Pancreatits after endoscopic retrograde cholangio-pancreatography

Pancreatitis is the most common complication after endoscopic retrograde cholangio-pancreatography (ERCP); the reported incidence of this complication varies from less than 1% to 40%, but a rate of 4%-8% is reported in most prospective studies involving non-selected patients. Differences in criteria for defining pancreatitis, methods of data collection, and patient populations (i.e. number of high-risk patients included in the published series) are factors that are likely to affect the varying rates of post-ERCP pancreatitis. The severity of post-ERCP pancreatitis (PEP) can range from a minor inconvenience with one or two days of added hospitalization with full recovery to a devastating illness with pancreatic necrosis, multiorgan failure, permanent disability, and even death. Although, most episodes of PEP are mild (about 90%), a small percentage of patients (about 10%) develop moderate or severe pancreatitis. In the past, PEP was often viewed as an unpredictable and unavoidable complication, with no realistic strategy for its avoidance. New data have aided in stratifi cation of patients into PEP risk categories and new measures have been introduced to decrease the risk of PEP. As most ERCPs are performed on an outpatient basis, the majority of patients will not develop PEP and can be discharged. Alternatively, early detection of those patients who will go on to develop PEP can guide decisions regarding hospital admission and aggressive management. In the last decade, great efforts have been addressed toward prevention of this complication. Points of emphasis have included technical measures, pharmacological prophylaxis, and patient selection. This review provides a comprehensive, evidence-based assessment of published data on PEP and current suggestions for its avoidance.

Performance of a three-phase shunt active power filter(SAPF)relies on the capability of the controller to track the reference ***,designing an accurate current controller is crucial to guarantee satisfactory SAPF *** paper presents a model predictive current controller(mPCC)for a low-cost,four-switch,shunt active power filter for power quality improvement.A four-switch,B4,converter topology is adopted as an SAPF,hence offering a simple,robust,and low-cost *** addition,to further reduce overall cost,only two interfacing filter inductors,instead of three,are used to eliminate switching current *** proposed SAPF model mPCC is detailed for implementation,where simulation and experimental results validate effectiveness of the proposed control algorithm showing a 20%improvement in total harmonic distortion compared with a conventional hysteresis band current controller.

BACKGROUND Type 2 diabetes is one of the most prevalent chronic diseases worldwide,significantly impacting patients'quality of *** treatment options like metformin(mET)effectively counteract hyperglycemia but fail to alleviate diabetes-associated complications such as retinopathy,neuropathy,nephropathy,hepatopathy,and cardiovascular *** To propose the supplementation of cholecalciferol(CHO)and taurine(TAU)to enhance mET efficacy in controlling diabetes while minimizing the risk of associated *** The study involved sixty rats,including ten non-diabetic control rats and fifty experimental rats with type 2 diabetes induced by *** experimental rats were further subdivided into positive control and treatment *** four treatment groups were randomly allocated to a single mET treatment or mET combined with supplements either CHO,TAU,or *** Diabetic rats exhibited elevated levels of glucose,insulin,Homeostasis model Assessment of Insulin Resistance(HOmA-IR),glycated hemoglobin percentage,lipid markers,aspartate aminotransferase,and malondialdehyde,along with reduced levels of antioxidant enzymes(catalase and superoxide dismutase).The administration of CHO and TAU supplements alongside mET in diabetic rats led to a noticeable recovery of islet *** antioxidative,anti-inflammatory,and anti-apoptotic properties of the proposed combination therapy significantly ameliorated the aforementioned *** The supplementation of CHO and TAU with mET showed the potential to significantly improve metabolic parameters and protect against diabetic complications through its antioxidative,anti-inflammatory,and antiapoptotic effects.

Geochemistry of island arc assemblage in the Eastern Desert of Egypt and the role of Pan-African magmatism in crustal growth of the Arabian–Nubian Shield:A review

Neoproterozoic island arc assemblage of the Arabian–Nubian Shield(ANS)in the Eastern Desert(ED)of Egypt comprises juvenile suites of metavolcanics(mV),large amounts of meta-sedimentary rocks(mS),and voluminous metagabbros-diorites(mGD)and syn-tectonic intrusions of older granitoids(OG).We report here the updates of these four rock units in terms of classification,distribution,chemical characteristics,geodynamic evolution,metamorphism,and *** addition,we discuss these integrated data to elucidate a reasonable and reliable model for crustal evolution in the *** main features of these rock units indicate their relation to each other and the geodynamic environment dominated by early immature oceanic island arcs to primitive continental *** information of the island arc metavolcanic and plutonic rocks(gabbros,diorites,tonalites,and granodiorites)furnish evidence of the genetic *** include proximity and a coeval nature in the field;all protolith magmas are subalkaline in nature following calc-alkaline series with minor tholeiitic affinities;common geochemical signature of the arc rocks and subduction-related magmatism;their similar enrichment in LREEs;and similar major element compositions with mafic melts derived from metasomatized mantle *** volcano-sedimentary and the OG rocks underwent multiphase deformation events whereas the mGD complexes deformed *** on the magmatic,sedimentological,and metamorphic evolutions constrained by geochronological data as well as the progressive evolutionary trend from extensional to compressional regimes,a possible gradual decrease in the subducted slab dip angle is the most infl uential in any geodynamic model for arc assemblage in the ED of Egypt.

Depositional Environment, Petrophysical Evaluation and Electrical Properties of Zeit Formation, Northwestern Shore of Gulf of Suez, Egypt

The Zeit sand reservoir is one of the most prolific formations at Northwestern side of the Gulf of Suez. In this research we will try to coordinate between electrical, petrophysical properties, depositional environment and facies discrimination in order to evaluate the hydrocarbon potentiality of studied Zeit Formation. The statistical parameters for potassium（K）, thorium（Th） and Th/U ratio contents have a general increase towards northwestern parts, whereas uranium（U） content has a general increase towards southeastern parts. The sandstone facies is distinguished from the other facies by its thorium content >4 ppm. U has high carbonate content（U ≥ 1 ppm）. Rocks' electrical properties vary greatly depending on a number of factors. Electrical measurements were taken at frequencies range of(5 × 10-4Hz–100 kHz) for fully saturated samples（clayey sandstone） with NaCl（20 gm/L）. As salinity, clay content, and frequency increase, consequently does the electrical properties. The continental condition are present in northwestern part（back-sea） which is distinguished by high K percent, high Th, high Th/U ratio, and low U contents. Low K, Th, and Th/U ratio contents, with high U contents, characterize the marine depositional environment that existed around the east and southeastern parts（foresea coincide with the dipping of strata）. Furthermore, the studied Zeit Formation has good petrophysical properties that coincide with marine conditions. The middle and eastern parts（around ISS-94 and CSS-288） is a good reservoir（porosity 36%–39%, shale content <15%, hydrocarbon saturation 71%–92%, and net pay thickness 17–63 feet）.

Ultralong hydroxyapatite-based forward osmosis membrane for freshwater generation

Increasing global water shortages are accelerating the pace of membrane manufacturing,which generates many environmentally harmful *** challenges need a radical rethink of developing innovative membranes that can address freshwater production without generating environmentally harmful *** work utilized the synthesized ultra-long hydroxyapatite(UHA)by the solvothermal method using the green solvent oleic acid in preparing UHA-based forward osmosis *** membranes were developed using different loading ratios of graphene oxide(GO)by vacuum-assisted filtration *** prepared GO/UHA membranes were identified using X-ray diffraction,scanning electron microscope,Fourier-transform infrared spectroscopy,and X-ray photoelectron *** contact angle and pore size distribution were determined for the obtained GO/UHA *** obtained hierarchical porous structure in the prepared membranes with interconnected channels results in a stable water flux with reverse salt *** best water flux rate of 42±2 L·m^(–2)·h^(–1)was achieved using the 50 mg GO/UHA membrane,which is 3.3 times higher than the pristine membrane,and a reverse salt flux of 67 g·m^(-2)·h^(–1).The obtained results showed a promising capability of a new generation of sustainable inorganic-based membranes that can be utilized in freshwater generation by energy-efficient techniques such as forward osmosis.

Unraveling the nature of sensing in electrostatic mEmS gas sensors

This paper investigates the fundamental sensing mechanism of electrostatic mEmS gas *** compares among the responsivities of a set of mEmS isopropanol sensors before and after functionalization,and in the presence and absence of electrostatic fields when operated in static and dynamic detection *** the static mode,we found that the sensors do not exhibit a measurable change in displacement due to added *** the other hand,bare sensors showed a clear change in displacement in response to isopropanol *** the dynamic mode,functionalized sensors showed a measurable frequency shift due to the added mass of isopropanol *** the presence of strong electrostatic fields,the measured frequency shift was found to be threefold larger than that in their absence in response to the same concentration of isopropanol *** enhanced responsivity of dynamic detection allows the sensors to measure the vapor mass captured by the functional material,which is not the case for static *** detection of isopropanol by bare sensors in static mode shows that change in the medium permittivity is the primary sensing *** enhanced responsivity of dynamic mode sensors when operated in strong electrostatic fields shows that their sensing mechanism is a combination of a weaker added mass effect and a stronger permittivity *** findings show that electrostatic mEmS gas sensors are independent of the direction of the gravitational field and are,thus,robust to changes in *** is erroneous to refer to them as‘gravimetric’sensors.
