Impact of Early versus Late Amniotomy on Induction of Labor in Nulliparous Women after Vaginally Administered Misoprostol: A Randomized Clinical Trial

Objective::To investigate the effect of early versus late amniotomy after induction of labor(IOL)with vaginally administered ***::This randomized clinical trial was conducted at the Department of Obstetrics and Gynecology,Menoufia University,from May 2019 to March 2020,and included 120 nulliparous women at term(≥37 weeks’gestation)undergoing ***-generated randomization was used to randomize the participants into either the early amniotomy group(3 cm cervical dilatation;n=60)or the late amniotomy group(7 cm cervical dilatation;n=60).All participants received misoprostol(25μg)vaginally to induce *** primary outcome was the induction-to-delivery interval,defined as the time from the initiation of IOL to the time of ***::Women in the early amniotomy group had a shorter duration of labor(12.60±5.36 h)than those in the late amniotomy group(16.67±7.26 h).The mean time from rupture of the fetal membrane to delivery was significantly shorter in the late(2.51±0.36 h)than in the early amniotomy group(3.1±0.89 h).There was no statistically significant difference between the groups in terms of maternal complications(fever,nausea,vomiting,and uterine hyperstimulation)or neonatal complications(meconium-stained liquor,APGAR score<7 at 1 and 5 min,and neonatal intensive care unit admission).Conclusions::IOL using vaginally administered misoprostol followed by early amniotomy was accompanied by a shorter duration of labor and decreased use of *** was no significant difference between the early and late amniotomy groups in terms of the rate of cesarean section or maternal and neonatal complications.

The Analytical and Numerical Solutions of Differential Equations Describing of an Inclined Cable Subjected to External and Parametric Excitation Forces

The analytical and numerical solutions of the response of an inclined cable subjected to external and parametric excitation forces is studied. The method of perturbation technique are applied to obtained the periodic response equation near the simultaneous principal parametric resonance in the presence of 2:1 internal resonance of the system. All different resonance cases are extracted. The effects of different parameters and worst resonance case on the vibrating system are investigated. The stability of the system are studied by using frequency response equations and phase-plane method. Variation of the parameters α2, α3, β2, γ2, η2, γ3, η3, f2 leads to multi-valued amplitudes and hence to jump phenomena. The simulation results are achieved using MATLAB 7.6 programs.

Evaluation the Mechanical Properties of Nanofilled Composite Resin Restorative Material

This study was designed to evaluate the wear resistance, fracture toughness and flexural strength of a nanofilled composite resin restorative material in comparison with a conventional hybrid composite resin. A total of 60 specimens were prepared from both types composite resins. Specimens were cured with a light curing device according to the manufacturer’s instructions. Wear resistance was evaluated through subjecting the specimens to wear testing at 0.1 bar wet pressure against carbide abrasive counter-body using wear testing machine under water as lubricant. The test conditions were;speed = 265 rpm, load = 0.1 bar, time = 5 min. Flexural strength and fracture toughness were tested using three-point bending test in universal testing machine at a cross head speed of 2 mm/min until failure occurred. The nanofilled composite resin material exhibited higher wear resistance than the hybrid composite resin material. On the other hand, there were no significant differences between the two materials in values of flexural strength and fracture toughness. It was concluded that the nanofilled composite resin was harder but it does not stronger than the conventional hybrid composite resin.

Microleakage of Nanofilled Composite Resin Restorative Material

The role of nanofillers in reducing the microleakage of dental composite resins has not been previously investigated. So this study was designed to evaluate microleakage of nanofilled composite resin in comparison to the conventional hybrid composite. Twenty extracted sound molars were selected. Class II cavities were prepared. All cavities were etched (enamel and dentin) with 37% phosphoric acid. Dentin bonding agents were applied to etched tooth surfaces and restored with nanofilled and hybrid composite restorative materials. The restored teeth were thermocycled. Specimens were immersed in 2% methylene blue dye, sectioned along the mesio-distal direction;dye penetration of occlusal and gingival margins of each section was evaluated using a stereo-microscope. No significant difference was found between the microleakage of nanofilled and hybrid composite restorations at occlusal / enamel and at gingival / dentin margins. Also, there were no significant differences for nanofilled composite restorations at occlusal / enamel margins and gingival / dentin margins. On the other hand, there were a significant differences for hybrid composite restorations at occlusal / enamel margins and gingival / dentin margins.

Objective: The aim of the study was to assess the hormonal profile level in females with pelvic tumors after surgical transposition of the ovaries during surgery followed by postoperative radiotherapy to the pelvis. Methods: Fifteen patients with rectal, bladder and cervical cancer were included in the study with their age ranged between 25 and 40 years old, 7 patients with cancer rectum, 3 patients with bladder cancer and 5 had cancer cervix. Ovarian transposition was done for patients during surgery followed by external beam radiotherapy to the pelvis, hormonal assessment was done for the patients before and after exposure to radiation. Results: No intra-operative or post-operative morbidity was observed in patients treated by laparotomy. Patients were found to have regular menstrual cycles. Their FSH, LH and progesterone levels were within normal ranges. Conclusion: Proper transposition of ovaries during laparotomy away from the radiation field can preserve the ovarian function in patients with pelvic tumors.

Study of Stability and Vibration Reduction in Multi-Tool Ultrasonic Machining under Simultaneous Primary and Internal Resonance

The main object of this paper is the mathematical study of the vibration behavior in ultrasonic machining (USM) described by non-linear differential equations. The ultrasonic machining (USM) consists of the tool holder and the absor-bers representing the tools. This leads to four-degree-of-freedom system subject to multi-external excitation forces. The aim of this project is the reduction of the vibrations in the tool holder and have reasonable amplitudes for the tools represented by the multi-absorbers. Multiple scale perturbation method is applied to obtain the solution up to the second order approximation and to study the stability of the steady state solution near different simultaneous resonance cases. The resulting different resonance cases are reported and studied numerically. The stability of the steady state solution near the selected resonance cases is studied applying both frequency response equations and phase-plane technique. The effects of the different parameters of the system and the absorbers on the system behavior are studied numerically. Optimum working conditions for the tools were obtained. Comparison with the available published work is reported.

AIM To assess the levels of different immune modulators in patients with hepatocellular carcinoma(HCC),in relation to other hepatic *** Eighty-eight patients were included in the current study and represented patients with HCC(20),liver cirrhosis(28) and chronic hepatitis(CH;25),and normal controls(NC;15).Peripheral blood was isolated for immunophenotyping of active myeloid dendritic cells(m DCs;CD1 c and CD40),mature inactive myeloid cells(CD1 c and HLA),active plasmacytoid cells(p DCs;CD303 and CD40),mature inactive p DCs(CD30 and HLA),active natural killer(NK) cells(CD56 and CD161),active NK cells(CD56 and CD314) and inactive NK cells(CD56 and CD158) was done by flow *** levels of interleukin(IL)-2,IL-10,IL-12,IL-1β,interferon(IFN)-α,IFN-γ and tumor necrosis factor(TNF)-αR2 were assessed by *** Active m DCs(CD1 C+/CD40+) and inactive m DCs(CD1 c+/HLA+) were significantly decreased in HCC patients in relation to NC(P < 0.001).CD40+ expression on active p DCs was decreased in HCC patients(P < 0.001),and its level was not significantly changed among other *** p DCs(CD303+/HLA+),inactive NKs(CD56+/CD158+) and active NKs(CD56+/CD161+) were not statistically changed among the four groups studied;however,the latter was increased in CH(P < 0.05).NKG2 D was statistically decreased in HCC,CH and cirrhosis(P < 0.001),and it was not expressed in 63%(12/20) of HCC *** was significant decrease of IL-2,IFN-α and IFN-γ(P < 0.001),and a significant increase in IL-10,IL-1β,and TNF-αR2(P <0.01,P < 0.001 and P < 0.001;respectively) in HCC *** was inverted correlation between IL-12 and IL-1β in HCC(r =-0.565,P < 0.01),with a strong correlation between p DCs(CD303+/CD40+) and NKs(CD56+/CD161+;r = 0.512,P < 0.05) as well as inactive m DCs(CD1 c+/HLA+) and inactive NK cells(CD56+/CD158+;r = 0.945,P < 0.001).CONCLUSION NKG2 D,CD40,IL-2 and IL-10 are important modulators in the development and progression of HCC.

A Prospective Study of the Effect of Different Palliative Radiotherapy Fractionation Schedules on Tumor Response and Toxicity in Advanced Non-Small Cell Lung Cancer (NSCLC) Patients

Background: The optimal dose of palliative radiotherapy (RT) in symptomatic advanced lung cancer is unclear. Patients and methods: Patients with advanced NSCLC who were indicated for thoracic palliative RT with age up to 65 y and Performance Status (PS) 0 - 2 and no significant cardiac or lung co-morbidities were randomized into two fractionation arms: arm A: 30 Gy/10 over 2 weeks and arm B: 27 Gy/6 over 3 weeks (2 fractions per week) using 2 anterior posterior (AP-PA) fields in both arms. Primary end points were symptomatic and radiological tumor response, respiratory functions assessment. Secondary end point was toxicity. Results: From December 2014 to October 2015, 40 patients were randomized, 20 patients in each arm. There was statistically insignificant higher symptomatic improvement in arm B. Four weeks after treatment, 12 out of 40 patients (30%), 6 patients in each arm, had radiological Partial Response (PR) of the primary thoracic lesion without significant difference between the two arms. There was a tendency for improvement in the post treatment mean Forced Vital Capacity (FVC) and Forced Expiratory Volume in one second (FEV1) in each arm without statistical significance. There were no reported skin reactions or esophagitis in both arms up to 4 weeks after treatment. Eleven out of the 40 patients (27.5%), 6 in arm B and 5 in arm A, had radiological signs of radiation pneumonitis without significant difference between both arms. Conclusion: The two RT fractionation schedules showed equal efficacy in terms of symptoms relief, radiological response of the primary thoracic tumor, respiratory functions and toxicity. Thus the 27 Gy/6 fractionation arm appears preferable compared to 30 Gy/10 arm to minimize the patients’ visits and load on the machines.

Non-Linear Analysis of Vibrations of Non-Linear System Subjected to Multi-Excitation Forces via a Non-Linear Absorber

The dynamic response of mechanical and civil structures subjected to high-amplitude vibration is often dangerous and undesirable. Sometimes controlled vibration is desirable as in the machinery used in the formation of rigid hard material as Ceramics and diamond. This process is done via the passive control methods. The main purpose of this paper is to how reduction of vibration of nonlinear system subjected to multi-excitation forces via a nonlinear absorber. The nonlinear differential equations describing the model, which describe ultrasonic cutting machine are solved by using perturbation method. The effects of different parameter on the response of the system are studied. The stability of the numerical solution is investigated by using frequency response equations and phase-plane method. The simulation results are achieved using Matlab and Maple programs. A comparison is made with the available published work.

Endometrial Preparation for Frozen Embryo Transfer by Supplemented Natural Cycle versus Hormone Replacement Cycle in Regularly Cycling Women

BACKGROUND: The first successful pregnancy from a frozen-thawed embryo transfer (FTET) was reported in 1983 and after more than three decades, significant changes have been introduced into clinical practice. OBJECTIVE: To compare endometrial preparation for frozen thawed embryo transfer (FTET) in supplemented natural cycle (NC) by hCG (human chorionic gonadotropin) versus HRT (hormonal replacement therapy) by estrogen and progesterone in regularly cycling women. METHODS: A prospective trial was conducted on 40 patients that were divided into two treatment groups. The first group underwent endometrial preparation by supplemented natural cycle by HCG, and the second group underwent endometrial preparation by HRT (estrogen and progesterone). RESULTS: No significant difference was revealed between the two groups regarding ongoing clinical pregnancy rate. CONCLUSION: This study concluded that there is no significant difference in clinical pregnancy rate between the two methods of endometrial preparation for frozen thawed embryo transfer;supplemented natural cycle by hCG or HRT by estrogen and progesterone, in regularly cycling women.
