disease was found in seven (7) cases. Abdominal angioscan was performed in fifteen (15) cases. There was a suspected diagnosis upon request in ten (10) cases. The main intestinal lesions were the lack of parietal enhancement (13 cases) and parietal thickening (12 cases). There were sixteen (16) cases of acute mesenteric ischemia and four (4) cases of ischemic colitis. The acute mesenteric ischemia was arterial in eleven (11) cases, venous in two (2) cases and mixed in two (2) cases. Diagnosis was made at the stage of intestinal infarction in eleven (11) cases. Ten (10) patients were operated and 10 others were treated medically. Thirteen (13) cases of death were registered. Conclusion: Acute intestinal ischemia is a rare but serious disease.
