Alginate/gelatin/boron-doped hydroxyapatite-coated Ti implants:in vitro and in vivo evaluation of osseointegration

In this study,boron-doped hydroxyapatite(BHT)-loaded alginate/gelatin-based(A/G)hydrogel coating on Ti was fabricated to support bone integration through triggering osteoinduction,vascularization and immunomodulation.Initially,highly reproducible,cheap and time-effective BHT was produced,which significantly promoted higher osteogenic and angiogenic maturation,while a mild innate immune response was observed.The immense potential of BHT was evidenced by the production of a gap-filling A/G/BHT interphase on Ti implants to mimic the osseous extracellular matrix to achieve functional bridging and exert control over the course of innate immune response.We initially aminosilanized the implant surface using 3-aminopropyl triethoxysilane,and then coated it with 0.25%w/v alginate with 20 mM 1-ethyl-3-(3-dimethylaminopropyl)carbodiimide and N-hydroxysuccinimide to allowthe A/G/BHT pre-gel to disperse evenly and covalently attach on the surface.The pre-gel was added with 0.2 M NaCl to homogeneously blend BHT in the structure without inducing ionic crosslinking.Then,the coated implants were freeze-dried and stored.The coated layer demonstrated high cohesive and adhesive strength,and 8-month-long shelf-life at room temperature and normal humidity.The A/G/BHT was able to coat an irregularly shaped Ti implant.Osteoblasts and endothelial cells thrived on the A/G/BHT,and it demonstrated greatly improved osteogenic and angiogenic capacity.Moreover,A/G/BHT maintained macrophage viability and generated an acute increase in immune response that could be resolved rapidly.Finally,A/G/BHT was shown to induce the robust integration of implant in a rabbit femur osteochondral model within 2months.Therefore,we concluded that A/G/BHT coatings could serve as amultifunctional reservoir,promoting the strong and rapid osseointegration of metallic implants.

Co-doping of hydroxyapatite with zinc and fluoride improves mechanical and biological properties of hydroxyapatite

Hydroxyapatite(HA) co-doped with Zn2+ tand F- ions was synthesized by precipitation method for the first time in this study.FTIR spectroscopy revealed Zn2+ tand F ions incorporation into HA structure.Co-doping of Zn2+ tand F- ions decreased unit cell volume of HA and decreased grain sizes.Zn2+ tor 5 mol% F addition into HA significantly improved its density.Microhardness was increased with Zn2+ taddition and further increase was detected with F co-doping.Zn2+ tand F co-doped samples had higher fracture toughness than pure HA.Zn2+ tincorporation to the structure resulted in an increase in cell proliferation and ALP activity of cells,and further increase was observed with 1 mol%F addition.With superior mechanical properties and biological response 2Zn1 F is a good candidate for biomedical applications.

Arterial complication of irreversible electroporation procedure for locally advanced pancreatic cancer

Irreversible electroporation(IRE) is a non-thermal ablation technique used especially in locally advanced pancreatic carcinomas that are considered surgically unresectable.We present the first case of acute superior mesenteric artery(SMA) occlusion secondary to pancreatic IRE procedure that has not been reported before in the literature.A 66-year-old man underwent neoadjuvant chemoradiotherapy for locally advanced pancreatic ductal adenocarcinoma.IRE procedure was applied to the patient during laparotomy under general anesthesia.After finishing the procedure,an acute intestinal ischemia was detected.A conventional vascular angiography was performed and a metallic stent was successfully placed to the SMA and blood flow was maintained.It is important to be careful in such cases of tumor involvement of SMA when evaluating for IRE procedure of pancreatic tumor.

Outcomes of Laparoscopic Cholecystectomy in Patients with End-Stage Renal Disease

Purpose: In this study, we aimed to discuss the laparoscopic cholecystectomy in patients with end stage renal disease compared to the general population. Materials and Methods: We retrospectively evaluated a group of patients with (n = 45) and without (n = 90) end-stage renal disease undergoing laparoscopic cholecystectomy. The groups were compared in terms of length of surgery;duration of hospitalization after surgery;use of blood derivatives;mortality rates;and perioperative, postoperative, and postdischarge complications. Results: Patients with end-stage renal disease exhibited a higher frequency of associated diseases;lower hemoglobin levels;and elevated alkaline phosphatase, blood urea nitrogen, and creatinine values. Statistically significant differences were found between the two groups regarding length of surgery (83.6 ± 14.88 vs. 71.7 ± 11.42 minutes;p p p = 0.011), postoperative (p p = 0.011) rates. Among all patients with end-stage renal disease, 12 (26.7%) were converted to an open procedure (p < 0.001). Conclusion: Despite higher complication rates of laparoscopic cholecystectomy in end-stage renal disease patients, laparoscopic cholecystectomy could be performed safely in patients with end-stage renal disease with low levels of complications and no associated mortality.

PURPOSE:The aim of this study was to investigate the effects of lateral internal sphincterotomy on quality of life in patients with chronic anal fissure using the Gastrointestinal Quality of Life Index and the Fecal Incontinence Quality of Life Scale.METHODS:Adult patients with chronic anal fissure underwent lateral internal sphincterotomy with the open technique.Two hundred forty-four patients completed the Gastrointestinal Quality of Life Index questionnaire at admission and at 12 months postoperatively.The Fecal Incontinence Severity Index score was calculated preoperatively and at 2 and 12 months postoperatively.The Fecal Incontinence Quality of Life Scale was administered to any patient who had a Fecal Incontinence Severity Index score greater than 0 at 12 months postoperatively.RESULTS:The mean preoperative Gastrointestinal Quality of Life Index score was 118.34± 6.33,which developed to 140.74 ± 2.38 postoperatively(P< 0.001) .At the two-month follow-up,18 patients(7.38 percent) had a Fecal Incontinence Severity Index score greater than 0.By 12 months,the number of patients with Fecal Incontinence Severity Index score greater than 0 was reduced to seven(2.87 percent) .These seven patients had a Gastrointestinal Quality of Life Index score similar to that of the group with postoperative Fecal Incontinence Severity Index score of 0,and only three patients(1.22 percent) had evident deterioration in the Fecal Incontinence Quality of Life Scale.The 12-month total Gastrointestinal Quality of Life Index score of the three patients who developed anal abscess/fistula after sphincterotomy(139.33 ± 3.21) was similar to the Gastrointestinal Quality of Life Index score of those without complications.However,the Gastrointestinal Quality of Life Index score of the recurrent cases(111.53 ± 3.53) was apparently low.CONCLUSION:The gastrointestinal quality of life improved significantly following lateral internal sphincterotomy,regardless of the surgical complications or postoperative disturbances of continence.Only 1.2 perce
