Comparison of posttraumatic growth and posttraumatic stress of nurses with physicians and medical students after caring of COVID‑19 patients

Objectives:The present study compared the prevalence and psychosocial factors affecting posttraumatic growth(PTG),posttraumatic stress disorder(PTSD),and psychological distress in nurses,physicians,and medical students.Materials and Methods:In a cross‑sectional study,three groups of medical staff including nurses(n=57),physicians(n=40),and medical students(n=34)who were responsible for the care/treatment of COVID‑19 patients admitted to a general hospital for 4 months responded to Posttraumatic Growth Inventory,Brief Symptom Inventory‑18,Resilience Questionnaire,PTSD Screen,and Social Capital‑Integrated Questionnaire.Results:After the exposure of medical staff to at least 4 months of treatment/care of COVID‑19 patients,PTG prevalence was higher than PTSD(38.2%vs.14.6%),but they experienced some degrees of psychological distress(65.5%).The nurses had 8.33(confidence interval[CI]:2.5–26.7)times higher PTG rate than medical students(P<0.001).Physicists also experienced 5.00(CI:1.4–26.7)times higher PTG than medical students(P<0.001).PTG was aided by age,married status,strong resilience,and high social capital,but gender had no influence.Resilience played an important protective role to prevent the incidence of psychological distress in nurses,medical students,and physicians.Conclusion:Despite the fact that the PTSD and psychological distress were same in the three groups of medical staff,the nurses had a greater rate of PTG than physicians and medical students.

Objective:Paclitaxel(PAC)is a chemotherapy drug and has an active role in the treatment of lung,breast,and ovarian cancers;however,its use causes increased levels of free radicals,leading to severe and irreversible organ damage.Thymus vulgaris is a species of flowering plant that contains various antioxidant chemical compounds.This study aimed to investigate the effect of T.vulgaris on PAC-induced oxidative damage in mice testis and sperm parameters.Methods:In this study,40 BALB/c mice were divided into five groups:normal control(NC);positive control(20 mg/kg of PAC);and three treatment groups(4.5,9,and 18 mg/kg doses of T.vulgaris extract+20 mg/kg PAC).Treatments were administered intraperitoneally daily for 50 days.Finally,the levels of serum luteinizing hormone(LH),follicle-stimulating hormone(FSH),and testosterone were measured using immunoassay method.Testicular stereological features and sperm parameters were also calculated.Antioxidant parameters were measured using nitrite oxide(NO)and perioxidation levels and ferric-reducing ability of plasma assay.The expressions of p53,caspase-3,Bax,and Bcl-2 were measured through real-time quantitative polymerase chain reaction.Results:T.vulgaris+PAC treatments at all doses significantly increased all parameters in NC and three treatment groups compared with the positive control group(20 mg/kg of PAC)(P<0.05),except the LH,FSH,and NO levels,which were decreased.Further,significantly downregulated levels of p53,caspase-3,and Bax genes and upregulated levels of Bcl-2 gene expression were noted in NC and three treatment groups in comparison with the positive control group(P<0.05).Conclusions:T.vulgaris administration attenuates the toxic effects of PAC on male reproductive parameters.

Squeezing ground in tunneling is associated with large deformation of the tunnel face. In this study, squeezing characteristics of the ground and rock conditions in Golab water conveyance tunnel, Iran, are discussed and the classification of squeezing behavior around zones where the problems occurred is presented. The squeezing conditions were investigated using empirical and semi empirical methods. In the next step, creep convergence of the tunnel with Burger's model was simulated by the numerical method. Numerical analysis showed that wall displacement(64.1 mm) of the Golab tunnel was more than allowable strain(1% of the tunnel diameter), therefore, it was found that squeezing phenomenon could exist, leading to the failure of the support system. Numerical analysis at the phyllite-slate zone also showed squeezing conditions due to the weakness of rock mass and high overburden that this situation cause failure in the segmental lining. In this research, failure in segmental lining in phyllite-slate zone verified the results of the numerical modeling.

Enclosed parking garages require mechanical ventilation fans to dilute concentrations of pollutants emitted from vehicles,which contributes to energy use and peak electricity demand.This study develops and applies a simulation framework combining multi-zone airflow and contaminant transport modeling,fan affinity laws,and realistic assumptions for vehicle traffic patterns and carbon monoxide(CO)emissions to improve our ability to predict the impacts of various ventilation control strategies on indoor air quality and fan energy use in parking garages.The simulation approach is validated using measured data from a parking garage case study and then applied to investigate fan energy use,peak power demand,and resulting CO concentrations for four different ventilation control strategies in a model underground parking garage under a variety of assumptions for model inputs.The four ventilation control strategies evaluated include one simplistic schedule(i.e.,Always-On)and three demand-based strategies in which fan speed is a function of CO concentrations in the spaces,including Linear-Demand Control Ventilation(DCV),Standardized Variable Flow(SVF),and a simple On-Off strategy.The estimated annual average fan energy consumption was consistently lowest with the Linear-DCV strategy,resulting in average(±standard deviation)energy savings across all modeled scenarios of 84.3%±0.4%,72.8%±3.6%,and 97.9%±0.1%compared to SVF,On-Off,and Always-On strategies,respectively.The utility of the framework described herein is that it can be used to model energy and indoor air quality impacts of other parking garage configurations and control scenarios.

Validation of nutritional risk index method against patientgenerated subjective global assessment in screening malnutrition in colorectal cancer patients

Objective：To validate malnutrition screening tool of nutrition risk index （NRI） against patent-generated subjective global assessment （PG-SGA） as a gold standard tool in colorectal cancer patients before radiotherapy.Methods：Nutritional status of 52 volunteer colorectal cancer patients with a mean age of 54.1±16.8 years who referred to radiotherapy center were assessed by PG-SGA （gold standard method） and NRI.Serum albumin levels of patients were determined by colorimetric method.A contingency table was used to determine the sensitivity,specificity,and predictive value of the NRI in screening patients at risk of malnutrition,in comparison with the PG-SGA in patients before radiotherapy.Results：The findings of PG-SGA and NRI showed that 52％ and 45％ of patients in our study were moderately or severely malnourished respectively.The NRI had a sensitivity of 66％ and a specificity of 60％ against PG-SGA.The positive predictive value was 64％ and the negative predicative value was 62％.The agreement between NRI and PG-SGA was statistically insignificant （kappa =0.267; P＞0.05）.Conclusions：The findings of present study showed that the prevalence of malnutrition was high in patients with colorectal cancer.Moreover,NRI method had low sensitivity and specificity in assessing nutritional status of patients with cancer.It seems that the combination of anthropometric,laboratory parameters and a subjective scoring system may be helpful tools in screening of malnutrition in cancer patients.

Objective: To predict the trend of cutaneous leishmaniasis and assess the relationship between the disease trend and weather variables in south of Fars province using Seasonal Autoregressive Integrated Moving Average(SARIMA) model,Methods: The trend of cutaneous leishmaniasis was predicted using Mini tab software and SARIMA model,Besides,information about the disease and weather conditions was collected monthly based on time series design during January 2010 to March 2016,Moreover,various SARIMA models were assessed and the best one was selected,Then,the model's fitness was evaluated based on normality of the residuals' distribution,correspondence between the fitted and real amounts,and calculation of Akaike Information Criteria(AIC) and Bayesian Information Criteria(BIC),Results: The study results indicated that SARIMA model(4,1,4)(0,1,0)(12) in general and SARIMA model(4,1,4)(0,1,1)(12) in below and above 15 years age groups could appropriately predict the disease trend in the study area,Moreover,temperature with a three-month delay(lag3) increased the disease trend,rainfall with a four-month delay(lag4) decreased the disease trend,and rainfall with a nine-month delay(lag9) increased the disease trend,Conclusions: Based on the results,leishmaniasis follows a descending trend in the study area in case drought condition continues,SARIMA models can suitably measure the disease trend,and the disease follows a seasonal trend.

Effects of Royal Jelly Supplementation on Glycemic Control and Oxidative Stress Factors in Type 2 Diabetic Female:A Randomized Clinical Trial

Objective： It has been proposed that royal jelly has antioxidant properties and may improve oxidative stress and glycemic control. Therefore, we investigated the effects of royal jelly supplementation in diabetic females. Methods： In this pilot, parallel design randomized clinical trial, 50 female volunteers with type 2 diabetes were randomly allocated to the supplemented （25, cases） and placebo （25, cases） groups, based on random block procedure produced by Random Allocation Software, given a daily dose of 1,000 mg royal jelly soft gel or placebo, respectively, for 8 weeks. Before and after intervention, glycemic control indices, antioxidant and oxidative stress factors were measured. Results： After royal jelly supplementation, the mean fasting blood glucose decreased remarkably （163.05_± 42.51 mg/dL vs. 149.68 ± 42.7 mg/dL）. Royal jelly supplementation resulted in significant reduction in the mean serum glycosylated hemoglobin levels （8.67%± 2.24% vs. 7.05% ± 1.45%, P=0.001） and significant elevation in the mean insulin concentration （70.28 ± 29.16 pmol/L vs. 86.46± 27.50 pmol/L, P=0.01）. Supplementation significantly increased erythrocyte superoxidase dismutase and glutathione peroxidase activities and decreased malondialdehyde levels （P〈0.05）. At the end of study, the mean total antioxidant capacity elevated insignificantly in both groups. Conclusions： On the basis of our findings, it seems that royal jelly supplementation may be beneficial in controlling diabetes outcomes. Further studies with larger sample size are warranted.

Development of a new method for RMR and Q classification method to optimize support system in tunneling

Rock mass classification system is very suitable for various engineering design and stability analysis. JH classification method is confirmed by Japan Highway Public Corporation that this method can figure out either strength or deformability of rock mass, further appropriating the amount of rock bolts, thickness ofshotcrete, and size of pitch of steel ribs just after the blasting procedure. Based on these advantages of JH method, in this study, according to data of five deep and long tunnels in Iran, two equations for estimating the value of JH method from Q and RMR classification systems were developed. These equations as a new method were able to optimize the support system for Q and RMR classification systems. From JH classification and its application in these case studies, it is pointed out that the JH method for the design of support systems in underground working is more reliable than the Q and RMR classification systems.
