Effectiveness and safety of generic and brand direct acting antivirals for treatment of chronic hepatitis C

BACKGROUND Direct acting antivirals(DAAs)are a very effective treatment for hepatitis C virus(HCV).However,brand DAAs are *** licensing of cheaper generic DAAs may address this issue,but there is a lack of clinical studies comparing the efficacy of generic vs brand DAA *** To compare the efficacy and safety of generic against brand DAAs for chronic hepatitis C treatment in *** This was a retrospective observational study involving 289 patients with chronic HCV infection during 2016 to *** were 149 patients who were treated with brand DAAs,while 140 patients were treated with generic *** used DAAs were Ombitasvir/Paritaprevir/Ritonavir±Dasabuvir±Ribavirin,and Sofosbuvir/Daclatasvir±*** at 12 wk post treatment was the main outcome *** Overall,87 patients(30.1%)had cirrhosis and 68.2%had genotype 1 HCV *** 12 wk post treatment,SVR was achieved by 271(93.8%)of the *** patients who were treated with generic medications,134(95.7%)achieved SVR at 12 wk post treatment,compared to 137(91.9%)among those treated with brand medications(P=0.19).Having cirrhosis[odds ratio(OR):9.41,95%confidence interval(CI):2.47–35.84]and having HCV genotype 3(OR:3.56,95%CI:1.03–12.38)were significant independent predictors of not achieving *** transaminase,gamma-glutamyl transpeptidase,and total bilirubin levels decreased significantly following therapy with both generic and brand *** Generic and brand DAAs demonstrate comparable effectiveness in the treatment of chronic hepatitis C *** are safe and equally effective in improving biochemical markers of hepatic inflammation.

Validation of a serum microRNA panel as biomarkers for early diagnosis of hepatocellular carcinoma post-hepatitis C infection in Egyptian patients

AIM To investigate the prospective importance of serum micro(mi)RNAs(mi R-125 b, mi R-138 b, mi R-1269, mi R-214-5p, mi R-494, mi R375 and mi R-145) as early biomarkers for the diagnosis of hepatitis C virus(HCV)-related hepatocellular carcinoma(HCC).METHODS Two-hundred and fifty HCV4 a patients, 224 HCV4 aHCC patients, and 84 healthy controls were enrolled in the study. Expression levels of mi R214-5p, mi R-125 b, mi R-1269 and mi R-375 were quantified using quantitative real-time *** Expression of the selected mi RNAs in serum wassignificantly lower in HCC patients than in the healthy controls, except for mi R-1269 and mi R-494. There was a significant difference between HCC and HCV patients, in particular for HCC and late stage fibrosis, rather than HCV patients and early fibrosis. It is obvious that mi R-1269 was significantly upregulated in HCC cases compared to hepatic fibrosis cases. Each mi RNA can show HCC progression. Multivariate logistic regression analysis indicated that the tested panel of mi RNAs(mi R214-5p, mi R-125 b, mi R-1269 and mi R-375) represent accurate and specific indictors of HCC *** This study presents a panel of mi RNAs with strong power as putative diagnostic and prognostic biomarkers for HCV-induced HCC. Moreover, mi R-214-5p and mi R-1269 could be considered as early biomarkers for tracking the progress of liver fibrosis to HCC.

Rice straw(RS)collected from Egyptian agricultural environment(al-Masied village-alsharqia Governorate-Egypt)was used as substrate for three fungal isolates namely:4b,1Asp,Pleurotus ostreatus NRRL 3501 and mixed culture of(4b,1Asp).Isolates number 4b and 1Asp were identified using the molecular technique(18S rRNA).Isolate 4b and 1Asp,were identified as Trichoderma saturnisporum MN1-EGY and Aspergillus niger MN2-EGY,*** saturnisporum MN1-EGY extract exhibited the highest antimicrobial activity compared to the other fungal *** phenolic,antimicrobial and antioxidant activities were also determined from the ethyl acetate extract of rice straw inoculated by these *** antioxidant scavenging concentration(SC50)values of the tested samples were 88.84,91.45,94.65 and 100.0μg/ml,respectively for Pleurotus sp.,Aspergiluus niger,Aspergiluus niger+Trichoderma sp.,and Trichoderma sp.,after 15 days incubation,compared to ascorbic acid as positive control with SC50 value equal to 8.0μg/*** phenolic contents(TPCs)were maximum in Pleurotus *** and found to be the most polyphenolic enriched sample(380.64 mg GAE/g dry extract)compared to extracts of Aspergiluusniger,Aspergiluus niger+Trichoderma sp.,and Trichoderma sp.(261.89,198.52,and 119.80 mg GAE/g dry extract),*** GC-MS analysis has been also performed for the promising extracts.

The interrelationship between insulin resistane and alzheimer development

Abnormalities in insulin metabolism, characteristic of T2DM, are among the major factors thought to mechanistically influence the onset of AD. These abnormalities are thought to play a role in AD via their influence on the synthesis and degradation of Aβ and as a consequence of the cascade of neuronal alterations resulting from the effects of danger/alarm signals from oligomeric amyloid species. Additionally, recent studies have indicated that certain signal transduction pathways downstream of the InsR may also promote the generation of Aβ peptides by modulating the cleavage of the parent Aβ precursor protein (AβPP) at the γ-secretase site, a cleavage site necessary for Aβ amyloidogenicity. Glucose homeostasis is critical for energy generation, neuronal maintenance, neurogenesis, neurotransmitter regulation, cell survival and synaptic plasticity. It also plays a key role in cognitive function. In an insulin resistance condition, there is a reduced sensitivity to insulin resulting in hyperinsulinemia;this condition persists for several years before becoming full blown diabetes. Toxic levels of insulin negatively influence neuronal function and survival, and elevation of peripheral insulin concentration acutely increases its cerebrospinal fluid (CSF) concentration. Peripheral hyperinsulinemia correlates with an abnormal removal of the amyloid beta peptide (Aβ) and an increase of tau hyperphosphorylation as a result of augmented cdk5 and GSK3β activities. This leads to cellular cascades that trigger a neurodegenerative phenotype and decline in cognitive function. Chronic peripheral hyperinsulinemia results in a reduction of insulin transport across the BBB and reduced insulin signaling in brain, altering all of insulin’s actions, including its anti-apoptotic effect. However, the increase in brain insulin levels resulting from its peripheral administration at optimal doses has shown a cognition enhancing effect on patient with AD.

BACKGROUND Nonalcoholic steatohepatitis-related cirrhosis is one of the liver complications in type 2 diabetes mellitus(T2DM)and reported to be a risk factor for developing hepatocellular carcinoma(HCC).A reliable screening biomarker of liver cirrhosis(LC)and HCC among T2DM patients is important to reduce the morbidity and mortality of this ***(miRNA)is considered a key player in HCC and T2DM,and it might be a hidden culprit in diabetes-associated HCC,making it a promising reliable prognostic *** To investigate the signature of serum miRNAs as early biomarkers for the screening of HCC among diabetic *** Expression profiles of miRNAs in serum samples of diabetic LC and diabetic HCC patients were assessed using Illumina sequencing;then,RT-qPCR was used to validate significantly altered miRNAs between the two *** miRNAs were tested in serum samples of 200 T2DM patients,270 LC patients,200 HCC patients,and 225 healthy control ***,receiver operating characteristic(ROC)analysis,with area under the curve(AUC),was performed to assess the diagnostic performance of the screened miRNAs for discriminating HCC from LC and nonmalignant patients(LC+T2DM).RESULTS Expression of the sequenced miRNAs in serum was different in HCC vs LCpositive T2DM *** miRNAs(miR-34a,miR-221)were significantly upregulated and five miRNAs(miR-16,miR-23-3p,miR-122-5p,miR-198,miR-199a-3p)were significantly down-regulated in HCC compared to LC *** of ROC curve demonstrated that the combination of these seven miRNAs can be used as a reliable biomarker for detection of HCC in diabetic patients,as it could identify HCC with high diagnostic accuracy in diabetic LC patients(AUC=0.993)and in diabetic nonmalignant patients(AUC=0.961).CONCLUSION This study validates a panel of serum miRNAs that can be used as a reliable noninvasive screening biomarker of HCC among T2DM cirrhotic and noncirrhotic *** study recommends furt

The Impact of Adding Magnesium Sulfate to Bupivacaine versus Bupivacaine alone in Fascia Iliaca Compartment Block in Burn Patients Undergoing Skin Grafting Procedures;Comparative Study

Purpose: The purpose of the study is to evaluate the analgesic effect of adding magnesium sulfate to bupivacaine in Fascia Iliaca Compartment Block (FI-CB). Patients and Methods: Ninety-six burn patients scheduled for skin-grafting procedures were allocated into two equal groups. After induction of general anaethesia, forty-eight patients received US guided FICB using 35 ml bupivacaine 0.25% + 5 ml isotonic sodium chloride solution with a total volume of 40 ml. The other forty-eight patients received 35 ml bupivacaine 0.25% + 250 mg of magnesium sulfate with a total volume of 40 ml. Results: Total postoperative pethidine consumption in the first 24 h post-operative showed a highly statistically significant decrease in the magnesium group in comparison to the bupivacaine group. Conclusion: Adding magnesium sulfate to bupivacaine in FICB in skin grafting procedures decreased the pain scores post-operative, delayed the first request of analgesia and reduced the total analgesic consumption in the first 24 h post-operative without any significant side effects.

Non-Linear Analysis of Vibrations of Non-Linear System Subjected to Multi-Excitation Forces via a Non-Linear Absorber

The dynamic response of mechanical and civil structures subjected to high-amplitude vibration is often dangerous and undesirable. Sometimes controlled vibration is desirable as in the machinery used in the formation of rigid hard material as Ceramics and diamond. This process is done via the passive control methods. The main purpose of this paper is to how reduction of vibration of nonlinear system subjected to multi-excitation forces via a nonlinear absorber. The nonlinear differential equations describing the model, which describe ultrasonic cutting machine are solved by using perturbation method. The effects of different parameter on the response of the system are studied. The stability of the numerical solution is investigated by using frequency response equations and phase-plane method. The simulation results are achieved using Matlab and Maple programs. A comparison is made with the available published work.
