Objective: To compare the inhibitory effect between DL-tryptophan and bovine lactoferrin on biofilm formed by isolated Pseudomonas aeruginosa strains. Methods: The study was carried out on 40 patients suffering from surgical site infection. Wound pus was collected using sterile swabs after isolation, and identified by common bacteriological methods. Isolated Pseudomonas aeruginosa strains were grown on biofilm enhancing materials, and then the inhibitory effects of different concentrations of DL-tryptophan and lactoferrin were tested using scanning electron microscopy and microtitre plate methods. Results: There was no significant difference in the inhibitory effect between DL-tryptophan and lactoferrin at 0.5 mg/mL. While in concentration of 1 mg/mL and 2 mg/mL, tryptophan showed more significant inhibitory effect than lactoferrin. Conclusions: Both DL-tryptophan and bovine lactoferrin have inhibitory effect on Pseudomonas aeruginosa biofilm formation in a dose dependent manner, and the inhibitory effect of DL-tryptophan is stronger.

AIMTo evaluate the reversibility of minimal hepatic encephalopathy (MHE) following liver transplantation (LT) in Egyptian cirrhotic patients. METHODSThis prospective study included twenty patients with biopsy-proven liver cirrhosis listed for LT and twenty age- and sex-matched healthy control subjects. All underwent neuro-psychiatric examination, laboratory investigations, radiological studies and psychometric tests including trail making test A (TMT A), TMT B, digit symbol test and serial dotting test. The psychometric hepatic encephalopathy score (PHES) was calculated for patients to diagnose MHE. Psychometric tests were repeated six months following LT in the cirrhotic patient group. RESULTSBefore LT, psychometric tests showed highly significant deficits in cirrhotic patients in comparison to controls (P P P P = 0.005). More patients in whom reversal of MHE was observed had a pre-transplant MELD score CONCLUSIONReversal of MHE in cirrhotic patients could be achieved by LT, especially in those with a MELD score

The wax gel grease (S0) was prepared from a mixture of 2.3:1 base oil blend (base oil grade 260/290, transformer oil), and microcrystalline wax in the presence of 0.1% - 2% of polyoxyethylene sorption nano-palmitate as antioxidant and 2,2 methylene bis (4-methyle-6-tertiary butyl phenol) as anticorrosion. It was found that the prepared Wax gel grease has inconvenient physico-chemical and dielectric properties, so in order to improve its physico-chemical properties (viscosity, penetration, dropping point and water resistance) and dielectric Properties (dielectric constant, dielectric loss and volume resistivity), Butyl rubber, isoprene rubber and bitumen were added separately as thickening agents to the prepared wax gel in certain proportion at certain frequency range 1 - 1000 KHz at 35°C. The best dielectric properties were achieved by adding butyl rubber to the prepared wax gel.

Validation of prognostic indices in Egyptian Budd-Chiari syndrome patients: A single-center study

AIMTo compare predictive ability of Budd-Chiari syndrome (BCS) prognostic indices (PIs) for one-year survival and Transjugular intrahepatic portosystemic shunt (TIPS) *** retrospective study enrolled 194 Egyptian patients with primary BCS who presented to the Budd-Chiari Study Group of Ain Shams University Hospital. Calculation of the available PIs was performed using Child-Pugh and model for end-stage liver disease scores, BCS-specific PIs (Clichy, New Clichy and Rotterdam) for all patients, and BCS-TIPS PI only for patients who underwent TIPS. The overall one-year survival rate and the one-year shunt patency rate for TIPS were *** overall one-year survival rate was 69.6%, and the New Clichy PI revealed the best validity for its prediction at a cut-off value of 3.75, with sensitivity and specificity of 78% and 73.3%, respectively [area under receiver operating characteristic curve (AUC) = 0.806]. The one-year survival rate post-TIPS was 89.7%, and the BCS-TIPS score demonstrated validity for its prediction at a cut-off value of 3.92 (sensitivity and specificity were 71.4% and 64.5%, respectively) (AUC = 0.715). Logistic regression analysis revealed that the New Clichy PI (P = 0.030), high serum total bilirubin (P = 0.047) and low albumin (P < 0.001) were independent factors for predicting mortality within one year. The one-year shunt patency rate in TIPS was 80.2%, and none of the PIs exhibited significant validity for its *** New Clichy score could independently predict the one-year survival in Egyptian BCS patients.

Objective: To evaluate the effectiveness of cryotherapy in managing the pain at the puncture site of Arterio-Venous Fistula (AVF) among children undergoing maintenance hemodialysis (HD).Methods: A one-group pre-post quasi-experiment was performed in two HD centers affiliated with Cairo *** experiment involved 40 children with AVF undergoing *** puncturing,cryotherapy was applied using 2 cm-3 cm pieces of frozen distilled water in a plastic *** was assessed subjectively and objectively in two dialysis sessions before and after cryotherapy.A part from a physiological assessment of vital signs,pain was assessed using the Wong-Baker Faces Pain and the Observed Pain Behavior rating *** research ethics were ***: HD had a median duration of four years,while cryotherapy had a median application time of 8.8 *** Wong-Baker Faces Pain score and almost all observed pain behaviors significantly decreased after *** improvements were observed in respiratory rate before and after needle puncture and in oxygen saturation after needle puncture.A lower skin dryness was observed after cryotherapy (12.5%) than before cryotherapy (52.5%;p < 0.001).Conclusions: Cryotherapy can effectively reduce the venipuncture pain among children with AVF undergoing maintenance ***,the confounding effects of distraction and the non-randomized design used must be both considered when interpreting the *** study recommends the use of cryotherapy in managing needle puncture *** research must adopt a randomized trial design with a placebo to support further the benefits of this procedure.

Ground penetrating radar, electromagnetic terrain conductivity, and electric tomography have proven to be effective tools if they are combined together to investigate archeological sites. We have conducted a geophysical survey at the Akhmim archaeological site, the main objective of our survey is to locate additional buried structures for further excavation. Geophysical data were acquired in the area using the GEM-300 multi-frequency terrain conductivity profiler, the SIR 2000 ground penetrating radar, and the Syscal R2 resistivity meter systems. The results of the integrated interpretation show a number of buried features and a strong linear zone about 1 m wide that coincides with the suspected trend of a buried wall. There appears to be two parallel ridges of strong reflections on either side, indicating two parallel walls extended East-West and a room is identified at the bottom left comer of the site. Moreover, the interpretation results of some selected GPR and dipoledipole resistivity profiles adjacent to the open-air museum suggest the existence of a second statue of Ramses Ⅱ to the right of the previously discovered statue which could still be buried in the sand.

Prospective Assessment of the Rockall Risk Scoring System in Egyptian Patients with Upper Gastrointestinal Bleeding

Background: One of the major challenges in managing acute upper gastrointestinal bleeding involves the identification of patients who are at high risk of rebleeding and death;conversely, the identification of patients who are suitable for early discharge and outpatient endoscopy is also important for effective resource. The use of Rockall system has been shown to represent accurate and valid predictor of rebleeding and death. The aim of this prospective study was to evaluate validity of Rockall score for prediction of rebleeding and mortality in Egyptian patients with upper gastrointestinal bleeding. Patients and Methods: 50 patients presented with upper GI bleeding were subjected to detailed clinical, laboratory, sonographic and endoscopic examination together with Rockall score calculation. Results: According to the findings of upper GI endoscopy patients were further subdivided into two subgroups as follows: Group 1 (n = 32): Patients with portal hypertension manifested by variceal bleeding and/or portal hypertensive gastropathy;and Group 2 (n = 18): other causes of acute upper gastrointestinal bleeding (peptic ulcer disease, Mallory Weiss syndrome…). Clinical condition of the patients was assessed after 48 hours and mortality was recorded. Rockall score was the highest in mortality group, and higher in rebleeding group than stable group (P < 0.01). Rockall score was higher in Child C than Child B and Child A among hepatic patients (P < 0.01). The cutoff value of Rockall score = 3.5 was the best for prediction of rebleeding with sensitivity 100%, specificity 53%, while the cutoff value = 6.5 was the best for prediction of mortality with sensitivity 100% and 93.5% specificity. Conclusion: Rockall score is clinically useful in prediction of rebleeding and mortality in patients with upper gastrointestinal bleeding. Background:Epidemiological studies have demonstrated associations between higher levels of vascular risk factors in midlife and later development of dementia,particularly Alzheimer’s disease(AD).However,in elderly subjects with dementia,some studies have shown that these associations may decrease or even ***,the study aimed to find the association between late life cardiovascular risk factors and neurodegenerative dementia in general and AD in ***:It is a retrospective case control study using electronic medical records that included elderly patients that were reviewed in Ahmadi hospital geriatric clinic,Kuwait,from the period of 1/7/2019 to 1/2/*** hundred and three(203)elderly patients with neurodegenerative dementia(study group)were recruited for this retrospective study and compared to two hundred and one(201)controls with normal cognition for the presence of vascular risk ***:The study included 404 *** ranged from 60 to 107 years(mean age 78.79,±8.13 SD).AD was found to be the most prevalent type of dementia in the study group,as 49.3%(100/203)of the demented patients were diagnosed with *** significant statistical association was found between vascular risk factors and dementia(P>0.05),except for obesity which showed a negative association(P<0.001).Regarding AD,no statistical significance was found between AD and diabetes,hyperlipidemia,smoking nor atrial *** the other hand,the authors found obesity and hypertension more prevalent in the normal cognition group(negative association with P value<0.001,0.05 respectively).Conclusions:The results of the study support an emerging concept that,while elevated levels of vascular risk factors in midlife increase the risk of development of dementia and AD later in life,once dementia begins,these associations may be diminished or reversed in the elderly.
