Background and objective: Nasal obstruction is the most frequent presentation to the otolaryngologist all over the world. Symptoms are multisystem involvement such as headache, epistaxis, ophthalmological, otological, upper and lower respiratory tract infections;and many more. Septoplasty is commonly performed for nasal septal deviation as a primary indication of nasal airway obstruction. Physical examination, anterior rhinoscopy and nasal endoscopy are considered the gold standard for detecting septal deviation. Computed tomography (CT) can also be used to evaluate the nasal septum, anatomical variants and unveils the associated sinonasal diseases. This study aimed to find out the advantages of CT in endoscopic septoplasty for all out benefits to the patients. Patients and methods: 168 patients of nasal septal deviation were selected from 2015 to 2018 in the Bangladesh Medical and Popular Medical College Specialized Hospital. Age ranged between 10 years to 70 years of age. CT scanning in both coronal, axial and sagittal sections had been done in all patients after proper history taking, physical examination, anterior rhinoscopy and nasal endoscopy. Endoscopic septoplasty alone and/or other ancillary procedures were performed in all 168 patients under general anesthesia. Results: Amongst 168 patients, male was 116 (69%) and female was 52 (31%) with a ratio 2.2:1. Maximum 58 (35%) reported in 31 to 40 years and 48 (29%) was found in 21 to 31 years of age. Hypertrophied inferior turbinate (HIT) 54 (32%) and 48 (29%) concha bullosa (CB) were associated with 168 septal deviation. Associated pathologies like maxillary antral cyst 68 (40%) and maxillary sinusitis 62 (37%) were documented. Conclusion: CT scanning unveils the abnormal anatomic variants and associated sinonasal pathologies along with nasal septal deviation. With the COVID-19 pandemic, disparities between the infection rate and death rate in different countries become a major concern. In some countries, lower mortality rate compared to others can be explained by better testing capacity and intensive care facilities. Complete SARS-CoV-2 genome sequences from different countries of the world are continually submitted to Global Initiative for Sharing All Influenza Data using Next Generation Sequencing method. A SARS-CoV-2 variant with a D 614G Mutation in the spike (S) protein has become the most dominant form in the global pandemic. There are a number of ongoing studies trying to relate this mutation with the infectivity, mortality, transmissibility of the virus and its impact on vaccine development. This review aims to accumulate the major findings from some of these studies and focus its future implication. Some studies suggested D 614G strain has increased binding capacity, it affects more cells at a faster rate, so has a high transmissibility. Patients infected with this strain were found with high viral load. But still now there is no such evidence that this strain produces more severe disease as well as increased mortality. The structural change of spike protein produced by D 614G mutation was minor and did not hamper the vaccine efficacy. Some studies showed antibodies produced against D614 strain can neutralize G614 strain and vice versa. Whenever a mutation occurs in spike protein there are always chances of affecting the infectivity, transmissibility, vaccine efficacy. Therefore, more studies are required to find out the overall effect of D 614G mutation.

本试验旨在评估在不同品种蛋鸡日粮中添加大蒜剂对蛋黄中胆固醇浓度和总体生产性能影响的潜力。试验选用海蓝褐、伊莎褐、罗曼、星杂、巴布谷、星杂-579共六个品种的28日龄蛋鸡36只,每个品种六只,日粮中分别添加0（对照组）、2、4、6、8、10%的晒干的大蒜剂,饲喂6周。试验期为六周,试验期间各日粮和品种的产蛋鸡的平均蛋重、总蛋重、采食量、饲料转化率、日增重都无明显差异（P＞0.05）。但添加大蒜组蛋黄重增加,不同品种增加幅度不同（P＜0.05）。与其他品种相比,巴布谷品种的产蛋量和蛋黄重都有显著提高。随日粮中大蒜的添加水平的增加血清和蛋黄中胆固醇含量成线性递减（P＜0.05）。不同的品种的血清和蛋黄中胆固醇浓度也不同（P＜0.05）。研究结果表明:产蛋鸡日粮中添加大蒜剂可减少血清和蛋黄中胆固醇的含量,但对产蛋量无显著影响。

Background: By the dawn of this modern era of science, the prime challenge of physician is cardiovascular disease (CVD). The most important modifiable risk factors of CVDs are unhealthy diet, physical inactivity and tobacco use. The effects of unhealthy diet and physical inactivity include abnormal blood lipid, obesity and hypertension. We tried to evaluate and correlate the pattern of lipid profile in obese and non-obese hypertensive patients. Objectives: This study was conducted at medicine department of Cumilla Medical College Hospital. The principal aim was to evaluate the lipid profile in obese and non-obese adult hypertensive patients. Methodology: During this cross sectional analytical study, a total of 100 adult hypertensive patients were taken by purposive sampling. Among them 50 (group 1) patients were taken those were obese and 50 (group 2) patients taken those were non-obese according to BMI measurement on operational definition. Diagnosis of hypertension would be established with the help of ambulatory BP measurements two occasions few minutes apart. The staging of hypertension was done according to JNC7 Criteria. Morning blood samples were taken after 8 - 12 hours of fasting and lipid profiles were done on authentic laboratories. The laboratory values were interpreted according to the operational definition of dyslipidaemia. The ethical research and review committee approved the study protocol and signed informed consent was obtained from the participants. Aim: The purpose of this article is to advocate standard skull base technique for removing the clinoidal meningioma and to delineate the technique's advantages that aid in achieving an improved extent of tumor resection and enhancing the patients' overall outcome, specially their visual outcome. Methods: A retrospective analysis was performed on 10 consecutive patients with clinoidal meningiomas who underwent surgical resection at the Bangabandhu Sheikh Mujib Medical University and other private clinics between May 2013 and July 2016. A standard pterional craniotomy technique consisting of extradural anterior clinoidectomy, coupled with optic canal unroofing and optic nerve sheath opening was used in all patients. All patients had thorough preoperative and postoperative ophthalmological evaluations. The follow-up period ranged from 6 to 42 months. Results: Total resection was achieved in 5 (50.0%) of the 10 patients in this series. The majority of the patients with preoperative visual impairment experienced significant visual improvement 7 of 10 patients;70.0%). Conclusion: In the majority of patients with clinoidal meningiomas, total resection may be achieved with minimal complications. For large tumors encasing the optic nerve and internal carotid artery, or for those tumors causing preoperative visual impairment, use of the cranial base technique delineated in this study may lead to significant improvement in the patients' visual and overall outcomes.

Penetrating injuries to the subclavian arteries as well as post traumatic pseudoaneurysm involving Subclavian artery (SCA) are very much uncommon. We present one case of a 21 year-old male sustained a physical assault with a post stab injury left subclavian artery pseudoaneurysm. He was referred to our facility, two days after sustaining a penetrating chest trauma over left infraclavicular area of the chest. The presentation, diagnostic procedures and surgical approach for management of this very rare injury are discussed.

A study was conducted to investigate the land-use pattern change over a period of 18 years （1988-2006） by using remote sens- ing and Geographical Information System （GIS） technologies, in the West Bhanugach Reserved Forest, a hill forest, in Sylhet Forest Divi- sion of Bangladesh, The images were processed using ERDAS Imagine software. Both supervised and unsupervised approaches were ap- plied and ground control points were collected using a GPS. Maps were prepared using GIS software. Results showed that vegetation cover drastically decreased from the year 1988 to 1996 （l 826 ha to l 714.85 ha）, but increased gradually from the year 1996 to 2006 （l 714.85 ha to l 847.83 ha） due to the initiation of co-management practice involving local communities. Change in bare land was inversely proportionate to the amount of vegetation cover changes unless any other land-uses were converted into bare land. The area of water bodies increased from the year 1988 to 1996 （307.67 ha to 379.53 ha）, but decreased from the year 1996 to 1997, then remained invariabile from the year 1997 to 2006. Some recommendations were also made for applying the RS and GIS techniques to study the land-use pattern change in the Bhanugach Reserved Forest and to create a GIS data base for the study area.
