Introduction: Appendicitis is the most common surgical emergency in childhood. It’s a common surgical disease that can be presented with a wide variety of atypical clinical features. Clinical Case: We report a case of a 7-year-old female patient admitted for abdominal pain dating back 20 days, radiological exploration suggested a right ovarian teratoma, while laparoscopic exploration has objective an appendicular abscess. Conclusion: This case has allowed us to clarify an atypical case of complicated appendicitis, and also to show the contribution of laparoscopy.

BACKGROUND Cardiac conduction disorders and electrocardiographic(ECG)changes may occur as a manifestation of coronavirus disease 2019(COVID-19),especially in severe *** To describe conduction system disorders and their association with other electrocardiographic parameters in patients who died of *** In this cross-sectional study,electrocardiographic and clinical data of 432 patients who expired from COVID-19 between August 1st,2021,and December 1st,2021,in a tertiary hospital were *** Among 432 patients who died from COVID-19,atrioventricular block(AVB)was found in 40(9.3%).Among these 40 patients,28(6.5%)suffered from 1st degree AVB,and 12(2.8%)suffered from complete heart block(CHB).Changes in ST-T wave,compatible with myocardial infarction or localized myocarditis,appeared in 189(59.0%).Findings compatible with myocardial injury,such as fragmented QRS and prolonged QTc,were found in 91 patients(21.1%)and 28 patients(6.5%),*** patients who died of COVID-19,conduction disorder was unrelated to any underlying medical *** QRS,axis deviation,and ST-T changes were significantly related to conduction system disorder in patients who died of COVID-19(P value<0.05).CONCLUSION Conduction system disorders are associated with several other ECG abnormalities,especially those indicative of myocardial ischemia or *** patients(73.14%)who died of COVID-19 demonstrated at least one ECG abnormality *** a COVID-19 patient's ECG gives important information regarding their cardiac health,our findings can help develop a risk stratification method for at-risk COVID-19 patients in future studies.

Objective: To assess the relation between acute gastrointestinal disease and Helicobacter pylori (H. pylori) infections. Methods: Over the 18-month period, a total of 323 patients referred to three hospitals in Babol (north of Iran) were enrolled in this cross-sectional study. H. pylori status (rapid urease test), endoscopic findings in the patients, personal habits (smoking or alcohol intake) and administration of drugs, such as non-steroidal anti-inflammatory drugs (NSAIDs) were analyzed using standard Chi-square test and multinomial logistic regression analysis. Results: Results showed that acute gastric ulcer patients had a significant association with alcohol (P=0.001, OR=6.183), opium (P=0.022, OR=2.823), smoking (P=0.016, OR=2.579) and NSAIDs (P=0.046, OR=2.071). However, patients with in acute duodenal ulcer have a significant association with opium (P=0.023, OR=2.326) and alcohol (P=0.003, OR=3.888). As well as, gastric cancer had significant association with alcohol (P<0.05, OR=6.937), smoking (P=0.012, OR=2.738), family history (P=0.005, OR=4.380) and gender (P≤0.05, OR=5.103). Conclusions: Current investigation shows that H. pylori infection, alcoholism, male gender, age and family history have an additive impact on the incidence of gastric cancer. In addition, alcoholism, opium usage, NSAIDs and family history have more impact on the incidence of acute gastric ulcer and acute duodenal ulcer in patients.

Aim: To study serum progranulin (PGRN) level in children with obesity and its relationship to inflamamatory markers and visceral fat. Methods: Fifty obese children and 50 controls aged 10-18 years were recruited. Demographic, anthropometric and biochemical features were collected according to a standard protocol. Serum progranulin levels, serum IL-6 and hsCRP were measured using ELISA. Insulin resistance was calculated by the homeostasis model (HOMA-IR) using the following formula: HOMA-IR = fasting insulin (mU/L) × fasting glucose (mmoL/L)/ 22.5. The maximum visceral fat thickness (VFT) and the minimum subcutaneous fat thickness (SFT) were measured by ultrasonography. Results: In the obese group, a significant increase was found in serum PGRN (48.87 ± 42.33 ng/mL) compared to control group (30.18 ± 23.82 ng/mL). Progranulin correlated significantly with VFT (r = 0.475), IL6 (r = 0.368), Insulin(r = 0.440) and HOMA-IR (r = 0.379). The mean serum progranulin in the high tertile VFT group was significantly higher than those in the low tertile and middle tertile groups (P = 0.030 and P = 0.039 respectively). VFT was highly positively correlated to progranulin, SFT, IL6, insulin, HOMA-IR and hsCRP (P = 0.001, 0.000, 0.001, 0.001, 0.003 and 0.003). However, the correlation coefficient between SFT and progranulin was insignificant. Summary: we demonstrated for the first time that serum PGRN concentrations increased in Egyptian obese children. The concentrations of serum PGRN correlated closely with visceral fat and IL6. Conclusion: PGRN may contribute to the pathogenesis of chronic inflammation in obesity. It could be a novel marker of visceral fat in obesity. Thus PGRN could be a potential therapeutic target for management of chronic inflammation in obesity.

Statistical and Geospatial Assessment of Groundwater Quality in the Megacity of Karachi

Inserting Groundwater quality variability and sources potentially contributing to aquifer recharge was evaluated in metropolitan Karachi. Selected sampling sites were characterized by large waste dumping sites, industrial zones, and the presence of open streams receiving heavy loads of industrial and domestic wastes. Levels of pH, electrical conductivity (EC), fluoride (F-), chloride (Cl-), bromide (Br-), nitrate-N (NO-3-N), sulfate (SO2-4), sodium (Na+), potassium (K+), calcium (Ca2+), magnesium (Mg2+), and ammonium (NH+4) were determined and compared with the WHO permissible limits. Concentrations of the measured ions were in the order of Cl- > Na+ > SO2-4 > Mg2+ > Ca2+ > NO-3-N > K+ > F- > Br-. EC values were above the WHO guidelines, representing the presence of high ionic concentration in the groundwater. The health risk index (HRI) for indicated that inhabitants of Karachi are at risk of high exposure. Ingestion of high concentrations of NO-3-N in water can cause methemoglobinemia and birth defects. Results of multivariate statistical analysis, principal component analysis (PCA), cluster analysis (CA), and geographic information system (GIS) map analysis revealed that human activities are leading to adverse effects on the existing groundwater quality in Karachi.

We report on the use of titanium dioxide nanoparticles (NPs) coated with poly-thiophene for the preconcentration of copper and silverions. The NPs were prepared by first modifying the surface of TiO2 NPs with vinyl groups and then copolymerizing them with vinyl thiophen. The resulting TiO2-polythiophene core-shell NPs were characterized by thermogravimetry, differential thermal analysis, scanning electron microscopy, Fourier transform infrared spectrometry and X-ray diffraction. The experimental conditions such as pH value, adsorption and desorption time, type, concentration and volume of the eluent, break through volume, and effect of potentially interfering ions were optimized. The ions were then desorbed with hydrochloric acid and determined by FAAS. The limits of detection are 0.4 and 1.2 μg·L_1 for Cu(II) and Ag(I), respectively, and recoveries and precisions are >98.0%
