Hand-foot syndrome (HFS), also known as palmar-plantar erythrodysesthesia, is a skin toxicity that could be observed during target therapies such as with tyrosine-kinase inhibitors (TKI). It usually develops within the first 2 - 4 weeks of drug administration. We present a case of HFS induced by Pazopanib after 2 months of treatment, in patients previously treated with Sunitinib, suggesting a possible cumulative toxicity of two drugs. The clinical and therapeutic management of skin adverse reactions during TKI therapy usually requires 25% dose reduction and adequate local treatment. It is important for the clinicians to recognize clinical signs and symptoms of such skin toxicities. Attention should be paid especially when two or more drugs from the same class are used in combined treatment.

香蕉、可可是厄瓜多尔重要的两大农产品。香蕉产业在厄瓜多尔国民经济中占有十分重要的地位,是仅次于石油行业的第二大支柱产业。2019年,厄瓜多尔香蕉收获面积18.33万公顷,总产量658.35万吨。可可种植业是厄瓜多尔另一农业支柱产业,咖啡和可可出口在厄瓜多尔农产品出口中占有50%以上的份额。多年来,厄瓜多尔经济高度依赖石油出口,但近年来农产品出口额增长迅速,已经成为第一大外汇收入来源。厄瓜多尔已经证明了农业部门在国民经济中的重要性,并与繁荣时期和出口危机密切相关。最近,人们意识到香蕉和可可出口对加强厄瓜多尔经济的重要性日益提高。在过去的16年中,该生产部门经历了由低水平的生产和产品需求引起的不规则危机。这些产品在市场上的数量和价格都不稳定,具有很高的风险和不确定性。本研究的目的是评估过去30年来农产品出口（香蕉和可可）与厄瓜多尔农业经济增长的趋势关系以及对农业经济增长的影响。重点研究农产品出口与农业经济增长之间是否存在长期的因果关系,以期为提升农产品贸易水平提供理论参考。本文首先对厄瓜多尔农业经济和农产品贸易的发展进程进行了回顾。30年来,厄瓜多尔农业经济经历了繁荣到萧条的发展循环,农产品出口贸易波动性较大,近10年来出口总体呈现明显的增长趋势。厄瓜多尔香蕉出口长期位居世界第1位,可可出口跻身世界第4位。国际市场价格波动、签订双边贸易协议、降低关税以及国内政策变化等是影响厄瓜多尔香蕉、可可出口的主要因素。其次,本文基于索洛新古典增长理论,选取农业生产总值、香蕉出口额、可可出口额、农业固定资本形成、农业劳动力等指标,利用1987年至2017年的年度时间序列数据,通过单位根检验、约翰逊协整检验以及向量自回归模型（VAR）等方法,对农产品贸易对厄瓜多尔农业经济增长的影响进行了计量分析。研究结果表明,厄瓜多尔的农业出口与农业经济增长之间没有长期关系,短期内香蕉出口、可可出口和农业资本形成等对农业经济增长具有显著的促进作用,农业劳动力对农业经济增长的作用不显著。为此,本文提出政府应实施旨在提高香蕉和可可作物生产力和质量的政策,要培训生产者进一步提高产品的质量标准,以充分发挥国际市场上的优势。建议政府通过不断完善法律制度,鼓励农产品出口并寻求多样化,建立香蕉和可可出口协会拓展市场。同时,积极实施援助项目,与国际社会共同提升可可价值链,生产可可加工产品以供出口,从而为农业部门做出贡献并促进国家经济的发展。

The clinical diagnosis of classic Crohn's disease (CD)of the small bowel is based on a typical history, tender right lower quadrant fullness or mass, and characteristic radiographic findings of the terminal ileum. Appendicitis may as well present with chronic or recurrent symptoms and this presentation may be confused with CD. We herein describe the case of a young teenage girl with a presumptive diagnosis of CD, who was ultimately diagnosed as having chronic nongranulomatous appendicitis. The literature on the subject is reviewed.

The preservation of the traditional subsistence farming systems and the conservation of natural values (habitats, fauna and flora), in articulation with local development, was the key issues in this paper. The farmers have been able to manage the systems so as to cope with adverse conditions, but their future is quite uncertain. The methodology included: (1) field observation visits and cartographic information analysis of land use; (2) structured interviews with animal production and agro-forestry specialists; and (3) meetings with the participation of stakeholders. The paper concludes that depopulation is responsible for the increasing decline of agriculture and livestock. The decrease in agricultural activity in general and the consequent practices and traditional systems of agriculture causes changes in the landscape and has negative effects on nature conservation and biodiversity. Thus, we believe that the preservation and development of traditional economic activities linked to agriculture, agro-forestry and food processing are essential to maintain the typical landscape and to preserve natural values of montemuro Site. The paper shows that in mountain areas, particularly those classified as nature network sites, farming, economic development and nature conservation are not antagonistic objectives.

Complete Kawasaki Disease in a 21-Year-Old Caucasian Patient

Kawasaki disease is a rare condition in adults and it may remain unrecognized because many pa-tients do not fulfil all the diagnostic criteria for the diagnosis of “complete” Kawasaki disease. The authors report a case of 21-year-old male Caucasian patient complaining fever and presenting a diffuse maculopapular and erythema multiforme-like eruption, erythema and edema of the hands, feet and ears, non-exudative bilateral conjunctivitis, injected lips, strawberry tongue and symmetrical widespread lymphadenopathy. On the basis of clinical presentation, laboratory results and diagnostic criteria, a diagnosis of complete Kawasaki disease has been done. After treatment with systemic corticosteroids and aspirin (100 mg trice daily), the patient showed clinical improvement with typical large scale desquamation of the hands. The patient did not present any complication during one month follow-up. It is important to be familiar with the signs and symptoms of Kawasaki disease, because early diagnosis and adequate treatment is crucial for the prevention of cardiovascular morbidity and mortality.
