Five Year Follow up after Surgical Treatment of Type 2 Diabetes with Laparoscopic Sleeve Gastrectomy Associated with a Duodenal Ileal Interposition

Introduction: Type 2 diabetes mellitus (T2D) is a chronic disease, primarily caused by a combination of defective insulin secretion and the inability of insulin-sensitive tissues to respond to insulin. Laparoscopic sleeve gastrectomy associated with duodenal ileal interposition (SGDII) has been shown to be a feasible treatment option for patients with T2D, as it provides improvement and control of glycemia, dyslipidemia and arterial hypertension. The aim of this study was to evaluate the mid and long-term results of SGDII for the treatment of diabetic patients, considering diabetic remission, weight loss and postoperative complications. Materials and Methods: Retrospective study with 96 patients with T2D submitted to SGDII, between 2010 and 2016. The glycated hemoglobin (HbA1c) value Results: Sixty-one patients (62.8%) were male, and 36 patients (37.2%) were female. The average age was 50.9 years. Median BMI was 33.43 kg/m2. The incidence of major postoperative complications in the first 30 days was 2.1%, with no mortality. Dyslipidemia control was sustained, after five years, in 62% of the patients. The five years follow-up demonstrated that 80% of patients had T2D remission at one year, 74.5% at three years and 61.8% at five years. Univariate analysis demonstrated that preoperative values of HbA1c and BMI, preoperative use of insulin, gender, and 30-day complication were not predictors of remission at all study intervals. The average duration of the disease was nine years and the mean glycated hemoglobin before surgery was 8.95%. Conclusion: SGDII resulted in good glycemic control at 5-years follow-up and represents a valid alternative for the treatment of T2D.

Robotic Surgery: An Effective Treatment Option for Epiphrenic Diverticulum Associated with Nutcracker Esophagus

Background: Epiphrenic Diverticulum is frequently associated with esophageal motility disorders, such as nutcrackers esophagus. The diagnosis is usually made using imaging studies such as a Barium esophagogram, and esophageal manometry. Surgical treatment options for epiphrenic diverticulum and EN include diverticulectomy and wide myotomy. Aim: The resection of three epiphrenic diverticula and extensive myotomy were performed by robotic thoracoscopy uneventfully. Case presentation: A 65-year-old female complaining of dysphagia for solid foods, Chest pain and regurgitation. Esophagogastroduodenoscopy (EDG) with difficulty in advancing the endoscope at 25 cm and demonstrating an ED, no hiatal hernia and normal stomach and duodenum. Barium Esophagogram showed multiple diverticula and tortuosity throughout the esophagus. Conclusion: With robotic surgery, surgeons can perform highly precise operations with enhanced 3D vision and control. Through this cutting-edge approach, the treatment of ED associated with EN can be drastically changed, promising better outcomes for patients.

Testicular prostheses-impact on quality of life and sexual function

Orchiectomy is the recommended treatment for many testicular ***,testicular prosthesis placement is not always performed for different *** this study,we aimed to evaluate patients'opinions and the impact on sexual function and quality of *** retrospective observational single-center study included patients who underwent orchiectomy between January 2014 and december 2020 at the department of Urology,Braga's Hospital(Braga,Portugal),where testicular implants were always available and the decision to undergo the procedure was made fully independent of *** completed four questionnaires that assessed demographic data,satisfaction,self-esteem,and sexual *** the 96 patients who underwent orchidectomy,59 replied to the questionnaires,and of these patients,86.4%decided to undergo silicone-based testicular prostheses *** remaining 13.6%refused the implant based on concerns about complications(37.5%),because they felt that it was unnecessary(37.5%),or because it was not offered by the doctor(25.0%).Overall,96.1%of these patients were satisfied with the implant;however,25.5%classified it as"too firm".No statistically significant differences were found in sexual function(all P>0.05).However,it can be observed that there are more patients with prostheses presenting normal sexual activity compared to patients without prostheses(74.0%vs 50.0%),and none of them reported severe erectile dysfunction(0 vs 16.7%).Regarding self-esteem,both patients with and without prostheses present very similar average scores with no statistically significant *** present study highlights the highest level of satisfaction among patients who received testicular *** prostheses implantation is a safe procedure that does not hamper sexual function after orchiectomy.

Percutaneous nephrostomy versus ureteral stent in hydronephrosis secondary to obstructive urolithiasis:A systematic review and meta-analysis

Objective: To assess if there is a preferable intervention between retrograde ureteral stent (RUS) and percutaneous nephrostomy (PCN) tube, in cases of upper urinary tract stone obstruction with complications requiring urgent drainage, by evaluating outcomes regarding urinary symptoms, quality of life (QoL), spontaneous stone passage, and length of hospital stays, since there is no literature stating the superiority of one modality over the ***: We searched MEDLINE and other sources for relevant articles in June 2019 without any date restrictions or filters applied. The selection was done first by the title and abstract screening and then by full-text assessment for eligibility. Only randomized controlled trials or cohort studies in patients with hydronephrosis secondary to obstructive urolithiasis that presented comparative data between PCN and RUS placement concerning at least one of the defined outcome measures were included. Lastly, MEDLINE database and PubMed platform were screened again using the same terms, from June 2019 until November ***: Of 556 initial articles, seven were included in this review. Most works were considered of moderate-to-high quality. Three studies regarding QoL showed a tendency against stenting, even though only one demonstrated statistically significant negative impact on overall health state. Two works reported significantly more post-intervention urinary symptoms in stenting patients. One article found that PCN is a significant predictor of spontaneous stone passage, when adjusted for stone size and location. Findings on length of hospital stays were not consistent among ***: PCN appears to be the intervention better tolerated, with less impact on the patient’s perceived QoL and less post-operative urinary symptoms, in comparison with RUS. Nevertheless, further studies with larger samples and a randomized controlled design are suggested.

Success in speed swimming depends on the efficiency of the anaerobic system for the production of cellular energy,especially during muscle power *** the adolescent athletes much is unknown with regards to the relationships between relative power of upper and lower limbs with speed swimming *** aim the present study was to identify differences in relative muscle power of upper and lower limbs in adolescent swimmers and relate these to speed swimming *** adolescents,of both sexes(50%female,50%male,30 swimmers and 30 controls),were *** relative upper limb power(ULP[W/kg])was assessed by a medicine ball test and the relative lower limbs power(LLP[W/kg])by a jump test on a jumping *** mass of the upper and lower limbs was assessed by dual-energy X-ray absorptiometry(DXA)(g).Sport performance was assessed during national level competition(50-m swimming time[in seconds]).Biological maturation(BM)was indexed by years from attainment of peak height ***(W/kg)was higher than LLP(W/kg)in both groups(pcluded that ULP(W/kg)is higher than LLP(W/kg)in adolescent *** and lower limb lean mass and BM were both positively associated with increased ULP(W/kg)and LLP(W/kg).

This review shows the importance of WO_(3)photoanode as a potentially low-cost,efficient,stable,and photoactive material for light-driven water *** such,this manuscript aims to review the most recent publications regarding the strategies to improve the phoelectroactivity of WO_(3)films for water *** addition,this review aims to graphically highlight and discuss the general trendings of the photocurrent density response and stability test of the recent outstanding studies in the literature for photoelectrochemical water splitting *** strategies covered in this review will not only concern the WO_(3)morphology and crystal plane growth,but also the many arrangements possibilities to improve the WO_(3)efficiency for water photoelectrooxidation,such as defect engineering based on oxygen vacancies,doping,decorations,and homo and *** these strategies are compared by the photocurrent density results and by the stability of these *** best results in this sense were observed in cases where the use of heterojunction was applied together with a desired morphology and crystal plane of the WO_(3)***,the modifications that caused a decrease in the photocurrent density reaching values that are even lower than the pure WO_(3)were also *** this way,this review intends to improve the knowledge about the synthesis and design of WO_(3)photoanodes to further obtain an efficient photocatalyst to minimize the recombination losses or losses across the interfaces and improve the photoelectroactivity for water splitting in the large-scale application.

Beam design for voice coil motors used for energy harvesting purpose with low frequency vibrations:A finite element analysis

In this study,a numerical approach is established to design a beam coupled to a Voice Coil Motor(VCM)with the aim to maximize the displacement in the inductive transducer.A finite element model is developed to simulate a VCM with different beams applying a harmonic *** VCM is extracted from a recycled hard disk drive(HDD)and a parametric modal analysis is performed to identify the material parameters of the HDD and the *** parameters are obtained comparing the real vibration modes and natural frequencies(VCM-beam)with those determined from the finite element model.A numerical-experimental case study is carried out to demonstrate that if a beam is designed for a specific low frequency vibration between 0 and 10 Hz,the displacements are maximized in the *** this purpose,real acceleration measurements taken from three individuals are used to provide the vibration signals in the numerical model.A beam is designed for one of the individuals using the natural frequency values determined from the measured *** show that the displacements are maximized in the model which coincides with the natural frequency of the chosen *** main purpose of this research is to establish a design tool for energy harvesting purposes with VCM based on low frequency vibration sources as for example gait motions.

Situs Inversus during Bariatric Surgery: A Retrospective Analyze

Background: We were reporting three cases of Situs Inversus in morbidly obese patients that were submitted a laparoscopic gastric bypass (LGBP). The condition itself was not a serious hazard to normal, healthy patients but, may cause difficulties in the diagnosis and management of abdominal pathology. The incidence of surgical disease did not seem to be different as in the general population, but its presentation, diagnosis and the operative procedure were. This was particularly true for laparoscopic surgery. Methods: From January 1999 to June 2015, 3047 morbidly obese patients underwent. Three patients with Situs Inverse (SI), two of the three with Situs Inversus Totalis (SIT), and one with Situs Inversus Parcialis (SIP) were submitted to a laparoscopic Roux-en-Y gastric bypass with no major difficulties. The mean age was 45 years (39 - 49), BMI 37.7 kg/m2, two of the above patients were female. Results: The LGBP was completed successfully in two of the three patients with SI. The mean duration of surgery was a 167 minutes, ranging from 110 - 210 minutes. The hospital stay was 5 days (3 - 8). Conclusions: The benefits of surgery were evident, even in the patients with SI;as there was no need for ICU admission and patients had an early hospital discharge. Morbidly obese patients with SI might be submitted to LGBP. This procedure was feasible and safe when the diagnosis was pre-operatively confirmed, and the surgical team made proper adjustments.

Gastroenteroanastomosis Techniques for Laparoscopic Gastric Bypass: Linear vs Circular Stapler

Background: Laparoscopic Roux-en-Y gastric bypass (LRYGB) is an effective and well-accepted procedure for the treatment of morbid obesity but has complications such as stenosis of the gastroenteroanastomosis (GE), GE leak, surgical site infection, and stapling malfunction. This study evaluated the efficiency of weight loss and the incidence of short- and mid-term postoperative complications in patients undergoing LRYGB in which anastomosis was performed using a linear stapler (LSA) or a circular stapler (CSA). Methods: Prospective observational study conducted between April 2016 and March 2019. The data were extracted from a hospital database that includes patients undergoing LRYGB in two different GE techniques, assessing postoperative complications and excess body weight loss. Results: Data from 457 patients were analyzed, of which 216 were in the LSA group and 241 were in the CSA group. There were four cases (1.7%) of GE stenosis in the CSA group and only one (0.5%) in the LSA group. Stapler malfunction occurred in both groups: CSA (0.4%) and LSA (0.5%), and a GE leak developed only in the CSA group (0.4%). Surgical site infection was found in five patients in the CSA group (2.1%) and two in the LSA group (0.9%). No statistical difference was found between the two groups in any of the variables analyzed (p > 0.05). Conclusions: Both stapling techniques resulted in a similar loss of excess body weight during the follow-up period. Although the LSA group had fewer total complications, these were not statistically significant, which substantiates the fact that both techniques are safe and feasible, provided they are performed by a surgeon with a long learning curve in laparoscopic bariatric surgery.

Updating of the hierarchical rock mass rating(HRMR) system and a new subsystem developed for weathered granite formations

The RMR system is still very much applied in rock mechanics engineering context. It is based on the evaluation of six weights to obtain a final rating. To obtain the final rating a considerable amount of information is needed concerning the rock mass which can be difficult to obtain in some projects or project stages at least with accuracy. In 2007 an alternative classification scheme based on the RMR, the Hierarchical Rock Mass Rating(HRMR) was presented. The main feature of this system was the adaptation to the level of knowledge existent about the rock mass to obtain the classification of the rock mass since it followed a decision tree approach. However, the HRMR was only valid for hard rock granites with low fracturing degrees. In this work, the database was enlarged with approximately 40% more cases considering other types of granite rock masses including weathered granites and based on this increased database the system was updated. Granite formations existent in the north of Portugal including Porto city are predominantly granites. Some years ago a light rail infrastructure was built in the city of Porto and surrounding municipalities which involved considerable challenges due to the high heterogeneity levels of the granite formations and the difficulties involved in their geomechanical characterization. In this work it is intended to provide also a contribution to improve the characterization of these formations with special emphasis to the weathered horizons. A specific subsystem applicable to the weathered formations was developed. The results of the validation of these systems are presented and show acceptable performances in identifying the correct class using less information than with the RMR system.
