Ocular disorders in children with spastic subtype of cerebral palsy

AIM: To document common ocular abnormalities in children with spastic subtype of cerebral palsy (CP) and to find out whether any correlation exists between their occurance and etiologic ***: Totally 194 patients with the diagnosis of spastic type CP were enrolled in this retrospective study. Detailed ophthalmic examinations were performed. Demographic data and neuroradiological findings were documented. Kruskal-Wallis, Mann Whitney U, Pearson Chi-square tests and Student’s t tests were used in the statistical ***: The mean age was 64.7±44.2 months on the first ophthalmic examination. Prevalences of diplegia (47.4%) and tetraplegia (36.1%) were found to be higher than the frequency of hemiplegia (16.5% ) in our study population. Etiologic factor was asphyxia in 60.8% of the patients. Abnormal ocular findings were present in 78.9% of the patients. Statistically significant poor vision was detected in tetraplegia group among all the spastic ubtypes of CP (P =0.000). Anisometropia and significant refractive error were found in 14.4% and 70.1% of the patients, respectively. Thirty-six children (18.6% ) had nystagmus and 107 children (55.2% ) had strabismus. Lower gestational age and birth weight were statistically higher in patients with esotropia than exotropia (P=0.009 and P =0.024, respectively). Abnormal morphology of the optic disc was present in 152 eyes (39.2% ). Severe periventricular leukomalacia (PVL) was found in 48 patients and statistically significant poor vision was detected in the presence of PVL (P =0.000).CONCLUSION:Spastic diplegic or tetraplegic CP patients with positive neuroradiological symptoms, younger gestational age and lower birth weight ought to have detailed ophthalmic examinations as early as possible to provide best visual rehabilitation.·

Long-term success of ahmed glaucoma valve in refractory glaucoma

AIM:To evaluate the long-term results and complications of ahmed glaucoma valve (AGV) implantation in refractory glaucoma.· METHODS:A retrospective review of 13 patients (13 eyes) with refractory glaucoma who underwent AGV implantation and had a minimum follow-up of 18 months was *** patients underwent a complete ophthalmologic examination and intraocular pressure (IOP) measurement before surgery and at 1 month,3 months,6 months,1 year after surgery and yearly *** and the number of antiglaucoma medications needed were recorded.· RESULTS:Mean age was 27.3±16.0 *** eyes (100%) had at least one prior incisional *** follow-up was 61.3±30.8 *** was reduced from a mean of 35.0±7.0mmHg to 18.2±7.9mmHg at 12 months and to 17.0±4.1mmHg at 96 months (P <0.05) with a lower number of medications from baseline,76.9% patients required additional procedures to achieve the success criteria set by previously published *** most common complications were encapculated cyst formation in eight eyes (61.5%) and tube exposure in four eyes (30.8%).· CONCLUSION:Encapsulated cyst formation was the most common complication which hindered succesful IOP control after AGV implant insertion for refractory *** cyst excision with anti-fibrotic agents,successful IOP reduction was not achieved in 76.9% of the patients without antiglaucoma medication.

No evidence of predator odor avoidance in a North American bird community

Recent advances in our understanding of avian chemical communication have highlighted the importance of olfaction in many aspects of avian *** studies investigating predator avoidance behaviors in response to predator odor cues have produced mixed results across species and *** we assess if a community of birds in eastern Pennsylvania displays avoidance behaviors towards predator odor cues in a natural foraging *** use clay caterpillars to measure foraging activity by birds in the presence of predator(bobcat)urine,non-predator(rabbit)urine,and water controls in two different environmental contexts(field ***).Although we detected a weak trend for birds to forage less at predator urine-treated sites,we found no significant difference in avian foraging between the site *** did find that foraging rates between environmental contexts changed significantly over the course of the experiment,with forest sites showing decreasing foraging rates and field sites showing increasing foraging *** results reinforce the published literature that avoidance of predator odors by birds may not be ubiquitous across contexts and species.

Is There a Role for Intravitreal Triamcinolone Acetonide Injection in the Treatment of Purtscher-Like Retinopathy? Report of Two Cases

We hereby described two cases with unilateral Purtscher-like retinopathy. The first case occurred following an uneventful spontaneous vaginal delivery. The second case had no discernable cause despite having meticulous systemic work-up. Both eyes had low presenting visual acuity of hand movements and 20/200 consecutively. We elected to inject 1 mg triamcinolone acetolamide in-travitreally for the treatment. Though, there was some subjective visual and clinical improvement within time in both patients, we were not convinced that intravitreal injection was superior to no treatment approach.

评估Ahmed青光眼阀(Ahmed glaucoma valve,AGV)植入治疗难治性青光眼的长期疗效及并发症。方法:回顾性分析13例13眼接受Ahmed青光眼阀植入,最短随访18mo的难治性青光眼患者。所有患者均在术前及术后1,3,6mo,1a及1a以后行全面的眼科检查,眼压测量,记录并发症及抗青光眼的药物需求。结果:患者平均年龄27.3±16.0岁。所有患眼(100%)接受至少一次前置小切口手术。平均随访61.3±30.8mo。在少量药物的作用下,12mo时眼压由35.0±7.0mmHg降至18.2±7.9mmHg,96mo时降至17.0±4.1mmHg,76.9%患者需其他辅助治疗以达到预期的成功疗效。最常见的并发症包括包裹性囊肿8眼(61.5%),置管脱出4眼(30.8%)。结论:包裹性囊肿是最常见的并发症,阻碍了Ahmed青光眼阀植入治疗难治性青光眼。虽然囊肿被抗纤维化药物切除,但没有抗青光眼药物的治疗,眼压仍旧不能很好控制。

Selenium: The Cancer Shield

Introduction: Breast cancer is one of the most common types of cancer in the world and the treatments are being improved day by day. Although chemotherapy agents have begun to be used, side effects, resistance development and toxicity seen with these drugs are still steps that limit treatment. Objective: Our aim in this study is to show whether sodium selenate (NaS), which has different effects on many different cells, has antiproliferative and apoptotic effects on MCF-7 breast cancer cells, considering the dose-time relationship, and to reveal its effect on oxidant stress parameters. Methods: 10, 20, 30, 40 and 50 μM sodium selenate was applied to the cells for 24, 48 and 72 hours. MTT test was applied to show the proliferative effect. Apoptosis was also measured with Annexin V/7AAD in MCF7 cells. Malondaldehyde (MDA) and Glutathione (GSH) levels were studied to reveal the oxidant/antioxidant balance. Results: It has been shown that as the NaS dose increases in MCF-7 cells, cell viability decreases (p 0.05), but at all subsequent increasing NaS doses, viability was found statistically significant decreased (p: 0.001, p 0.05, respectively). Conclusion: Considering that NaS has an antitumor effect on breast cancer, increases oxidative stress and increases apoptosis by supressing the antioxidant protection system, and does not have a toxic effect on normal body cells at certain doses. When all these features evaluated, we think, selenium should be considered as a natural option in the treatment of breast cancer.

Subsequent pregnancy outcomes and fertility rates in the case series that underwent bilateral hypogastric artery ligation(BHGAL)due to severe postpartum hemorrhage

Objective:To evaluate the effect of bilateral hypogastric artery ligation(BHGAL)on women's pregnancy outcomes and fertility rates in cases with severe postpartum ***:Patients hospitalized in a tertiary care center between 2007 and 2018 were included in the *** records of cases were obtained from the hospital database,*** demographic data,morbidities or complications associated with BHGAL surgery,any additional haemostatic interventional or surgical procedures,preoperative admission and lowest postoperative haematocrit-hemoglobin values,blood products given,type of anaesthesia,intensive care unit(ICU)and length of hospital stay were *** cases without surgical sterilization,future fertility and subsequent pregnancy outcomes were assessed for at least two years after *** was obtained through face-to-face or telephone interviews with patients whose long-term fertility and subsequent pregnancy outcomes were ***:Sixty-eight patients with BHGAL surgery history were included in the study.26 Of 37 cases(70%)that did not undergo surgical sterilization had a desire for pregnancy after the *** of these cases(23%)were diagnosed with secondary *** fertility rate of these cases was 77%,and subsequent pregnancy was detected in 20 women.A total of 25 pregnancies,including first and second pregnancies,developed,and 15 of these pregnancies resulted in term delivery,six preterm births,one stillbirth,and three ***:The postoperative pregnancy rate was found to be higher than the rate of women with infertility problems,and this surgery does not seem to adversely affect pregnancy *** is a tertiary center experience and cohort studies with large patient series are needed.

Epidemic management in a measles outbreak in 2023,the return of vaccine-preventable diseases:A single center,retrospective observational study

Objective:To investigate the importance of immunization in preventing measles infection and to determine the most useful laboratory tests for confirmation of ***:This study included pediatric cases evaluated with a presumed diagnosis of measles between December 2022 and June 2023,at Marmara University Pendik Training and Research *** effects of vaccination status and underlying disease on the clinical course,treatments,and complications were ***:In total,117 patients were enrolled in the study with a median age of 80 months(IQR:32.5-125.0).Twelve patients with contact history were asymptomatic and had an underlying disorder,and intravenous immunoglobulin was given to them for post-exposure ***-one patients had confirmed measles *** treatment was given to three patients(a newborn,a girl with rhabdomyosarcoma,and a healthy boy)with respiratory ***-eight percent of confirmed measles cases were unvaccinated,and all hospitalized cases were unvaccinated or *** full-vaccinated children had confirmed measles *** PCR from nasopharyngeal swabs was negative in all of them,and their diagnosis was established with anti-measles IgM ***:The measles vaccine is the most effective way to protect from measles and measles-related *** measles can also occur in fully vaccinated patients,the disease is milder than in unvaccinated *** ELISA and RT-PCR tests together may be beneficial in patients with high clinical suspicion for early diagnosis.

Evaluation of Interferon-Gamma Release Assay Testing and Tuberculin Skin Test for Early Diagnosis of Tuberculosis in Children and Adolescents

Background: This study aimed to evaluate the diagnostic value of interferon-γ release assay (IGRA), a sensitive microbiological diagnostic method, in children and adolescents with suspected tuberculosis in a country with a high burden of tuberculosis. Method: This study included 581 children and adolescents aged 4 - 19 years who were suspected of having tuberculosis, were latently infected with Mycobacterium tuberculosis, and had received at least one dose of BCG vaccine between April 17, 2019, and February 24, 2021. The study evaluated the TST results of 106 patients who had a positive Quantiferon test and were suspected of having tuberculosis. Results: The study included 581 patients aged between 4 and 19 years. Of these, 106 patients tested positive for the Quantiferon test, while 19 were indeterminate and 456 were negative. The Quantiferon test positivity rate was 18.24%. Among the 106 QFT-Plus-positive cases, 23 patients also tested positive for TST. The difference in distribution was found to be statistically significant. Conclusion: The QFT-Plus test is considered an alternative to TST and other microbiological diagnostic methods for early tuberculosis diagnosis, particularly in children and adolescents.
