Microalbuminuria in hepatitis C-genotype 4:Effect of pegylated interferon and ribavirin

AIM:To study the relation between hepatitis C virus (HCV) genotype 4 and microalbuminuria and renal impairment in relation to hepatic histology, and viremia in the absence of cryoglobulinemia, and to examine the effect of treatment on ***: Three hundred subjects, including 233 HCV genotype-4 infected patients, were tested for cryoglobulinemia, microalbuminuria, albumin creatinine ratio (ACR), urea, creatinine, and estimated glomerular filtration rate (eGFR). The parameters were measured again in the HCV patients after 48 wk of treatment with pegylated interferon and ***: Significantly higher levels of microalbumin- uria were detected in HCV-positive patients compared to HCV-negative controls (median 9.5 vs 5.9, respectively, Kruskal-Wallis P=0.017). Log microalbuminuria was significantly correlated with hepatic inflammation (r=0.13, P=0.036) and f ibrosis (r=0.12, P=0.061), but not with viral load (r=-0.03, P=0.610), or alanine transaminase (r=-0.03, P=0.617). Diabetes mellitus neither significantly moderated (χ2=0.13, P=0.720), nor mediated (Sobel test P=0.49) the HCV effect. HCV status was signifi cantly associated with log microalbu-minuria (χ2=4.97, P=0.026), adjusting for age, gender, diabetes, cryoglobulinemia, urea and creatinine. A positive HCV status was not significantly associated with low eGFR (<60 mL/min every 1.73 m2) [odds ratio (OR): 0.5, 95% confidence interval (CI):0.2-1.4], nor with high ACR (OR: 1.7, 95% CI:0.7-4.1). End-of-treatment response (ETR) was achieved in 51.9% of patients. Individuals with ETR had significantly lower microalbuminuria post-treatment (χ2=8.19, P=0.004).CONCLUSION: HCV affected the development of microalbuminuria independent of diabetes or cryoglobulinemia. Combination therapy of pegylated interferon-ribavirin had a positive effect in reducing microalbuminuria.

Taurolock Effectively Reduces Tunneled Dialysis Catheter-Related Infection and Catheter Exchange Rates among Hemodialysis Patients

Aims: Catheter-related infection, which is one of the major side effects of the use of dialysis catheters, leads to increases in hospitalization, morbidity and mortality. Antibiotic lock is an option for reducing the incidence of these infections, but there are concerns regarding antibiotic resistance. A prior study demonstrated that Taurolock (a taurolidine lock) may reduce the rate of catheter-related infection. Methods and Material: This investigation was a prospective before-and-after study. During period one, patients continued to use a heparin lock (5000 units/ml) for 6 months. During period two, they were shifted to Taurolock (a solution of 1.35% taurolidine, 4% citrate, and 500 units/ml of heparin) for 6 months. The primary outcome was the incidence of tunneled catheter-related infection and/or catheter exchange, and the secondary outcomes were the effects of Taurolock on catheter flow rate, dialysis adequacy, and catheter malfunction rate. Results: A total of 49 patients were included in this study. During period one, the primary outcome occurred 17 times, with an incidence rate of 86.8 per 100 patient-years;during period two, the primary outcome occurred 7 times, with an incidence rate of 32.6 per 100 patient-years (incidence rate ratio: 2.65, 95% confidence interval (CI): 1.05 - 7.6, P = 0.023). There was no significant difference between the two periods with respect to mean catheter blood flow rate (P = 0.29). During period one, thrombolytic therapy (TPA) lock was indicated on 19 occasions, with an incidence rate of 97 per 100 patient-years;during period two, TPA lock was indicated on 53 occasions, with an incidence rate of 247.5 per 100 patient-years (incidence rate ratio: 0.3, 95% CI: 0.21 - 0.67, P = 0.0003). Conclusions: We demonstrated that Taurolock usage is safe and effective for the prevention of dialysis catheter-related infection and/or catheter exchange.

The Wireless Sensor Network(WSN)is regarded as the fastest expanding technological trend in recent years due its application in a variety of *** the monitoring region,several sensor nodes with various sensing capabilities are installed to gather appropriate data and communicate it to the *** proposed system of the heterogeneous WSN employing LoRaWAN-Zigbee based hybrid communication is explored in this research *** communicate in a network,two Long–Range Wide Area Network(LoRaWAN)sensor clusters and two Zigbee sensor clusters are employed,together with two Zigbee and LoRaWAN *** suggested Golden eagle shepherd optimization(GESO)method then forms Zigbee as well as LoRaWAN networking ***,depending on energy usage and data packet size,the fitness of each sensor node is assessed using the Dynamic Intelligent Reasoning Based Neural(DIRN)*** software is used to implement and execute this *** the Zigbee network’s transmission distance is 650 m and the LoRaWAN network’s transmission range is 3.5 km,the system can function with a packet loss rate of less than 0.04 *** study shows significant gains in the performance of the system when compared to traditional approaches based on digital findings obtained on software solutions.

Mother’s Nutritional Knowledge and Practice towards Children under Two Years of Age and Its Impact on Their Health in Abuzaid Albuluk Pediatric Specialized Hospital in 2023

Background: Nutritional knowledge and practise is an important measure to improve dietary habits and food choices. The daily food should contain balanced nutrition as needed to support optimal growth, avoid deficiency diseases and prevent the diseases which might interfere with the children health. Materials & Methods: A descriptive cross-sectional non-interventional hospital-based study was carried out among 122 mothers and 122 children by using a non-probability convenience sampling technique in the period of 15 March to 15 April 2023, and using interviews with structural questionnaires, the data was analyzed using SPSS version 26. Results: We found that most of the children had moderate health, while most of the mothers had moderate nutritional knowledge and practise towards their children, also we found that the relationship between mothers’ knowledge and practise and child health was significant (P Conclusion: Mothers’ knowledge and practise among children was moderate and children had moderate health. Wrong beliefs and cultural taboos play major roles in decreasing knowledge and practise.
