Clinical significance of low transaminase levels in children with inflammatory bowel disease

Background Low alanine aminotransaminase (LALT) levels may be seen in patients with inflammatory bowel disease (IBD),but there has been no study about the frequency and its clinical *** aimed to analyze the frequency of LALT,and its clinical significance in children with *** The study included the 89 patients with IBD without hepatobiliary *** was defined as ALT levels < 5 U/*** and clinical findings and outcome of the patients with and without LALT were *** LALT was found 47.1% of the *** initial examination,it was more common in female patients (92.3 vs.41.3%,P < 0.001) and patients with CD (57.7 vs.30.2%,P=0.01).75% of the patients with penetrating Crohn's disease (CD) had LALT (P=0.002).Hemoglobin (10.4±2.1 vs.11.7± 1.9 g/dL,P=0.01),folic acid (5.2±3.3 vs.8.6±5.9 ng/mL,P =0.02) and serum albumin levels (3.6 ± 0.8 vs.4.7 ± 5 g/dL,P =0.002) were significantly low in patients with *** was associted with the disease relapse within 2 weeks in 12 of the 16 patients with LALT whereas it was seen in 16 of the 73 patients without LALT during the follow-up (75 vs.21.9%,P < 0.001).Additionally,steroid dependency was more common in patients with LALT during the follow-up (62.5 vs.12.3%,P<0.001).Conclusions LALT is common in children with IBD especially in CD and associated with low hemoglobin,albumin and folic acid *** may be a marker of relapse and steroid dependency.

A new approach to the surgical treatment of parasitic cysts of the liver:Hepatectomy using the liver hanging maneuver

AIM： To review 11 patients with parasitic cysts of the liver, who were treated by hepatic Iobectomy using the liver hanging maneuver （LHM）.METHODS： Between January 2003 and June 2006, we retrospectively analyzed patients who underwent surgical treatment due to parasitic cysts of the liver, at the Ege University School of Medicine, Department of General Surgery. Of these, the patients who underwent hepatic lobectomy using the LHM were reviewed and evaluated for surgical treatment outcome.RESULTS： Over a three-year period, there were 102 patients who underwent surgical treatment for parasitic cysts of the liver. Of these, 11 （10%） patients with parasitic cysts of the liver underwent hepatic Iobectomy using the LHM. Presenting symptoms were abdominal pain, dyspepsia, and cholangitis. Cyst locations were as follows： right lobe filled with cyst, 7 （63%）; segmental location, 2 （18%）; and multiple locations, 2 patients （18%）. All patients underwent hepatic Iobectomy with an anterior approach using the LHM. The intraoperative blood transfusion requirement was one unit for 3 patients and two units for one patient. Postoperative complications included pulmonary atelectasy （2, 18%） and pleural effusion （2, 18%）. No significant morbidity or mortality was observed.CONCLUSION： We concluded that hepatic Iobectomy using the LHM should be considered, not only for hepatic tumors or donor hepatectomy, but also to treat parasitic cysts of the liver.

Comparison of early enteral nutrition in severe acute pancreatitis with prebiotic fiber supplementation versus standard enteral solution:A prospective randomized double-blind study

AIM: To compare the benefi cial effects of early enteral nutrition (EN) with prebiotic fiber supplementation in patients with severe acute pancreatitis (AP).METHODS: Thirty consecutive patients with severe AP, who required stoppage of oral feeding for 48 h, were randomly assigned to nasojejunal EN with or without prebiotics. APACHE Ⅱ score, Balthazar’s CT score and CRP were assessed daily during the study ***: The median duration of hospital stay was shorter in the study group [10 ± 4 (8-14) d vs 15 ± 6 (7-26) d] (P 0.05). Deaths occurred in 6 patients (20%), 2 in the study group and 4 in the control group. The mean duration of APACHE Ⅱ normalization (APACHE Ⅱ score unal EN with prebiotic fiber supplementation in severe AP improves hospital stay, duration nutrition therapy, acute phase response and overall complications compared to standard EN therapy.

Molecular approach to genetic and epigenetic pathogenesis of early-onset colorectal cancer

Colorectal cancer(CRC) is the third most frequent cancer type and the incidence of this disease is increasing gradually per year in individuals younger than 50 years old. The current knowledge is that early-onset CRC(EOCRC) cases are heterogeneous population that includes both hereditary and sporadic forms of the CRC. Although EOCRC cases have some distinguishing clinical and pathological features than elder age CRC, the molecular mechanism underlying the EOCRC is poorly clarified. Given the significance of CRC in the world of medicine, the present review will focus on the recent knowledge in the molecular basis of genetic and epigenetic mechanism of the hereditary forms of EOCRC, which includes Lynch syndrome, Familial CRC type X, Familial adenomatous polyposis, Mut YH-associated polyposis, Juvenile polyposis syndrome, Peutz-Jeghers Syndrome and sporadic forms of EOCRC. Recent findings about molecular genetics and epigenetic basis of EOCRC gave rise to new alternative therapy protocols. Although exact diagnosis of these cases still remains complicated, the present review paves way for better predictions and contributes to more accurate diagnostic and therapeutic strategies into clinical approach.

Effect of prayer on the self-care agency and hope in hemodialysis patients

Background:The level of self-care of hemodialysis patients is important in controlling the disease process and *** care has an important place in the process of resolving existing or developing problems of patients receiving hemodialysis treatment and in adapting to *** is not only a resource that encourages religious/spiritual coping,it is also a spirituality that strengthens ***:The objective of this study is to determine whether prayer has an effect on self-care agency and hope in hemodialysis *** and Methods:This is a descriptive and correlational study which was conducted with 180 patients in a private and a public dialysis *** data of 173 hemodialysis patients who met the inclusion criteria were collected using personal information form,Herth Hope Scale,Self-care Agency Scale,and Prayer Attitude ***:A positive,moderate,and statistically significant correlation was found between the Prayer Attitude Scale and total score(r=0.144,P=0.048)and sub-scale future(r=0.350,P=0.000)of Herth Hope *** there was positive correlation between the Prayer Attitude Scale and the total score of Self-care Agency Scale(r=0.599,P=0.000)and its five sub-dimensions drug use(r=0.566,P=0.000),diet(r=0.619,P=0.000),self-monitoring(r=0.522,P=0.000),hygienic care(r=0.576,P=0.000)and mental status(r=0.328,P=0.000).Conclusions:The study found that prayer had a positive effect on self-care agency and hope in hemodialysis patients.

Laparoscopic cholecystectomy in a patient with situs inversus totalis

Currently, laparoscopic cholecystectomy is an undoubtfully optimal treatment of cholelithiasis. What about performing this procedure on a patient with situs inversus totalis and what are the difficulties of this operation for a right-handed surgeon? We presented a 35-year-old man with unknown situs inversus totalis who was admitted with epigastric pain and digestive problems. Ultrasonography and computed tomography of the abdomen confirmed the diagnosis of a gallstone. Besides, the liver and gallbladder were on the left side and the spleen was on the right. All systems were left-right reversal as mirror image in all diagnostic studies. Laparoscopic cholecystectomy was safely performed, despite of difficulties of situs inversus. The patient was discharged on postoperative day 1. It should be considered that existence of other anomalies may easily cause uninvited injuries. In the patients with situs inversus, laparoscopic cholecystectomy can be safely managed by an experienced surgeon through laparoscopy, and also hepatobiliary surgery.

Influence of H pylori on plasma ghrelin in patients without atrophic gastritis

AIM:To determine the association between H pylori infection and serum ghrelin levels in patients without atrophic ***:Fifty consecutive patients(24 males and 26 females)with either H pylori-positive gastritis(n = 34)or H pylori-negative gastritis(n = 16)with normal gastric acid secretion determined by 24-h pHmetry and without atrophic gastritis in histopathology were enrolled in this ***-four H pylori-infected patients were treated with triple therapy consisting of a daily regimen of 30 mg lansoprazole bid,1 g amoxicillin bid and 500 mg clarithromycin bid for 14 d,followed by an additional 4 wk of 30 mg lansoprazol treatment.H pylori infection was eradicated in 23 of 34(67.6%)patients.H pylori-positive patients were given eradication *** acidity was determined via intragastric pH *** ghrelin was measured by radioimmunoassay(RIA).RESULTS:There was no signifficant difference in plasma ghrelin levels between H pylori-positive and H pylori-negative groups(81.10 ± 162.66 ng/L vs 76.51 ± 122.94 ng/L).In addition,there was no significant difference in plasma ghrelin levels and gastric acidity levels measured before and 3 mo after the eradication ***:H pylori infection does not influence ghrelin secretion in patients with chronic gastritis without atrophic gastritis.

Epidemiological and clinical features of paediatric open globe injuries in southwestern Turkey

AIM:To evaluate the epidemiologic,anatomic,and clinical features of open globe injuries in ***:The medical files of patients under the age of 16 who had been operated for an open globe injury at Akdeniz University Hospital’s Department of Ophthalmology were retrospectively ***:A total of 90 patients were evaluated in this *** these patients,26(28.9%)were female and64(71.1%)were *** mean age of the patients was7.7±4.2 *** male/female ratio was observed to increase with increasing age(P=0.006,P=7.48).Injuries were most likely to occur in spring and autumn(P=0.028).The time interval between the injury and the surgical repair was 9.36±***(44.4%)of the injuries occurred in the home,27(30%)occurred in the yard,and 21(23.3%)happened while playing in the *** most common causes of injury were sharp metal objects(P<0.001).Injury to the cornea occurred in47(52.2%)of the patients(P<0.001).The most common complication to occur was cataract *** operations were necessary for 37(41.1%)of the *** final visual acuity was correlated with both the initial visual acuity of the wounded eye prior to surgery and the length of the wound(P<0.001,r=0.502 and P<0.001,r=-0.442,respectively).CONCLUSION:Open globe injuries that are suffered in childhood generally occur either at home,in the yard,or on the street,with sharp metal objects being the most common cause of *** initial visual acuity and the length of the wound are the most important determinants of the final visual acuity.

Aluminum is an abundant metal in the earth’s crust that turns out to be toxic in acidic *** plants are affected by the presence of aluminum at the whole plant level,at the organ level,and at the cellular *** as a cash crop(Nicotiana tabacum L.)is a widely cultivated plant worldwide and is also a good model organism for *** there are many articles on Al-phytotoxicity in the literature,reviews on a single species that are economically and scientifically important are *** this article,we not only provide the biology associated with tobacco Al-toxicity,but also some essential information regarding the effects of this metal on other plant species(even animals).This review provides information on aluminum localization and uptake process by different staining techniques,as well as the effects of its toxicity at different compartment levels and the physiological consequences derived from *** addition,molecular studies in recent years have reported specific responses to Al toxicity,such as overexpression of various protective ***,this review discusses data on various organelle-based responses,cell death,and other mechanisms,data on tobacco plants and other kingdoms relevant to these studies.
