Operative complications and economic outcomes of cholecystectomy for acute cholecystitis

bACKGROUND Recent management of acute cholecystitis favors same admission(SA)or emergent cholecystectomy based on overall shorter hospital stay and therefore cost *** adopted the practice of SA cholecystectomy for the treatment of acute cholecystitis at our tertiary care center and wanted to evaluate the economic benefit of this *** hypothesized that the existence of complications,particularly among patients with a higher degree of disease severity,during SA cholecystectomy could negate the cost *** To compare complication rates and hospital costs between SA vs delayed cholecystectomy among patients admitted emergently for acute *** Under an IRb-approved protocol,complications and charges for were obtained for SA,later after conservative management(Delayed),or elective cholecystectomies over an 8.5-year *** were identified using the acute care surgery registry and billing *** was retrieved via EMR,operative logs,and Revenue Cycle *** severity of acute cholecystitis was graded according to the Tokyo ***18 categorizes acute cholecystitis by Grades 1,2,and 3 representing mild,moderate,and severe,*** were analyzed withχ2,Fisher’s exact test,ANOVA,ttests,and logistic regression;significance was set at P<*** Four hundred eighty-six(87.7%)underwent a SA while 68 patients(12.3%)received Delayed *** rates were increased after SA compared to Delayed cholecystectomy(18.5%vs 4.4%,P=0.004).The complication rates of patients undergoing delayed cholecystectomy was similar to the rate for elective cholecystectomy(7.4%,P=0.35).Mortality rates were 0.6%vs 0%for SA vs *** with moderate disease(Tokyo 2)suffered more complications among SA while none who were delayed experienced a complication(16.1%vs 0.0%,P<0.001).Total hospital charges for SA cholecystectomy were increased compared to a Delayed approach($44500±$59000 vs$35300±$16700,P=

Ab Initio Calculation of <sup>2</sup>H and <sup>4</sup>He binding Energies

The binding energies of all hydrogen isotopes have been calculated successfully for the first time in a previous paper [J Fusion Energy, 30 (2011) 377], using only the electric and magnetic Coulomb’s laws, without using the hypothetical shell model of the nucleus and its mysterious strong force. In this paper, an elementary calculation gives the order of magnitude of the nuclear interaction. The binding energies of the deuteron and the alpha particle are then calculated by taking into account the proton induced electric dipole in the neutron. The large binding energy per nucleon of 4He, as compared to that of 2H, has been explained by a larger electric attraction combined with a lower magnetic repulsion. The binding energies have been calculated without fitting, using only fundamental laws and constants, proving that the nuclear interaction is only electromagnetic.

Electromagnetic Nature of Nuclear Energy: Application to H and He Isotopes

The one million times ratio between nuclear and chemical energies is generally attributed to a mysterious strong force, still unknown after one century of nuclear physics. It is now time to reconsider from the beginning the assumptions used, mainly the uncharged neutron and the orbital motion of the nucleons. Except for the long range Coulomb repulsion, the electric and magnetic Coulomb’s forces between adjoining nucleons are generally assumed to be negligible in the atomic nucleus by the nuclear specialists. The Schrodinger equation with a centrifugal force as in the bohr model of the atom is unable to predict the binding energy of a nucleus. In contrast, the attractive electric and repulsive magnetic Coulomb forces alone explain quantitatively the binding energies of hydrogen and helium isotopes. For the first time, with analytical formulas, the precision varies between 1 and 30 percent without fitting, adjustment, correction or estimation, proving the electromagnetic nature of the nuclear energy.

Structural and chemical mechanisms governing stability of inorganic Janus nanotubes

One-dimensional inorganic nanotubes hold promise for technological applications due to their distinct physical/chemical properties,but so far advancements have been hampered by difficulties in producing single-wall nanotubes with a well-defined *** this work we investigate,based on Density Functional Theory(DFT),the formation mechanism of 135 different inorganic nanotubes formed by the intrinsic self-rolling driving force found in asymmetric 2D Janus *** show that for isovalent Janus sheets,the lattice mismatch between inner and outer atomic layers is the driving force behind the nanotube formation,while in the non-isovalent case it is governed by the difference in chemical bond strength of the inner and outer layer leading to steric *** our pool of candidate structures we have identified more than 100 tubes with a preferred radius below 35Å,which we hypothesize can display distinctive properties compared to their parent 2D *** descriptors have been identified to accelerate the discovery of small-radius tubes and a bayesian regression approach has been implemented to assess the uncertainty in our predictions on the radius.

A platform for high-repetition-rate laser experiments on the Large Plasma Device

We present a new experimental platform for studying laboratory astrophysics that combines a high-intensity, highrepetition-rate laser with the Large Plasma Device at the University of California, Los Angeles. To demonstrate the utility of this platform, we show the first results of volumetric, highly repeatable magnetic field and electrostatic potential measurements, along with derived quantities of electric field, charge density and current density, of the interaction between a super-Alfv′enic laser-produced plasma and an ambient, magnetized plasma.

Prevalence and Associated Factors of Postpartum Depression: A Study Conducted in Health Centres in Dakar (Senegal)

Introduction: Postpartum depression is the most common psychiatric disorder affecting women in the perinatal period. According to the WHO, it affects 10% - 15% of women who have just given birth. In Africa, and particularly in Senegal, epidemiological studies focusing exclusively on postpartum depression are rare or non-existent. For this reason, we conducted this screening study on postpartum depression to assess the socio-demographic characteristics and associated factors. Methods: This was a prospective cross- sectional study with descriptive and analytical aims. It was carried out in the twelve Dakar referral health centres during the period august 2021 to June 2022. Results: At the end of our survey we had recruited one hundred and twenty-two (122) participants with a PPD prevalence rate of 25.41%. The most common age group was 24 - 29 years. 40% of the participants were of Fulani ethnicity, followed by Wolof. Married women made up 93% of the sample. The school enrolment rate was 89%. Most participants were not in paid employment. Domestic violence was present in 14% of couples. 28% of mothers reported experiencing a stressful life event during their pregnancy. 22% of the women had a chronic medical condition, of which 55% had high blood pressure. 1% of the women reported a personal psychiatric history and 5% a family psychiatric history. Most women had between two and four healthy living children. The pregnancy was unwanted in 17% of cases and 12% of participants had a complication. 6% were dissatisfied with the quality of their antenatal care. The caesarean section rate was 18%. In terms of associated factors, eight factors were identified in a bivariate analysis: single marital status, lack of support from partner, domestic violence, conflict with in-laws, and the occurrence of a stressful life event during pregnancy, the presence of a personal medical history, lack of satisfaction with pregnancy follow-up, and mode of delivery by caesarean section. Domain Structure of the Selenocysteine-specific Translation Factor Selb in Prokaryotes

Translation factor Selb is the key component for the specific decoding of UGA codons with selenocysteine at the ribosome. Selb binds selenocysteyl-tRNASec, guanine nucleotides and a secondary structure of the selenoprotein mRNA following the UGA at the 3' side. A comparison of the amino acid sequences of Selb species from E. coli,Desulfomicrobium baculatum, Clostridium thermoaceticum and Haemophilus influenzae showed that the proteins consist of at least four structural domains from which the Nterminal three are well conserved and share homology with elongation factor Tu whereas the C-terminal one is more variable and displays no similarity to any protein known. With the aid of the coordinates of EF-Tu the N-terminal part has been modelled into a 3D structure which exhibits intriguing features concerning its interaction with guanine nucleotides and other components of the translational apparatus. Cloning and expression of fragments of Selb and biochemical analysis of the purified truncated proteins showed that the C-terminal 19 kDa protein fragment is able to specifically bind to the selenoprotein mRNA. Selb, thus, is a translation factor functionally homologous to EF-Tu hooked up to the mRNA with its C-terminal end. The formation by Selb of a quaternary complex in vivo has been proven by overexpression of truncated genes of Selb and by demonstration that fragments comprising the mRNA or the tRNA binding domain inhibit selenocysteine insertion

Inventory and Distribution of benthic Macroinvertebrates, Indicators of Water Quality in Four Forested Watercourses in Centre Region of Cameroon

The inventory of benthic macroinvertebrate species was carried out in some streams of the Mefou catchment area, from august 2021 to august 2022, in order to determine the biological groups that make up the population and characterise these environments. The dataset acquired during this study made it possible to update the existing database and contribute to the knowledge of the biological groups associated in these environments, and to provide additional information on the existing ones in relation to bioindication. Overall 80 families were identified and, on the basis of frequencies of occurrence greater than 75%, 22 families were retained as potential biological indicators. The remarkable diversity of insects and the low abundance of Diptera reflect the low degradation of environments and the good quality of water. The high diversity indices obtained reflect the maintenance of the favourable ecological conditions that favours the development of a balanced and, integrated biological community capable of adapting to changes.
