Determinants of invasive strategy in elderly patients with non-ST elevation myocardial infarction

Background Knowledge gaps across literature prevent current guidelines from providing the profile of elderly patients most likely to derive benefit fi^om invasive strategy （IS） in non ST-elevation myocardial infarction （NSTEMI）. Furthermore, the benefit of IS in a real-world elderly population with NSTEMI remains unclear. The aims of this study were to determine factors that lead the cardiologist to opt for an IS in elderly patients with NSTEMI, and to assess the impact of IS on the 6-month all-cause mortality. Methods This multicenter prospective study enrolled all consecutive patients aged 〉 75 years old who presented a NSTEMI and were hospitalized in cardiology intensive care unit between February 2014 and February 2015. Patients were compared on the basis of reperfusion strategy （invasive or conservative） and living status at six months, in order to determine multivariate predictors of the realization of an IS and multivariate predictors of 6-month mor- tality. Results A total of 141 patients were included; 87 （62%） underwent an IS. The strongest independent determinants of IS were younger age [odds ratio （OR）： 0.85, 95%-confidence interval （CI）： 0.78-0.92; P 〈 0.001） and lower ＂Cumulative Illness Rating Scale-Geriatric＂ number of categories score （OR： 0.83, 95%CI： 0.73-0.95; P = 0.002）. IS was not significantly associated with 6-month survival （OR： 0.80, 95%CI： 0.27-2.38; P = 0.69）. Conclusions In real-world elderly patients with NSTEMI, younger patients with fewer comorbidities profited more often from an IS. However, IS did not modify 6-month all-cause mortality.

Magnetic resonance imaging may predict deep remission in patients with perianal fistulizing Crohn's disease

To evaluate the imaging course of Crohn’s disease (CD) patients with perianal fistulas on long-term maintenance anti-tumor necrosis factor (TNF)-α therapy and identify predictors of deep *** patients with perianal CD treated with anti-TNF-α therapy at our tertiary care center were evaluated by magnetic resonance imaging (MRI) and clinical assessment. Two MR examinations were performed: at initiation of anti-TNF-α treatment and then at least 2 years after. Clinical assessment (remission, response and non-response) was based on Present’s criteria. Rectoscopic patterns, MRI Van Assche score, and MRI fistula activity signs (T2 signal and contrast enhancement) were collected for the two MR examinations. Fistula healing was defined as the absence of T2 hyperintensity and contrast enhancement on MRI. Deep remission was defined as the association of both clinical remission, absence of anal canal ulcers and healing on MRI. Characteristics and imaging patterns of patients with and without deep remission were compared by univariate and multivariate ***-nine consecutive patients (31 females and 18 males) were included. They ranged in age from 14-70 years (mean, 33 years). MRI and clinical assessment were performed after a mean period of exposure to anti-TNF-α therapy of 40 ± 3.7 mo. Clinical remission, response and non-response were observed in 53.1%, 20.4%, and 26.5% of patients, respectively. Deep remission was observed in 32.7% of patients. Among the 26 patients in clinical remission, 10 had persisting inflammation of fistulas on MRI (T2 hyperintensity, n = 7; contrast enhancement, n = 10). Univariate analysis showed that deep remission was associated with the absence of rectal involvement and the absence of switch of anti-TNF-α treatment or surgery requirement. Return to Sports after Lower Extremity Injuries:Assessment of Movement Quality

Numerous high performance athletes experience further damage or recurrent injuries even after successful rehabilitation. This is often caused by an insufficient movement quality, which has been very rarely assessed by test protocols which determine the point of reintegration into high performance sports (Return-to-Play, RTP). In order to assess the movement quality both objective and subjective test protocols exist. Objective methods like 3D-movement analysis are viewed as international gold standard, but are not the most practicable solutions for daily training routine. This study aims at reviewing the available literature on reliability and validity of existing subjective test protocols. Further their use within high-performance sports is evaluated. Up to now subjective methods fail to match sufficient validity. However some practical approaches for assessment of movement quality after injury are known. Based on selected criteria first recommendations for the use of different subjective screening test methods are given. In summary further research focusing on the validity of subjective tests is needed. The subjective testing methods should be used in combination with additional tests (e.g. strength testing) in order to identify other risk factors. Recurrent pre-injury screenings on movement quality should be carried out to enhance injury prevention.

Catalytically active, stable, and mechanically durable solid K/Al 2 O 3 catalysts for the transesterification of rapeseed oil with methanol was studied. In a batch reactor, high catalytic activity was accompanied by leaching of K species, caused by glycerol, and mechanical destruction of the solid catalyst as a result of contact with the stirrer. In a fixed-bed reactor, some leaching of K species into the liquid phases was also observed, but approached 0 during 30 h of time-on-stream; the activity of the K/Al 2 O 3 catalyst (∼83% ester yield) was stable for 100 h of time-on-stream and no mechanical destruction of the catalyst was observed. The populations of K 2 O and K-O-Al species for fresh and used K/Al 2 O 3 catalysts were compared using Fourier transform infrared spectroscopy. It was found that some K 2 O species leached into the liquid phases at the beginning of the reaction.

Bioactivity of mackerel peptides on obesity and insulin resistance,an in-vivo study

Of many metabolic irregularity risk factors,glucose intolerance,insulin resistance,and abdominal obesity are the main *** risk of vascular diseases and cancers is rather high in such individuals therefore its treatment and prevention are equally important for the health of *** including fish biopeptides have frequently shown to prevent and or ameliorate these risk *** our earlier studies,Atlantic mackerel hydrolysate and its fractions showed promising effects on immunity and ***,therefore,in the current study,aimed to examine the effects of mackerel hydrolysate along its cationic peptides,isolated at pH3 using electrodialysis with ultrafiltration membrane,on metabolic disorder factors in diet-induced insulin resistant obese *** were fed the chow and or the hypercaloric diet (high fat high sucrose) for the duration of *** the hypercaloric fed insulin resistant obese mice were gavaged daily either the vehicle and or the test material at the dosage of 208 mg of sample protein per kg mice body *** to our observations,mackerel peptides could not effectively modulate food intake,weight gain,adiposity,plasma glucose and insulin concentrations,hepatic cholesterol,and triglyceride ***,obesity and metabolic impairments could not be averted by the treatments in insulin resistant obese *** conclusion,anti-hypertensive,and immunomodulatory attributes of mackerel peptides at the tested concentrations do not seem to be correlated with its metabolic syndrome benefiting *** presume a general statement about the metabolic syndrome benefiting potency of Atlantic mackerel's biopeptides further research is warranted.
