Comparison of intravitreal bevacizumab and triamcinolone acetonide theraphies for diffuse diabetic macular edema

AIM: To compare therapeutic effects of intravitreal triamcinolone acetonide(IVTA) versus intravitreal bevacizumab(IVB) injections for bilateral diffuse diabetic macular edema(DDME). METHODS: Forty eyes of 20 patients with bilateral DDME participated in this study. For each patient, 4 mg/0.1 m L IVTA was injected to one eye and 2.5 mg/0.1 m L IVB was injected to the other eye. The effects of injection for diabetic macular edema(DME) were evaluated using best-corrected visual acuity(BCVA), central macular thickness(CMT) by optical coherence tomography(OCT)and intraocular pressure(IOP) by applanation *** underwent eye examinations, including BCVA,CMT, and IOP at pre-injection, 1, 4, 8, 12 and 24 wk after injection. During the follow-up, second injections were performed to eyes which have CMT greater than 400 μm at 12 wk for salvage ***: BCVA(logarithm of the minimum angle of resolution) at pre-injection, 1, 4, 8, 12 and 24 wk after injection was 0.71 ±0.19, 0.62 ±0.23, 0.63 ±0.12, 0.63 ±0.13,0.63±0.14 and 0.61±0.24 in the IVTA group and 0.68±0.25,0.61 ±0.22, 0.60 ±0.24, 0.62 ±0.25, 0.65 ±0.26 and 0.59 ±0.25 in the IVB group, respectively. CMT(μm) at pre-injection,1, 4, 8, 12 and 24 wk after injection was 544 ±125, 383±96,335 ±87, 323 ±87, 333 ±92, 335 ±61 in the IVTA group and514 ±100, 431 ±86, 428 ±107, 442 ±106, 478 ±112, 430 ±88 in the IVB group respectively. Reduction ratios of mean CMT were 29% at 1wk, 38% at 4wk, 40% at 8wk, 38% at12 wk, and 38% at 24 wk in the IVTA group. Second IVTA injections were performed to the 6 eyes(30%) at 12 *** ratios of mean CMT were 16% at 1wk, 17% at4wk, 14% at 8wk, 7% at 12 wk, and 16% at 24 wk in the IVB group. Relation of eye dominancy with color vision discrimination performance ability in normal subjects

AIM:To evaluate the performance of dominant eye(DE) for color vision discrimination ability among the medical students with normal color ***:Total of 50 students studying at Baskent University Faculty of Medicine,including 31 males(62%) and 19 females(38%),with visual acuity of 20/20 and without congenital color vision deficiency(CCVD) evaluated by Ishihara pseudoisochromatic plate test(IPPT) were recruited for this prospective comparative study upon their voluntary *** was determined by the Gündogan *** color discrimination ability was examined with the FarnsworthMunsell 100 hue(FM100) *** was applied by two days interval to all subjects for the three times while two eyes(TE),right eye(RE) and left eye(LE) were seeing for detecting red-green(r/g),blue-yellow(b/y) local color spectral regions error *** error scores were evaluated for both in DE and non-dominant(NDE).P values below 0.05 were considered to be statistically ***:The students aged 21.18±2.52 years(mean±SD).Without sex difference the RE and the LE dominancy were found 22(44%) and 28(56%) respectively and FM 100 test total error scores of DE in both r/g-b/y regions were found without gender difference 24.12±14.70,34.68±18.95,*** the NDE in both,r/gb/y regions error scores without gender difference were 32.20±19.21,36.24±17.56,*** difference of total error scores between the DE and NDE was found as 58.80±29.92,68.44±*** statistical differences among the DE and the NDE in r/g local region and total error scores were found significant in both genders(P<0.05,P<0.001). CONCLUSION:The color vision discrimination performance ability was found prominent for *** superiority was attributed to higher sensitivity of the r/g local color spectral *** conclude that DE has priority in r/g color spectral region,probably including inhibition of NDE.

目的:为了检查一项新的性能测试,对同以受试者用两种不同的方法进行检测和再测试,并比较和讨论这些检测方法的可靠性。方法:共179个大学生(平均年龄±标准差为19.37±1.62)a自愿参加这项由110名女性(61.5%)和69名男性(38.5%)组成的调查。优势眼由两种不同的方法检测,一个为McManus方法,一个为Gndo■an方法。利用SPSS的重测信度的可靠性分析保证该项调查的可靠性。结果:McManus方法检测128名(71.5%)参加者右眼为优势眼,无性别差异。同样利用Gndogˇan方法再检测发现110名(61.5%)参加者右眼为优势眼。两种方法的检测结果由Fisher确切概率法分析发现呈显著性相关(P<0.01),一致性数值为为κ=0.256,P<0.001。利用Mc-Manus法发现女性参加者中74名(67.3%)右眼为优势眼,36名(32.7%)左眼为优势眼。利用Gndog■an法再检测发现62名女(56.4%)右眼为优势眼,48名女(43.6%)左眼为优势眼。通过Fisher确切概率法分析发现McMa-nus和Gndo■an两种方法在女性优势眼检测方面呈显著性相关(P<0.05),一致性数值为κ=0.239,P<0.01。在男性参与者中,通过McManus法检测右眼优势眼为54眼(78.3%),左眼优势眼为15眼(21.7%),利用Gndo■an法再检测右眼优势眼为48眼(69.6%),左眼优势眼为21眼(30.4%)。利用χ2检验和Fisher确切概率法分析数据。利用Kappa一致性检验分析不同方法的一致性。比例的比较由z检验完成,P值小于0.05为差异显著。结论:通过检测观察到女性和男性均以右眼优势眼为主,没有性别差异。右眼优势这种功能性偏侧化现象可能是由于大脑左半球的主导作用造成的。这对于将来在一侧面偏重方面的研究很重要,也许会成为一个重要的模式系统。

Degradation of refractory organics in concentrated leachate by the Fenton process: Central composite design for process optimization

The primary aim of this study is inert COD removal from leachate nanofiltration concentrate because of its high concentration of resistant organic *** this framework,this study focuses on the treatability of leachate nanofiltration concentrate through Fenton oxidation and optimization of process parameters to reach the maximum pollutant removal by using response surface methodology(RSM).Initial pH,Fe2+concentration,H_(2)O_(2)/Fe^(2+)molar ratio and oxidation time are selected as the independent variables,whereas total COD,color,inert COD and UV254 removal are selected as the *** to the ANOVA results,the R^(2) values of all responses are found to be over 95%.Under the optimum conditions determined by the model(pH:3.99,Fe^(2+):150 mmol/L,H_(2)O_(2)/Fe^(2+):3.27 and oxidation time:84.8 min),the maximum COD removal efficiency is determined as 91.4%by the *** color,inert COD and UV254 removal efficiencies are determined to be 99.9%,97.2%and 99.5%,respectively,by the model,whereas the total COD,color,inert COD and UV254 removal efficiencies are found respectively to be 90%,96.5%,95.3%and 97.2%,experimentally under the optimum operating *** Fenton process improves the biodegradability of the leachate NF concentrate,increasing the BOD5/COD ratio from the value of 0.04 to the value of *** operational cost of the process is calculated to be 0.238€/g COD_(removed).The results indicate that the Fenton oxidation process is an efficient and economical technology in improvement of the biological degradability of leachate nanofiltration concentrate and in removal of resistant organic pollutants.

Wear Resistance and Non-Magnetic Layer Formation on 316L Implant Material with Plasma Nitriding

In this study, the applicability of plasma nitriding treatment in the production of non-magnetic and corrosion resistant layer on 316L stainless steel implant material was investigated. 316L stainless steel substrates were plasma nitrided at temperatures of 350 ℃, 375 ℃, 400 ℃, 425 ℃ and 450 ℃ for 2 h in a gas mixture of 50% N2-50% H2, respectively. It was determined that the treatment temperature is the most important factor on the properties of the corrosion resistant layer of 316L stainless steel. The results show that s-phase formed at the temperatures under 400 ℃, and at the temperatures above 400 ℃, instead of s-phase, CrN and y＇-Fe4N phases were observed in the modified layer. The electrical resistivity and surface roughness of the modified layer increase with treatment temperature. Under 400 ℃ the corrosion resistance increased with the temperature, above 400 ℃ it decreased with the increase in treatment temperature. It was analyzed that the electrical resistivity and the soft （ideal） ferro- magnetic properties of 316L stainless steel increased with treatment temperature during nitriding treatment. Also, plasma ni- triding at low temperatures provided magnetic behavior close to the ideal untreated 316L stainless steel.

Thermoluminescence Kinetic Parameters of TLD-600 and TLD-700 after 252Cf Neutron+Gamma and 90Sr-90Y Beta Radiations

The thermoluminescent(TL) properties such as glow curve structure, relative thermoluminescence sensitivity,dose response linearity of lithium fluoride thermoluminescent dosimeters 6 LiF:Ti,Mg(TLD-600) and 7 LiF:Ti,Mg(TLD-700) are investigated after irradiationCf neutron+gamma andSr-Y beta sources at room temperature and then the obtained results are compared. The kinetic parameters, namely the order of kinetics b,activation energy Ea and the frequency factor s,are calculated using the computerized glow curve deconvolution(CGCD) program. The effect of heating rate on the glow curves of dosimeters is also investigated. The maximum TL peak intensities and the total area under the glow curves decrease with the increasing heating rate. There is no agreement with the kinetic parameters calculated by the CGCD program for both radiation sources.

Effects of Annealing on Thermoluminescence Peak Positions and Trap Depths of Synthetic and Natural Quartz by Means of the Various Heating Rate Method

Some environmental and experimental variables such as annealing process alter the structure of glow curves of some thermoluminescence *** investigate the effects of annnealing on thermoluminescence peaks of synthetic and natural quartz at different heating *** experimental results show that the peak temperatures of glow peaks in annealed samples shift to higher temperature *** amount of shifting is higher for low temperature peaks than high temperature *** trap depths of annealed and unannealed samples of both synthetic and natural quartz are also obtained by various heating rate *** is noted that the annealing process affects the trap depth of all glow peaks and the trap depth increases after the annealing process.

Effect of a cervical collar on optic nerve sheath diameter in trauma patients

BACKGROUND:As advocated in advanced trauma life support and prehospital trauma life support protocols,cervical immobilization is applied until cervical spine injury is *** study aimed to show the difference in optic nerve sheath diameter(ONSD)between patients with and without a cervical collar using computed tomography(CT).METHODS:This was a single-center,retrospective study examining trauma patients who presented to the emergency department between January 1,2021,and December 31,*** ONSD on brain CT of the trauma patients was measured and analyzed to determine whether there was a difference between the ONSD with and without the cervical ***:The study population consisted of 169 *** CT imaging of patients with(n=66)and without(n=103)cervical collars,the mean ONSD in the axial plane were 5.43±0.50 mm and 5.04±0.46 mm respectively for the right eye and 5.50±0.52 mm and 5.11±0.46 mm respectively for the left *** results revealed an association between the presence of a cervical collar and the mean ONSD,which was statistically significant(P<0.001)for both the right and left ***:A cervical collar may be associated with increased *** effect of this increase in the ONSD on clinical outcomes needs to be investigated,and the actual need for cervical collar in the emergency department should be evaluated on a case-by-case basis.

Evolving role of the endoscopist in management of gastrointestinal neuroendocrine tumors

Neuroendocrine tumors(NETs) are uncommon gastrointestinal neoplasms but have been increasingly recognized over the past few decades. Luminal NETs originate from the submucosa of the gastrointestinal tract and careful endoscopic exam is a key for accurate diagnosis. Despite their reputation as indolent tumors with a good prognosis,some NETs may have aggressive features with associated poor long-term survival. Management of NETs requires full understanding of tumor size,depth of invasion,local lymphadenopathy status,and location within the gastrointestinal tract. Staging with endoscopic ultrasound or cross-sectional imaging is important for determining whether endoscopic treatment is feasible. In general,small superficial NETs can be managed by endoscopic mucosal resection and endoscopic submucosal dissection(ESD). In contrast,NETs larger than 2 cm are almost universally treated with surgical resection with lymphadenectomy. For those tumors between 11-20 mm in size,careful evaluation can identify which NETs may be managed with endoscopic resection. The increasing adoption of ESD may improve the results of endoscopic resection for luminal NETs. However,enthusiasm for endoscopic resection must be tempered with respect for the more definitive curative results afforded by surgical treatment with more advanced lesions.
