目的　评价单平面Simpson法计算右心室容积的可行性。方法　采用Toshiba多层螺旋CT对 15个人的右心室铸型进行扫描 ,然后沿右心室长轴重建出右心室矢状面的多平面重组(multiplanarreformattedimage ,MPR)图像 ,采用单平面Simpson法对右心室铸型的容积进行计算。右心室铸型的实际容积采用阿基米德原理计算。结果　右室铸型的实际容积为 (6 4 .2 3± 2 4 .5 1)ml,单平面Simpson法右心室容积计算值为 (5 3.18± 2 6 .2 2 )ml,平均低估右心室实际容积 (11.0 5± 13.0 9)ml(t=3.2 7,P =0 .0 0 6 )。单平面Simpson法计算的右心室容积值与右室铸型的实际容积显著相关 (r=0 86 9,P <0 .0 0 1)。结论　单平面Simpson方法可以用于右心室容积的计算。

目前，胸腺恶性肿瘤尚无国际抗癌联盟（International Union Against Cancer, UICC）和美国癌症联合委员会（American Joint Committeeon Cancer, AJCC）的官方分期。在2017年新版国际肿瘤分期提出较为广泛接受的分期之前，国际胸腺肿瘤协作组织（International Thymic Malignancy Interest Group, ITMIG）仍然建议选用经Koga等修订的Masaoka分期[1-3]。然而， Masaoka和Koga分期都存在一些模糊的术语定义，尤其是对某些细节未作出明确的定义，造成学术界的许多混乱。为此，ITMIG首先由核心工作组起草推荐定义，再交由扩展工作组提炼，并于2010年11月16日ITMIG举办的定义和术语研讨会上进行了进一步修订，最终经ITMIG全体成员讨论后于2011年2月经ITMIG批准并被采用。其灵魂内容是ITMIG对Masaoka-Koga分期系统的许多细节问题给出了较明确定义与解释，旨在使得大家在应用Masaoka-Koga分期的过程中更加一致，以利于相互合作、资源共享，同时便于前瞻性数据的正确收集，最终提出更合理的分期系统供临床使用。本文就此作一综述。

CT评估胆管癌患者的主动脉旁淋巴结转移

Background. The extent of paraaortic lymph node (PAN) metastasis parellels that of distant metastases in patients with biliary carcinoma. Accurate preoperative assessment of PAN metastasis has a crucial impact on surgical indications. In this retrospective study, we evaluated whether computed tomography (CT) scans were useful for diagnosing PAN metastases and excluding patients with PAN metastases from an indication for surgery. Methods. Between March 1999 and November 2003, 57 patients with biliary carcinoma underwent radical lymphadenectomy or surgical biopsy of PANs. Nine of these patients were diagnosed as having PAN metastasis microscopically. All patients had undergone abdominal CT scans before surgery. To diagnose PAN metastases, we used the following diagnostic criteria. (1) Size; when lymph nodes were greater than 12 mm, 10 mm, 8 mm, or 6 mm in longo or short- axis diameter, the nodes were considered metastatic. (2) Shape and size; when the axial ratio of a lymph node was greater than 0.5, 0.7, 1.0, and the maximum diameter of the long or short axis was greater than 12 mm, 10 mm, 8 mm, or 6 mm, the node was considered metastatic. (3) Internal structure; if the internal structure of a PAN was heterogeneous, the node was considered metastatic. A positive predictive value was calculated for each included criterion when patients numbered ten or more. Results. Positive predictive values using the above criteria ranged from 13% to 36% . Only one patient had PANs with heterogeneous internal structures. Conclusions. We were unable to determine surgical indications based on the morphological criteria revealed by a CT scan.

A 78-year-old male was admitted to our hospital because of *** demonstrated choledocholithiases with a maximum diameter of 13 mm, and we performed endoscopic papillary large balloon dilation(EPLBD) with a size of 15 *** following the balloon deflation, spurting hemorrhage occurred from the orifice of the duodenal *** we performed endoscopic hemostasis by compressing the bleeding point with the large balloon catheter, we could not achieve ***, we placed a 10 mm fully covered selfexpandable metallic stent(SEMS) across the duodenal papilla, and the hemorrhage stopped *** 1 wk of SEMS placement, duodenal endoscopy revealed ulcerative lesions in both the orifice of the duodenal papilla and the lower bile duct.A direct peroral cholangioscopy using an ultra-slim upper endoscope revealed a visible vessel with a longitudinal mucosal tear in the ulceration of the lower bile *** believe that the mucosal tear and subsequent ruptured vessel were caused by the EPLBD procedure.

We retrospectively evaluated the efficacy and safety of the combination drug piperacillin/tazobactam (PIPC/TAZ) in comparison with those of fourth-generation cephalosporins (4th Cephs) as initial empirical treatment in hematological malignancies patients with febrile neutropenia (FN). Among 200 patients assessed in this study, 49 had received PIPC/TAZ and 151 4th Cephs. Patient background characteristics were comparable between the two treatment groups. The overall efficacy rate in those receiving 4th Cephs and PIPC/TAZ was 57.0% (86/151 patients) and 59.2% (29/49 patients), respectively, with no significant difference detected between the two treatment regimens (P = 0.78). Treat-ment did not need to be discontinued or interrupted due to development of adverse drug reactions in any of the patients. Therefore in this study the efficacy and safety of PIPC/TAZ as initial antimicrobial treatment for FN in patients with hematological malignancies were not inferior to those of 4th Cephs. Based on the preliminary data obtained in this study, we propose to conduct a multicenter, prospective, controlled study to compare PIPC/TAZ versus CFPM given as empirical antimicrobial treatment against FN in patients with hematological malignancies.
