Background: Human Immunodeficiency Virus (HIV)/Acquired Immune Deficiency Syndrome (AIDS) has afflicted persons of all ages leading to increase spread of the virus. This study attempts to determine the prevalence of HIV infection among 50 years of age and above over the five years of study in Nnewi, Nigeria. Methods: Males and females who were 50 years and above who underwent HCT in Nnewi between January 2008 to November 2012 were counseled and tested for HIV antibodies after giving an oral consent. Results: Of the 57,090 clients offered HCT, 4384 (7.7%) were 50 years and above while 52,706 (92.3%) were 15 - 49 years of age. The crude HIV prevalence was 8.2%. HIV infection in the 50 - 59 years of age was 2.4 times higher than that in the 60 years of age and above. A higher prevalence was observed in females (8.8%) compared with 7.8% in males in our study. No female 75 years and over was found to have HIV infection in our study (0%) while males in the same age group had a prevalence of 3.0%. The prevalence in our study was higher compared to the 2010 ANC sentinel survey (8.7%) in Anambra state. Conclusion: Our findings show that offering HCT routinely is feasible and may increase linkage to HIV care and treatment for many individuals with HIV infections. Routine HCT should be an integral component of any expansion in HIV prevention, care and treatment services in Africa and other settings where the prevalence of HIV is high. House to house HIV testing will improve uptake of testing among older population in developing countries.

Rationally utilizing and developing synthetic units is of particular significance for the design of high-performance non-fullerene small-mole cule acceptors(SMAs).Here,a thieno[3,2-b]pyrrole synthetic unit was employed to develop a set of SMAs(ThPy1,ThPy2,ThPy3 and ThPy4) by changing the number or the position of the pyrrole ring in the central core based on a standard SMA of IT-4 Cl,compared to which the four thieno[3,2-b] pyrrole-based acceptors exhibit bathochromic absorption and upshifted frontier orbital energy level due to the strong ele ctron-donating ability of *** a result,the polymer solar cells(PSCs) of the four thieno[3,2-b] pyrrole-based acceptors yield higher open-circuit voltage and lower energy loss relative to those of the IT-4 Cl-based *** is more,the ThPy3-based device achieves a power conversion efficiency(PCE)(15.3%) and an outstanding fill factor(FF)(0.771) that are superior to the IT-4 Cl-based device(PCE=12.6%,FF=0.758).The ThPy4-based device realizes the lowest energy loss and the smallest optical band gap,and the ternary PSC device based on PM6:BTP-eC9:ThPy4 exhibits a PCE of 18.43% and a FF of ***,this work sheds light on the great potential of thieno[3,2-b]pyrrole-based SMAs in realizing low energy loss and high PCE.

在植物生理生态学中,叶片中碳13(^(13)C)含量负值较少(富集),表明叶片处于通过气孔的气体交换减少,比如在干旱胁迫下。此外,与叶片相比,δ^(13)C在非光合组织中的负值也较少。然而,对从叶片(光合器官)到树枝、树干和根(非光合器官)中的数值的关系知之甚少,特别是缺少在关联密切的多个树种间或者不同器官间,以及对生长在极端高温和干旱胁迫下的树木中进行测定。本研究测定了3种近缘沙漠相思树种(Acacia tortilis、A raddiana和***)从叶片到根的δ^(13)C含量。我们在以色列南部成树的叶片组织中测定了含量。与此同时,在试验果园进行了为期7年的3个水平的灌溉试验。在试验结束时,测定了叶片、树枝、树干和根的生长参数和含量。研究结果表明,叶片组织中“临含量约为-27‰,其同位素贫化程度远超过生长在地球上最干燥和最热环境中的沙漠树种的预期值。在不同的相思树种和不同器官中,所有灌溉水平处理中的δ^(13)C含量并没有富集(-28‰到ca.-27‰),证实了在成熟相思树中的测定结果。在不同器官中,叶片δ^(13)C临含量与树枝和根的^(13)C含量异常相似,甚至比树干的δ^(13)C含量负值更少。高度贫化的叶片δ^(13)C表明,尽管这些树木生长在极端干燥的生境中,但其气孔气体交换较高。非光合组织中缺乏δ^(13)C富集可能与叶片和异养组织生长的季节耦合有关。

A gas diffusion electrode (air electrode) with a high current efficiency of electro\|synthesizing H 2 O 2 using O 2 in air was prepared. The several systems with air electrode as cathode of ele ctro\|synthesizing H 2 O 2 on the reaction spot for degrading aniline in aqueous--electro\|Fenton system, photo\|excitation electro\|H 2 O 2 system and photo\|electro\|Fenton system, were developed. Th e rates of decomposition of H 2 O 2 and mineralization of anil ine were experimentally measured respectively under different conditions, and th e results indicated there has an excellent parallel relation between decompositi on rate of H 2 O 2 and mineralization rate of aniline. Especia lly, photo\|electro\|Fenton system, where H 2 O 2 is decompose d the fastest, is the best system of oxidizing and degrading organic toxicants. Compared photo\|electro\|Fenton system with photo\|Fenton system, important role is revealed in the interface of air electrode. In this paper, the mineralizatio n mechanism of aniline in the photo\|electro\|Fenton system was also discussed.

In randomized clinical trials, low-molecular-weight heparin enoxaparin has been shown to decrease ischemic complications in patients with acute coronary syndromes(ACSs) without ST elevations who are treated conservatively. Enoxaparin has been shown to be equally effective as unfractionated heparin in high-risk patients with an early invasive approach. Little is known about the use and efficacy of enoxaparin in unselected patients with non-ST-elevation ACSs in clinical practice. In a retrospective analysis of the prospective ACOS registry, we compared the outcome of patients who were >60 years of age with non-ST-elevation myocardial infarction or unstable angina with ST-segment changes who were treated with enoxaparin or unfractionated heparin. In total,4,806 patients(55%) with non-ST-ele-vation ACS fulfilled our inclusion criteria for this analysis; 1,178(25%) were treated with enoxaparin and 3,628(75%) with unfractionated heparin. There were no differences between groups in baseline characteristics. We observed a significant decrease in the combined end point of death or nonfatal reinfarction with enoxaparin in the entire study group(odds ratio 0.51, 95%confidence interval 0.37 to 0.70) and in subgroups treated with early percutaneous coronary intervention(n=1,333, odds ratio 0.36, 95%confidence interval 0.17 to 0.80), coronary artery bypass grafting during the index hospitalization(n=270, odds ratio 0.31, 95%confidence interval 0.04 to 2.42), or conservatively(n=3,203, odds ratio 0.57, 95%confidence interval 0.40 to 0.81). There was no significant increase in severe bleeding complications with enoxaparin(5.2%vs 4.5%). In conclusion, in clinical practice, in unselected high-risk patients with non-ST-elevation ACSs who are treated conservatively or with early percutaneous coronary intervention, early treatment with enoxaparin is associated with a significant decrease in the combined end point of in-hospital death and reinfarction, without a significant increase in s

目的研究氯胺酮的异构体S(+)氯胺酮[S(+)-Ket]和R(-)氯胺酮[R(-)-Ket]对中性粒细胞释放超氧阴离子的影响及机制。方法细胞色素C还原法测定3种刺激剂介导的中性粒细胞超氧阴离子释放量。用荧光分光法检测中性粒细胞内钙的水平。用Western Blot法检测中性粒细胞蛋白质p47phox磷酸化水平。结果S(+)-Ket和R(-)-Ket可抑制N-甲酰甲硫氨酰-亮氨酰-苯丙氨酸(N-formyl-methionyl-leucyl-phenylalanine,fMLP)介导的中性粒细胞释放超氧阴离子(P<0.01)。S(+)-Ket可抑制佛波豆蔻醚乙酸盐(phorbol12-myristate13-acetate,PMA)介导的中性粒细胞释放超氧阴离子和钙离子的增加,而R(-)-Ket可轻度增加PMA介导的中性粒细胞释放超氧阴离子和促进钙离子的增加。S(+)-Ket抑制PMA介导中性粒细胞蛋白质p47phox磷酸化,而R(-)-Ket增强上述作用;钙螯合剂EGTA可取消R(-)-Ket对PMA介导中性粒细胞蛋白质p47phox磷酸化的抑制作用。结论S(+)-Ket通过PKC-钙信息通道,抑制NADPH氧化酶p47phox,抑制超氧阴离子的产生。

文章以苏州地铁1号线深基坑工程为研究背景,采用GTS软件对基坑支护体系进行了有限元分析;考虑支护结构与土体的共同作用,计算和分析了非对称超载条件下支护结构的内力和变形,并与对称超载条件下的计算结果进行对比,得到了非对称超载条件下支护结构内力和变形的分布规律。计算和分析结果表明:非对称超载状态下,基坑支护体系超载侧支撑轴力略大于对称超载条件下的支护结构,但超载侧的位移较大,基坑的稳定性处于最不利的状态。研究结果将有助于提高深基坑设计水平,为类似工程的设计、施工和研究提供必要的数据。

煤矿井下平板硫化机机架有限元分析及优化

Background: Depression among adolescents has been recognized as a public health problem all over the world. In Nigeria, as in most developing countries, there is a need for more research on the epidemiology of adolescent depression as this will guide prevention, diagnosis and treatment. Objectives: The study sought to determine the one month prevalence and predictors of depression among a sample of in-school adolescents in a rural region in South West Nigeria. Materials and Methods: This was a cross sectional descriptive survey among 540 secondary school students randomly selected from the six secondary schools in the study area using a Socio demographic Questionnaire and the Patient Health Questionnaire modified for Adolescents. Subsequently, a proportion of them were interviewed with the Kiddies Schedule for Affective Disorders and Schizophrenia. Results: Data were analyzed using the SPSS version 17. Respondents were aged 10 - 19 years (mean age was 14.07 ± 1.84). The one month prevalence of depression among the study population was 16.3% (using weighting method). Logistic regression analysis showed that death of a mother (OR = 11.786, 95% CI, 1.990 - 23.184), being from a polygamous family (OR = 5.781, 95% CI, 3.253 - 24.371), low socioeconomic class (OR = 6.222, 95% CI, 4.705 - 23.379), having a single parent (OR = 2.236, 95% CI, 0.869 - 11.786), having witnessed frequent violence (OR = 12.411, 95% CI, 0.516 - 29.851) and positive history of sexual abuse (OR = 0.203, 95% CI, 0.003 - 0.529) were significantly and independently associated with depression in this sample of adolescents. Conclusion: The one month prevalence of adolescent depression within the study population is slightly higher than what has been reported in other parts of the world. Therefore, there is a need for government to design and implement policies which can help to prevent, detect early, and treat depression among youths especially in rural areas.
