Thiol-Disulphide Balance:Could Be a New Marker for Thyroid Cancer?

Objectives. Cancer is a very widespread disorder known in world wide since long, but its biochemical features remain unclear. Thyroid carcinomas are the most common endocrine cancer and its frequency continues to escalate. There is evidence that the serum concentration of TSH is an unreliant predictor for the diagnosis of thyroid cancer. The formation of the plasma thiol pool from low and large molecular weight proteins suggests that thiol/disulfide balance is important in cancerous cases. The aim of this study was to investigate an oxidative stress marker (thiol/disulphide homeostasis) and IMA (Ischemia modified albumin), Albumin, CEA (Carcinoembryonic antigen), TSH (Thyroid stimulate hormone), thyroxine (T4), free thyroxine (FT4), triiodothyronine (T3) and free triiodothyronine (FT3) in patients with thyroid cancer and compare the results with healthy controls for the first time in literature. Materials-Methods: A total of 43 participants including 23 patients with thyroid cancer and 20 healthy individuals were included in the study. Serum levels of TSH, T4, FT4, T3 and FT3 have been measured during treatment and follow-up of patients with thyroid carcinoma. Serum levels of TSH, T4, FT4, T3 and FT3, IMA, Albumin, CEA, Native thiol (-SH), disulfide (-S-S) and total thiol (TT) as well as disulphide/native thiol and disulphide/total thiol ratios were compared between the groups. Native thiol, disulfide and total thiol concentrations were measured with a novel automated method (Roche, cobas 501, Mannheim, Germany). Results and conclusion: This paper discusses an oxidative stress marker (thiol/disulphide homeostasis) and tumor markers IMA, Albumin, CEA, TSH, T4, FT4, T3 and FT3 in patients with thyroid cancer and compare the results with healthy controls. Mean age at participant was 41.73 years for thyroid cancer patients (21 females/2 males). A control group of 20 participants was included the study (19 females/1 male, mean age 51.75).

Effectiveness of Bemiparin Sodium for Preventing Pregnancy Loss in Patients with MTHFR C677T Mutation and Habitual Abortus

Objective: In this study the purpose is to evaluate the effectiveness of daily bemiparin sodium injection in preventing early pregnancy loss and in achieving live births in pregnant patients, not ahead of 6th week, who are diagnosed with habitual abortion in a case-controlled randomized prospective study. Study Design: In this prospective study, 120 pregnant patients not ahead of 6th week and aged between 21 - 36 and diagnosed with habitual abortion between April 2011 and April 2012 in our clinic were included. Habitual abortion diagnosis criteria was based on patient history of three or more miscarriages before the 20th week and a genetically documented homozygote or heterozygote mutation of the methylenetetrahydrofolate Reductase gene. Results: The rate and the number of live births in the study group was 60 (93.75%) and statistically higher than the control group 32 (57.14%). The difference was found to be statistically significant (p = 0.001). The rate and the number of abortions in the control group was 24 (42.85%) and statistically higher than the study group 4 (6.25%). The difference was found to be statistically significant (p = 0.001). Conclusion: In order to maintain a healthy pregnancy an effective uteroplacental circulation is a must and any defects in haemostasis mechanisms may have an adverse influence. A few non-randomized studies comparing pregnancy results of low dose salycilic acid and low molecular weight heparin usage in patients also show that prophylaxis might be beneficial.

Hysteroscopic septum resection in patients with recurrent abortions and infertility

Objective: The aim of this study was to assess the achievement of pregnancy as well as the reproductive outcome after hysteroscopic septum resection in women with recurrent abortions or infertility. Design: retrospective, clinical trial. Setting: University hospitals. Patient(s): Fourty women with a diagnosis of uterine septum who had a history of pregnancy loss or infertility. They were randomized into two groups. Group I includes 18 infertile women and group II includes 22 recurrent miscarriage women. Intervention(s): Hysteroscopic metroplasty was performed on all patients in both groups. Result(s): 40 patients who were diagnosed with a partial septate uteri had hysteroscopic septum resection and were divided into two groups: Group I (18 patients) presented with infertility and Group II (22 patients) with recurrent abortions. The mean post-operative follow up period was 24 months. Group I, 12 patients (66.6%) and Group II, 17 patients (77.2%) achieved pregnancy, with respective abortion rates of 2 (11.1%) and 4 (18.1%). Conclusion(s): Using hysteroscopy to perform metroplasty on septate uteri makes the procedure safer, easier, and less complicated. This procedure is recommended for all cases diagnosed with an uterine septum.

Association of Tetanus-Diphtheria Vaccination during Pregnancy with Preeclampsia

Introduction: The tetanus-diphtheria vaccine administered during pregnancy is used as a toxoid vaccine with an intact antigen structure. In this study, we investigated the association of tetanus-diphtheria vaccination (TDV) during pregnancy with adverse maternal and neonatal outcomes and particularly preeclampsia that may occur during pregnancy. Materials and Methods: From the patients who presented to our clinic between 01.01.2016 and 31.12.2016 and underwent a cesarean section, a total of 435 patients, 327 who received TDV and 108 who did not receive TDV, were included in the study. The groups were compared with respect to their demographic characteristics. The patient data on the gestational week, birth weight, normal newborn, preeclampsia, small for gestational age (SGA), early membrane rupture, preterm labor, diabetic mother’s baby, prematurity, temporary neonatal tachypnea, newborn’s need for intensive care, and neonatal mortality were recorded. Results: While 14.7% of the women who had been administered TDV had preeclampsia diagnosis during their pregnancy, 12% of those who had not been vaccinated were diagnosed with preeclampsia. This showed no significant correlation between having been vaccinated during pregnancy and preeclampsia (p = 0.474). The other outcomes we had specified had also no statistically significant correlations with vaccination during pregnancy. Having been vaccinated during pregnancy was found correlated only with the place of residence and nulliparity (p = 0.004 and p = 0.002, respectively). Conclusion: No correlation was found in the study between TDV during pregnancy and preeclampsia. According to the data obtained from this study, vaccination was found to have no negative effect on preeclampsia or the other outcomes.

Antioxidant Supplement Improves the Pregnancy Rate in Patients Undergoing in Vitro Fertilization for Unexplained Infertility

Objective: This is a retrospective study to evaluate the effect of using the antioxidants containing coenzyme Q10 (CoQ10) and omega-3 in patients undergoing in vitro fertilization and intracytoplasmic sperm injection (IVF-ICSI) for unexplained infertility. Materials Methods: A total of 299 patients undergoing IVF-ICSI for unexplained infertility at the IVF center of Ondokuz Mayis University were studied. The subject group consisted of 135 patients who used OMEPA Q10 (Co Q10 100 mg, omega-3 600 mg, eicosapentaenoic acid 300 mg, docosahexaenoic acid 230 mg) for two months before ovarian stimulation. A group of 164 patients who did not use the supplement were evaluated as the control group. Results: The pregnancy rate of patients using antioxidants was statistically higher than that of those who did not use the supplement (p 0.05). The pregnancy rates of patients in the two groups were 32.9% and 49.6% respectively. The pregnancy rate of patients using antioxidants was statistically higher than that of those who did not use the supplement (p 0.05) seemed not enough. Conclusion: We concluded that using antioxidants that contain Co Q10 and omega-3 in patients undergoing IVF-ICSI for unexplained infertility increased the clinical pregnancy rate. We recommend using antioxidants that contain Co Q10 and omega-3 in infertile patients.

Are Acute Phase Reactants and CA125 Useful for Pregnancy Prediction in Frozen Embryo Transfer?

Objective: This study assessed blood C-reactive protein (CRP), fibrinogen, procalcitonin, and CA125 levels and determined whether they have an effect on pregnancy. Material & Methods: Between Feb. and Sep. 2018, 50 consecutive women who had their first frozen embryo transfer at a tertiary referral IVF center in Turkey were included in the study. Serum samples were collected on the second day of the cycle and on the transfer day. The women were divided into two groups based on the results of pregnancy outcome: pregnant (group 1) and non-pregnant (group 2). Blood CRP, fibrinogen, procalcitonin, and CA125 levels were compared between the groups. Results: There were no statistically significant differences between the groups according to CA125, CRP, fibrinogen, and procalcitonin levels at the beginning of the cycle and on the transfer day. In comparison with levels at the beginning of the cycle and on the transfer day, fibrinogen and CRP were significantly higher on the transfer day than at the beginning of the cycle. Procalcitonin was significantly higher on the transfer day than at the beginning of the cycle in group 1. However, there was no statistically significant difference between the transfer day and the beginning of the cycle in group 2. Conclusions: However, the fibrinogen, CRP and procalcitonin levels were significantly higher on the transfer day than at the beginning of the cycle. The results indicated that acute phase reactants or CA125 should not be used to determine the time of embryo transfer or predict pregnancy.

A Different Method in the Treatment of Placenta Previa:A Comparison of Lower Uterine Segment Transverse Suture Technique and Bakri Balloon Application

Objective: The incidence of placenta previa (PP) has been increasing due to the rise in cesarean rates. The aim of this study is to determine the success of lower uterine segment compression suture and bakri balloon applications in patients diagnosed with placenta previa. Materials and Methods: 257 patients who underwent cesarean section due to placenta previa totalis (PPT) between the years of 2010-2018 in Ondokuz Mayis University were screened retrospectively. The patients were evaluated in four groups according to their treatment method: medical treatment, bakri balloon application, compression suture and hysterectomy. Results: In the study, between the years 2010-2015, postpartum hysterectomy was performed in the cases of adherent placenta previa, and hysterectomy was performed in 33 patients. Between the years of 2015-2018, first, bakri balloon or compression suture was applied;and if bleeding control was not achieved despite these, then, hysterectomy was performed. Between 2015 and 2018, a total of 27 patients underwent compression suture and 1 patient had a hysterectomy after all. The success rate was 96.3% with compression suture and 91.7% for bakri balloon application. Totally, 24 patients were treated with bakri balloon and yet, two patients underwent hysterectomy. When all the cases were examined, complications caused by urinary bladder and ureteral injury were seen in 5.6% of patients, and all of these complications were also seen in hysterectomy patients. Conclusion: Lower uterine segment transverse suture technique and bakri balloon application should be considered as a good alternative to prevent hysterectomy in patients with placenta previa totalis. In addition, they are effective treatment methods that can be used without increasing maternal morbidity.

Nano-phytosomes(NP),lipid-based nano-carriers,were used to encapsulate the ***(Phy)is known for its important biological properties with low bioavailability and low *** aim of the study was to prepare phosphatidylcholine(PC)based Phy loaded NP to overcome the bioavailability and storage stability *** characterization of NP such as entrapment efficiency(EE),particle size,poly-dispersity index(PDI),ζ-potential and microstructural properties was conducted and in vitro bioaccessibility and storage stability of bioactive properties were *** highest EE was determined as 81.92%however with a loading capacity(LC)of 58.12%.Phy-NPs showed good characteristics,such as low PDI values(<0.5),low particle size(106.13–134.10 nm),and non-dense spherical shape of *** NP complexes showed significant bioaccessibility with total phenolic content(TPC)values greater than 50%in intestinal ***,PC based NP did not show a clear-cut pattern of TPC and color protection of Phy via increased PC content.

Conduction Mechanism Analysis of Inversion Current in MOS Tunnel Diodes

Self inversion issue and excess capacitance phenomenon were observed for the first time in relatively thick silicon dioxide (SiO2) in the form of MOS (metal(Al)/SiO2/p type crystalline silicon) structure. Both phenomena were based on minority carriers (electrons in this case) and studied through DC current-applied bias voltage (I-V) and AC admittance measurements in dark/light condition as a function of ambient temperature (295 - 380 K). Either of the cases was the departure of traditional MOS analysis, manifesting themselves in the inversion regime of MOS diode. Increase in frequency/temperature/light intensity within dark and light conditions led to weaken the maxima of hump in C-V curves and finally turned into deep depletion mode after exceeding threshold value of frequency/temperature/light intensity. In resumed conditions, supplementary I-V measurements were carried out to describe the generation and conduction mechanism(s) for minority carriers (electrons).

目的:研究探通术与灌溉对儿童先天性鼻泪管阻塞(congenital nasolacrimal duct obstruction,CNDO) 的成功率及相关因素。方法:从2005/2011 年,患有CNDO的儿童235例261眼行鼻泪管探通术。患者按年龄分为3组:组1,131 眼为12 ～24月龄; 组2,82 眼为>24～48 月龄;组3,48 眼为>48～120 月龄。在全身麻醉下进行上、下泪小点探通术。术后1d; 1,2wk;1,3mo进行随访。通过Mann Whitney U检验和卡方检验分析手术成功率与年龄的相关性。结果:儿童235例(女性122例,男性113例)的平均年龄为27. 6±10. 7(12～120)mo。3组患儿一次探通术后的成功率分别为90.1%、85.4%和47.6%,并且组1 、组2成功率显著高于组3(P<0.05)。患儿53例(20.3%)再次行探通术,3组的成功率分别为:61.5%、58.3%和25.0%。结论:鼻泪管探通术是治疗CNDO的有效方法,特别是对于小于2岁的患儿。手术成功率随着年龄增加而降低,但是上下泪小管二次探通术可以提高术后效果。
