Marine Accident Analysis with GIS

In this study, marine casualties which are recorded Global Integrated Shipping Information System (GISIS) in 2007-2011, result in death, injury, economic loss and environmental pollution are discussed. The studied types of ships are container, bulk dry, general cargo, roro, ropax, passenger and tankers. The information contained in GISIS system is textual format and it is difficult to systematically analyze this information. For this reason, by creating a new data base, which is Microsoft Excel-based, ship accidents are classified according to name, flag state and type of ship, type, size and coordinates of the accidents that are evaluated by entering the Geographical Information System(GIS). In the study, all marine areas having been separated into ranges in ArcGIS 10 program, the marine areas with intensive marine accidents have been determined and marine accident chart has been created. As a result of the study, high risk marine areas are Strait of Dover and Hamburg in the North Europe, Belfast Shores in Ireland, the seas surrounding Great Belt, Kattegat and Copenhagen in Baltic Sea, In far east, Kanmon Strait, Urage Channel and Bungo Strait in Japan, Shanghai, Ningbo and Hong Kong in China.

Medical and alternative therapies in urinary tract stone disease

Nephrolithiasis is a serious problem for both patients and the health system. Recurrence stands out as a significant problem in urinary system stone disease, the prevalence of which is increasing gradually. If recurrence is not prevented, patients may go through recurrent operations due to nephrolithiasis. While classical therapeutic options are available for all stone types, the number of randomized controlled studies and extensive meta-analyses focusing on their efficiency are inadequate. Various alternative therapeutic options to these medical therapies also stand out in recent years. The etiology of urolithiasis is multifactorial and not always related to nutritional factors. Nutrition therapy seems to be useful, either along with pharmacological therapy or as a monotherapy. General nutrition guidelines are useful in promoting public health and developing nutrition plans that reduce the risk or attenuate the effects of diseases affected by nutrition. Nutrition therapy involves the evaluation of a patient's nutritional state and intake, the diagnosis of nutrition risk factors, and the organization and application of a nutrition program. The main target is the reduction or prevention of calculus formation and growth via decreasing lithogenic risk factors and increasing lithogenic inhibitors in urine. This review focuses briefly on classical medical therapy, along with alternative options, related diets, and medical expulsive therapy.

An Awesim Simulation Study： To Determine the Efficiency of Future Improvements on Tupras Izmit Oil Terminal

Oil and oil products occupy an important place in the industry and transportation sectors, both as bunker and raw materials. Most oil and oil products are transported by sea. The aim of this study was to evaluate the handling capacity of the TuprasIzmit Tanker Terminal, which is an important hub for Turkey with regards to the transportation of oil and refined oil products. To assess the capacity of the port and to determine the amount of queuing that could form with regards to tug services; the Awesim simulation program was used. Based on the study results, a comparison was performed between the port"s current annual handling capacity and the maximum amount of cargo it can handle. In addition, by using the study data, the maximum amount of queuing that can form in tug services was also calculated for days with unfavorable weather and sea conditions. This study thus represents a case study performed with the Awesim modeling program in order to determine port efficiency.

Endovascular treatment of carotid cavernous sinus fistula: A systematic review

Carotid cavernous sinus fistulas are abnormal communications between the carotid system and the cavernous sinus. Several classification schemes have described carotid cavernous sinus fistulas according to etiology, hemodynamic features, or the angiographic arterial architecture. Increased pressure within the cavernous sinus appears to be the main factor in pathophysiology. The clinical features are related to size, exact location, and duration of the fistula, adequacy and route of venous drainage and the presence of arterial/venous collaterals. Noninvasive imaging (computed tomography, magnetic resonance, computed tomography angiography, magnetic resonance angiography, Doppler) is often used in the initial workup of a possible carotid cavernous sinus fistulas. Cerebral angiography is the gold standard for the definitive diagnosis, classification, and planning of treatment for these lesions. The endovascular approach has evolved as the mainstay therapy for definitive treatment in situations including clinical emergencies. Conservative treatment, surgery and radiosurgery constitute other management options for these lesions.

Retroanastomotic hernias after gastroenterostomieseither antecolic or retrocolic-are extremely rare but are associated with high mortality rates due to delayed identification which precludes immediate surgical re-duction. In this report, we present a 77-year-old man with retroanastomotic herniation of the efferent loop segments that occurred 14 years after a Moynihan's gastroenterostomy.
