Impact of central venous port implantation method and access choice on outcomes

BACKGROUND Although the number of patients who need central venous ports for permanent vascular access is increasing,there is still no“gold standard”for the implantation *** To identify the implantation technique that should be *** Two hundred central venous port-implanted patients in a tertiary hospital were retrospectively *** were assigned into two groups according to the access *** first group comprised patients whose jugular veins were used,and the second group comprised patients whose subclavian veins were *** were evaluated regarding age,sex,application side,primary diagnosis,active follow-up period in the hospital,chemotherapy agents administered,number of complications,and the Clavien-Dindo severity *** distribution of the variables was tested with the Kolmogorov-Smirnov test and the Mann-Whitney U ***χ^(2) test was used to analyze the *** There was no statistically significant difference between the groups regarding age,sex,side,number of chemotherapy drugs,and duration of port usage(P>0.05).Only 2 patients in group 1 had complications,whereas in group 2 we observed 19 patients with complications(P<0.05).No port occlusion was found in group 1,but the catheters of 4 patients were occluded in group *** port was infected in group 1 compared to three infected ports in group *** port ruptures,two pneumothorax,one revision due to a mechanical problem,one tachyarrhythmia during implantation,and four suture line problems were also recorded in group 2 *** also showed that it would be sufficient to evaluate and wash ports once every 2 *** Our results robustly confirm that the jugular vein route is safer than the subclavian vein approach for central venous port implantation.

Our initial single port robotic cholecystectomy experience:A feasible and safe option for benign gallbladder diseases

BACKGROUND Although single-port laparoscopic cholecystectomy has been performed for over 25 years, it is still not popular. The narrow working space used in this surgery limits the movement of instruments and causes ergonomic challenges. Robotic surgery not only resolves the ergonomic challenges of single-port laparoscopic surgery but is also considered a good option with its additional technical advantages, like a three-dimensional display and not being affected by ***, the extent to which these technical and ergonomic advantages positively affect the surgical outcomes and how safe the single-port robotic surgeries need to be assessed for each particular *** To evaluate the feasibility and safety of single-port robotic cholecystectomy for patients with *** The electronic records of the first 40 consecutive patients with gallbladder lithiasis who underwent single-port robotic cholecystectomy from 2013 to 2021 were analyzed retrospectively. Influence of tribology on global energy consumption, costs and emissions

Calculations of the impact of friction and wear on energy consumption,economic expenditure,and CO2 emissions are presented on a global *** impact study covers the four main energy consuming sectors:transportation,manufacturing,power generation,and *** published four case studies on passenger cars,trucks and buses,paper machines and the mining industry were included in our detailed calculations as reference data in our current *** following can be concluded:-In total,～23％ (119 EJ) of the world's total energy consumption originates from tribological *** that 20％ (103 EJ) is used to overcome friction and 3％ (16 EJ) is used to remanufacture worn parts and spare equipment due to wear and wear-related failures.-By taking advantage of the new surface,materials,and lubrication technologies for friction reduction and wear protection in vehicles,machinery and other equipment worldwide,energy losses due to friction and wear could potentially be reduced by 40％ in the long term (15 years)and by 18％ in the short term (8 years).On global scale,these savings would amount to 1.4％ of the GDP annually and 8.7％ of the total energy consumption in the long term.-The largest short term energy savings are envisioned in transportation (25％) and in the power generation (20％) while the potential savings in the manufacturing and residential sectors are estimated to be ～10％.In the longer terms,the savings would be 55％,40％,25％,and 20％,respectively.-Implementing advanced tribological technologies can also reduce the CO2 emissions globally by as much as 1,460 MtCO2 and result in 450,000 million Euros cost savings in the short *** the longer term,the reduction can be 3,140 MtCO2 and the cost savings 970,000 million *** years ago,wear and wear-related failures were a major concern for UK industry and their mitigation was considered to be the major contributor to potential economic savings by as much as 95％ in ten years by the development and dep

Achieving superlubricity in DLC films by controlling bulk, surface, and tribochemistry

Superlubricity refers to a sliding regime in which contacting surfaces move over one another without generating much adhesion or friction[1].From a practical application point of view,this will be the most ideal tribological situation for many moving mechanical systems mainly because friction consumes large amounts of energy and causes greenhouse gas emissions[2].Superlubric sliding can also improve performance and durability of these *** this paper,we attempt to provide an overview of how controlled or targeted bulk,surface,or tribochemistry can lead to superlubricity in diamond-like carbon(DLC)***,we show that how providing hydrogen into bulk and near surface regions as well as to sliding contact interfaces of DLC films can lead to super-low friction and *** of hydrogen into bulk DLC or near surface regions can be done during deposition or through hydrogen plasma treatment after the *** can also be fed into the sliding contact interfaces of DLCs during tribological testing to reduce *** to favorable tribochemical interactions,these interfaces become very rich in hydrogen and thus provide super-low friction after a brief run-in *** of the method used,when sliding surfaces of DLC films are enriched in hydrogen,they then provide some of the lowest friction coefficients(i.e.,down to 0.001).Time-of-flight secondary ion mass spectrometer(TOF-SIMS)is used to gather evidence on the extent and nature of tribochemical interactions with *** on the tribological and surface analytical findings,we provide a mechanistic model for the critical role of hydrogen on superlubricity of DLC films.

A New Metaheuristic Approach to Solving Benchmark Problems: Hybrid Salp Swarm Jaya Algorithm

Metaheuristic algorithms are one of the methods used to solve optimization problems and find global or close to optimal solutions at a reasonable computational *** with other types of algorithms,in metaheuristic algorithms,one of the methods used to improve performance and achieve results closer to the target result is the hybridization of *** this study,a hybrid algorithm(HSSJAYA)consisting of salp swarm algorithm(SSA)and jaya algorithm(JAYA)is *** speed of achieving the global optimum of SSA,its simplicity,easy hybridization and JAYA’s success in achieving the best solution have given us the idea of creating a powerful hybrid algorithm from these two *** hybrid algorithm is based on SSA’s leader and follower salp system and JAYA’s best and worst solution *** works according to the best and worst food source *** this way,it is thought that the leader-follower salps will find the best solution to reach the food *** hybrid algorithm has been tested in 14 unimodal and 21 multimodal benchmark *** results were compared with SSA,JAYA,cuckoo search algorithm(CS),firefly algorithm(FFA)and genetic algorithm(GA).As a result,a hybrid algorithm that provided results closer to the desired fitness value in benchmark functions was *** addition,these results were statistically compared using wilcoxon rank sum test with other *** to the statistical results obtained from the results of the benchmark functions,it was determined that HSSJAYA creates a statistically significant difference in most of the problems compared to other algorithms.

Microencapsulated paraffin as a tribological additive for advanced polymeric coatings

Numerous tribological applications,wherein the use of liquid lubricants is infeasible,require adequate dry *** the use of polymers as an effective solution for dry sliding tribological applications,their poor wear resistance prevents the utilization in harsh industrial *** methods are typically implemented to tackle the poor wear performance of polymers,however sacrificing some of their mechanical/tribological ***,we discussed the introduction of a novel additive,namely microencapsulated phase change material(MPCM)into an advanced polymeric ***,paraffin was encapsulated into melamine-based resin,and the capsules were dispersed in an aromatic thermosetting co-polyester(ATSP)*** found that the MPCM-filled composite exhibited a unique tribological behavior,manifested as“zero wear”,and a super-low coefficient of friction(COF)of *** developed composite outperformed the state-of-the-art polytetrafluoroethylene(PTFE)-filled coatings,under the experimental conditions examined herein.

The biochemical effects of ischemia-reperfusion injury in the ipsilateral and contralateral testes of rats and the protective role of melatonin

Testicular torsion （TT） is a serious urologic emergency that is observed in adolescent males and that can lead to infertility if left untreated. The ischemia-reperfusion （I/R） injury due to TT has been implicated in the pathogenesis of testicular damage. We investigated the effects of melatonin on oxidative damage in the ipsUateral and contralateral testes of rats induced by unilateral TT. A total of 21 prepubertal male Wistar albino rats were divided into three groups, each consisting of seven rats, In Group 1 （SHAM group）： a sham operation to the left testis and bilateral orchiectomy were performed. In Group 2 （I/R group）： I/R injury was created by rotating the left testis 720° in a clockwise direction for 2 h and detorsing the testis after 2 h. Group 3 （I/R ＋ MEL group）： rats were subjected to I/R injury and one-shot melatonin injection （50mgkg-1, intraperitoneal （i.p.））. The testes of the rats were excised bilaterally in all groups. The testicular tissue activities of antioxidant catalase （CAT）, superoxide dismutase （SOD） and glutathione peroxidase enzymes （GSH-Px）, and the tissue levels of malondialdehyde （MDA）, protein carbonyl （PC） and nitric oxide （NO） were determined. Administration of melatonin caused a significant decrease in lipid peroxidation and enzyme activities in the ipsilateral testis when compared with the control group （P 〈 0.05）. All of the changes in the enzyme activities of the contralateral testis were insignificant （P 〉 0.05）. MDA levels were significantly altered in the contralateral testis （P = 0.009）, Melatonin administration decreased the deleterious effects of I/R injury in the ipsilateral torted testes of the rats. The contralateral testes were slightly affected by unilateral TT.

Influence of tribofilm on superlubricity of highly-hydrogenated amorphous carbon films in inert gaseous environments

In this study,we mainly focus on the structural morphology and inter-atomic bonding state of tribofilms resulting from a highly-hydrogenated amorphous carbon(a-C:H) film in order to ascertain the underlying mechanisms for its superlubric behavior(i.e.,less than 0.01 friction coefficient).Specifically,we achieved superlubricity(i.e.,friction coefficients of down to 0.003) with this film in dry nitrogen and argon atmospheres especially when the tribo-pair is made of an a-C:H coated Si disk sliding against an a-C:H coated steel ball,while the a-C:H coated disk against uncoated ball does not provide *** also found that the state of superlubricity is more stable in argon than in nitrogen and the formation of a smooth and uniformly-thick carbonaceous tribofilm appears to be one of the key factors for the realization of such ***,the interfacial morphology of sliding test pairs and the atomic-scale bond structure of the carbon-based tribofilms also play an important role in the observed superlubric behavior of a-C:H *** Raman spectroscopy and high resolution transmission electron microscopy,we have compared the structural differences of the tribofilms produced on bare and a-C:H coated steel *** the a-C:H coated ball as mating material which provided superlow friction in argon,structural morphology of the tribofilm was similar or comparable to that of the original a-C:H coating;while for the bare steel ball,the sp^2-bonded C fraction in the tribofilm increased and a fingerprint-like nanocrystalline structure was detected by high resolution transmission electron microscopy(HRTEM).We also calculated the shear stresses for different tribofilms,and established a relationship between the magnitude of the shear stresses and the extent of sp^3-sp^2 phase transformation.

Seeing the Balkan War Through the Memoirs of the Battle of Gallipoli

The twentieth century features numerous phenomena remarkable for Turkish history. Such wars as the Trablusgarp （Turco-ltalian） War （1911）, the Balkan War （1912-1913）, World War I （1914-1918）, and National War of Independence （1919-1922） caused traumas that remain vivid in the minds of Turkish people and at the time devastated their psychological well-being and social lives. However, military and social disappointment caused by the Balkan War played an important part in invigorating and motivating soldiers to fight in the Battle of Gallipoli, which cleansed the taint caused by the Balkan defeat and reminded Turkish soldiers of their glorious past. The current study attempts to reveal how the shame caused by the Balkan defeat disappeared in the trenches at the Battle of Gallipoli. To this end, wartime writings, memoirs, and diaries were analyzed and evaluated. The primary concern of this study is comparison of how these two battles featured in the memoirs and/or reports or columns written by officers, soldiers, and intellectuals who participated in and witnessed the two wars.
