Use of donepezil for neurocognitive recovery after brain injury in adult and pediatric populations:a scoping review

There are few pharmacologic options for the treatment of cognitive deficits associated with traumatic brain injury in pediatric *** inhibitors such as donepezil have been evaluated in adult patients after traumatic brain injury,but relatively less is known about the effect in pediatric *** goal of this review is to identify knowledge gaps in the efficacy and safety of acetylcholinesterase inhibito rs as a potential a djuvant treatment fo r neurocognitive decline in pediatric patients with traumatic brain *** queried PubMed to identify literature published from database inception thro ugh June 2023 desc ribing the use of donepezil in young adult traumatic brain injury and pediatric patients with predefined *** on preselected search criteria,340 unique papers we re selected for title and abstra ct ***-two reco rds were reviewed in full after eliminating preclinical studies and pape rs outside the scope of the *** adult traumatic brain injury,we review results from 14 papers detailing 227 subjects where evidence suggests donepezil is well tole rated and shows both objective and patient-reported efficacy for reducing cognitive *** children,3 pape rs report on 5 children recovering from traumatic brain injury,showing limited *** additional 15 pediatric studies conducted in populations at risk for cognitive dysfunction provide a broader look at safety and efficacy in 210 patients in the pediatric age *** its promise for efficacy in adults with traumatic brain injury and tole rability in pediatric patients,we believe further study of donepezil for children and adolescents with traumatic brain injury is warranted.

Eccentric exercise-induced muscle weakness abolishes sex differences in fatigability during sustained submaximal isometric contractions

Background:Females are typically less fatigable than males during sustained isometric contractions at lower isometric contraction *** sex difference in fatigability becomes more variable during higher intensity isometric and dynamic *** less fatiguing than isometric or concentric contractions,eccentric contractions induce greater and longer lasting impairments in force ***,it is not clear how muscle weakness influences fatigability in males and females during sustained isometric ***:We investigated the effects of eccentric exercise-induced muscle weakness on time to task failure(TTF)during a sustained submaximal isometric contraction in young(18-30 years)healthy males(n=9)and females(n=10).Participants performed a sustained isometric contraction of the dorsiflexors at 35°plantar flexion by matching a 30%maximal voluntary contraction(MVC)torque target until task failure(i.e.,falling below 5%of their target torque for>2 s).The same sustained isometric contraction was repeated 30 min after 150 maximal eccentric *** and antagonist activation were assessed using surface electromyography over the tibialis anterior and soleus muscles,***:Males were~41%stronger than *** eccentric exercise both males and females experienced an~20%decline in maximal voliuntary contraction *** was-34%longer in females than males prior to eccentic exercise-induced muscle ***,following eccentric exercise-induced muscle weakness,this sex-related difference was abolished,with both groups having an"45%shorter ***,there was~100%greater antagonist activation in the female group during the sustained isometric contraction following exercise-induced weakness as compared to the ***:This increase in antagonist activation disadvantaged females by decreasing their TTF,resulting in a blunting of their typical fatigability advantage over males.

Background:We sought to assess the overall benefit of laparoscopic versus open hepatectomy for treatment of colorectal liver metastases(CRLMs)using the win ratio,a novel methodological ***:CRLM patients undergoing curative-intent resection in 2001-2018 were identified from an international multi-institutional *** were paired and matched based on age,number and size of lesions,lymph node status and receipt of preoperative *** win ratio was calculated based on margin status,severity of postoperative complications,90-day mortality,time to recurrence,and time to ***:Among 962 patients,the majority underwent open hepatectomy(n=832,86.5%),while a minority underwent laparoscopic hepatectomy(n=130,13.5%).Among matched patient-to-patient pairs,the odds of the patient undergoing laparoscopic resection“winning”were 1.77[WR:1.77,95%confidence interval(CI):1.42-2.34].The win ratio favored laparoscopic hepatectomy independent of low(WR:2.94,95%CI:1.20-6.39),medium(WR:1.56,95%CI:1.16-2.10)or high(WR:7.25,95%CI:1.13-32.0)tumor burden,as well as unilobar(WR:1.71,95%CI:1.25-2.31)or bilobar(WR:4.57,95%CI:2.36-8.64)*** odds of“winning”were particularly pronounced relative to short-term outcomes(i.e.,90-day mortality and severity of postoperative complications)(WR:4.06,95%CI:2.33-7.78).Conclusions:Patients undergoing laparoscopic hepatectomy had 77%increased odds of“winning”.Laparoscopic liver resection should be strongly considered as a preferred approach to resection in CRLM patients.

Animals communicate with potential mates using species-specific signals,and pheromones are powerful sexual signals that modify conspecific behavior to facilitate mate *** the vertebrates,snakes are especially adept in mate searching via chemical trailing,which is particularly relevant given that many snake species are invasive outside their native *** signals used in mate choice are,thus,potentially valuable tools for management of invasive snake *** Burmese python(Python bivittatus)is an invasive snake in the Florida Everglades where it is negatively impacting native *** this study,we sought to:(i)determine if males can follow conspecific chemical trails in a Y-maze;and(ii)describe the mate searching behaviors exhibited by males while *** males consistently followed a single female scent trail in the maze,but when only a male scent trail was present they did not discriminate between the male and blank *** of tongue-flicking,a proxy for chemosensory sampling,was also marginally higher when males were following female versus male scent ***,when both female and male scent trails were simultaneously present in the Y-maze,males did not show a preference for the female arm,although the tongue-flick rate was higher in the female-only trial compared to female versus *** of multiple male behaviors individually and using an ethogram revealed that behaviors were more frequent and complex in the female-only trials compared to male-only *** behavioral trials are needed to determine if an effective pheromonal approach to Burmese python management is possible.

We compared small mammal community composition among undisturbed habitats and habitats disturbed by military operations on Warren Grove Gunnery Range(WGR)in the New Jersey *** is one of the largest tracts of protected land within this globally rare *** in the form of fire,mowing,soil disruption and logging has had a large effect on small mammal occurrence and *** the 14 small mammal species that occur in the Pinelands,9 live on WGR,including large populations of the southern bog lemming(synaptomys cooperi Baird,1858)and meadow jumping mouse[Zapus hudsonius(Zimmermann,1780)].simpson’s Index of Diversity was 0 for most disturbed sites and was generally greater in wetlands than in ***-footed mouse[Peromyscus leucopus(Rafinesque,1818)]was the most common species on WGR and had a dominant effect on species diversity and community similarity *** dominated upland habitats and was the only species to occur in several disturbed habitats,whereas all 9 species occurred in *** components analysis indicated that most variation in species diversity was explained by disturbance and differences between upland and wetland habitats,due to presence of white-footed mice in disturbed and upland *** jumping mice,southern bog lemmings and red-back voles[Myodes gapperi(Vigors,1830)]were positively correlated with wetland habitats,and pine voles[Microtus pinetorum(Le Conte,1830)],short-tail shrews[Blarina brevicauda(say,1823)]and eastern chipmunks[Tamias striatus(Linnaeus,1758)]were associated with *** heterogeneity at WGR,including extensive undisturbed wetlands and uplands supported a rich diversity of small mammal species.

social relationships formed within a network of interacting group members can have a profound impact on an individual’s behavior and fitness. However, we have little understanding of how individuals perceive their relationships and how this perception relates to our external measures of interactions. We investigated the perception of affiliative and agonistic relationships at both the dyadic and emergent social levels in two captive groups of monk parakeets (Myiopsitta monachus,n =21 and 19) using social network analysis and playback experiments. At the dyadic social scale, individuals directed less aggression towards their strong affiliative partners and more aggression towards non-partner *** the emergent social scale, there was no association between relationships in different social contexts and an individual’s dominance rank did not correlate with its popularity rank. Playback response patterns were mainly driven by relationships in affiliative social contexts at the dyadic scale. In both groups, individual responses to playback experiments were significantly affected by strong affiliative relationships at the dyadic social scale, albeit in different directions in the two groups. Response patterns were also affected by affiliative relationships at the emergent social scale, but only in one of the two groups. Within affiliative relationships, those at the dyadic social scale were perceived by individuals in both groups, but those at the emergent social scale only affected responses in one group. These results provide preliminary evidence that relationships in affiliative social contexts may be perceived as more important than agonistic relationships in captive monk parakeet groups. Our approach could be used in a wide range of social species and comparative analyses could provide important insight into how individuals perceive relationships across social contexts and social scales.

Aims:To determine whether day and time of admission influences the practice patterns of the admitting general surgeon and subsequent outcomes for patients diagnosed with small bowel ***:A retrospective database review was carried out,covering patients admitted with the presumed diagnosis of partial small bowel obstruction from 2004–***:A total of 404 patients met the inclusion *** hundred and thirty-nine were admitted during the day,93 at night and 172 on the *** 30.2%of the patients were managed operatively with no significant difference between the groups(P=0.89);however,of patients taken to the operating room,patients admitted during the day received operative intervention over 24 hours earlier than those admitted at a weekend,0.79 days vs 1.90 days,respectively(P=0.05).Overall mortality was low at 1.7%,with no difference noted between the groups(P=0.35).Likewise there was no difference in morbidity rates between the three groups(P=0.90).Conclusions:Despite a faster time to operative intervention in those patients admitted during the day,our study revealed that time of admission does not appear to correlate to patient outcome or mortality.
