We analyzed the relationship between the clinical characteristics, comorbidity, average relative dose intensity (aRDI) and outcome in patients of over 75 years old with malignant lymphoma. Of the 98 patients studied, the mean age was 79.9 years, and 68 patients (69.4%) had B-cell lymphomas, corresponding to mainly diffuse large B-cell lymphoma. The 5-year overall survival rate was 32.2% in 97 malignant lymphoma patients. T/NK subtype, poor performance status (PS) and high-intermediate/high international prognostic index (IPI) were found to be predictive of significantly poorer overall survival, as is the case in young patients. Correlation between comorbidity index and survival rate was not observed. We also analyzed the aRDI of cyclophosphamide and pirarubicin for 64/97 patients. The proportion of patients receiving ≤84% of the planned DI during five cycles gradually increased. Most patients could not maintain aRDI ≥85%. However, overall survival was not significantly different between patients with aRDI ≥0.85 and those with aRDI ≤0.84. In conclusion, the prognoses of very elderly patients with malignant lymphoma were not so poor when they were appropriately treated with modification of the applied dose and the duration of chemotherapy according to their status.

To compare a novel, fully synthetic, polyurethane based glue (MAR-1) to fibrin sealant in a partial liver resection rat model. METHODSAfter 50% resection of the lateral left liver lobe in male Wistar rats (n = 7/group/time point), MAR-1, Fibrin or NaCl was applied. After 14, 21 and 90 postoperative days, sealant degradation, intra-abdominal adhesions were scored, and histological examination of liver tissue was performed. RESULTS(Mean ± SEM) (MAR-1 vs Fibrin vs NaCl). Bleeding mass was significantly higher in NaCl (3.36 ± 0.51 g) compared to MAR-1 (1.44 ± 0.40 g) and Fibrin (1.16 ± 0.32 g). At 14 and 90 d, bleeding time was significantly lower in MAR-1 (6.00 ± 0.9 s; 13.57 ± 3.22 s) and Fibrin (3.00 ± 0.44 s; 22.2 ± 9.75 s) compared to NaCl (158.16 ± 11.36 s; 127.5 ± 23.3 s). ALT levels were significantly higher in MAR-1 (27.66 ± 1 U/L) compared to Fibrin (24.16 ± 0.98 U/L) and NaCl (23.85 ± 0.80 U/L). Intrabdominal adhesions were significantly lower in MAR-1 (11.22% ± 5.5%) compared to NaCl (58.57% ± 11.83%). Degradation of the glue was observed and MAR-1 showed almost no traces of glue in the abdominal cavity as compared to the Fibrin (10% ± 5% 14 d; 7% ± 3% 21 d). Survival showed no significant differences between the groups. CONCLUSIONCompared to Fibrin, MAR-1 showed similar hemostatic properties, no adverse effects, and is biocompatible. Further studies on adhesion strength and biodegradability of synthetic sealants are warranted.

Flow Characteristics of Two-Dimensional Supersonic Under-Expanded Coanda-Reattached Jet

When there is a wall near the jet, it deflects and flows while being attached to the wall owing to the Coanda effect. The flow characteristics of the incompressible and two-imensional (2D) Coanda-reattached jets have been considerably explained. However, 2D supersonic under-expanded jets, reattached to side walls, have not been sufficiently investigated. These jets are used in gas-atomization to produce fine metal powder particles of several micrometers to several tens micrometers. In this case, the supersonic under-expanded jets are issued from an annular nozzle, which is set around a vertically installed circular nozzle for molten metal. The jet flow at the center cross-section of the annular jet resembles a 2D Coanda-reattached jet that deflects and attaches on the central axis. In this study, the flow characteristics of a supersonic under-expanded Coanda air jet from a 2D nozzle that reattaches to an offset side wall are elucidated through experiment and numerical analysis. For numerical analysis, we show how much it can express experimental results. Furazolidone-based triple therapy for H pylori gastritis in children

AIM： To evaluate the furazolidone-based triple therapy in children with symptomatic H pylori gastdtis. METHODS： A prospective and consecutive open trial was carried out. The study included 38 patients with upper digestive symptoms sufficiently severe to warrant endoscopic investigation. H pylori status was defined based both on histology and on positive ^13C-urea breath test. Drug regimen was a seven-day course of omeprazole, clarithromycin and furazolidone （100 mg, 200 mg if over 30 kg） twice daily. Eradication of H pylori was assessed two months after treatment by histology and ^13C-urea breath test. Further clinical evaluation was performed 7 d, 2 and 6 mo after the treatment. RESULTS： Thirty-eight patients （24 females, 14 males） were included. Their age ranged from 4 to 17.8 （mean 10.9 ± 3.7） years. On intent-to-treat analysis （n = 38）, the eradication rate of H pylori was 73.7% （95% CI, 65.2%-82%） whereas in per-protocol analysis （n = 33） it was 84.8% （95% CI, 78.5%-91%）. All the patients with duodenal ulcer （n = 7） were successfully treated （100% vs 56.2% with antral nodularity）. Side effects were reported in 26 patients （68.4%）, mainly vomiting （14/26） and abdominal pain （n = 13）. Successfully treated dyspeptic patients showed improvement in 78.9% of H pylori-negative patients after six months and in 50% of Hpylori-positive patients after six months of treatment. CONCLUSION： Triple therapy with furazolidone achieves moderate efficacy in H pylori treatment. The eradication rate seems to be higher in patients with duodenal ulcer.

Morus alba L., (cv Ichinose) was cultivated by in-room hydroponics. The flavor and texture of leaves were markedly improved enough to be edible. When the contents of 1-deoxynojirimycin (DNJ) and polyphenols were measured in the hydroponic cultivar, DNJ increased in the leaf compared to the field grown M. alba. However, polyphenols, in contrast, decreased compared to the field cultivar. HPLC profiling revealed marked difference in leaf components between hydroponic and field cultivars indicating relative contents of lipophilic polyphenols were increased. The polyphenols contents, especially, lipophilic polyphenols in the root were remarkably high compared to So-Haku-Hi (Sang Bai Pi in Chinese). The anti-obesity effect of the hydroponically grown Morus was further studied in rats by feeding high-fat, high-sucrose (HFHS) diet with and without supplementation of dried leaf and root powders for 15 weeks. As the result, both the leaf and root from the hydroponic cultivar showed potential anti-obesity and anti-hyperlipidemic functions through amelioration of insulin resistance. Differential effects of leaf and root powders indicated that besides DNJ, the lipophilic polyphenols may play a crucial role in the anti-diabetic function of hydroponically grown Morus alba L. The hydroponics will provide an alternate way to cultivate a novel resource of Morus for developing functional foods and medicines.

Influences of Network Delay on Quality of Experience for Soft Objects in Networked Real-Time Game with Haptic Sense

In this paper, we investigate the influences of network delay on QoE (Quality of Experience) such as the operability of haptic interface device and the fairness between players for soft objects in a networked real-time game subjectively and objectively. We handle a networked balloon bursting game in which two players burst balloons (i.e., soft objects) in a 3D virtual space by using haptic interface devices, and the players compete for the number of burst balloons. As a result, we find that the operability depends on the network delay from the local terminal to the other terminal, and the fairness is mainly dependent on the difference in network delay between the players’ terminals. We confirm that there exists a trade-off relationship between the operability and the fairness. We also see that the contribution of the fairness is larger than that of the operability to the comprehensive quality (i.e., the weighted sum of the operability and fairness). Assessment results further show that the output timing of terminals should be adjusted to the terminal which has the latest output timing to maintain the fairness when the difference in network delay between the terminals is large. In this way, the comprehensive quality at each terminal can be maintained as high as possible.
