Contralateral eye study of refractive,topographic and aberrometric outcomes after femtosecond assisted Myo Ring implantation and DALK for management of keratoconus

AIM：To evaluate the efficacy of femtosecond laser assisted MyoRing intrastromal corneal implant and deep anterior lamellar keratoplasty （DALK） for management of moderate to advanced keratoconus regarding the degree of changes in visual acuity, refraction, corneal asphericity and ***：A prospective non comparative interventional case study was conducted in Ophthalmology Department,Ain Shams University Hospital in the period from January 2015 to February 2017. The study included 30 eyes of moderate to advanced keratoconus. MyoRing was implanted in one eye （Group Ⅰ） and DALK operation was performed in the contralateral eye of the same patient （Group Ⅱ）.Preoperative and 6mo post-operative uncorrected visual acuity（UCVA）, corrected distance visual acuity （CDVA）, spherical equivalent （SE）, corneal and refractive astigmatisms,keratometry and Q-value using topography images were acquired. Quality of vision was assessed in all eyes including total corneal, anterior corneal high order aberrations analysis at 5 mm pupil size and the Strehl ratio of point spread function （PSF） as an objective measure of ***：Mean postoperative UCVA, CDVA, SE, refractive astigmatism,keratometryreadingsandasphericity were statistically improved compared to preoperative parameters in both groups （P〈0.05）. Significant reduction of all corneal aberrations following both techniques（P〈0.05） was achieved except mean trefoil and mean PSF in Group Ⅰ （P〉0.05）. Postoperative corneal aberrations were significantly lower in Group Ⅱ compared to Group Ⅰ.A statistically significant negative correlation was found in Group Ⅰ between the mean change in CDVA （logMAR）and the mean preoperative and mean postoperative total corneal aberrations root mean square （RMS; r=-0.78,P=0.04）. Also a statistically significant negative correlation was found between mean preoperative coma RMS and mean post PSF （r=-0.86, P=0.01）. AIM： To evaluate the safety and efficacy of posterior sub-Tenon＇s carboplatin injection compared to intravitreal melphalan injection in the management of retinoblastoma （RB） with secondary vitreous seeds. The outcome measures were vitreous seeds regression, need for other treatment modalities to achieve ocular salvage and treatment side effects. METHODS： A prospective interventional comparative nonrandomized study included RB eyes developed secondary vitreous seeds during the period of follow up. They subdivided into two groups： study group Ⅰ where posterior sub-Tenon＇s carboplatin （20 mg/2 mL） was injected and study group Ⅱ where intravitreal melphalan （20 μg/0.1 mL） was injected. The injections repeated every 2-4wk. RESULTS： Thirty-three eyes were included in the study. Seventeen eyes （16 patients） in study group Ⅰ and 16 eyes （16 patients） in study group Ⅱ. Ten eyes （30.3%） were completely salvaged following local chemotherapies, Ocular salvage was 23.5% following posterior sub-Tenon＇s carboplatin injection versus 37.5% following intravitreal melphalan raised to 47.1% and 75% with addition of external beam radiotherapy （EBR） with no statistically significant difference between the study groups （P=0.16）. A statistically significant correlation was found between ocular salvage rate and type of vitreous seeds either dust, spheres and clouds （r=0.42, P=-0.015） and eyes harbor new solid tumor growth （r=0.35, P=0.045）. The mean and median follow up periods following local chemotherapy injections were 2.0y in the study group Ⅰand 2.37y in the study group Ⅱ. Few complications were reported： periorbital edema in all eyes and ocular motility disturbances in 13 eyes （76.5%） following posterior sub-Tenon＇s carboplatin injection. Vitreous hemorrhage developed in 2 eyes （12.5%） and localized retinopathy in 5 eyes （31.25%） following intravitreal melphalan. ~ CONCLUSION： Local chemotherapy for treatment of RB with secondary vitreous seeds is safe and can salvage 30.3% of eyes

Safety and efficacy of photodynamic therapy using BCECF-AM compared to mitomycin C in controlling post-operative fibrosis in a rabbit model of subscleral trabeculectomy

AIM： To evaluate the safety and efficacy of cellular photoablation using BCECF-AM [2, T-bis-（2-carboxyethyl） -5-（and-6）-carboxyfluorescein, acetoxymethyl ester mixed isomersl as a method to control postoperative fibrosis in subscleral trabeculectomy （SST） compared to mitomycin C （MMC） in a rabbit model. METHODS： A comparative prospective case-control animal study was conducted. Fourteen rabbits were subjected to SST with intraoperaUve use of wound modulating agents （MMC or BCECF-AM） of the right eye （study groups I and II respectively） and SST without use of intraoperative wound modulating agents for the left eye （control group II）. Two rabbits 4 eyes were considered as control group I with no surgical intervention. BCECF-AM was injected subconjunctivally 30min before surgery followed by intraoperative illumination with diffuse blue light for 10min. Antifibrotic efficacy was established by clinical response and histological examination. Clinical response was assessed by measuring intraocular pressure （lOP） at day 1, 3, 5, 7, 14, 21 postoperatively, Success was defined by 〉20.0% reduction in lOP from the preoperative values without anti-glaucoma medications. RESULTS： The mean percentage of reduction was 35.0% in the study group I with only one eye （14.3%） had 12.5% reduction. The mean percentage of reduction was 28.0% in the study group U with two eyes （28.6%） in study group II had 14.2% reduction each. Regarding the control group II, the mean percentage of reduction was 14.3% with 64.3% eyes had 〈20.0% reduction. There was a highly statistically significant difference between each of the study groups （right eyes） and the corresponding control group II （left eyes） as regards the mean postoperative lOP values started from day 5 in both study groups and this highly significant difference remained so till the end of the follow up period. AIM： To evaluate the effect of punctal occlusion using thermosensitive（smart plug） versus silicone plug for management of aqueous deficient dry eye on cornea sensitivity, ocular surface health and tear film ***： A comparative prospective interventiona case study included 45 patients with bilateral severe form of aqueous deficient dry eye. In each patient, the smar plug was inserted in the lower punctum of the right eye which was considered as study group 1 and silicone plug was inserted in the lower punctum of the left eye o the same patient which was considered as study group2. All patients were subjected to careful history taking and questionnaire for subjective assessment of severity of symptoms. Corneal sensitivity, corneal fluorescein rose bengal staining, Schirmer＇s I test, tear film break up time and conjunctival impression cytology were performed pre and 1, 3 and 6mo post plug ***： A statistically significant improvement in subjective and objective manifestations occurred following treatment with both types of plugs（P 〈0.01）The thermosensitive plug caused significant overal improvement, decrease in frequency of application o tear substitutes and improvement of conjunctiva impression cytology parameters in the inserted side（P 〈0.01）. Canaliculitis was reported in two eyes（4.4% following punctal occlusion using thermosensitive plug（study group 1）. Spontaneous plug loss occurred in 21eyes（46.6%） in the silicone plug group（study group 2）.CONCLUSION： Improvement of subjective and objective manifestations of aqueous deficient dry eye occurs following punctal plug occlusion. Thermosensitive plug has good patient＇s compliance with fewer complications and lower rates of loss compared to the silicone plug.

Inverse microemulsion system of cetyltrimethylammonium bromide（CTAB） molecules is utilized for virtually monodispersed and controlled growth of HCl polyaniline（PANI） nanoparticles at room temperature（ca.300 K）.The templated electroconductive polymer reveals lamellar crystalline structure under X-ray diffraction signifying marked sub-chain alignment of the polymerized *** nanostructured polymer has spherically symmetric morphology in a size range of 2.0 nm to 6.0 nm under electron microscope *** permeation chromatography gives polydispersity index of 1.02 for nanostructured polymer in agreement with the size monodispersity transpired by electron *** *** conductivityσ_（dc） of PANI at room temperature is 10.11 S/cm whereas the variation of conductivity with temperature in the range 227-303 *** that the conducting mechanism can be considered as three-dimensional variable-range-hopping（3D-VRH）.UV-Vis spectrum indicates two broad absorption bands due to polaron formation that contributes to enhanced electrical conductivity of the *** fundamental absorption edge in the polymer is formed by direct allowed transitions to the extent that the optical band gap value was found to be 2.35 *** crystalline nanostructure and homogeneous doping attained in the cationic template of amphiphile are argued as contributing factors to the enhanced conductivity of the polymer.

A modified adaptive two-phase sliding mode controller for the synchronous motor drive that is highly robust to uncertain- ties and external disturbances is proposed in this *** proposed controller uses two-phase sliding mode control (SMC) where the 1st phase mainly controls the system in steady states and disturbed states-it is a smoothing *** 2nd phase is used mainly in the case of disturbed ***,it is an autotuning phase and uses a simple adaptive algorithm to tune the gain of conventional variable structure control (VSC).The modified controller is useful in position control of a permanent magnet synchronous drive.

New model for standpipe pressure prediction while drilling using Group Method of Data Handling

The continuous evaluation of the measured Stand Pipe Pressure(SPP)against a modeled SPP value in real-time involves the automatic detection of undesirable drilling events such as drill string washouts and mud pump *** theoretical and experimental studies have been established to calculate the friction pressure losses using different rheological models and based on an extension of pipe flow correlations to an annular ***,it would not be feasible to employ these models for real-time applications since they are limited to some conditions and intervals of application and require input parameters that might not be available in real-time on each *** this study,The Group Method of Data Handling(GMDH)is applied to develop a trustworthy model that can predict the SPP in real-time as a function of mud flow,well depth,RPM and the Fan VG viscometer reading at 600 and 300 *** order to accomplish the modeling task,3351 data points were collected from two wells from Algerian *** and statistical assessment criteria disclosed that the model predictions are in excellent agreement with the experimental data with a coefficient of determination of 0.9666 and an average percent relative error less than 2.401%.Furthermore,another dataset(1594 data points)from well-3 was employed to validate the developed correlation for *** obtained results confirmed that the proposed GMDH-SPP model can be applied in real-time to estimate the SPP with high ***,it was found that the proposed GMDH correlation follows the physically expected trends with respect to the employed input ***,the findings of this study can help for the early detection of downhole problems such as drill string washout,pump failure,and bit balling.

Distribution and ecological risk assessment of some heavy metals in coastal surface sediments along the Red Sea,Egypt

The ecological risk assessment for Al,Zn,Cu,Ni,V,Pb,Cd,and Hg in surface sediment collected from the Egyptian Red Sea coast was evaluated using the Geo-accumulation Index(Igeo).Sediment Enrichment Factor（SEF） and Potential Ecological Risk Index（PERI） *** predominant heavy metal,aluminum,showed high concentrations along both of Aqaba Gulf（4378.8 ± 2554.1 μg/g） and southern part of the Red Sea（2972.8 + 1527.5 μg/g）.while it recorded the lowest concentration in Suez Gulf（829.7 ± 398.2 μg/g）.The determined heavy metal concentrations had the order of Al > Zn >-Ni > V > Pb > Cu > Cd > *** statistical analyses showed some correlations among the heavy metals *** international sediment quality guidelines were used to estimate the quality of the collected ***,the recorded average heavy metals concentrations were lower than those of the permissible contents for sediment quality *** Geo-accumulation index calculations(Igeo) proved that the investigated region could be classified as an unpolluted *** Enrichment Factor(KSEF) study showed high values in Suez Gulf *** single pollution index analysis of heavy metals in the sediments（Ci_f） indicated that Al,Zn,V,and Pb were of natural origin,while Ni,Cd and Hg were seriously affected by human ***,amongst,all the determined heavy metals,Cd and Hg gave moderate ecological risk indicators.

In this editorial,we discuss the article in the World Journal of *** article conducts a meta-analysis of the diagnostic accuracy of the urea breath test(UBT),a non-invasive method for detecting Helicobacter pylori(***)infection in *** is based on radionuclide-labeled *** methods,both invasive and non-invasive,are available for diagnosing *** infection,inclu-ding endoscopy with biopsy,serology for immunoglobulin titers,stool antigen analysis,and *** guidelines recommend UBTs as the primary choice for diagnosing *** infection and for reexamining after eradication *** is used to be the first choice non-invasive test due to their high accuracy,specificity,rapid results,and ***,its performance remains unaffected by the distribution of *** in the stomach,allowing a high flow of patients to be *** its widespread use,the performance characteristics of UBT have been inconsistently described and remain incompletely *** are two UBTs available with Food and Drug Administration approval:The 13C and 14C *** tests are affordable and can provide real-time *** may prefer the 13C test because it is non-radioactive,compared to 14C which uses a radioactive isotope,especially in young children and pregnant *** there was heterogeneity among the studies regarding the diagnostic accuracy of both UBTs,13C-UBT consistently outperforms the *** makes the 13C-UBT the preferred diagnostic ***,the provided findings of the meta-analysis emphasize the significance of precise considerations when choosing urea dosage,assessment timing,and measurement techniques for both the 13C-UBT and 14C-UBT,to enhance diagnostic precision.
